1.Assessment of bone metabolic biochemical indexes measurement in gynecological clinical practice

Objectives To evaluate prospectively the bone markers of serum osteocalcin (BGP),bone-specific alkaline phosphatase (BAP),C-terminal peptide of type I collagen (CICP),urinary pyridinoline (PYD),urinary calcium to creatinine value (uCa/Cr),and hydroxyprolin to creatinine ratio (uHOP/Cr),and to assess their responsiveness to subtle changes of bone metabolism,and furthermore to compare which of the bone markers are more sensitive to estrogen shortage caused by menopause.Methods Markers of above bone formation and bone resorption were measured in 146 women classified into three groups: group I: 51 normal premenopausal women,group II:42 postmenopausal women,and group III: 53 bilateral ovariectimized women.Correlation analysis was made between these indexes and bone mineral density (BMD).Results The mean values of the four bone markers of BGP,BAP,CICP,PYD in group II and III were significantly higher than those in group I (P<0.05).All correlations between the biochemical markers of BGP,BAP,CICP and PYD for bone turnover were highly significant and exhibited a significant connection to BMD.The best correlation was found between BGP and PYD,BGP and BAP.However,the levels of Ca/Cr and HOP/Cr made no difference in the three groups,and there was no correlation with any other indexes.Conclusion In postmenopausal women (natural menopause and surgical menopause) suffering from severe E2 dificiency,BGP,BAP,CICP and PYD are significantly increased,indicating a clear correlation between E2 diffciency and these markers of bone turnover,so they are clinically useful in the assessment of bone turnover during changes of the estrogen status that modify the metabolic activity of the skeleton.But the traditional markers of Ca/Cr and HOP/Cr have limitations in gynecological practice because of lack of sensitivity and specificity.
2.Peropective comparison of hydroxyapatite orbital implant between primary and second drilling procedure

Objective To evaluate the clinical effect of the procedure that involves drilling and inserting a motility peg at the time of hydroxyapatite orbital implant.Methods 89 patients who needed hydroxyapatite orbital implant were divided into groups A and B.The patients in group A were drilled and inserted a motility titanium peg when hydroxyapatite implantation.The patients in group B were drilled and inserted a motility titanium peg 6 months after hydroxyapatite implantation.Data were recorded.including the rate of postoperative pain and conjunctive edema,postoperative facial appearance,motility of the artifieial eye,the rate of hydroxyapatite exposure,excursion and extrusion of titanium peg and other complications.Results The mobility of the prosthesis was 44.97%in group A,and 45.34%in group B.There was no difference in appearance and the motility of their artificial eye between the two groups.The rate of excursion of titanium peg was 4.2% in group A and 7.3%in group B and no statistical difference was found between the two groups.There were no prosthesis exposure,extrusion of titanium peg,secondary infeetion and other complication in one-year follow-up.There were no difference in the rate of postoperative pain and eonjunctive edema in first operative stage.The postoperative reaction in group A was lighter than that in group B in secondary operanve stage.Conclusion The hydroxyapatite orbital implant primary procedure drilling is simple,safe,convenient and effective.It is worth to recommend that procedure that involves drilling and inserting a motility peg at the time of hydroxyapatite orbitaI primary implant.
3.Application of PMMA artificial eye in ophthalmosteresis

Objective To evaluate the application of the polymethyl methacrylate (PMMA) artificial eye in acquired ophthalmostewesis. Methods We summarized the experience in the application of PMMA in prosthesis for 8 years. 1155 patients were involved in the artificial eye custom-made program. The artificial eye was individually prepared based on the patient's condition and characters of the healthy eye.Results PMMA artificial eye associated with hydroxyl apatite implantation gave good cosmetic results with good motility, and there was no rejective reaction. The artificial eye was almost same to the real one in color and transparency. Conclusion It is suggested that PMMA is one of the best materials for manufacture of artificial eye at present.
4.Deep vein hemodynamics of the lower limbs in obesity

Objective To evaluate lower extremity deep venous hemodynamic changes and lower extremity deep venous disease in abdominal obesity.Methods To compare venous flow parameters of the lower limbs as assessed by Duplex ultrasound scanning in obese and nonobese individuals according to body mass index(BMI).Venous hemodynamics were studied in a prospective cohort study in nonobese( BMI < 25 kg/m2) and obese individuals( BMI > 30 kg/m2 ).Diameter,peak,mean,and minimum velocities were assessed.Results The study examined 45 limbs in 24 nonobese individuals and 44 limbs in 22 obese individuals.The diameter of the femoral vein was significantly greater in obese vs nonobese limbs ( 10.4 ± 1.3) mm vs (7.1 ± 1.0) mm,P < 0.01 ).Calculation of venous amplitude and shear stress showed significantly higher values in nonobese vs obese limbs [ ( 11 ± 5 ) vs ( 8 ± 3 ) cm/s ],( P < 0.01 ) and [ (0.50 ± 0.17 ) vs ( 0.30 ± 0.09 ) ] dyn/cm2,( P < 0.01 ).Spearman rank correlation revealed a significant inverse correlation between waist-to-hip ratios and waist circumference and velocities amplitude (peak ve locity-minim velocity),and shear stress.Conclusions Lower limb venous flow parameters differ significantly between obese and nonobese individuals.These findings suggest the mechanical role of abdominal adipose tissue potentially leading to elevated risk for both chronic venous insufficiency and venous thromboembolism.
5.Prognostic value and influencing factors of active movement of hemiplegic limbs in patients with acute stroke

