1.Monocyte chemotactic protein-1 and ischemic stroke

Inflammatory reaction plays an important role in the occurrence and development of ischemic stroke. Monocyte chemotactic protein-1 (MCP-1) is one of the subfamily members of the CC chemokine class. It can attract and activate a variety of cells, such as monocytes and T lymphocytes, promote the expression of cytokines, and participate in the occurrence of ischemic brain injury. This article reviews the progresses on the MCP-1 and ischemic stroke-related research.
2.Evaluation and cell warning information for clinical application of five-part differential hematology analyzer

Objective To investigate the technical charactenstics ot SYSMEX XS1000i 5-part differential automated hematological analyzer and its clinical applications.Methods 209 samples were analyzed with the analyzer of XS1000i and compared to the results from Beckman-Coulter LH750 analyzer.The main parameters from XS1000i,such as precision within-run,day to day precision and carryover contamination rates etc,were recorded respectively in 509 samples to compare the difference between the instrumental and optical examination.Results All variation coefficients of precision from XSI000i were within the manufacturer.The carryover contamination rates of WBC,RBC,Hb,HCT and PLT were 0.19%,0.93%,0,-0.88%,-0.76%,respectively.The parameters of XS1000i were correlated with the results of LH750 except basophil granulocyte.The correlation coefficients of WBC,RBC, HGB and PLT were 0.994 5,0.996 8,0.997 0 and 0.974 6 ,respectively.The sensitivity of warning flags in immature leukocytes was 100% and the specificity was 69.7%,the sensitivity of warning flags in atypical lymphocyte was 100% and the specificity was 66.7%.Especially in 3 leukocytopenia that was induced by chemotherapy in patients with leukemia who had only few immature cells existed in the peripheral blood,the parameters of XS1000i were positive,and correlated with the results of detection of minimal residual disease with flow cytometry.Conclusions The warning system of XSIO00i provides more valuable information for manual microscopic examination.If it is combined with flow cytometry,the advantages in detection of residual leukemic cell will be fully displayed.
3.The post -operation rehabilitation nursing of re-establishing anterior cruciate ligament of the knee by using allogeneic tendon

Objective To approach rehabilitation nursing measures in using the allogeneic ten-don to re-establish the anterior cruciate ligament of knee through the tibia twin tunnel under arthroscopy.Methods Mental nursing and direct functional exercises was given to 30 patients who received this kind of operation from November,2006 to July,2007,and functional exercises of flection and extention of knee joint by steps,functional exercises of muscle force of quadficeps muscle and back muscle group of legs and the correct using of artificial brace of knee joint.Results Range of joint motion could reach 95°after 2 weeks and 125°after 4 weeks postoperation.Score of joint function(Lysholm)could reach(74.8±5.7)after 6 weeks,(80.0±2.3)after 9 weeks,(91.8±3.4)after 12 weeks, and(94.5±2.2)after 1 year.Conclusion Selective perioperative rehabilitation nursing instruction is the important guarantee of function recovery.
4.Clinical analysis of 45 cases of drug-induced hypersensitivity syndrome

Objective:To analyze clinical characteristics of drug-induced hypersensitivity syndrome (DIHS) , and to compare the European, Japanese and Chinese diagnostic criteria.Methods:A total of 45 patients confirmedly diagnosed with DIHS according to the DIHS criteria originally proposed by Bocquet, were collected from the First Affiliated Hospital (Southwest Hospital) of Army Medical University between January 2009 and January 2019. Clinical data on the 45 patients were retrospectively analyzed, clinical characteristics were summarized and re-evaluated according to the European, Japanese and Chinese diagnostic criteria separately, and differences were analyzed in terms of the latency period, time to rash regression, eosinophil count, liver function indices, etc. One-way analysis of variance was used to compare means among multiple groups, and t test to compare means between two groups. Results:Of the 45 patients, 38 presented with eruptive drug eruptions, and 44 were accompanied by liver damage, 40 by elevated counts of peripheral white blood cells, 38 by eosinophilia, 21 by lymphadenectasis, and 4 by mucosal damage. Common culprit drugs included allopurinol (10 cases) , anti-tuberculosis drugs (7 cases) , cephalosporins (7 cases) , and Chinese medicine (4 cases) . Forty patients were treated with glucocorticoids, and 17 with glucocorticoids and intravenous gamma globulin. After treatment, 44 patients received improvement and 1 died. According to the European diagnostic criteria, there were 29 patients with suspected DIHS and 16 with confirmed DIHS; according to the Japanese diagnostic criteria, 37 patients could be confirmedly diagnosed with DIHS, but 8 could not be confirmedly diagnosed; according to the Chinese diagnostic criteria, 17 patients could be confirmedly diagnosed, but 28 could not be confirmedly diagnosed. According to the Japanese diagnostic criteria, the latency period was significantly longer in the patients with a confirmed diagnosis (36.91 ± 21.73 d) than in those without (20.00 ± 20.82 d, P = 0.04) . Conclusions:Common culprit drugs for DIHS include allopurinol, anti-tuberculosis drugs and cephalosporins. Most patients with DIHS are accompanied by liver damage, and the European diagnostic criteria are preferentially recommended for DIHS.
5.Characteristics of erythrocytes washing with the autologous transfusion system

