1.Preparation and Quality Control of Fumeisong Cream

OBJECTIVE: To prepare fumeisong cream and establish its quality control method. METHODS: Ingredients including carbamide, triamcinolone acetonide acetate, camphor, and laurocapram were mixed in an oil-in-water matrix for preparation of fumeisong cream. HPLC was adopted to determine the content of triamcinolone acetonide in the cream. RESULTS: The prepared cream was white in color, with test items all in line with specifications stipulated in 2005 version of China Pharmacopoeia. The linearity concentration range of triamcinolone acetonide was 2.06~20.6 ?g?mL-1(r=0.999 6) and the average recovery rate was 99.6% (RSD=1.0%,n=9). CONCLUSION: The cream is simple and feasible in preparation techniques, and its quality is stable and controllable.
2.Characteristics of attempted suicide individuals from an emergency room in a seven-year period

Objective To study the demographics and background of attempted suicide individuals,such as age,gender,mode of suicide,seasonality,etc.so as to improve prevention strategy.Methods By sequential sampling analysis,there were data of 895 survivors from committed suicide taken from 907 attempted suicide individuals recorded in an emergency room of a local hospital between 2002 and 2008 for retrospectively study.Results The sample was composed of 191 males and 704 females with a male to female ratio of 1∶3.69.The adolescents and young adults aged 15-34 years accounted for the vast majority of attempted suicide individuals with the peak at the age of 24 years.The number of committed suicide remarkably reduced after the age of 47 years.The major mode of committed suicide was ingestion of poisons.The incidence of committed suicide declined significantly from 2002 to 2005,and then increased between 2005 and 2008.It was more often occurred in the seasons of Spring and Summer. Conclusions Age,gender,and seasonality are associated with attempted suicide in china,and these risk faetors should be the focuses of suicide prevention in China.Additionally,control of pesticides and psychological intervention should be the parts of the work to prevent suicide in the this cohort of special individuals with propensity of committed suicide.
3.The Early Diagnostic Value of Thyroid Hormone Level in Patients With Left Ventricular Diastolic Dysfunction

Objective: To explore the early diaghostic value of thyroid hormone level in patients with left ventricular diastolic dysfunction. Methods: Our work included 4 groups:①Normal control group, n=40,②Impaired left ventricular diastolic function
group, n=40,③Failure of left ventricular diastolic function group, n=41 and④Impaired left ventricular systolic function group, n=40. The cardiac function was assessed by color Doppler lfow imaging and tissue Doppler imaging for the early diastolic mitral lfow velocity (E), the peak early diastolic mitral annular velocity (Em) and the ratio of E/Em. The blood levels of NT-proBNP and FT3, FT4, TSH, rT3 were examined in clinical laboratory.
Results: Blood levels of FT3 and rT3 were different in pair wise comparison among 4 groups, P<0.05, and FT4 and NT-proBNP were different in pair wise comparison among 4 groups, except for Normal control group and Impaired left ventricular diastolic function group, P<0.05. The TSH level was similar among 4 groups, P>0.05. The ratio of E/Em was negatively related to FT3 (r=-0.724, P<0.05) and FT4 (r=-0.49, P<0.05), while positively related to rT3 (r=0.87, P<0.05) and NT-proBNP (r=0.83, P<0.05).
Conclusion: The changes of thyroid hormone level may exist in patients with impaired diastolic function and it may vary with the severity of the cardiac dysfunction. Blood levels of NT-proBNP with thyroid hormone might be used as a reference index for early diagnosis of diastolic dysfunction.
4.Effects of Omeprazole Combined with Somatostatin on Intra-Abdominal Pressure and Intestinal Mucosal Barrier Function of Elderly Patients with Severe Acute Pancreatitis

Objective:To explore the effects of omeprazole combined with somatostatin on intra-abdominal pressure and intestinal mucosal barrier function of elderly patients with severe acute pancreatitis.Methods:Selected 82 cases of elderly patients with severe acute pancreatitis from our hospital.They were divided into research group (48 cases) and control group (34 cases).The control group was treated with basic therapy,and the research group was treated with omeprazole combined with somatostatin on the basis of the control group.Analyzed and compared the clinical effects,intestinal mucosal barrier function and symptom indexes of patients in the two groups.Results:①The total effective rate of the research group was significantly better than that of the control group,and the difference was statistically significant (P=0.005).②After treatment,the D-lactose and diamine oxidase (DAO) levels of both groups were significantly lower than before treatment,and the difference was statistically significant (both P<0.001).In addition,after treatment,the D-lactose and DAO levels were significantly lower in the research group than in the control group,and the differences were statistically significant (mean P < 0.001).③After treatment,patients of the research group showed better results in abdominal pain and abdominal distension relief time,bowel recovery time,hospital stay and blood amylase index than those in the control group,and the difference was statistically significant (mean P < 0.001).④After treatment,the intra-abdominal hypertension of two groups was significantly lower than before treatment,with statistical difference (P < 0.001).Yet it was significantly lower in the research group than in the control group,with statistical difference (P < 0.001).Conclusion:The combined treatment of omeprazole and somatostatin can effectively improve the clinical efficacy of severe acute pancreatitis in elderly patients,correct intra-abdominal hypertension and improve the intestinal mucosal barrier function of patients,so it is worthy of reference and promotion.
5.Effects of continuous renal replacement therapy on refractory hypertension in maintenance hemodialysis patients

