1.Epidemiological characteristics related to the morbidity of carcinoma of large intestine

OBJECTIVE: To probe into the effects of living habit, diet, drug and hereditary factors related to daily life so as to reduce the incidence of carcinoma of large intestine through changing the life style.DATA SOURCES: Articles on carcinoma of large intestine published between January 1980 and December 2003 were retrieved in NCBI Entrez PubMed with the key words of "carcinoma of large intestine, epidemiology and prevention" and the language restricted to English. Meanwhile, articles on carcinoma of large intestine published between January 1994 and December 2003 were searched for in CNKI database with the Chinese key words of "carcinoma of large intestine, epidemiology and prevention" and the language restricted to Chinese.STUDY SELECTION: The related literature was selected by the primary tions, hormone level and genetic factors on the morbidity of carcinoma of excluding blind method was not required. Exclusion criteria: review articles and papers of meta-analysis and replicated studies.DATA EXTRACTION: Totally 84 related articles were collected, including 17 retrieved with retrospective approach. Fifty-eight articles met the inclusion criteria, and 26 papers were excluded. Among the excluded papers, 6 papers were about the basic biological and chemical research on carcinoma of large intestine, 12 were about different pathological types of carcinoma of large intestine and distribution of hospitalized cases, 4 were meta-analysis, and 4 were of popular science and delayed diagnosis due to physicians or patients themselves. The collected data showed that the morbidity and distribution of large intestine carcinoma were related to region,age, dietary factor, verminosis, heredity, hormone and chronic intestinal diseases and long-term stress.characteristics, trace element intake and living habits on the morbidity of responding preventive measures for reducing the incidence of large intestine carcinoma.CONCLUSION: With the improvement of living standard, the occurrence .and distribution of carcinoma of large intestine increase, presenting a tendency to occur in young people. Developing good life style and dietary habit and doing moderate exercise can prevent the occurrence of carcinoma of large intestine.
2.Probe into sleep quality in the patients with irritable bowel syndrome

Objective To investigate the sleep features in the patients with irritable bowel syndrome (IBS) and compare the sleep quality between those IBS patients who were with and without anxiety and depression.Methods Pittsburgh sleep quality index questionnaire (PSQI), self-rating anxiety scale (SAS) and self-rating depression scale (SDS) were measured in the 145 IBS patients and 59 regular physical examination volunteers.IBS patients were also divided into two subgroups-patients with or without anxiety and depression based on cutoff scores of SAS and SDS.Comparisons of sleep quality were made between subgroups, and between IBS patients and volunteer controls.Results Compared with the controls,the SAS raw score, SDS raw score and SAS positive incidence in IBS patients were shown statistically significant differences (P < 0.05 ), while the SDS positive incidence had no statistically significant difference(P > 0.05 ).PSQI total scores were significantly higher in the IBS patients without anxiety and depression (P < 0.05), 3 domains (sleep quality, sleep disturbances and daytime function disorder) were also found statistically significant differences ( P < 0.05 ), compared with the controls.The IBS patients with anxiety and depression were statistically significantly different from the controls ( P < 0.05 ) in 6 domains (sleep quality, sleep latency, sleep efficiency, sleep disturbances, sleep time and daytime function disorder) and significantly higher PSQI total scores( P < 0.05 ).Statistically significant differences (P <0.05) were also found in all 7 domains and with higher PSQI total scores in IBS patients with anxiety and depression, compared with IBS patients without anxiety and depression.Conclusions IBS patients were more likely to have sleep abnormality, mainly in sleep quality, sleep disturbances and daytime function disorder and PSQI total scores.The abnormalities of these factors were independent of emotional disorder.However, emotional disorder worsened the sleep disorder in IBS patients.
3.Evaluation of the effect of human biological stimulation feedback instrument for screening of metabolic syndrome

