1.Diagnosis and treatment of hepatic endometriosis misdiagnosed as hepatic echinococcosis

		                        			Hepatic endometriosis is a rare form of atypical endometriosis,only 30 cases were reported worldwide.Most cases of hepatic endometriosis were misdiagnosed due to the lack of typical symptoms.One patient with space-occupying lesions in the liver diagnosed by color Doppler ultrasonography were admitted to the Fuzhou General Hospital of Nanjing Military Command on November 5,2012.The patient was preliminarily diagnosed as with hepatic echinococcosis and then received operation.The result of postoperative pathological examination confirmed hepatic endometriosis.
		                        		
		                        		
		                        		
		                        	
2.Construction and application of pharmacophore model of benzoylurea derivatives as beta-tubulin inhibitors.

		                        			Ten pharmacophore models of beta-tubulin inhibitors were established from the training set of seventeen beta-tubulin inhibitors (two categories) with comformer analysis by using the Catalyst software. The optimal pharmacophore model with two hydrophobic units and two hydrogen bond acceptor units were confirmed (RMS = 0.43, Correl = 0.98, Weight = 2.06, Config = 15.97). This pharmacophore model is able to predict the activity of known beta-tubulin inhibitors and can be further used to identify structurally diverse compounds with higher activity.

		                        			Objective To study the influence of the different tidal volume on hemodynamics,pulmonary ventilation and pulmonary mechanics during mechanical ventilation in patients with acute lung injury.Methods To adopt contrast method,making use of the VIIP +0.196 kPa of P-V curve to decide PEEP,then according to the VUIP of P-V curve,obtaining the three different data of humidity 100%Vt,85% Vt and 70 Vt.At the same time,monitoring the change of static compliance,the changing of hemodynamics and P-V curve.Results 85%Vt group was better than 100%Vt group and 70%Vt group in the heart rates,CVP,PaO_2,the peak airway pressure(PIP) and mean pressure(Pm),Cst,PaO_2/FiO_2.Conclusions When insuring the PEEP data in low inflection point pressure (Pinf) of breathing system P-V curve,and 85% humidity in upper inflection point pressure,which is in accordance with individual protecting breathing strategy.The curative effect is the best for improving static compliance and decreasing the harm of lung injury.The change of pressure-volume curve should be paid attention to in nursing.
		                        		
		                        		
		                        		
		                        	
4.Analysis on Chemical Compositions and Antioxidant Activity of Different Processing Decoction Pieces of Rosae Laevigatae Fructus

		                        			Objective To investigate the honey and bran of processing for chemical compositions of Rosae Laevigatae Fructus.Methods With total polyphenols, total saponins, total polysaccharides and amino acids as the evaluation indexes, changes of the chemical compositions of these four chemical components in Raw product, honey products, bran products of Rosae Laevigatae Fructus were studied; in vitro activity of DPPH radical clearance for evaluation index, antioxidant activity of raw products, honey products, bran products of Rosae Laevigatae Fructus were compared.Results Raw products, honey products, and bran products of total saponin content were 3.332%, 4.880%, and 4.572%; total phenolic contents were 1.92%, 6.38%, and 7.30%; total polysaccharide contents were 35.99%, 40.38%, and 36.86%; the total amino acid contents were 0.67%, 0.76%, and 0.96%. Total polyphenol, total saponins, total polysaccharides, and total amino acids of Rosae Laevigatae Fructus after processing increased, in which the total polyphenol was most obvious; DPPH radical scavenging IC50 of raw products, honey products, and bran products were 0.47, 0.51, and 0.22 mg, respectively.Conclusion Compared with raw product, the content of chemical fractions in honey products and bran products of Rosae Laevigatae Fructus increase, and oxidation resistance is enhanced.