BACKGROUND:Autologous shed blood washing with the autologous transfusion system involving recovery, anticoagulation, centrifugation, concentrating and washing has been widely used in clinical practice.
OBJECTIVE:To clarify the characteristics of erythrocytes washing with autologous transfusion system, including recovery rate and hematocrit, the changes of shape, deformability, flow properties and in vivo half-life, oxygen carrying and delivering capacity and erythrocyte immunity and immunereceptor expression.
METHODS:The literatures published from January 1987 to January 2013 were retrieved by the first author in Wanfang and PubMed databases. Key words were“blood transfusion, autologus, blood preservation, erythrocytes”in English and Chinese. A total of 200 literatures relating to the erythrocyte characteristics in autologous blood transfusion were found by the computer, 60 of which were retained for further analysis after eliminating repetitive researches.
RESULTS AND CONCLUSION:Because of the mechanical force, such as negative pressure suction, centrifugal separation, and inflammatory mediators, enzymes, activated complements released by various damaged tissues and cells, the col ected erythrocytes were damaged to some extent. As a result, the total recovery rate of erythrocytes depended on the recovery rate, storage breakage rate and cleaning loss rate. The oxygen carrying capacity of erythrocytes was not influenced significantly by this procedure, so the recycled erythrocytes had the same oxygen carrying capacity with normal erythrocytes. To some extent, the number of surface receptors and immune function of recycled erythrocytes descended, but they were better than the erythrocytes preserved for 2 weeks. Studies suggested that blood recovery technology should be improved to reduce the functional decline in immune adherence of the recycled erythrocytes.
6.Influence of different extracellular DNA levels in Staphylococccus epidermidis isolates on the ability of biofilm formation

ObjectiveTo determine the ability of biofilm formation of Staphylococcus epidermidis isolates and study the influence of different extracellular DNA(eDNA) levels in S.epidermidis isolates on the ability of biofilm formation.MethodsDetect the biofilm-formation ability of 227 S.epidermidis isolates with adhesion assays,amplify the icaA gene fragment with PCR.The S.epidermidis isolates were divided into biofilm formation(BF) group and non-biofilm formation (NBF) group according to adhesion assays and icaA amplification.Detect eDNA levels of S.epidermidis in planktonic culture and microtitre plate static culture.The eDNA in S.epidermidis biofilms stained by AO-PI was observed by CLSM.Results26 isolates were positive in adhesion assays and 32 isolates existed icaA gene among 227 S.epidermidis isolates.Select 20 isolates with positive adhesion assays and positive icaA amplification for BF group.Select 19 isolates with negative adhesion assays and negative icaA amplification for NBF group.The eDNA levels were (32.2±10.1)μg/ml,(33.6±11.9) μg/ml,(34.3±10.0) μg/ml in BF group when cultured in planktonic condition for 2,4,6 h,while the eDNA levels in NBF group were (28.7±8.9) μg/ml,(31.5±11.7) μg/ml,(31.8±12.7) μg/ml respectively.There were no significant differences between the two groups for these three phases(P>0.05),though the eDNA levels of BF group were higher than that of NBF group.The eDNA levels were (740.0±264.4) ng/A600 in BF group when cultured in static microtitre plate,higher than that of NBF group,(80.1 ±31.1) ng/A600,and the difference between these two groups was significant.The eDNA in BF isolate Y36 biofilms could be visualized by staining with AO and PI when observed by CLSM,while neither biofilm structure nor eDNA appeard when NBF isolate Y26 was cultured for 24 h.ConclusionS.epidermidis isolates have the ability of biofilm formation.eDNA is one of the important matrix components in the S.epidermidis biofilm-forming process.The eDNA of static culture in microtitre plate was more efficient than planktonic culture in the case of estimating the ability of biofilm formation of S.epidermidis.
7.Influence of intensive nursing on working memory dysfunction in patients with early Parkinson's disease

Objective To examine the influence of 3R intensive nursing on working memory dysfunction of patients with early Parkinson's disease. Methods 30 patients with early Parkinson's disease were assessed by Working Memory Test Battery,and acceped 3R- nursing. The condition of working memory dysfunction was compared before and after nursing intervention. Results The patients with early Parkinson's disease had evident difference than the normal controls, especially the visualspatial working memory. After 3R- nursing, the subtest scores and dimensions were improved apparently. Conclusions The clinical effect of 3R- nursing method in patients with early Parkinson's disease has significant predominance ,so it is worth popularizing.
8.The relationship between serum CC10 protein and ventilator-associated lung injury in rats

