1.Interventional therapy for Budd-Chiari syndrome: Analysis of 138 cases

Objective To investigate the selection and effectiveness of interventional treatment for different types of Budd-Chiari syndrome (BCS). Methods A total of 138 cases of BCS were divided into 4 types (type Ⅰ, membranous stenosis and obstruction of the inferior vena cava; type Ⅱ, segmental stenosis of the inferior vena cava; type Ⅲ, segmental obstruction of the inferior vena cava; type Ⅳ, patent inferior vena cava) and 9 subtypes according to results of angiography of the hepatic vein or the inferior vena cava. Therapeutic methods included percutaneous transluminal angioplasty (PTA) of the inferior vena cava and stent placement by way of the femoral vein, right cervical vein or subclavian vein, and PTA of the hepatic vein by way of the transhepatic approach, right cervical vein or inferior vena cava. Results The success rate was 100% (83/83) in type Ⅰ patients, 75.0% (9/12) in type Ⅱ patients, 81.6% (31/38) in type Ⅲ patients, and 60% (3/5) in type Ⅳ patients. One patient died (0.7%, 1/138). Complications occurred in 4 patients (2.9%, 4/138), including 2 cases of pericardial injury, 1 case of stent detachment, and 1 case of hepatic hemorrhage. Follow-up observations in 126 successful cases for 3~96 months (mean, 26 months) showed 8 recurrent cases, 2 of which expired, and 2 fatal cases because of hepatocarcinoma. Conclusions Interventional therapy, as a simple, safe and effective minimally invasive method, is the first choice for the treatment of BCS.
2.Hemocompatibility of polyoxymethylene used for bileaflet heart valve.

The possibility of polyoxymethylene (POM) as heart valve leaflet material was investigated by comparing the hemocompatibility with that of 316L stainless steel and low-temperature isotropic pyrolytic carbon (LTIC). Surface hydrophobicity was characterized by water contact angle measurement. Platelet adhesion, APTT/PT/TT and hemolysis rate tests were applied for evaluating hemocompatibility. The results showed that POM was hydrophobic and had a low hemolytic rate, adhesion amount and activation degree of platelets on POM surface were less than 316L stainless steel, and was similar to LTIC. This research pointed out potential application of POM as heart valve leaflets.

4.Effects of HO-1 modified rat bon e marrow mesenchymal stem cells on lymphocytes in vitro

Objective To investigate the immunomodulatory effects of heme oxygenase-1 ( HO-1) modified rat bone marrow mesenchymal stem cells ( BM-MSCs) on lymphocytes .Methods Rat BM-MSCs were cultured and identified in vitro.HO-1 gene was transduced into BM-MSCs via recombinant adenovirus (rAdV) to construct HO-1/BM MSCs, which were then co-cultured with rat spleen lymphocytes .The cellu-lar activity of lymphocytes was detected by using MTT method .The cell cycle distributions were analyzed by propidium iodide staining and flow cytometry .ELISA was used to detect the level of cytokines in the super-natant of cultured cells .The expression of lymphocyte activation markers , CD25 and CD71, were measured by using fluorescence labeled antibodies and flow cytometry .Results Compared with the control group , the lymphocytes co-cultured with HO-1/BM MSCs showed a suppressed proliferation and cell cycle progression . HO-1/BM MSCs inhibited the secretion of IL-2, TNF-αand IFN-γ, but enhanced the secretion of IL-10 in lymphocytes.Furthermore, the expression of CD25 and CD71 were also down-regulated by HO-1/BM MSCs. Conclusion Compared with BM MSCs , HO-1/BM MSCs show higher immunosuppressive effects on lym-phocytes .

6.Clinical Analysis of Knee Osteoarthritis Treated Mainly by Acupuncture

Purpose: To observe the curative effects of different treatments on genual osteoarthritis.Methods: The 148 cases were randomly divided into 4 groups: acupuncture group; Chinese herbs plus spectrograp irradiation group; comprehensive treatments group and western medicine group. Results:The total effective rate in these groups were 78.9%, 72.7%, 92.3% and 78.9% respectively, and there was not a significant difference among the groups (P>0.05). The marked effective rate in comprehensive treatments group was higher than that in other three groups (P<0.05). Conclusion: Acupuncture and Chinese herbs plus sepectrograp irradiation had certain effect on gonitis. Comprehensive therapies could coordinate the actions of acupuncture, and Chinese herbs plus spectrograp irradiation, and raise the curative effect.
7.Clinical Observation on Regulating Effect of Pravastatin on Blood Lipid and Blood Pressure in Patients with Coronary Heart Disease