6.Invasive pulmonary aspergillosis: 20 cases

		                        			Objective To analyze the clinical, radiological, pathological and microbiological features of invasive pulmonary aspergillosis (IPA) to improve clinical management.Methods Retrospective analysis of 20 pathologically and/or microbiologically confirmed IPA cases in our hospital from January 2005 to August 2008. Results Group A (with underlying diseases) included 13 patients (underlying malignancy in 9 patients, including 5 cases of hematological malignancy, COPD in 2 patients, pulmonary tuberculosis and bronchiectasis in 1 each). Group B (without underlying disease) included 7 patients (2 patients with a long time of fluffy toy contact, another 1 had exposure to moldy rice, and 3 had exposure to polluted water). All these 20 patients had pulmonary invasion revealed by CT imaging. Multiple changes were identified in 16 patients. Bilateral pulmonary infiltrates and/or consolidation were revealed in 7 patients. Multiple nodules were seen in 9 patients. Four patients had solitary lesions, including isolated nodules in 2 patients and segment consolidation in the other 2 patients. Pulmonary cavity without fluid level was found in 8 patients (40.0%). Eighteen cases received antifungal therapy. The overall efficacy rate was 55.6%. The efficacy rate in group A and B was 45.5% and 5/7 respectively. The average time to symptomatic relief was (12.0±2.8) days. The time to lung lesion improvement on CT was (17.4±2.9) days. The time to significant CT improvement was (34.3±9.9) days. The time to the resolution of active lesion was (56.4±6.2) days.Conclusions IPA may occur in immunocompetent patients without underlying disease. Most IPA patients have bilateral multiple pulmonary nodules and cavities on CT. 7.Relationships between the levels of serum bone-specific alkaline phosphatase, cross-linked telopeptide of type Ⅰ collage and bone metastasis in patients with breast cancer

		                        			Objective To analysis the relationships between bone markers, bone-specific alkaline phosphatase (BAP) and cross-linked telopeptide of type Ⅰ collage (ICTP), and bone metastasis of breast cancer.Methods A total of 217 patients' serum were collected.The 217 cases were divided into two groups:109 cases with bone metastasis, 108 cases without bone metastasis. Serum BAP and ICTP was measured by ELISA. The relationships between factors of bone metastasis and serum levels of BAP, ICTP were analyzed.Results The levels of serum BAP and ICTP in bone metastases group were significantly higher than those in non-bone metastasis group[BAP:24.8 μg/L(7.60-213.70 μg/L) vs 21.2 μg/L(7.3~68.8 μg/L),ICTP:7.0μg/L(1.4~32.4 μg/L) vs 4.1 μg/L(0.0~15.8 μg/L) (P=0.003,P=0.000)].The level of serum BAP and ICTP in patients with multiple bone metastasis was significantly higher than that in patients with single bone metastasis[BAP:32.3 μg/L(9.A~213.7 μg/L) vs 18.1 μg/L(7.6~60.0 μg/L),ICTP:7.6 μg/L(1.4~32.4 μg/L) vs 4.9 μg/L(1.8~10.5 μg/L),(P=0.001,P=0.010)].The sensibility of BAP and ICTP was 45.0 % (49/109)and 46.8 % (51/109),respectively.The specificity of ICTP and BAP was 83.3 % (90/108)and 84.3 % (91/108),respectively.Joint detection of BAP and ICTP had improved sensibility in the diagnosis of bone metastasis in breast cancer patients. 8.Preliminary clinical study of recombinant human endostatin combined with radiotherapy in the treatment of brain metastases of non-small cell lung cancer and the patients suitable for this therapy

		                        			Objective To investigate the therapeutic effects of recombinant human endostatin (RHES) combined with radiotherapy on brain metastases (BM) of non-small cell lung cancer (NSCLC) and the patients suitable for this therapy.Methods Eighty patients with BM of NSCLC were randomly divided into RHES combined with radiotherapy group (combination group) and radiotherapy alone group (each group with 40 patients).The short-term effective rate,overall survival time,cerebral edema index and adverse reactions were observed and the expressions of vascular endothelial growth factor receptor 2 (VEGFR2) protein in primary lesions were detected with immunohistochemical method in all patients.Results Compared with radiotherapy alone group,brain edema was significantly relieved (t=4.9,P=0.000) and there were no marked adverse reactions in combination group.In short-term effective rate,there was no statistical significance in total population (n=80,90% vs.75%,x2=3.11,P=0.07),but there was statistical significance in the patients with positive VEGFR2 (93% vs.67.7%,x2=6.31,P=0.012).In overall survival time,there was no statistical significance in total population (n=80,P=0.35,95% CI:0.25-1.30) or in the patients with positive VEGFR2 (P=0.109,95% CI:0.40-1.34).Conclusion Compared with radiotherapy alone,RHES combined with radiotherapy can relieve brain edema in the patients with BM of NSCLC and obtain better short-term effective rate in the patients with positive VEGFR2.
		                        		