Objective To investigate the relationship between serum CC10 protein and lung injury induced by mechanical ventilation.Methods Forty healthy Wistar rats of both sexes weighing 200-250 g were randomly divided into 5 groups(n=8 each):group Ⅰ control;group Ⅱ mild lung injury[VT=7 ml/kg,duration of mechanical ventilation(t):2 h];group Ⅲ moderate lung injury(VT=7 ml/kg,t=4 h);group Ⅳ severe lung injury(VT=40 ml/kg,t=2 h);group Ⅴ extremely severe lung injury(VT=40 ml/kg,t=4 h).The animals were anesthetized with intraperitoneal 10% chloral hyrdrate 3.5 ml/kg and tracheostomized.Group Ⅰ received no mechanical ventilation.The animals in group Ⅱ-Ⅴ were mechanically ventilated with air(FiO2=21%,RR=40 bpm,I:E=1:2).The animals were sacrificed at the end of mechanical ventilation.The lungs were immediately removed for microscopic examination and determination of W/D lung weight ratio.The left lung was lavaged.The CC10 protein level in broncho-alveolar lavage fluid(BALF)and serum were determined by Western blotting.The Clara cells in the bronchiolar epithelium were examined by immno-histochemistry.Results The level of CC10 protein in BALF was significantly lower in group Ⅳ and Ⅴ while the serum CC10 protein level was significantly higher in group Ⅱ-Ⅴ than in group Ⅰ.The serum CC10 protein level was positively correlated while the CC10 protein level in BALF was negatively correlated with the severity of lung injury and W/D ratio.Conclusion The serum CC10 protein is closely related to the severity of lung injury induced by mechanical ventilation.
9.Study on relationships between the expressions of cidA and lrgA and biofilm formation of Staphylococcus epidermidis clinical isolates

Objective To investigate the relationships between the expressions of cidA and lrgA and the biofilm formation of Staphylococcus epidermidis clinical isolates. Methods Thirty-nine S. epidermidis clinical isolates were divided into biofilm formation group and non-biofilm formation group according to adhesion assays and icaA amplification, 20 strains for biofilm formation group and 19 strains for another group respectively. Amplify cidA and lrgA fragment with PCR, detect mRNA levels of cidA and lrgA with RT-PCR and then calculate the ratios of cid/lrg mRNA. Results All of the 39 isolates of S. epidermidis existed cidA and lrgA gene.Select 2 strains of S. epidermidis, Y36 and Y26, at random for detecting the cidA and lrgA mRNA levels of different phases, the highest level of cidA mRNA shows on 4 h's culture while lrgA shows on6 h. The relative expression levels of cidA of 39 isolates of S. epidermidis were 0.340 + 0.250 in biofilm formation group and 0. 406 +0. 408( P =0. 541 ) in non-biofilm formation group for 4 h culture, while the relative expression levels of lrgA were 0.325 ± 0.218 and 0.253 ± 0.211 respectively ( P = 0. 299). There were no significant differences between these two groups. The ratios of cid/lrg mRNA were 1.067 ±0.529 and 1.958 ±1.877 respectively, the difference of population distribution was significant( P = 0.001 ). Conclusion The expressions of cidA and lrgA of S. epidermidis clinical isolates may be time-limited. There were no significant differeces of expressions of cidA and lrgA between biofilm formation group and non-biofilm formation group. The ratios of cid/lrg mRNA maybe have some biological significance.
10.Vancomycin flush solution for prevention of catheter-related infections in oncology patients with PICC: a systematic review of the effectiveness

Objective To assess the efficacy of vancomycin flush solution for prevention of catheter-related infections in oncology patients. Methods Randomized controlled trials(RCTs) were searched and identified from Cochrane library, PubMed, EMBASE, CBM, CNKI and related journals were also scanned. In accordance with inclusion and exclusion criteria, we evaluated the quality of the included studies by Jadad scale and analyzed the data of catheter- related sepsis, catheter colonization, exit- site infection by RevMan 5.0. Results We included 5 randomized controlled trials (400 patients). Meta analysis showed that there were statistical differences between vancomycin- heparin mixture and heparin solution on the incidence of catheter-related sepsis, catheter colonization, but no difference was found on the incidence of exit-site infection. Conclusions The current evidences show that contrast with heparin flush solution, van comycin - heparin flush solution can significantly reduce the incidence, of catheter - related sepsis and catheter colonization. But vancomycin-heparin flush solution was more effective, and the results should be further confirmed in prevention of Gram-positive bacterial and fungal catheter-related infections or in non- neutropenic patients. Before draw the conclusions, further high-quality, large scale, double-blind randomized controlled trials are still needed.