BACKGROUND:About 10%-49%of maintenance hemodialysis patients are accompanied by refractory hypertension and common drug therapy is ineffective. Continuous renal replacement therapy could clear the middle and giant molecule toxins in the blood plasma of patients, it is theoretical y treat refractory hypertension through reducing the toxin levels.
OBJECTIVE:To observe the effect of continuous renal replacement therapy on maintenance hemodialysis patients with refractory hypertension.
METHODS:A total of 45 maintenance hemodialysis patients with refractory hypertension were randomly divided into two groups, hemodialysis group (n=22) and continuous blood purification group (n=23). Hemodialysis group underwent conventional hemodialysis treatment. Continuous blood purification group underwent conventional hemodialysis therapy, and treatment of continuous blood purification weekly.
RESULTS AND CONCLUSION:After 3 months of treatment, the renin activity, endothelin, angiotensin II, C-reactive protein, interleukin-6, and tumor necrosis factor-αlevels in continuous blood purification group were significantly lower than the levels before the experiment, while 24-hour mean blood pressure levels were significantly decreased (P<0.05). In hemodialysis group, renin activity, endothelin, angiotensin II, C-reactive
protein, interleukin-6, and tumor necrosis factor-αlevels and 24-hour mean blood pressure did not change (P>0.05). After the test, continuous blood purification group showed significantly lower levels of the above index than the hemodialysis group (P<0.05). Continuous renal replacement therapy plus hemodialysis significantly reduce the blood pressure in maintenance hemodialysis patients with refractory hypertension, the potential mechanisms are mediated by lowering the middle and giant molecule toxin as wel as inflammatory factor in plasma.
6.Autogenous fibular transplantation and reconstruction of radiocarpal joint after en bloc excision of giant cell tumor of distal radius

Objective To evaluate the method and curative effect of reconstruction with vascularized or nonvascularized autogenous fibular transplantation of radiocarpal joint after en bloc excision of giant cell tumor of distal radius.Methods Seventeen cases with giant cell tumor of distal radius bone were treated by en bloc resection and reconstruction of wrist joint with vascularized or non-vascularized autogenous fibular transplantation.Postoperative wrist joint function and radiographic outcomes were evaluated.Results Seventeen cases were followed up from 2.0 to 5.5 years (average 3.6 years).Three cases were treated by peroneal artery anastomosis and other 3 cases by inferior lateral genicular artery anastomosis.All cases achieved primary healing of incision and the radial average length that was en bloc excised was 7.6 cm.All of the transplanted fibulas were healed well.The bone healing average time of the vascularized fibular graft was 3.7 months,while that of the non-vascularized fibular graft was 7.9 months.No tumor recurrence or lung metastases was observed during the follow-up,but 1 wrist joint was narrowed and another had degeneration of the wrist.After 2 years of surgery,the average range of motion of the wrist joint was as follows:dorsiflexion 47°,flexion 30°,ulnar deviation 23°,radial deviation 14°,pronation 55°,supination 62°.The grip force was from 40% to 80% of the contralateral upper limb.The MSTS score averaged 87.6 % with 6 excellent,eight good and 3 satisfactory results.Disabilities of the Arm,Shoulder,and Hand (DASH) questionnaire score averaged 3.48.Conclusion The method of autogenous fibular transplantation and reconstruction of radiocarpal joint after en bloc excision of giant cell tumor of distal radius can save the wrist joint function in a certain extent.It is an effective method of the wrist joint function reconstruction and beneficial to bone healing with vascularized transplantation which has less complications.
7.The radiosensitizing effects of cryptotanshinone on H22 hepatoma-bearing mice

