[Objective]To investigate the treatment of thoracolumbar vertebral fracture without nerve symptoms.[Method]Thirty cases of thoracolumbar vertebrae fracture without nerve and spinal cord symptoms were treated with vertebra body restitution,pedicle screw fixation and posterolateral bone fusion.[Result]All the cases were followed up for 12-27 months,with an average of 15 months.There was no loosening and breaking of internal fixation.A great part of the height of the fractured vertebra body and the intervertebral body angle was restored(P

[Objective]To evaluate the clinical efficacy of posterior pedicle screw instrumentation followed by anterior debridement and bone grafting through the lateral edge of the rectus abdominal muscle and retroperitoneal approach for the treatment of tuberculosis of the lower lumbar spine. [Methods]From June 2002 to January 2007,27 patients suffering from lower lumbar tuberculosis were treated with this technique.[Results]The surgical average time and amount of bleeding was 4.3 h and 438 ml respectively.The length of anterior incision was 5~7 cm.The average time and the amount of bleeding were 21 min and 48 ml respectively during the anterior lesion exposure.The follow-up period were 12~37 months,mean 17.3 months.The average preoperative,immediate postoperative,and final follow-up local lordotic angles of the affected segments were-2.1??4.0?,4.9??1.4?,3.7??3.0?.All patients had healed of disease and there was no recurrence at final follow up.Bony fusion was obtained in all patients.[Conclusion]Anterior exposure through the lateral edge of the rectus abdominal muscle and retroperitoneal approach is simple and less invasive.The spinal stability is achieved through short-segment pedicle screw fixation while reducing the sacrifice of additional motion segments.This technique is safe and effective for the treatment of tuberculosis of the lower lumbar spine.
3.Clinical Value of ~(18)F-FDG PET-CT Fusion Imaging in Hepatocellular Carcinoma after Transcatheter Arterial Chemoembolization with Lipiodol

Objective To discuss the clinical value of ~(18)F-FDG PET-CT fusion imaging in hepatocellular carcinoma after transcatheter arterial chemoembolizationTACE)with lipiodol. Methods 15 cases of hepatocellular carcinoma diameter 4~10 cm) were investigated with ~(18)F-FDG PET-CT imaging within three months after TACE. The findings of imaging were compared with the results of anteriography and clinical follow-up.Results After TACE, regions of absent~(18)F-FDG uptakes could be found in all 15 patients. Increased ~(18)F-FDG uptakes could be found in liver of 11 cases. The other 4 patients had no increased~(18)F-FDG uptakes in liver. The regions of increased ~(18)F-FDG uptakes were residual viable tumor confirmed by anteriography and clinical follow-up , and most of them locate around region of absent ~(18) F-FDG uptakes.~(18)F-FDG PET-CT fusion imaging showed that there were no correlation between increased ~(18)F-FDG uptakes and lipiodol dense distribution. After another TACE given or radiotherapy based on ~(18)F-FDG PET-CT fusion imaging, the areas of increased ~(18)F-FDG uptakes reduced or disappeared. In 4 patients without Increased ~(18)F-FDG uptakes ,DSA showed that there were still viable tumor in the peripheral zone of tumor.Conclusion Residual viable tumor can still be found in patients with hepatocellular carcinoma after TACE.~(18)F-FDG PET-CT imaging can characterize and locate the most residual viable tumor, monitor response and be a guide in following therapy, but some viable tumor can not be found by ~(18)F-FDG PET-CT imaging.
4.Observation of discoloration of chemical indicator cards in different positions in pressure steam sterilization bags and sterilization effect

ObjectiveTo observe the discoloration of chemical indicator cards in different positions in pressure steam sterilization bags and stetilization effect.MethodsFrom January 2009 to January 2011,168 packages needing sterilization were divided into group A and group B randomiy with 84 packages in each.We put chemical indicator cards on different positions in the packages.Then discoloration of chemical indicator cards and the qualified rate of bacteriology detection were observed.ResultsThere were 9cases of uneven discoloration in group A,and the incidence rate was 10.71%,higher than group B.The unqualified rate of bacteriology detection in group A was 2.38%,higher than group B(1.19%),but the difference was not statistically significant.ConclusionsChemical indicator cards could reflect the effect of sterilization,but placing them in different positions can appear different results.The chemical indicator cards that placed between the two packing layer could provide powerful evidence for sterilization effects for clinicians,and avoid unnecessary economic waste in Sterilization and Supply Center,which is worthy of being widely used in Sterilization and Supply Center.
5.Prevalence of benign prostatic hyperplasia in Beijing: a multicentre community-based cross-sectional survey

