1.Mini-invasive treatment of cystitis glandularis with affected ureteric meatus

Objective To investigate the minimal invasive management of cystitis glandularis with invasion of the ureteric meatus.Methods The clinical data of 18 cases were reviewed.Among the 18 cystitis glandularis patients,12 cases were invasion of bilateral ureteric meatus and 6 of unilateral ureteric meatus.Operation or drug treatment was performed on the diseases that can cause cystitis glandularis such as bladder stone,bladder neck stegnosis,external urethral meatus stegnosis and benign prostate hyperplasia.Sensitive antibiotic was administrated in all cases.After placing ureter catheter,transurethral plasma electro-resection was carried out in five patients whose ureteric meatus could be identified.In addition,of thirteen patients with ureteral orifice unable to be identified,there were ten cases with normal renal function,mitomycin was injected under affected membrana mucosa,and then the patient with ureteral orifice identified underwent transurethral plasma electro-resection after placing ureter catheter.On the other hand,the patient whose ureteral orifice still could not be recognized undertaken transurethral electro-resection at first,during which the ureter catheter was put once ureteral orifice had been detected,otherwise,the ureter catheter should be placed through cystoscope if nephritic colic emerged and hydronephrosis aggravated after operation.In those patients with kidney dysfunction,the ureter catheter was put by ureter discission or ureter replantation at first,the transurethral electro-resection could not be executed until the renal function recovered.Following all these procedure above,bladder instillation of drugs regularly,anti-infection and symptomatic treatment were administrated.Results One patient combined with bladder adenocarcinoma received cystectomy,of the other patients,six cases recurred and underwent electrotomy again resulting in no relapse.All nephrohydrops vanished or relieved obviously,nevertheless,urinary tract infection,haematuria and bladder-ureter backstreaming as the cardinal complication developed in some cases.Conclusion In the management of cystitis glandularis encroaching ureteric meatus,total or partial cystectomy can be avoided if ureter draining freely can be ensured,motivation removal,antiinfection,injection of drug under mucosa and preoperative diuresis conduce to the achievement of ureter catheter placing,transurethral plasma electro-resection is still effective methods in treating these cystitis glandularis.
2.Application of bioactive artificial vertebrae of nano-hydroxyapatite and polyamide 66 to anterior approach of osteoporotic thoracolumbar burst fractures

BACKGROUND: During thoracolumbar anterior operation, implant collapse is an important factor for affecting outcome in patients with thoracolumbar fracture. In particular, osteoporotic patients commonly developed implant collapse, nail channel loose, bone disunion, kyphosis deformation. Bioactive artificial vertebrae of nano-hydroxyapatite and polyamide 66 (n-HA/PA66) has good biocompatibility and biological safety and is an ideal substitute for vertebral body bone graft. OBJECTIVE: To investigate the efficacy of the bioactive artificial verteificial vertebrae of nano-hydroxyapatite and polyamide 66 (n-HA/PA66) for osteoporotic thoracolumbar burst fracture. DESIGN, TIME AND SETTING: A retrospective case analysis was conducted at the Department of Spinal Surgery, Affiliated Hospital of Luzhou Medical College from January 2004 to January 2008. PARTICIPANTS: A total of 20 patients with moderate and severe osteoporotic thoracolumbar fracture (6 males and 14 females) aged 51 -82 years, with an average of 69 years were enrolled in this study. There were 17 cases of fresh fracture and 3 cases of old fracture. n-HAyPA66 was produced by the Sichuan Guona Science and Technology Co., Ltd. This artificial vertebral body was in 10-35 mm diameter, 30-100 mm length, circular cylinder shape, 3-12 mm midheaven diameter, 2.5-6.5 mm tubal wall thickness, with the presence of 2 mm holes surrounding the vertebral body. The contact area of the vertebral body was 78.5-176.7 mm2. Fixity was anterior nail plate system (titanium alloy material) produced by Foshan Shitaibao Company. METHODS: Following conventional tracheal intubation and general anesthesia, at right arm recumbent, according to various fracture-involved segments, different regions were incised and treated with anterior decompression, implanted with artificial vertebrae with the bioactive artificial verteificial vertebrae n-HA/PA66 and fixed with steel plate. MAIN OUTCOME MEASURES: Fracture healing and implant loose were observed by radiograph. Cobb angle, the height of damaged vertebral body and spinal cord function score were compared prior to surgery, 3 months following surgery and the last follow-up. RESULTS: All the patients were successfully operated. Bleeding 200-800 mL during surgery and operation time ranged from 2.0 to 3.0 hours. There were 1 case of pulmonary infection and 1 case of delayed wound healing. A total of 20 patients were followed up for 6-42 months (averagely 18 months). Postoperative radiograph review demonstrated that closed vertebral body was healed three to four months. No significant displacement of implant was found. There was less loss of reestablished vertebral body height. Internal fixation position was good, without nail breakdown or internal fixation loose/displacement. At 3 months following surgery, significant differences in Cobb angle, the height of damaged vertebral body and spinal cord function score were detected compared with that before surgery (P < 0.05). No significant difference in postoperative two follow-up was detected (P > 0.05). CONCLUSION: The bioactive artificial vertebrae of n-HA/PA66 can increase the fusion area, reduce local pressure and prevent loosening of implants and sinking into the vertebrae, finally resulting in restoration of vertebral body height.
3.Effect of independent breath-holding technology on target geometry in radiotherapy for liver cancer