Objective To assess the effect of a noninvasive equipment (the human biological stimulation feedback instrument) for the screening of metabolic syndrome (MS). Methods The samples were selected from the International Health Care Center of the Second Affiliated Hospital of Zhejiang University College of Medicine. From January to December 2012, according to the inclusion criteria, a total of 5674 consecutive participants (3 437 men, 2 237 women) without medical history of diabetes mellitus underwent general health screening in this hospital, which included questionnaire investigation, all MS components and the risk assessment of the human biological stimulation feedback instrument. MS was defined according to 2004 the Chinese Diabetes Society (CDS) criteria. Results The 5 674 subjects were divided into three groups in accordance with the human biological stimulation feedback instrument risk score: the normal group, metabolic abnormalities low risk group, metabolic abnormalities high risk group. The detection rate of MS corresponding to the three groups was 8.73% (251/2 876), 29.32% (612/2 087), 32.77% (233/711), respectively (trend χ2=372.14,P<0.001). With the increase of risk score, the detection rate of MS increased. A total of 1 096 (male 879, female 217) were diagnosed MS by CDS criteria. The area under the curve (AUC) of the test was 0.674 (95%CI 0.658-0.691, P<0.000 1)for predicting MS. The corresponding cutoff value was 25.00%, the sensitivity, specificity, positive predictive value and negative predictive value of diagnosis of MS was 77.80%, 56.00%, 30.20%and 91.30%, respectively;409 subjects who had no history of MS and diabetes mellitus follow up for two years, the AUC of the test was 0.719 (95%CI 0.644-0.794)for predicting MS. The corresponding cutoff value was 25.00%, too. And the sensitivity and specificity of diagnosis of MS was 78.00%and 59.80%, respectively. The detection rate of MS was positively correlated with the risk score. Conclusion We found that the human biological stimulation feedback instrument is a potential screening approach for early predicting MS in health checkup with simple and noninvasive procedure and high negative predictive value, although its diagnostic efficiency is not so good. And it might have a warning effect on the progress of MS.
5.Association between cyberbullying and depression in college students

Objective:
To explore the relationship between cyberbullying and depression in college students, and to provide a scientific basis for the prevention of cyberbullying.
Methods:
A total of 3 914 college students from 6 universities in Shanxi Province were selected as research objects. The students’ cyberbullying behavior scale and depression self-rating scale were used to investigate the students.
Results:
The overall reported rate of cyberbullying among college students was 17.4%. Boys who suffered from online verbal bullying and online fraud scored higher than girls. Students whose parents divorced scored higher than those whose parents did not divorce in all dimensions of cyber-bullying. There was a statistically significant difference in the overall scores of students with different degrees of education and time spent online( P <0.05). Depression was reported in 15.6% of participants. The depression scores of female students were higher than that of male students.College students whose parents were divorced had higher depression scores than those whose parents were not divorced.There were statistically significant differences in depression scores among college students with different fathers’ education levels and different time spent online every day( t / F =2.05, 6.64, 3.91 , 19.52, P <0.05). Pearson correlation analysis showed that online speech bullying, online fraud and identity concealment bullying were positively correlated with depression scores( r s =0.40, 0.36, 0.46, P <0.01). Multiple linear regression analysis showed that three forms of cyberbullying were risk factors for the increase of depression in college students( β =0.89, 0.38, 0.38, P <0.01).
Conclusion
Cyber-bullying is one of the risk factors for increasing the severity of depression among college students.Strengthening the management of Internet use, promotion college students’ interpersonal conununication and active cooperating between society, school and family are great significance to reduce the occurrence of cyberbullying.
6.Relationship between childhood maltreatment and suicide ideation of college students

Objective:
To explore the relationship between childhood maltreatment and suicide ideation of college students,so as to provide basis for physical and mental health of college students.
Methods:
Students from 6 universities in Shanxi Province(3 854 college students provided eligible questionniare for childhood abuse and 3 882 for suicidal ideation) were selected by multi-stage random cluster sampling from May to July 2018, the Chinese Version of the Childhood Trauma Questionnaire and the Self-rating Idea of Suicide Scale were used in the survey.
Results:
Of all subjects,42.4% had at least one kind of abuse experience in childhood and 3.9% had suicidal ideation in the past week. By chi square test,the detection rate of suicidal ideation of college students who suffered physical abuse,emotional abuse,sexual abuse,physical neglect,emotional neglect in childhood was higher than that of college students without maltreatment (χ2=13.78,12.97,17.10,56.56,66.58,P<0.01);Spearman correlation analysis showed that there was a positive correlation between each type of maltreatment in childhood and college students’ suicidal ideation(r=0.06,0.06,0.07,0.12,0.13,P<0.01);Multivariate Logistic regression model found that after controlling other influencing factors,physical neglect and emotional neglect in childhood were independent risk factors for suicide ideation of college students (OR=2.18,2.07,P<0.05).
Conclusion
Physical neglect and emotional neglect in childhood can increase the risk of college students’ suicidal ideation. Paying enough attention and care to them in early childhood may help to prevent their suicidal ideation in adulthood.
7.Correlation between experience of childhood abuse and implementing cyberbullying in college students