Objective To investigate the prevalence of benign prostatic hyperplasia (BPH) in Beijing. Methods A cross-sectional study was performed at Beijing communities in men aged over 50 years. The International Prostate Symptom Score (IPSS) questionnaire was used to evaluate the symptoms. Transabdominal ultrasonography was used to measure prostate size and the residual urine volume(RUV). Uroflowmetry was performed to record the maximum flow(Q_(max)). One-way ANOVA was used to analysis the differences. Results 1644 subjects were enrolled and the mean age was 64. 5 years (range 50 - 93 years). The mean IPSS, prostate weight andQ_(max) were 9. 9±8. 2, (30. 8± 19.4) and (14. 7 ± 7. 5)ml/s, respectively. They had correlation with age (r=0. 388,0. 262, - 0.371). The prevalence of moderate-severe lower urinary tract symptoms (LUTS) was 50. 8% (835/1644). Prostate volume was greater than 20 ml in 69. 7%(1146/1644) of the subjects.Q_(max) was less than 15 ml/s in 53. 8% (883/1642) of the subjects. Conclusions In men age over 50 years in Beijing, there is positive correlation between LUTS, prostate size and age, and negative correlation between Q_(max) and age. The prevalence of BPH, defined as IPSS greater than 7, maxium flow rate less than 15 ml/s and prostate size greater than 20 ml, is 26. 8%(441/1644).

7. Observation of xenoplanted VX2 tumors in rabbits by 18F-FDG PET/CT imaging

Objective: To study the changes in metabolism of (18F-fluoro-2-deoxyglucose) by positron emission tomography over computed tomography (PET/CT) imaging at different time points after inoculation of VX2 tumor cells into rabbits and determine the difference in FDG metablism between various inoculation regions. Methods: VX2 tumor cells were inoculated into rabbits. PET/CT imaging was performed at d 14 and d 19 postinoculation. The tumor growth and SUV (standard uptake value) changes were recorded. Tumor volume was defined in CT images. SUVmax was used in the calculation of retention index (RI). The distribution of 18F-FDG was observed in tumor samples and pathological examination was performed. The correlations of tumor volume and SUV value at various time points was analyzed. Results: Continuous dynamic PET/CT imaging found that VX2 tumor grew rapidly and the volume of tumor body gradually increased with the prolongation of postinoculation time. A linear increase in SUV was observed when the postinoculation time was prolonged. The tumor volume and SUV value had significant correlation with postinoculation time (r = 0.89, r = 0.93, P < 0.05). However, the RI had no significant difference at various postinoculation time points (P > 0.05). SUV value was not associated with tumor volume at different time points (P > 0.05). Pathological examination validated that the tissues with accumulation of 18-FDG were enriched with tumor cells. Conclusion: VX2 xenografted model in rabbit is an ideal animal model for PET/CT imaging. Similar tumor growth and invasion style are observed regardless of different postinoculation days and body part. The efficacy of radiochemothery should be mornitored by PET/CT imaging in the period when the metablism of 18-FDG increases in linear.
8. Pretreatment of donor liver with salvianolate reduces biliary injury in rats after liver transplantation

