Diabetic retinopathy (DR) is a common cause of blindness,its occurrence and development are the synergic results of multiple factors.Current studies suggest that inflammation and inflammatory factor has an important role in the pathogenesis of DR.The occurrence and development of DR are closely related with interleukins,intercellular adhesion molecules,hasten factors,tumor necrosis factor,C-reactive protein etc.Mesenchymal stem cells (MSCs) are pluripotent cells derived from the mesoderm and have multiple differentiation potentials,and anti-inflammatory and immunosuppressive function.Recent studies shown that MSCs transplantation can protect damaged retina by inflammatory regulation,which become a new research direction for DR treatment.

Microvesicles (MVs) is small membrane vesicles released from different cell types under different conditions.Studies have shown that MVs may mediate vascular inflammation,angiogenesis,and other pathological processes.MVs may play an important role in the pathogenesis of diabetic retinopathy (DR) by mediating endothelial cell injury,thrombosis and neovascularization.The plasma MV level may be an effective parameter to monitor the development of DR.This article will summarize the research progress of the relationship between MVs and DR in recent years.

OBJECTIVETo study clinical effects of inversive LISS (less invasive stabilization system, LISS) plate combined with steel wire for the treatment of Seinsheimer type V subtrochanteric femoral fractures.

METHODSFrom January 2009 to Janurary 2012, 11 patients with Seinsheimer type V subtrochanteric femoral fractures were treated with inversive LISS plate combined with steel wire, including 8 males and 3 females, ranging in age from 35 to 61 years old (averaged, 51 years old). Causes of injury: 8 patients caused by accident and 3 patients caused by falling down. According to X-ray examination, all the patients were type V subtrochanteric femoral fractures and were treated with LISS plate combined with steel wire. Merle d'Aubigne scores were used to evaluate therapeutic effects on hip.

RESULTSThe mean period of follow-up was 16 months (ranged, 12 to 28 months). All the patients obtained bone union for average 3.8 months (ranged from 3 to 4.3 months). There were no complications such as deep infection,deep vein thrombosis, pulmonary embolism and bone nonunion. According to Merle d'Aubigne scores, 7 patients got an excellent result and 4 good.

CONCLUSIONInversive LISS plate combined with steel wire for the treatment of Seinsheimer type V subtrochanteric femoral fractures is satisfactory, Which can provide stable fixation for fracture end and provide an ideal internal fixation method for treatment of this kind of fractures in the future.

OBJECTIVE:A HPCE method for determination of Quercetin in acanthopanax and acanthopanax injection was developed.METHODS:Measurements were carried out in a buffer solution containing 50mmol/L boric acid solution and 50mmol/L SDS(pH 8.5),at 20kV running voltage applied to a 50cm?50?m capillary.The detection wavelength was 260nm.RESULTS:The linear range of quercetin was 0.0 125～1.0mg/ml.The recovery of acanthopanax was (98.99?0.63)%.The recovery of acanthopanax injection was (98.62?0.42)%.CONCLUSION:The method is simple,rapid,accurate and suitable for determination of quercetin in acanthopanax and acanthopanax injection.

Objective To evaluate the effect of vitreoretinal surgery with lens sparing technique in treating the detachment with giant retinal tear(GRT) associated with proliferative vitreoretinopathy(PVR). Methods Thirty one consecutive eyes with GRT underwent vitrectomy were analysed retrospectively. Operative techniques included peeling of pre retinal membrane, injection of perfluorodecalin liquid, retinotomy and retinectomy,endolaser,and silicon oil or C 2F 6 gas tamponade. Lens sparing vitrectomy was performed in 28 phakic eyes. Follow up period ranged from 11 to 34 months. Results Anatomic retinal attachment was achieved intraoeratively in 29 eyes. In 16 eyes of 28 eyes with postoperative cataract formation,3 eyes underwent cataract surgery with or without intraocular lens implantation. The corrected final visual acuity ranged from 0.4 to 0.01. Conclusion Most phakic eyes of retinal detachment with GRT PVR can be successfully operated on with an outcome of improving the visual acuity by using techniques of lens sparing vitrectomy, perfluorodecalin liquid and silcone oil tamponade.

Objective To present a new method of osteotomy for malar complex hypertrophy .Methods The operative procedures was transpositional osteoplasty of the malar complex by coronal approach. A lateral orbital osteotomy began at the level of the frontozygomatic suture line,preserving the lateral orbital rim. It continued far down inferoposteriorly across the zygomaticomaxillary suture line. The zygomatic arch was obliquely cut at 1 cm anterior from the articular tubercle. The bone was not separated from the soft tissue to preserve the blood supply to the zygoma and slid upward, inside and setback. The single microplate,screw and steelwire fixation was done on the repositioned zygomatic body.To zygomatic body prominence,it was obtained a satisfactory effect that only shaved the protruded zygomatic body with chisels by means of an intraoral approach.Operative procedures for reductive malar complex plasty were performed in 12 cases. The patients consisted of 3 males and 9 females whose ages ranged from 21 to 36 years.Results Postoperative follow up for 3～24 months showed satisfactory results using this procedure.Conclusion This modified method has an ideal effect without the possibility of bone resorption of the malar complex.

In Qin Han periods,the long-storaging method for Chinese materia medica(CMM) has been used in traditional Chinese medicine.People have named the Prescription with "chen" in the Song Dynasty,such as "Er Chen Tang".With the traditional Chinese medicine storing and treating with different methods in appropriate period,medicine becomes old medicine from the new,there by,reaching property of a medicine of reducing the poisonous or over-strong property,strengthening the medicine function and processing effect,getting rid of stench taste,increasing fragrant purpose,so as to further consistent with the demands of clinical treatment.By now,this traditional and distinct processing technique is on the verge of extinct.In order to fully reflect the superiority of long-storaged medicine by therapy,it is imminent to conserve,investigate and develop such medicine and such method.We should reinforce to research the changing of its effective component and pharmacology effect before and after processing drugs,discuss the contribution of the long-storaging method on the quality of CMM,so as to make this method applying and developing further.

Objective To compare the clinical effect of two hemostatic methods for patients with pelvic fracture combined with hemorrhagic shock in the early stage.Methods A retrospective analysis was done on clinical data of 90 patients with unstable pelvic fracture combined with hemorrhagic shock managed by damage control resuscitation from January 2008 to December 2012.Unstable blood pressure and abdominal distension were noted postoperatively.Forty patients in Group A received laparotomy and bilateral internal iliac artery ligation from January 2008 to January 2010.Fifty patients in Group B underwent internal iliac artery embolization from February 2010 to December 2012.Comparative measurement was made on parameters of mortality,24-hour blood transfusion volume,24-hour lactic acid value,postoperative systolic blood pressure,postoperative body temperature,and postoperative prothrombin time (PT).Results Following parameters differed significantly between Groups A and B (P ＜ 0.05):mortality rate (53％ vs 12％),24-hour blood transfusion volume[(3 865.5 ±451.3)ml vs (2 108.8 ±336.4)ml],24-hour lactic acid value[(13.0 ± 2.0)mmol/L vs (5.4 ± 1.2)mmol/L],postoperative systolic blood pressure [(80.50 ± 22.73) mmHg vs (113.23 ± 20.89) mmHg],postoperative body temperature [(32.4 ± 0.2)℃ vs (36.1 ±0.3)℃],postoperative PT [(24.5 ±3.6)s vs (18.4±2.1)s].Conclusion For pelvic fracture combined with hemorrhagic shock,if the indications of abdominal viscera rupture are unclear,the interventional embolization can gain advantage over laparotomy in improving treatment success rate and reducing mortality and complications.
