1.Analysis on structured electronic medical record first page data in clinical application

Objective:Study on the clinical application and analysis of structured electronic medical records in our hospital in last.Methods: Discharged patients data selected from July 2012 to June 2014, using statistical methods for data processing, 8000 case files were divided into two groups: Group A and Group B.Results: After comparing the two groups of two groups of data, and ultimately disease coding accuracy rate 94.88%, fill in the correct rate increased 88.38%, missing rate decreased 4.63%, two groups had no significant difference data; The quality of writing the doctor increased gradually, the error rate tends to be stable, structured clinical application to control.Conclusion: Sequential specification writing medical doctor, scientific analysis of medical record information, the clinical application of structured electronic medical records the error rate is reduced in the operation, improve the accuracy, more standardized, accurate, standardized.
2.Effect of adenovirus-mediated bcl-2 transfer on regeneration and repair of peripheral nerves in rats

Objective To explore the effect of adenovirus-mediated bcl-2 transfer on the regeneration and repair of peripheral nerves in rats. Methods The sciatic nerve-transected model was selected and sutured end to end with epineurium. Then, adenovirus/sense-B-lymphoma/leukemia-2 (Ad/s-bcl-2), adenovirus/anti-sense-B-lymphoma/leukemia-2 (Ad/as-bcl-2), adenovirus/?-galactosidase (Ad/LacZ) and normal saline (NS) were injected into the sciatic nerve 0.5 cm away from the sutured point respectively. The rats were perfused with 4% paraformaldehyde at the 48th hour, 7th, 15th and 30th days respectively after operation. The spinal cords L 4-6 were harvested, frozen sectioned and given x-gal staining, bcl-2 in situ hybrid and immunohistochemical staining and GAP-43 immunohistochemical staining. The postoperative functional recovery of the rear limbs of rats was dynamically observed by using sciatic nerve functional index (SFI). Results The GAP-43 expression in the motor neurons of anterior horn of spinal cord in group Ad/s-bcl-2 was higher than that in other groups (P0.05). Conclusions Gene transfer of bcl-2 can promote regeneration and functional recovery of the peripheral nerves following sciatic nerve injury.
3. In vitro evaluation of biocompatibility and surface modification of 3 kinds of artificial graft material

Objective: To evaluate the biocompatibility of 3 kinds of artificial, biodegradable graft materials, including T20 [Poly (butylene terephthalate)-co-poly (cyclohexylene dimethanol terephthalate)-b-poly (ethylene glycol)], PHBV (Poly3-hydroxybutyrate-Poly3-hydroxyvalerate) and PEGT/PBT [Poly (butylene terephthalate)/poly (ethylene glycol) terephthalate], and to study their effects on canine smooth muscle cells(SMCs) growth after surface modification. Methods: The contact angles were measured to compare the hydrophilicity of T20, PHBV, and PEGT/PBT scaffolds. Then SMCs were cultivated on these biodegradable scaffolds; their proliferation and growth were assayed by MTT assay, FITC and electron microscopic observation for evaluation of their in vitro biocompatibility. The growth of SMCs was also assayed on the 3 scaffolds after they were pretreated with gelatin or Poly-1-lysine. Results: The contact angles of 3 scaffolds were all less than 90°(PEGT/PBT>T20>PHBV). MTT assay showed that SMCs adhered to and grew well on T20, PHBV, and PEGT/PBT scaffolds. The cell viability began to increase 24 h after cultivation (PEGT/PBT>T20>PHBV, P<0.01), followed by a decrease, but increased again 72 h after cultivation (T20>PEGT/PBT>PHBV, P<0.01). FITC observation showed that SMCs grew well and electron microscopic observation showed a confluent growth of SMCs with abundant extracellular matrix. The viability of SMCs was significantly higher on scaffolds pretreated with gelatin or Poly-1-lysine than on those without pretreatment(P<0.01). Conclusion: T20, PHBV, PEGT/PBT have good in vitro biocompatibility; the scaffolds made of these materials can obviously improve cell attachment after surface modification.
4.Effect of Auricular Acupuncture Combined with Physical Therapy on Motor Function of Stroke Patients