		                        		
		                        		
		                        	
9.Human umbilical cord mesenchymal stem cells are induced in vitro to differentiate into fibroblasts

		                        			BACKGROUND:The umbilical cord mesenchymal stem cells possess multipotent differentiation capacity, but less research focus on its differentiation into fibroblasts. 
 OBJECTIVE:To investigate the capacity of human umbilical cord mesenchymal stem cells differentiating into fibroblasts. 
 METHODS:Using adherent method, human umbilical cord mesenchymal stem cells were isolated, and flow cytometric analysis of the surface antigen was performed. Passage 3 cells were selected for osteogenic and 
 adipogenic differentiation, and cells differentiated into fibroblasts under the induction of basic fibroblast growth factor. RESULTS AND CONCLUSION:Adherent stem cells were stably isolated from the umbilical cord. Human umbilical cord mesenchymal stem cells lowly expressed CD31, CD45, CD40, HLA-DR, but strongly expressed CD29, CD90, CD44, CD105. Oil red O staining showed red droplets were ful of the cytoplasm after adipogenic induction;alizarin red staining showed red calcium nodules after osteogenic induction. After induced by basic fibroblast growth factor, the type I col agen expression was significantly higher than that in the control group. These findings indicate that the adherent human umbilical cord mesenchymal stem cells are reliably isolated with high purity;basic fibroblast growth factor can induce differentiation of umbilical cord mesenchymal stem cells into fibroblasts.
		                        		
		                        		
		                        		
		                        	
10.Evaluation and prediction of medical complications after hip fracture surgery

		                        			Objective To analyse the risk factors of medical complications after hip fracture surgery.Methods Clinical data of 327 hip fracture patients undergoing surgical treatment in Beijing Haidian Hospital from January 2009 to December 2013 were retrospectively studied.There were 114 males and 213 females with a mean age of (75.1 ±15.6) years, 129 patients(39.4%) had femoral neck fractures, and 198 patients ( 60.6%) had intertrochanteric fractures.Fifty four patients had medical complications during hospitalization ( complication group) and 273 patients did not have complications ( non-complications group ) . The clinical features, presurgical comorbidities and operative parameters were documented and compared between two groups.The risk factors of medical complications were assessed by regression analysis.Results Univariate analysis showed that age ( t =2.85, P=0.007), lying in bed before fracture(χ2 =12.86,P =0.000), the history of chronic obstructive pulmonary diseases(COPD) (χ2 =9.45, P=0.000), coronary heart disease(χ2 =10.67,P=0.000), heart failure(χ2 =15.85,P=0.000), diabetes (χ2 =3.22,P=0.024), cerebrovascular disease(χ2 =4.21,P=0.008), anesthesia method(χ2 =8.67,P=0.018), American Society of Anesthesiologists(ASA) score ≥3 level(χ2 =2.01, P=0.042) and the delay time of operation (t=4.24, P=0.037) were significantly different between two groups.Multivariate regression analysis revealed that age(β=0.030, OR=1.030), lying in bed before fracture (β=2.303, OR=10.716), the history of COPD (β=0.831, OR =1.995), coronary heart disease(β=0.858, OR=2.392), heart failure(β=1.971, OR=7.333)and ASA score ≥3 level(β=0.809,OR=2.583) were the independent risk factors of medical complications after hip fracture surgery. Conclusion Age, lying in bed before fracture, the history of COPD, coronary heart disease, heart failure and ASA score≥3 level would increase the risk of postoperative medical complications for patients with hip fracture.