8.The value of differential diagnosis of the configuration of QRS complex in lead aVR in patients with inferior wall myocardial infarction

Objective To investigate the value of differential diagnosis of the configuration of QRS complex in lead aVR in patients with inferior wall myocardial infarction. Methods The configuration of QRS in 52 patients with pathological Q-wave both in lead Ⅲ and aVF were analyzed and the result of selective coronary arteriography was compared. Results 13 patients with the configuration of QRS in lead aVR appeared rS ( s), while 10 patients appeared QS(qs) and 29 Q(q)r,correlated with 12,4 and 0 patients with coronary arteriography showed stenosis or occlusion lesion in fight coronary artery or left circumflex artery (χ2 = 35.56, P = 0.000). Conclusions The con-figuration of QRS in lead aVR is helpful to differential diagnosis of the patients with pathological Q-wave both in lead Ⅲ and aVF. Patients with the configuration of QRS in lead aVR appear rS(s) could be diagnosed as old myocardial infarction,but excluded from old myocardial infarction while appearing Q(q)r.
9.Preliminary study of semi-quantitative and quantitative dynamic contrast-enhanced MRI in evaluating the response to concurrent chemoradiotherapy in patients with non-small cell lung cancer.

OBJECTIVETo investigate the capability of semi-quantitative and quantitative parameters of dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI) to predict the response to concurrent chemoradiotherapy( CCRT) in patients with non-small cell lung cancer (NSCLC).
METHODSA total of 24 patients with stage III or IIIB NSCLC, who underwent 3.0T DCE-MRI before CCRT, were enrolled in this study. Semi-quantitative and quantitative parameters were calculated by Funtool and Omnikinetics software. The relationship between these obtained parameters and tumor response was evaluated by Spearmen' s correlation analysis. The patients were classified into two groups according to the tumor regression rate after treatment, as response group (group A) and non-response group ( group B). Mann-Whitney U test was used to compare the parameters of responders and non-responders. The value of the parameters on predicting response was calculated by receiver operating characteristic curve (ROC).
RESULTSThe tumor regression rate after treatment was negatively correlated with time to peak (TTP) and the extravascular-extracellular volume fraction (Ve), and was positively correlated with signal enhancement ratio (SERmax) and volume transfer constant (Ktrans) (P < 0.05 for all). Statistical significant differences were found between group A and group B both in semi-quantitative and quantitative parameters (P < 0.05). Group A had a lower TTP value [(34.66 ± 16.37) s vs. (44.09 ± 17.41) s] and Ve value [(0.19 ± 0.03) vs. (0.25 ± 0.05)] than group B, whereas group A had a higher SERmax [(166.50 ± 44.95)% vs. (113.57 ± 46.62)%] and Ktrans [(0.41 ± 0.17) min(-1) vs. (0.28 ± 0.12) min(-1)] than group B (P < 0.05 for all). The ROC analysis indicated that when setting the threshold of Ve on ≤ 0.21 for predicting response, the specificity, sensitivity and accuracy were 85.7%, 80.0% and 83.3%, respectively, with an area under curve of 0.875 (P < 0.001).
CONCLUSIONSBoth the semi-quantitative and quantitative DCE-MRI parameters are helpful for predicting the response after CCRT of NSCLC. Quantitative parameters seem to be more meaningful than semi-quantitative parameters.

10.Clinical effect of transurethral resection of bladder tumor with upside down loop and plsamakinetic in the prevention of obturator nerve reflex

Objective To observe the results and reality of transurethral resection of bladder tumor(TUR-Bt) with upside down loop and plsamakinetic in the prevention of obturator nerve reflex.Methods A total of 211 patients with lumbar anesthesia received TUR-Bt.Firstly,the tumor tower above bladder wall was cut with the traditional way.When intraoperative findings were obturator nerve reflex,or to the obturator nerve mapping area,operation should be changed to superimpulse plasma column electrode vaporization to cut off residual tumor tissue,then after flipping the loop,the residual tumour was resected with upside-down loop from the tumour side wall.Results In 211 cases,192 cases had a little obturator nerve reflex,but the movement of body was not obvious,which did not influence the operation.The serious complication,such as perforation of bladder and injury of nerve and vessel,did not occur.Conclusions The lateral resection of bladder side wall tumour with upside-down loop can effectively avoid strong obturator nerve reflex,which is safe,uncomplicated,and efficacious and it does not increase the additional outlay and hurt.
