1.Laparoendoscopic examination in ascites of unknown origin

Objective To investigate the diagnostic value of laparoendoscopic exploration and peri-toneum biopsy in patients with ascites of unknown origin and to study the feasibility of replacing hard laparo-endoscope with ultrathin gastroendoscope. Methods A total of 35 patients with ascites of unknown origin underwent laparoendoscopic exploration and biopsies were taken at suspected sites on peritoneum. Results Definite diagnosis was made in 33 patients after laparoendoscopy (94. 3% ), among which there were 10 tuberculosis peritonitis (28. 6% ), 7 metastatic peritoneal carcinoma (20%), 5 liver cancer, 3 liv-er cirrhosis, 3 primary peritoneal carcinoma, 2 ovary cancer, 1 hepatovascular sarcoma, 1 spleen lymphoma and 1 acidophilic enteritis. No procedure related complications were observed. Conclusion Laparoendosco-py is safe and effective in target peritoneal biopsy, and is of great importance in diagnosis of ascites of un-known origin.
2.Retrospective study on distribution rule of manifestations in patients with carotid artery atherosclerosis

ObjectiveTo explore clinical distribution rule of manifestations in patients with CAS,to provide objective basis of standardization,clinical diagnosis and treatment.MethodsEpidemiological methods were used to investigate on guanan'men hospital patients with CAS.Altogether 298 records of CAS patients were enrolled in this study,the frequency of main syndromes and the distribution characteristics of accompanying symptoms were analyzed.ResultsThe main syndromes of CAS were sputum stasis syndrome,qi stagnation and blood stasis syndrome,and deficiency syndrome of both liver and kidney yin,all together accounted for 39.3%.The major symptoms were vertigo or dizziness (64.1%),chest distress (60.1%),dry mouth (63.8%),amnesia (58.7%),and insomnia (50.0%) separately.ConclusionSputum stasis syndrome,Qi stagnation and blood stasis syndrome and deficiency syndrome of both liver and kidney yin group were as a combine form in patients with CAS,and dizziness or vertigo was its main clinical manifestation.

4.Immunoscreening and Sequence Analysis of a cDNA Library of Adult Trichinella spiralis

Objective To obtain an antigenic gene of adult Trichinella spiralis. Methods cDNA library of the adult Trichinella spiralis was screened using the sera of immunized and infected rabbits. The gene sequence was analyzed by DNAstar software and GenBank database. Results Nine positive clones were identified by immunoscreening. The clone Ts87 was sequenced and a cDNA with 1 172 bp full length was obtained using 5′ RACE technique, encoding 347 amino acids. Some possible antigen epitopes were predicted. Conclusion A novel antigenic gene of Trichinella spiralis was obtained.
5.Efficacy of aseptic processing on dental unit waterlines

Objective To compare the effect of different ways of water supply and whether or not drain water from waterlines for overnight on bacterial counts in dental unit waterlines(DUWLs).Methods In the first phase,6 sets of DUWLs were randomly divided into 2 groups (external storage tank water supply group and municipal water sup-ply group);in the second phase,6 sets of DUWLs were all changed to use external storage tank and randomly di-vided into 2 groups (draining water for overnight group and without draining water for overnight group),bacterial count before and within one week of disinfection between two groups at two phases were compared respectively. Results In the first phase,handpiece water of DUWLs was most seriously contaminated by bacteria,the average colony count was 4117 CFU/mL,qualified rate was 15.38%.Before disinfection,no significant difference in bacte-rial count were found among each groups (all P>0.05),bacterial count of DUWLs of all groups severely exceeded the standard(all >3000 CFU/mL).Comparison of bacterial count in DUWLs from different water supply routes after disinfection was not significantly different on day 1(P>0.05),but were significantly different at day 2-7(all P<0.05).On the second day after disinfection,municipal water supply group began to exceed bacteria standard;on the third day after disinfection,external storage tank group began to exceed bacteria standard.Bacterial count in DUWLs after disinfection between draining water for overnight group and without draining water for overnight group was no significantly different on day 1(P>0.05),but were significantly different on day 2-7(all P<0.05). On the fourth day of disinfection,bacterial count of without draining water for overnight group exceeded standard.On day 7 of disinfection,bacterial count in draining water for overnight group exceeded>100 CFU/mL.Conclusion Use of external storage tank,daily change of sterile distilled water,and daily emptying water for overnight can effectively reduce bacterial count in DUWLs.
6.Cisplatin blood concentration of patients with malignant bone tumor of lower extremity with out discarding blood after hyperthermic antiblastic perfusion