Limb rehabilitation is one of the key issues for improving the quality of life in patients with stroke. Studies have demonstrated that the retention and recovery of active movement of hemiplegic limb have predictive value for the prognosis in acute stroke. The responsible lesion sites and rehabilitation training in the acute phase may be an important factor in influencing the recovery of active movement ability of hemiplegie upper limb. However, there is still lack of a large, multi-center study, and further related studies are needed.
6.Glutamate-mediated signaling pathway regulates the invasion and growth of malignant melanoma

Objective To investigate the action mechanism of glutamate-mediated signaling pathway in malignant melanoma. Methods WM451LU melanoma cells in log phase were classified into 6 groups, negative control group treated with PBS (100 μl), MK801 group treated with the non-competitive N-methyl-D-aspartate (NMDA) receptor antagonist MK-801 (100 μmol/L), CPCCOEt group treated with non-competitive metabotropic glutamate receptor 1 (mGluR1) antagonist CPCCOEt, MAP2 group transfected with adenovirus vector containing microtubule associated protein 2a (Ad-MAP2a), MK801 + MAP2 group treated with MK801 of 100 μmol/L and transfected with Ad-MAP2a, CPCCOEt + MAP2 group treated with CPCCOEt of 10 μmol/L and transfected with Ad-MAP2a. Western blot was performed to detect the expression of an ionotropic glutamate receptor, i.e., N-methyl-D-aspartate receptor type 2A (NMDAR2A) in WM451LU cells transfected with Ad-MAP2a. Scratch motility assay and cell invasion assay were conducted in vitro to detect the changes in migration and invasion ability of WM451LU cells after treated with Ad-MAP2a, MK-801, CPCCOEt alone or in combination. In vivo study was carried out to compare the inhibitory effect of the above treatments on melanoma. Results Western blot revealed a decrease in the expression of NMDAR2A in WM451LU cells after transfected with Ad-MAP2a. The scratch motility assay showed that the number of migrating cells per high power field was 117.04 ± 2.76 in MAP2 group,107.64 ± 6.50 in MK801 group,97.36 ± 4.79 in CPCCOEt group, 43.28 ± 3.02 in MK801 + MAP2 group,30.76 ± 3.97 in CPCCOEt + MAP2 group,significantly different from that in the negative control group (152.3 ± 5.75,all P < 0.01 ). Cell invasion assay demonstrated that the average number of invading cells per high power field in the negative control was significantly higher than that in MAP2 group, MK801 group, CPCCOEt group, MK801+MAP2 group and CPCCOEt + MAP2 group (170.43 ±8.72 vs. 98.26 ± 3.84, 97.22 ± 5.54, 112.23 ± 7.21, 42.89 ± 5.06, 58.25 ± 6.68, P < 0.05, 0.05, 0.05, 0.01and 0.01, respectively).A significant decrease was observed in the average volume of experimental melanoma in mice of MAP2 group, MK801 group, MK801 + MAP2 group, CPCCOEt group and CPCCOEt + MAP2 group compared with the negative control group (224.02 ± 46.19 mm3, 160.33 ± 33.91 mm3, 91.49 ± 21.48 mm3,202.30 ± 52.37 mm3, 111.13 ± 69.81 mm3 vs. 342.70 ± 60.92 mm3, all P < 0.01 ). Conclusions To block the glutamate signaling pathway in vitro can inhibit the invasion and migration of melanoma cells, and to block the pathway in vivo can inhibit the growth of malignant melanoma and alter the morphology of melanoma cells.
7.Experimental Study on Dependence of Diffusion Tensor-derived Parameters upon Diffusion Time

Objective To investigate the relationship between diffusion time and diffusion tensor-derived parameters. Method Diffusion tensor data sets were obtained from 11 healthy volunteers using 8 different levels of diffusion time while keeping the diffusion sensitive gradient magnetic field strength fixed. Diffusion tensor-derived parameters with variant diffusion time for six regions of interest were calculated and compared. Result There was statistically significant difference in mean diffusivity (MD) of variant diffusion time (P0.05). Conclusion There is no diffusion time dependent in FA, but MD are diffusion time dependent. The reason may be attributed to the difference of the diffusion properties between the intra- and extra-cellular water molecules.