ObjectiveTo observe the effect of auricular acupuncture combined with physical therapy on motor function of stroke patients.Methods40 stroke patients were divided randomly into trial group and control group with 20 cases in each group. Patients of trail group were treated with auricular acupuncture combined with physical therapy; cases of control group were treated only with physical therapy. Motor function of all patients were assessed with Fugl-Meyer Assessment (FMA) before and after treatment.ResultsFMA scores of patients of trial group increased significantly compared with that of control group (P<0.01).ConclusionEffect of auricular acupuncture combined with physical therapy is better than that of single physical therapy on motor function of the stroke patients.
5.The post -operation rehabilitation nursing of re-establishing anterior cruciate ligament of the knee by using allogeneic tendon

Objective To approach rehabilitation nursing measures in using the allogeneic ten-don to re-establish the anterior cruciate ligament of knee through the tibia twin tunnel under arthroscopy.Methods Mental nursing and direct functional exercises was given to 30 patients who received this kind of operation from November,2006 to July,2007,and functional exercises of flection and extention of knee joint by steps,functional exercises of muscle force of quadficeps muscle and back muscle group of legs and the correct using of artificial brace of knee joint.Results Range of joint motion could reach 95°after 2 weeks and 125°after 4 weeks postoperation.Score of joint function(Lysholm)could reach(74.8±5.7)after 6 weeks,(80.0±2.3)after 9 weeks,(91.8±3.4)after 12 weeks, and(94.5±2.2)after 1 year.Conclusion Selective perioperative rehabilitation nursing instruction is the important guarantee of function recovery.
6.Effects of Levocarnitine on Peripheral Blood T Cell Subsets and Inflammatory Factors in Diabetic Patients Underwent Peritoneal Dialysis

OBJECTIVE:To investigate the effects of levocarnitine on peripheral blood T cell subsets and inflammatory factors in diabetic patients underwent peritoneal dialysis. METHODS:A total of 100 diabetic patients underwent peritoneal dialysis in se-lected as research objects were divided into conventional therapy group and levocarnitine group according to random number table, with 50 cases in each group. Conventional therapy group received peritoneal dialysis and conventional therapy. Levocarnitine group was additionally given levocarnitine 1.0 g added into 0.9% Sodium chloride injection 20 mL intravenously,3 times a week,on the basis of conventional therapy group for 12 weeks. CD3+,CD4+,the proportion of Th1 and Th2,the contents of TNF-α,INF-γ, IL-10 and IL-4 in supernatant were all detected before and after treatment. The expression of T-bet and GATA-3 were compared be-tween 2 groups. RESULTS:After treatment,CD3+,CD4+,Th2 cells,GATA-3 expression and IL-10,IL-4 levels were significantly increased,the expression of Th1 cells,T-bet content and TNF-αand INF-γwere significantly reduced,and levcarnitine group was sig-nificantly better than conrentional therapy group,with statistically significant (P<0.05). CONCLUSIONS:Levocarnitine can im-prove inflammatory reaction and the expression of related transcription factors by promoting T cell subsets and Th1/Th2 cell subsets.
7.Association of HSP70 gene polymorphisms with type 2 diabetes mellitus in Hui and Han people in Ningxia

Results There was no significant difference in the distribution of genotypes and allele frequencies of HSP 70-1 and HSP70-hom between case group and control group (P>0.05), but there was significant difference in the distribution of genotypes and allele frequencies of HSP70-2 between the two groups(P<0.05).There was no significant difference in the distribution of genotypes and allele frequencies of HSP70-1,HSP70-2 and HSP70-hom between Hui and Han people (P>0.05)or in the distribution of genotypes of HSP 70-2 between males and females in case group, but there was significant difference in the distribution of allele frequencies of HSP 70-2 in case group .There was no significant difference in the distribution of genotypes and allele frequencies of HSP 70-1 and HSP70-hom between males and females in case or control group .Logistic analysis confirmed that the WC, TG, TC, LDL-C, SBP, family history of diabetes and G allele were risk factors of T 2DM. Conclusion GG genotype and G allele of HSP 70-2 (+1267 ) SNP may be genetic markers for the susceptibility of type 2 diabetes.There is no significant difference in the distribution of genotypes and allele frequencies of HSP 70-1,HSP70-2 and HSP70-hom between Hui and Han nationalities .Family history of diabetes, LDL-C, TC, and G allele of HSP70-2 (+1267) SNP are the main risk factors of T2DM while HDL-C is a protective factor.
8.Long-term effective rate,retention rate and tolerability of the ketogenic diet in paediatrics drug-resistant epilepsies