Objective:
To explore the relationship between childhood abuse and cyberbullying among college students and to provide theoretical support for early intervention of cyberbullying among college students.
Methods:
A total of 3 850 sophomores and juniors from 6 universities in Shanxi Province, randomly selected by stratified cluster sampling method, were investigated by using the Questionnaire on Cyberbullying and the Chinese Version of the Child Abuse Questionnaire.
Results:
Male students reported higher rates of sexual abuse, emotional neglect and physical neglect than female students(χ2=5.22, 4.39, 7.53 P<0.05). The child abuse report rate of college students whose parents divorced was higher than that of those whose parents were not divorced(χ2=86.80, 134.06, 130.18, 175.64, 118.46,P<0.05). In addition to physical neglect, childhood abuse rate of only children was higher than that of non-only children, with statistically significant differences(χ2=9.44, 12.44, 21.18, 21.26, all P<0.05). The scores of all factors and total scores of cyberbullying implemented by male students were higher than those by female students(t=9.35, 5.59, 5.83, 7.57,P<0.05); the scores of all factors and total scores of cyberbullying implemented by only-child students were higher than those by non-only-child students(t=2.79, 3.74, 4.78, 4.40,P<0.05); the scores of all factors and total scores of cyberbullying implemented by students whose parents were divorced were higher than those by students whose parents were not divorced, with statistically significant differences(t=6.99,6.78, 8.04,11.33, P<0.05). Multiple linear regression model showed that there was a positive correlation between the childhood abuse of college students and the implementation of cyberbullying, and regression coefficient differences of all factors were statistically significant(β=0.10, 0.11, 0.05, 10.08,0.06, P<0.05).
Conclusion
Childhood abuse experiences increase the risk of college students practicing cyberbullying.
8.Exploration on training model of general practitioners in county medical community with the outreaching of medical consortium

In recent years, a number of policies have been implemented to strengthen the cultivation of general practitioners in China. However, the development of community-level health professionals is still lagging behind, the development is uneven among regions, the overall number of general practitioners is insufficient, and the quality of medical services needs to be improved. Based on the Shanhai (Mountain and Sea) promotion project, with the close cooperation between the Second Affiliated Hospital and the medical consortium unit Suichang County People′s Hospital, a " Joint Cultivation by Dual-Teachers " model has been applied in training general practitioners for Suichang county community since March 2021. In this article we discuss the optimization and integration of medical and health resources through the outreaching assistance of high-quality personnel, management and system of medical service, to comprehensively upgrade the primary care and the quality of grass-roots general practitioners in remote mountainous areas.
9.A case of neurofibromatosis complicated with small bowel obstruction.

Neurofibromatosis type 1(NF-1) with gastrointestinal stromal tumor (GIST) is a rare case in clinical practice. But it is even rarer that the GIST tumor in this case is so large that it cause small intestinal obstruction. Here we report such a male case of NF-1,with positive family history of NF-1 and nodules all over the patient's skin and abdominal cavity. The patient came to hospital with a sudden upper abdominal pain and nausea and vomiting over 5 d. Abdominal computerized tomography (CT) showed that there were numerous nodules of different size in his abdominal cavity. And the largest one is about 10 cm in diameter,which oppressed the small bowel and caused the small bowel mesenteric volvulus and obstruction of the bowel. Finally,the tumor was pathologically proved to be a GIST tumor but not NF nodule. The patient stays healthy until now after operation.
10.Controlled attenuation parameter for steatosis assessment in health checkup groups

Objective:To evaluate the quantitative diagnostic value of controlled attenuation parameter (CAP) in health checkup groups with asymptomatic nonalcoholic fatty liver disease.Methods:A multicenter prospective study was conducted among Chinese individuals undergoing regular health checkups; a total of 173 subjects were investigated. Human body indexes such as height, weight, and blood pressure were measured, and complete blood count, liver function, blood lipid, FibroScan, and MRI-PDFF examinations were performed. Correlation between MRI-PDFF and CAP was described using Spearman′s and Pearson′s coefficients. Diagnostic efficacy of the CAP was evaluated using the subject work characteristic curve and the area under this curve, and the optimal cut-off value was determined according to the Youden index.Results:The average age and body mass index of the subjects were 45.0±10.5 years and 25.8±4.0 kg/m 2, respectively. A linear correlation was found between CAP and lg transformed magnetic resonance imaging-based proton density fat fraction results (Pearson′s coefficient 0.772, P<0.001). When optimized for ≥90% sensitivity, the CAP cutoff for staging ≥S1 steatosis was 244 dB/m. Conclusions:The CAP result was significantly correlated with the liver fat fraction measured by MRI-PDFF, and capable of differentiating steatosis grades. CAP can be used as a tool for screening fatty liver in health checkup groups.