Objective To investigate the roles of glutamate signaling pathway in melanin transfer.MethodsEpidermal melanocytes and keratinocytes were isolated from human foreskin tissue followed by purification and primary culture.Immunofluorescence microscopy was conducted to observe the intracellular distribution of N-methy-D-aspartate receptor 1 (NMDAR1) and NMDAR2A in melanocytes.Some melanocytes were classified into 4 groups to be pretreated with MK801 (the NMDAR antagonist dizocilpine maleate) at 100μmol/L for 5 minutes followed by treatment with NMDA(an NMDAR agonist) at 100 μmol/L (MK801-pretreated group 1),pretreated with MK801 at 100 μmol/L for 1 hour followed by treatment with NMDA at 100μmol/L (MK801-pretreated group 2),treated with MK801 at 100 μmol/L for 5 minutes (MK801 group),treated with NMDA at 100 μmol/L for 5 minutes (NMDA group),respectively,then,confocal microscopy was performed to measure the intracellular calcium (Ca2+) concentration of the melanocytes.The distribution of β-tubulin was visualized by confocal microscopy in melanocytes treated with MK801 at 100 μmol/L for 24 hours.Some melanocytes and keratinocytes were cocultured with or without MK801 at 100 μmol/L for 24 or 48 hours,then,scaning microscopy was carried out to observe the junction structure between melanocytes and keratinocytes,and alkali method coupled with spectrophotometric analysis to determine melanin content in keratinocytes.Results The intracellular calcium concentration of melanocytes was decreased by MK-801,but increased by NMDA at 100 μmol/L,and the increase was blocked by the pretreatment with MK-801 for 5 minutes or 1 hour.After incubation with MK-801 at 100 μmol/L for 24 hours,a more intense staining for β-tubulin was observed around the nuclei of melanocytes.There was a significant reduction in the number of filopodia on the surface of and between melanocytes and keratinocytes after treatment with MK-801 at 100 μmol/L for 48 hours.Also,the content of melanin(represented as the absorbance value at 375 nm) transferred from melanocytes into keratinocytes was statistically reduced in coculture system treated with MK-801 at 100 μmol/L compared with that without treatment (0.158 ± 0.003 vs.2.203 ± 0.006,t =6.323,P < 0.01 ).Conclusions The glutamate signaling pathway exerts a regulatory effect on intracellular calcium concentration of distribution of β-tubulin in,filopodia formation of melanocytes and melanin transfer between melanocytes and keratinocytes.
9.Detecting underlying malignancy with 18F-FDG PET/CT in patients with suspected paraneoplastic neurological syndrome

Objective To evaluate 18F-fluorodeoxyglucose (FDG) PET/CT in detecting occult malignancy in patients with suspected paraneoplastic neurological syndrome (PNS).Methods Twenty consecutive patients who underwent PET/CT scanning with the indication of suspected PNS were retrospectively reviewed.The gold standard of PNS was either cytology or clinical follow-up, and the final diagnosis was compared with PET/CT findings.Results Of the 20 patients, six were PNS.PET/CT detected nine cases.Six were true positive and three were false positive.The sensitivity, specificity, accuracy, positive predictive value and negative predictive value of PET/CT were 100% (6/6), 78.57% (11/14), 85.00% (17/20),66.7% (6/9) and 100.00% ( 11/11 ) respectively.The treatment plan was modified based on the PET/CT results in 4 patients.Conclusions 18F-FDG PET/CT may play a role in detecting the underlying malignancy of PNS.It is also valuable in staging of the malignancy thus providing information for therapy decision making.
10.Effect of the side of cerebral lesion on the outcome in patients with acute ischemic stroke

Objective To investigate the effect of the side of cerebral lesion on the outcome in patients with ischemic stroke.Methods A total of 407 consecutive anterior circulation ischemic stroke patients within 14 days after symptom onset were recruited prospectively.The basic data ofthe e,~ISes were collected,such as the National Institutes ofHealth Stroke Scale (NU-ISS)and the side ofcerebral lesion.The modified Rankin Scale(mRS)was used to evaluate the prognosis of the patients at 6 raomhs.Results Of the 407 patients recruited,230 patients (56.5%)Were left hemisphere stroke,177(43.5%)were fight hemisphere stroke.After multivariable logistic recession analysis,the age(odds ratio[OR]1.022,95% confidence interval[CI]1.001-1.043,P=0.040),the side of lesion(OR 1.999.95%CI1.179.3.389.P=0.010),the time from onset to admission(OR1.006,95%(7/1.002-1.010,P=0.007),the outcome of the anterior circulation ischcmic stroke at 6 months aftel"onset.The outcome of the right hemisphere stroke Was significantly worse than that of the left hemisphere stroke.The onset-admission time in patients with right hemisphere stroke(median 12 h,median 39.61 h)was significantly delayed compared to the patients with left hemisphere stroke(median 12 h,median 22.72 h;Z=-2.962,P=0.003).Condusions The outcome of the left hemisphere stroke at 6 months after onset is superior to the right hemisphere stroke,and it mau be associated with the delayed admission.