Objective To evaluate the long-term effective rate,retention rate and tolerability of the ketogenic diet (KD) in pediatric drug-resistant epilepsies.Methods Data of 36 children who were treated in Department of Pediatrics,the Second Affiliated Hospital of Xi'an Jiaotong University from Nov.2011 to Dec.2013 and had continuous follow-up of at least 12 months after initiation of the KD were analyzed prospectively.Response was defined as 50% seizure reduction.The effective rate,retention rate, outcome-predictive value of various clinical factors were also assessed.The causes of the patients withdrew from KD and side effects were recorded and analyzed.Results Thirty-six children(29 boys,7 girls; mean age of 2.84 years)were included.The effective rate was 50.0%,52.8%,47.2% and 41.7% at the 1,3,6,12 months;the retention rate respectively was 94.4%,91.2%,69.4% and 52.8%.Seventeen cases withdrew from KD.Seven cases (41.2%)ceased KD becau~ of a lack of compliance,5 patients because of poor outcome,and 2 cases because of repeated infection.None of the age,disease duration,etiology and seizure type could be used as the predictor for the favourable treatment for outcome.The antiepileptic drugs before KD might be predicted the efficacy of the KD.The less amount of the antiepileptic drugs,the more opportunity of the KD might have to succeed.The side effects on the start-up period were drowsiness,week and digestive discomfort,hyperlipoidemia,hypoglycemia and hepatic dysfunction.The side effects on the maintenance period were digestive discomfort,susceptibility to infection,hyperlipoidemia,the deficiency of trace elements.Conclusions The KD is a safe and potentially effective method in treatment of refractory epilepsy patients who do not respond to customary medication therapies.
9.Effects of cyclooxygenase-2 selective inhibitor Celecoxib on the expression of P-glycoprotein in rat with status epilepticus

Objective To study the effect of cyclooxygenase-2 (COX-2)selective inhibitor Celecoxib on the expression of P-glycoprotein(P-gp)in the brain of rats with status epilepficus,in order to assess the therapeutic value of intractable epilepsy.Methods Sixty adult male SD rats were randomly divided into blank control group,the epilepsy model group and Celecoxib intervention group.The status epilepticus was induced in rats by injecting Lithium pilocarpine.Forty-eight rats were included in the experiment.There were 16 rats in each of the blank control group,epilepsy model group and Celecoxib intervention group,respectively.Immunohistochemical method and Western blot method were used to detect the expression of P-gp in experimental group in the frontal cortex and hippocampus.Results Immunohistochemistry result showed that the expression of P-gp was significantly higher in epilepsy model group than the blank control group,and the difference was statistically significant (P < 0.01);The P-gp expression in the Celecoxib intervention group was lower than that in the epilepsy model group,and the difference was statistically significant (P <0.01).Western blot results suggested that the expression of P-gp could be found both in the frontal cortex and hippocampus in each group.Compared with the blank control group,the P-gp expression was significantly higher than that in the epilepsy model group,and the expression of the P-gp was lower after the Celecoxib intervention than that in the epilepsy model group,and the difference was statistically significant (P < 0.05).Conclusions COX-2 inhibitor Celecoxib could decrease the expression of P-gp in brain tissue with status epilepticus,which may provide a new method for the treatment of intractable epilepsy.
10.Precolumn derivatization LC-MS/MS method for the determination and pharmacokinetic study of glucosamine in human plasma and urine

A selective precolumn derivatization liquid chromatography-tandem mass spectrometric (LC-MS/MS) method for the determination of glucosamine in human plasma and urine has been developed and validated.Glucosamine was derivatized by o-phthalaldehyde/3-mercaptopropionic acid.Chromatographic separation was performed on a Phenomenex ODS column (150 mm × 4.6mm,5μm) using linear gradient elution by a mobile phase consisting of methanol (A),and an aqueous solution containing 0.2% ammonium acetate and 0.1% formic acid (B) at a flow rate of 1 mL/min.Tolterodine tartrate was used as the internal standard (IS).With protein precipitation by acetonitrile and then the simple one-step derivatization,a sensitive bio-assay was achieved with the lower limit of quantitation (LLOQ) as low as 12 ng/mL for plasma.The standard addition calibration curves suitable for clinical sample analysis showed good linearity over the range of 0.012-8.27 μg/mL in plasma and 1.80-84.1 μg/mL in urine.The fully validated method has been successfully applied to a pharmacokinetic study of compound glucosamine sulfate dispersible tablets in health Chinese volunteers receiving single oral doses at 500,1000 and 1500 mg of glucosamine sulfate,as well as multiple oral doses of 500 mg t.i.d.for 7 consecutive days.