Objective To use the fusion image of the end-inhalation holding (EIH) phase and endexhalation holding (EEH) phase to define the target volume of individual patient with liver cancer,and to evaluate the target geometry,feasibility,and clinical significance of the technology.Methods Eighteen patients with liver caucer who were treated in our hospital from 2012 to 2013 were enrolled as subjects.With the same posture and scan range,all patients underwent contrast-enhanced three-dimensional computed tomography (3DCT) scans in the phases of free breathing (FB),EIH,and EEH.Gross tumor volume (GTV),clinical target volume (CTV),and organ of risk (OAR) were delineated on the above images.CTVFB was defined as GTV on the FB phase image (GTVFB) plus a margin of 10 mm,while planning target volume (PTVFB) was defined as CTVFB plus a margin of 10 mm in the right-left and anterior-posterior directions and a margin of 20 mm in the superior-inferior direction.GTVEI and GTVEE were defined as GTV on the EIH and EEH images,respectively.Based on the EEH images,the registered EEH and EIH images were fused to form GTVEI+EI.CTVEI+EE was defined as GTVEI+EI plus a margin of 10 mm,while PTVEI+EE was defined as CTVEI+EE plus a margin of 5 mm in the right-left and anterior-posterior directions and a margin of 10 mm in the superior-inferior direction.The Pinnacle3 v8.0m treatment planning system was used to design two 3D conformal radiotherapy plans for each patient.The volume,degree of inclusion (DI),matching index (MI),and central displacement of CTVFB and CTVEI+EE,as well as PTVFB and PTVEI+EE,were compared between the two plans.Results In the 18 patients,the mean CTVFB was significantly smaller than the mean CTVEI+EE(149.00±87.54 cm3 vs.188.17± 125.72 cm3,P=0.014);there was no significant difference between the mean PTVFB and PTVEI+EE (276.68± 146.41 cm3 vs.253.66± 117.35 cm3,P=0.080).DI of CTVFB to CTVEI+EF,PTVFB to PTVEI+EE,CTVEI+EE to CTVFB,and PTVEI+EE to PTVFB were (99.83±0.09)%,(84.55±8.45) %,(80.83± 12.31) %,and (99.78±0.08) %,respectively.MI of CTVEI+EE to CTVFB and PTVEI+EE to PTVFB were 0.83± 0.07 and 0.87± 0.03,respectively.The central displacements of CTVEI+EE from CTVFB in x,y,and z axes were 0.55± 1.07 cm,0.76±3.02 cm,and-0.26± 1.98 cm,respectively (P =0.432,0.971,0.587).Conclusions In the treatment of liver cancer,the target volume delineation and image fusion using 3DCT images in EIH and EEH phases may avoid target omission due to respiratory movement,making it possible to increase radiation dose to target volume and improve the efficacy of radiotherapy.
4.Differences in carbon source usage by dental plaque in children with and without early childhood caries

Early childhood caries(ECC)is a considerable pediatric and public health problem worldwide.Preceding studies have focused primarily on bacterial diversity at the taxonomic level.Although these studies have provided significant information regarding the connection between dental caries and oral microbiomes,further comprehension of this microbial community's ecological relevance is limited.This study identified the carbon source metabolic differences in dental plaque between children with and without ECC. We compared the microbial community functional diversity in 18 caries-free subjects with 18 severe ECC patients based on sole carbon source usage using a Biolog assay.The anaerobic microbial community in the ECC patients displayed greater metabolic activity than that of the control group.Specific carbon source metabolism differed significantly between the two groups.Subjects from the two groups were well distinguished by cluster and principal component analyses based on discriminative carbon sources. Our results implied that the microbial functional diversity between the ECC patients and healthy subjects differed significantly.In addition,the Biolog assay furthered our understanding of oral microbiomes as a composite of functional abilities,thus enabling us to identify the ecologically relevant functional differences among oral microbial communities.
5.Peri-operative management of off-pump coronary artery bypass grafting