Objective To explore the possibility of non-discarding blood after cisplatin hyperther-mic antiblastic perfusion(HAP)in treating the patients with malignant bone tumor of lower extremity.Methods Forty patients consisted of 31osteosarcomas and9malignant fibrous histiocytomas of bone.The clinical stages were wholly classified intoⅡB stage according to Ennekings surgical staging system for muscu-loskeletal tumors.The patients were divided into three groups:group1:discarding blood group(10cases),in whom200ml of the400ml blood remaining in heart-lung machine was discarded after HAP;group2:Non-discarding blood group(25cases),the blood remaining in heart-lung machine was not discarded after HAP and re-infused into systemic circulation;group3:systemic chemotherapy group(5cases),cisplatin was infused intravenously.The regional and the systemic blood cisplatin concentrations were determined at3,30and60minutes during HAP,and the systemic blood cisplatin concentrations were determined at0,0.5,1,2,4,8,12,24,48and72hours after HAP in the group1and group2.While1h after cisplatin infusion in-tra venously in the group3,the blood cisplatin concentration were also measured.Results The regional blood cis platin concen tra tions were high er than that in systemic blood at the time during HAP.The systemic blood cisplatin concen trations after HAP,ex cept at0minute in the group2were higher than that in the group1,there were sig nificant statistic differences between the two groups.The systemic blood cisplatin con-cen trations after HAP in the group2were simi lar to that after systemic chemotherapy.No severe adverse ef-fects in the group1were found.The Ennek ings surgical stagings for the tumors were turned fromⅡB intoⅡA in all the patients.Conclusion The method of non-discarding blood after HAP is available,it is not only local treatment ,but also systemic chemotherapy in the patients with malignant bone tumor of lower ex-tremi ty for limb-salvage surgery.
7.Correlation of expressed TopoⅡ? and Ki-67 with the prognosis of uterine sarcoma

However, in high LI groups, the overall survival rate was lower than those in low LI groups (P=0.002, P=0.004, Log-Rank test), the volume of recurred masses was bigger (P=0.005, P=0.000) and recurrence time was shorter (P=0.000, P=0.000) than those in low LI groups. There existed a significant linear correlation between the expressions of TopoⅡ? and Ki-67 (r=0.935 5, P=0.000). The expressions of TopoⅡ? and Ki-67 were related with pathological category and staging (P0.05). Conclusions Both TopoⅡ? and Ki-67 could be used as specific markers of cell proliferation, but the specificity of TopoⅡ? might be higher than that of Ki-67 in accurately reflecting the cell proliferation. TopoⅡ?, combined with pathological category and staging, may play an important role in prognostic prediction of uterine sarcoma.
8.Incidence and Drug-Resistance of Helicobacter pylori from Patients with Peptic Ulcer

OBJECTIVE To investigate incidence and drug susceptibility of Helicobacter pylori(Hp) in local area.METHODS Strains of Hp were isolated and cultivated from gastric antrum mucosa of 141 peptic ulcer disease(PUD) patients diagnosed by gastroscope.The drug sensitive test of Hp strains was examined by K-B method.RESULTS The isolation rate of Hp in 141 PUD patients was 75.2%.Hp isolates showed a relative hypersensitivity to clinical commonly used drugs(ie furazolidone,amoxicillin,levofloxacin,cefuroxime and moxifloxacin).On the other hand,Hp isolates exhibited resistance to metronidazole and clarithromycin.CONCLUSIONS Hp is an important etiologic factor of PUD,and showing an increasing resistance to common clinical antibiotics.
9.Preparation and biodistribution of paclitaxel-loaded nanoparticles after intravascular infusion

ObjectiveTo prepare paclitaxel-loaded nanoparticles (NPs),and to observe drug biodistribution after intravascular infusion of the NPs using a DispatchTM catheter into New Zealand rabbit abdominal aorta models.Methods Paclitaxel-loaded NPs were prepared by ultrasonication/emulsificcation/solvent evaporation technique using biodegradable poly (lactic-co-glycolic acid)(PLGA) as drug carrier.NP size and morphology was assessed by submicro-laser defractometer and scanning electron microscopy.In vitro release of paclitaxel from the NPs was performed by shaking in PBS at 37℃.The NPs was delivered into New Zealand rabbit abdominal aorta using a DispatchTM catheter.ResultsThe diameter of paclitaxel NPs was around 246 nm with very narrow size distribution.The NPs showed good spherical shape with smooth uniform surface.Paclitaxel loading in the NPs was about 19.06% with encapsulation efficiency about 93.25%.The NPs maintained a sustained in vitro drug release for 30 days in PBS.After in vivo NP infusion,paclitaxel was detected in the vascular tissue around the infusion site and it retained in the site for 21 days.ConclusionPLGA nanoparticles as local drug delivery carrier showed great potential to maintain a high local drug concentration and prolonged drug resident time in animal model in vivo.
