37 free second molar residual crowns were repaired by glass fiber post and resin core(GF group),and another 37 by cobalt chromium alloy casting post and core(CCA group).The success rate of 1 ~5 years of GF group was 1 00%,1 00%,91 .9%,86.5% and 81 .1 %;that of CCA group 97.3%,91 .9%,83.8%,81 .1 % and 73.0%,respectively(P >0.05).

The aging of the population and improvements in outcomes after cardiovascular surgery have resulted in a worldwide growing demand of complex surgical intervention for elderly patients. We briefly review the up-to-date English-language literature with particular focus on cardiovascular surgery in elderly patients. With earlier referral, careful preoperative evaluation, strategic planning, and the continuing efforts in optimizing surgical techniques, operative mortality and morbidity following primary or reoperative coronary artery bypass grafting and valvular interventions are expected to fall in this high-risk patient subset. Importantly,accumulating evidence indicates that elderly patients may benefit from improved functional status and quality of life after cardiovascular surgical therapy

Objective To construct an X-gene defect and IFN gene contained HBV expression plasmid and explore its expression effect in HepG2 cell line. Methods By employing pBR322-B-HBV plasmid which contained type adrⅠHBV DNA, we destroyed HBV X-gene by inserting mutation and connected the defect HBV with IFN sequence. After recombination, it was introduced into a mammalian plasmid and was constructed as the objective plasmid. At the same time, two control groups were constructed. One was pcDNA3-ES-HBV2, which contained the full length of HBV DNA. The other was pcDNA3-KN-F1F2, which contained the X-gene defect HBV DNA. Then we transfected HepG2 cells with all the plasmid. After the cells were screened by G418, the X-gene defect and IFN gene contained HBV(KN-F1F2-IFN) and IFN protein from culture medium were detected by fluorescence- quantitative-PCR and ELISA respectively. Results pcDNA3-KN-F1F2-IFN, as the objective plasmid, was constructed successfully; contrary with the control virus, the amounts of both HBV and HBsAg were lower; both IFN mRNA and IFN protein were detected.Conclusions An X-gene defect and IFN gene contained HBV expression plasmid was constructed successfully and it can be expressed in HepG2 cell line.

Object To establish an HPLC method for the determination of dehydroxypresenegenin for the quality control of Polygalae tenuifolia Willd. Methods Dehydroxypresenegenin was separated by ODS column (125 mm ? 4 mm, 5 ?m) with a mobile phase of methanol 0 05% phosphoric acid (74∶26) and detected at a wavelength of 210 nm. Results The calibration curve was linear in the range of 0 585～11 7 ?g,with a correlation coefficient of 0 999 99 . The average recovery was 99 21% (n=5), and RSD=0 25% (n=5). Conclusion The method is simple and accurate, with good repeatability and can be used for the quality control of Polygalae tenuifolia Willd and compound preparations of Radix Polygalae.

Objective To investigate the therapeutic efficacy and safety of stellate ganglion block (SGB) with ropivacaine to treat Renault's disease.Methods Thirty seven patients with Renault's disease were randomly assigned into two groups:the lidocainegroup (L group,18 patients) treated by SGB with 5 ml of 1％ lidocaine,and the Ropivacaine-group (R group,19 patients) treated by SGB with 5 ml of 0.2％ ropivacaine.The efficacy,adverse reactions,and the recurrence after 3 months of two groups were observed and compared.Results The total effective rate of 89.5 ％ in group R was significantly higher that 55.6％ in group L (P ＜ 0.05).There were less times(22 times) presenting no Homers'sign in group R compared to group L (43 times).The incidence of adverse reaction and the recurrence after 3 months in group R were not statistically significantly different form group L (P ＞ 0.05).Conclu-sions SGB with 0.2％ ropivacaine could obtain better efficacy compared to lidocaine,and had a higher security.

Objective To evaluate the efficacy of endovenous laser treatment(EVLT) plus punctiform stripping therapy for lower extremity varicose veins.Methods Sixty-eight limbs in fifty patients with varicosed vein underwent EVLT plus punctiform stripping therapy in department of vascular surgery of the first affiliated hospital of Anhui medical university.Results All patients were followed for 2～27 months and all the patients had successful occlusions of varicosed veins with no recurrence were observed.Local skin paresthesia were found in 2 patients.No local recurrence and deep vein thrombosis were found.Conclusion EVLT plus punctiform stripping therapy is a simple and effective treatment for lower extremity varicosed veins with fewer complications and recurrences.

Descending necrotizing mediastinitis that has an abdominal pain as a main clinical manifestation is seldom. Here one case is reported. At the beginning, the patient had pharyngalgia and his swallowing was not smooth. After that, abdominal pain became a main symptom. Pharyngalgia relieved . However CT showed mediastinal infection. Surgical drainage,antibiotics treatment and nutritional support were performed. The patient was cured.

Objective To study the distribution characteristics of radon progeny at different air quality index (AQI) and evaluate health risks for the exposed population.Methods EQF3120 Monitor was used to monitor the concentration of radon and its daughters.The relevant air quality data was provided by the environmental monitoring station at Suzhou Industrial Park.SPSS 16.0 software was used for statistical description,principal component analysis and simple correlation analysis.Environmental radon exposure-caused effective dose to lung region was estimated by using the radon dose formulas.Results Radon progeny in fog haze weather in winter of Suzhou Industrial Park had relationships with NO2,SO2,O3,PM10 and PM2.5.The correlation coefficient of 214Bi were 0.741,0.681,-0.431,0.597 and 0.675.The correlation coefficient of radon progeny with PM2.5 was greater than that with PM10.When AQI ＞ 200,the effective dose to residents outdoor from radon and its short-lived progeny was 0.63 mSv/a.Conclusions Higher AQI results in higher concentration of attached radon progeny.There is little variation in concentration of unattached radon progeny.Fog and haze can increase exposure of the population to inhalated radon.

Objective To observe the effect of buprenorphine transdermal patch combined with gabapentin in patients with diabetic peripheral neuralgia (DPN).Methods Sixty patients were divided randomly into experimental (A) and control (B) groups.Patients in group A were treated with buprenorphine transdermal patch combined gabapentin.Patients in group B were orally administered pregabalin only.The period of treatment was 8 weeks.The analgesic effect [visual analogue scale (VAS) scores] , sleep scores, daily doses of gabapentin, and adverse reactions were evaluated before treatment, and the 2nd weeks, 4th weeks and 8th weeks after treatment.Results The VAS scores of group A were significantly lower than group B at 4th weeks and 8th weeks after treatment [(2.8 ± 1.6) points vs (3.8 ± 1.5) points,(2.6 ± 0.5) points vs (3.5 ± 1.6) points, P ＜ 0.05].The sleep scores of group A were significantly higher than group B (P ＜ 0.01).The daily doses of gabapentin were significantly lower [(853.0 ± 349.8) mg vs (1575.8 ± 351.0) mg, (845.1 ± 342.7) mg vs (1434.2 ± 440.8) mg, (826.9 ± 298.5) mg vs (1389.8 ±356.6)mg, P ＜ 0.05].The incidence of adverse reactions was significantly lower in group A (10.7％ vs 25.9％ , 10.7％ vs 29.6％ , P ＜ 0.05).Conclusions The combination of buprenorphine transdermal patch and gabapentin is effective and safe for the patients with DPN.