Objective To retrospectively analyse the clinical data of off-pump coronary artery bypass grafting surgery(OPCAB)in our department and summarize the clinical experience of peri-operative management.Methods From January 2007 to May 2008,253 patients underwent OPCAB.Seventy-six cases with left main disease(including 18 cases of single left main lesion and 58 cases of complex lesion);13 cases had single vessel disease;32 cases had double vessels disease and 190 cases had triple vessels disease.Eighteen cases received emergency OPCAB.All vessel anastomosis were performed with coronary artery stablizer and shunt through median sternotomy.Resuits Three cases died after operation(1.2%).Intra-aortic balloon pump was used in 11 cases.Mechanical ventilation time was 3-168 h[(24.1±22.3)h],and ICU stay time was 1-14 day[(2.8±2.0)d].Postoperative hospital stay time was 8-42 days[(15.6±6.3)d].Conclusion OPCAB is reliable for most bypass cases.With appropilate peri-operative management,a satisfied result will be acceptable.
6.Clinical analysis of 1386 patients undergoing off-pump coronary artery bypass grafting

Objective To summarize the clinical experiences of 1386 patients undergoing off-pump coronary artery bypass grafting(OPCAB)during Jan 2002 to Mar 2008.Methods OPCAB was done through midsternumtomy under ordinary-temperature anesthesia after radial artery,left breast artery and vena saphena magna were taken.Patients over 70 years old accounted for 32.35%,and the oldest was 99 years old.Emergency and subemergency OPCAB was done in 111 cases,re-do CABG with off-pump technique was done in 52 patients.Results 24 cases died during perioperation,with mortality of 1.73%and all other patients discharged from the hospital.Following operation,the symptoms of angina pectoris were relieved and the life quality was much beaer than that before operation.Conclusion OPCAB is feasible for older patients,who are complicated with other chronic diseases for elder patients.Skillful technique of surgeon,right indications for OPCAB and management of peri-and postoperation are key to success of operation.
7.Expression and clinical significance of ADAM-12 and PCNA in bladder carcinoma

Objective To investigate the expression of adamalysin-12 (ADAM-12) and PCNA in human bladder carcinoma and to investigate their correlation with different grades and stages of bladder cancer.Methods Biopsies of 15 normal bladder and 43 bladder tumors were analyzed.Immunohistochemistry was conducted to detect the expression of ADAM12 and PCNA in the biopsies.Results Postive expression signals of ADAM12 were detected significantly higher in the bladder cancer biopsies than that in the normal ones (P = 0.010).Those with lower histological grade had a higher expression level of ADAM-12 compared to the higher histological grades (P <0.001 ).Positive expression signals of PCNA were detected significantly higher in the bladder cancer biopsies than that in the normal ones (P = 0.026).Those with lower histological grade had a higher expression level of PCNA (P =0.014).There was a positive correlation between the expression of ADAM-12 and PCNA in bladder cancer (r =0.997,P < 0.001 ).Conclusions The overexpression of ADAM-12 and PCNA in the biopsies of bladder tumors shows that protein expression of ADAM-12 and PCNA correlated with tumor stage and grade.Furthermore,ADAM-12 may be a promising biomarker of bladder cancer in the clinical implication.
8.Clinical observation on effect of shenqi guchong recipe in treating peri-menopausal anovulatory dysfunctional uterine bleeding.