Background and purpose:Quinonoids can change the cell cycle distribution of tumor cells, and effect the radiosensitizing. This study aimed to investigate the radiosensitization effects, cell cycle and apoptosis of cryptotanshinone on H22 hepatoma-bearing mice. Methods:The mouse hepatoma H22 model was established, then divided into blank control group, irradiation alone group, high dose of cryptotanshinone group, cryptotanshinone (low, medium and high)+IR groups. After irradiated, observed the growth of tumor’s conditions, record epigenetic tumor irradiation time, calculated the delay time of tumor growth and enhancement factor (EF). After 22 days, mice were killed, stripped tumor, and calculated the inhibition rate. The cell cycle distribution and apoptosis were measured by lfow cytometry. Results:Cryptotanshinone (low, medium and high) groups inhibited the tumor growth better than the blank group, and had the signiifcant radiosensitizing effect. The enhancement factor was 1.22, 1.43,2.19, respectively. Cells were treated with cryptotanshinone which had significant effects on cell cycle, and induced apoptosis, which indicated signiifcant G2/M phase arrest and a decrease in S phase. Conclusion:Cryptotanshinone inhibited the tumor growth and had the radiosensitizing effects on H22 hepatoma-bearing mice. One of the mechanism may be that it might make significant G2/M phase arrest and S phase decreased, and induced apoptosis. So cells were more sensitive to radiation.
8.Observation of oral Streptococcus oligofermentans colonization in rats

Objective:To study the colonization ability of Streptococcus oligofermentans (S.oligofer-mentan)in the condition of high sucrose in oral cavity of rats.Methods:In this study,48 SPF-SD rats aged 21 days were selected.From 24th to 27th days,the rats were fed with water of antibiotic and fed with high glucose diet continuously.On the 28th day,the rats were divided into four groups randomly,12 rats per group.From the 28th day to 30th day,the first group (SMgroup)was inoculated with S.mutans,the second group (SO group)with S.oligofermentan,the third group (SO+SM group)with mixture of S. mutans and S.oligofermentan,the control group not with any bacteria.On the next day and the 10th day after inoculation of bacteria,the samples of dental plaque of the rats were acquired by scrubbing occlu-sal,buccal and lingual surfaces of bilateral mandibular molars with sterile swabs.The samples of SM group were inoculated on MSB and BHIS,of SO group on MSAE,of SO+SMgroup on MSB,MSAE and BHIS,of the control group on MSB and MSAE.S.mutans were screened and calculated on MSB,the suspected colonies of S.oligofermentan were screened and identified by the analysis of 16S rDNA.Re-sults:On the next day,the detection rate of S.oligofermentan was 33.3% (4/12)in the group of SO;in the group of SO+SM,the detection rate of S.oligofermentan was 0,the detection rate of S.mutans 100.00%,and the proportion of S.mutans 14.70%±4.53%;in the group of SM,the detection rate of S.mutans was 100.00%,the proportion of S.mutans 12.42%±4.27%.On the 10th day,in the group of SO,the detection rate of S.oligofermentan was 0;in the group of SO+SM,the detection rate of S.oligofermentan was 0,the detection rate of S.mutans 100.00%,and the proportion of S.mutans 15.78%±5.10%;in the group of SM,the detection rate of S.mutans was 100.00%,and the propor-tion of S.mutans 17.08%±5.75%.Conclusion:In the condition of the experiment where high glucose was maintained in the oral cavity in rats,S.oligofermentan appeared transiently and couldn’t colonize in the rats.
9.Analysis on drugs application in elderly patients with renal insufficiency

Objective To summarize the clinical medicine treatment in elderly patients with renal dysfunction,in order to discover the existing problems and analyze the reasons.Methods One hundred and eighty cases of senile patients with renal insufficiency were randomly selected in the second half of 2012 to analyze medication states based on the medication standard.Results Medical advices prescribed by doctors showed that 7.5 % of drugs were forbidden used in patients with severe renal insufficiency,and 34.1% of drugs should be used with caution or reduction requirements.Each department and system had irrational drug using.Conclusions There are a large problem in the clinical medication in patients with renal insufficiency,and a reasonable medication standard needs to be established to find out potential irrational drug using so as to reduce the possibility of adverse reactions.
10.Periphery analgesic effect of myricetin on a rat model of inflammatory pain

Aim To investigate the periphery analge-sic effect of myricetin on a rat model of inflammatory pain and the mechanism. Methods Rat models of in-flammatory pain were induced by complete Freund ’ s adjuvant ( CFA) injection in left hindlimb plantar cen-ter. The thermal withdrawal latency ( TWL) was meas-ured before and after CFA or myricetin treatment. Elec-trophysiological method was used to identify the effect of myricetin on the action potential frequency and the voltage dependent potassium channel currents in small DRG neurons. Results Rats with CFA injected showed thermal hyperalgesia ( P <0. 05 ) and TWL in-creased significantly after myricetin intraperitoneally in-jected ( P <0. 05 ) . Current clamp recording showed the action potential frequency of small DRG neurons in rats was inhibited by myricetin ( P<0. 01 ) and voltage calmap recording showed the inhibitory effect of myr-icetin was enhanced by calcium depended potassium channel currents ( P<0. 05 ) . Conclusion Myricetin exerts periphery analgesic effect by enhancing calcium depended potassium channel currents and inhibiting excitability of small neurons of dorsal root ganglion.