Objective: To evaluate the effect of pretreating donor liver with salvianolate on biliary injury after orthotopic liver transplantation (OLT) in rats, and to discuss the underlying mechanisms. Methods: According to the preservation solutions of the liver grafts, the recipients rats were allocated in three groups: UW solution group, salvianolate group (UW solution + 0.4 mg/ml salvianolate), and saline group (UW solution + saline). In experiment 1, the recipient rats underwent arterialized OLT with biliary extra-drainage, and the bile flow and bile salt secretion were recorded at 1, 6, and 24 h post-transplantation. In experiment 2, the recipient rats underwent arterialized OLT, and the serum alkaline phosphatase, bile salt, total bilirubin, and the incidence of biliary complications were examined at 2 weeks post- transplantation. Meanwhile, the mRNA and protein expression of bile salt export pump (BSEP) in the liver samples were also detected using real-time RT-PCR and Western blotting analysis, respectively. Results: Compared with the UW solution and saline groups, salvianolate group exhibited significantly increased bile flow and bile salt secretion at 1, 6, and 24 h post-transplantation (P<0. 05). At 2 weeks post-transplantation, salvianolate group had significantly lower serum alkaline phosphatase, bile salt, total bilirubin(P<0. 05), and incidence of biliary complications(P<0. 01), and significantly higher expression of BSEP mRNA and protein compared with the other two groups (P<0. 05). Conclusion: Salvianolate addition in UW preservation solution can ameliorate biliary injury and reduce the incidence of biliary complications after OLT in rats, which might be associated with increased expression of BSEP in liver graft.
9. Salvianolate negatively regulates maturation and immunologic functions of mouse bone marrow-derived dendritic cells

Objective: To observe the effect of salvianolate on the maturation and immunologic functions of mouse bone marrow-derived dendritic cells (BMDCs). Methods: Mouse BMDCs were cultured with GM-CSF and 1L-4 for 5 days to obtain immature DCs (imDCs),then the imDCs were cultured in the presence of LPS (1 μg/ml) or LPS (1 μg/ml) plus salvianolate (50, 100 and 200 μg/ml) for 48 h to obtain DCLPS , SVL , SVM and SV H. The immunophenotypes and antigen phagocytosis capabilities of imDCs, DCLPS, SVL, SVM and SVH were analyzed by flow cytometry and FITC-Dextran intake. The IL-12 p40 contents in the supernatants were determined by enzyme linked immunosorbent assay (ELISA). The stimulatory activities of imDCs, DCLPS, SVL ,SV M and SVH on allogeneic T cells in mixed lymphocyte reactions (MLR) were tested by incorporation of 3H-TdR; the contents of IL-10,IL-2 and IFN-γ in the supernatants of MLR were examined by ELISA. Results: Compared with DCLPS,SVL,SVM and SVH had lower expression of MHC-II, CD40,CD80 and CD86 molecules, secreted less IL-12 p40 in the supernatant, and exhibited stronger antigen phagocytosis capability; moreover, SVL ,SVM and SV H showed significantly weaker activity in stimulating proliferation of allogeneic T cells(P<0.05,0.01). Compared with DCLPS, SV L, SVM and SVH groups had significantly decreased IL-2 and INF-γ levels in MLR supernatants(P<0.05), but there was no significant difference in the secretion of IL-10. Conclusion: Salvianolate (50-200 μg/ml) can inhibit the maturation of mouse BMDCs in vitro, and thus has a negative effect on immunologic function of BMCDs.
10.Percutaneous Fixation of Distal Tibia Comminuted Fracture by Using Normal Anatomical Plate

Objective To explore efficacy of minimally invasive percutaneous plate osteosynthesis (MIPPO) for the treatment of distal tibia comminuted fracture by using normal anatomical plate. Methods Between January 2007 and July 2008,18 cases of distal tibia facture were treated by MIPPO using anatomical plate. The clinical data of the patients were reviewed. Results A mean of 6.5 cm incision (5.0-8.5 cm) was made in the patients; the intraoperative blood loss ranged from 60 to 300 ml (mean,145 ml);and the operation time ranged from 30 to 120 min with a mean of 63 min. After the surgery,2 patients developed mild skin necrosis,and was then cured by conventional therapy; no patient had nonunion of the fracture,failure of internal fixation,or delayed wound healing. The 18 cases were followed up for a mean of 8 months (range,4 to 10 months); all of them were healed clinically and could walk without crutch in 4 months postoperatively. According to Johner-Wruhs score system,11 were excellent and 7 were good; the excellent-good rate was 100%. Conclusions MIPPO with anatomical plate is an optimal treatment for distal tibia comminuted fracture with advantages in protecting the soft issues and bony blood supply,promoting the wound-healing process,and reducing the rate of complications.