OBJECTIVETo investigate the effect and hemostatic mechanism of Shenqi Guchong Recipe (SGR) in treating peri-menopausal anovulatory dysfunctional uterine bleeding (ADUB) of qi deficiency blood stasis syndrome type (QDBS).
METHODSPatients of ADUB-QDBS were assigned to two groups, the 47 patients in the treated group administering orally with SGR and the 31 in the control group treated with Kunning Oral Liquid. The hemostatic effect, and the changes before and after treatment of 6-ketoprostaglandin 1alpha (6-K-PGF1 alpha), thromboxane B2(TXB2), T-lymphocyte subsets (CD3 and CD8, CD4/CD8 ratio) in menstrual blood, and serum calcium level in patients were observed.
RESULTSThe hemostatic effect in the treated group [85.1% (40/47)] was better than that in the control group [51.6% (16/31)], significant difference was shown between groups (P < 0.05). Changes of 6-K-PGF1 alpha TXB2, and 6-K-PGF1 alph/TXB2 ratio in the treated group were significant after treatment and showed significant difference as compared with those in the control group (P < 0.05 or P < 0.01). Meantime, serum Ca2+ level was increased and parameters of T-lymphocyte subsets improved significantly in the treated group (P < 0.05 or P < 0.01), also statistically different to those in the control group (P < 0.05).
CONCLUSIONSGR shows obvious hemostatic effect in treating patients with ADUB of QDBS type, and the mechanism may be related to its actions for improving microcirculation, enhancing immune function, and improving uterine local micro-environment, etc.

9.Histiocytic sarcoma:a clinicopathologic study of 4 cases.

OBJECTIVETo investigate the clinicopathologic features, immunophenotyping, differential diagnoses and prognosis of histiocytic sarcoma (HS).
METHODSThe clinical and pathologic findings of 4 cases of HS were reviewed. The samples were used for paraffin section, HE stain, immunohistochemistry stain by EnVision method, electron microscope observation. Follow-up information was available in all patients.
RESULTSThe age of patients, 2 males and 2 females, ranged from 22 to 65 years old (median, 43.25 years). The sites of involvement included lymph node (2 cases), skin or soft tissue (1 case) and colon (1 case). The tumor cells were widespread infiltration, diffused distribution, no adhesion to each other. Tumor cells were middling and large, round, orbicular-ovate, polygon, epithelium appearance, plentiful cytoplasm and acidophilia, cystose. Nucelus was round, orbicular-ovate, dissymmetry. Nuclear chromatin was vacuole appearance, basophilia nucleolus, caryocinesia and pathological mitotic figure. Three of the cases showed conjugate nuclei, increased pleomorphism with multinucleated tumor giant cell formation. Focal cytoplasmic with foamy appearance was identified in 2 cases. One case demonstrated foci of spindly sarcomatoid appearance. Hemophagocytosis was identified in 2 cases. The tumor cells of 4 cases were often accompanied by various numbers of inflammatory cells. Immunohistochemical study showed that all cases were diffusely positive for α-1-ACT, CD68, CDl63 and lysozyme. Three of 4 cases also expressed CD45, CD45RO. The electron microscope results of 4 cases showed that the tumor cells were plentiful cytoplasm and a few cytolysosome in the cytoplasm, and no birbeck cytorrhyctes, cell-cell junction and digitation. Amongst the 4 patients with follow-up information available, three died of the disease 6-13 months after diagnosis. One patient, whose lesion was localized at the skin and soft tissue, survived at the present time.
CONCLUSIONHS was a scarce malignant tumor with mature histiocyte morphology and immunophenotype character. The diagnosis should be based on tissue morphology, immunohistochemistry and electron microscope observation to exclude other disorders.

Objective To explore the expressions and significance of brain natriuretic peptide (BNP),hypersensitive C-reactive protein (hs-CRP),human fibrinogen (Fib),D-Dimer and blood gas analysis in patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD) and pulmonary hypertension (PH).Methods Totally 135 patients with AECOPD were divided into single AECOPD group (n =75) and AECOPD + PH group (n =60).The value of pulmonary systolic pressure (PASP) was measured by echocardiography.Serum BNP,hs-CRP,D-Dimer,Fib levels and arterial blood gas analysis indexes such as p(CO2) and p(O2) were compared between two groups.Results The levels of hs-CRP,BNP,D-Dimer and p(CO2) in AECOPD + PH group were significantly higher than those in AECOPD group (P < 0.05),while p (O2) in AECOPD + PH group was significantly lower than that in AECOPD group.Multiple linear regression analysis showed that the levels of hs-CRP,Fib,BNP,D-Dimer and p (CO2) were positively correlated with pulmonary artery pressure (P < O.05),and p (02) was negatively correlated with PASP (P < 0.05).Conclusion The hs-CRP,Fib,BNP,and D-Dimer participate in the formation of AECOPD complicated with pulmonary hypertension,and systemic inflammation plays an important role in the pathogenesis of AECOPD complicated with pulmonary hypertension.
