1.Comparison of Two Quantitative Assays for Determination of Rh-anti-CD20 zumab and Their Application to Pharmacokinetic Study

An enzyme linked immunosorbent assay ( ELISA ) and a flow cytometry assay ( FCA ) based on Wil2-S cells were developed and systematically compared for quantification of recombinant anti-CD20 humanized monoclonal antibody ( rh-anti-CD20zumab) in biological matrix. The specificity, precision and accuracy of each method at correspondingly different linear range showed good results. For ELISA, the precisions of intra-day and inter-day were both <19 . 5%, the relative error was from-18 . 2% to 17 . 6%;For FCA, the precisions of intra-day and inter-day were both <19. 0%, the relative error was from -18. 9% to 18. 4%. The sensitivity of ELISA was significantly higher than that of FCA. The quantitative ranges of ELISA and FCA methods were 0. 04-5. 0 mg/L and 3. 1-200 mg/L, respectively. The concentrations in serum samples and pharmacokinetics analysis were determined by both of two methods after vein drip administration of rh-anti-CD20zumab in rhesus monkeys. Pharmacokinetics data showed that there was excellent consistency between results obtained by two methods at the given dose. We believe that the novel FCA with high speed and high sensitivity can be used to perform PK and PD study of cell surface antigen-targeted antibody derivatives.
2.Efficacy of cold preservation solution containing desferrioxamine in protecting rat hearts

Objective To evaluate the efficacy of cold preservation solution containing desferrioxamine (DFO) in protecting the rat hearts.Methods Pathogen-free male Sprague-Dawley rats,weighing 300-350 g,were used in the study.Thirty-two isolated rat hearts were equally and randomly divided into control group (C group) and DFO group using a random number table.Hearts of rats were stored for 6 h in 4 ℃ histidine-tryptophan-ketoglutarate (HTK) solution in group C.DFO was added to HTK solution (DFO concentration 100 μmol/L),and hearts of rats were stored for 6 h in 4 ℃ HTK solution in group DFO.At 6 h of cold storage,creatine kinase and lactate dehydrogenase activities in the cold preservation solution were determined.Myocardial specimens were obtained from the apex,cut into sections which were stained with haematoxylin and eosin,and examined under a microscope.The pathological changes of myocardial tissues were scored.The content of malondialdehydc in myocardial tissues was determined using thiobarbitnric acid method,and hypoxia-inducible factor-1α protein and mRNA expression in myocardial tissues was detected by real-time reverse transcriptase polymerase chain reaction and Western blot.Results Compared with group C,the creatine kinase and lactate dehydrogenase activities in the cold preservation solution,malondialdehyde content in myocardial tissues,and pathological scores were significantly decreased,and the expression of hypoxia-inducible factor-1α protein and mRNA expression in myocardial tissues was significantly up-regulated in group DFO (P< 0.05).Conclusion Cold preservation solution containing DFO can protect the rat hearts effectively,and the mechanism is related to up-regulation of the expression of hypoxia-inducible factor-1α.
3.Combined lung tumor resection and heart surgery

Objective To assess the safety of combined lung resection and cardiac surgery in patients with synchronously occurring lung tumor and heart disease. Methods Fourteen patients with synchronously lung tumor and heart disease received lung resection combined with open heart surgery between 2003 and 2008 in our hospital. There were 11 men and 3 women with a mean age of 64 years. Lung resection consisted of lobectomy in 8 patients, sleeve lobectomy in 1, and wedge resection in 5.Pulmonary tumor pathology was squamous cell in 4 patients, adenocarcinoma in 6, undifferentiated carcinoma in 1, hamartoma in 2 and sclerosing hemangioma in 1. Cardiac procedure included coronary artery bypass grafting (off-pump) in 12 cases, mitral valve replacement in 1 and valve repair in 1. Results No patient died or needed to do re-exploration for bleeding. One patient developed atrial fibrillation postoperatively and recovered soon. One patient developed pneumonia and ARDS who bad to receive tracheostomy but finally he discharged from hospital one month later after the operation. Conclusion Combined lung tumor resection and cardiac surgery are relatively safe in selected patients, especially concomitantly with off pump coronary artery bypass grafting. Lung resection and system medinstinal lymph node dissection for lung canner through second incision had low and acceptable operative morbidity.
4.An Outbreak of Seasonal Influenza Viruses A(H1N1) in Changsha Was Diagnosed by Laboratory and the HA Gene Characteristic Was Analyzed

To determine the etiologic agent of an outbreak of influenza viruses from Changsha Foothill Mountain International School in 2009, and to analyze the HA Gene Characteristic of the H1N1 influenza viruses. Twenty-five nasopharyngeal swab specimens from the outbreak of influenza viruses were tested by conventional RT-PCR and influenza viruses isolated simultaneously. Virus isolated (A/Yuelu/314/2009) from the outbreak was sequenced by CEQTM 8000 Genetic Analysis System and the sequencing results submitted to GenBank (Accession No: FJ912843), then the sequencing data was analyzed by ClustalX and Mega4.1softwares. Results showed the influenza viruses A(H1N1) of positive were 18 cases by influenza viruses isolated tests and 21 cases by conventional RT-PCR, respectively. The nucleotide and amino acid sequence homology of the HA gene of A/Yuelu/314/2009 are 99% compare with the vaccine strain (A/Brisbane/59/2007) in 2008~2009 years. The HA sequence data also showed that had 6 amino acid mutations (V148A, S158N, G202A, I203D, A206T, W435R), and the S158N located at antigenic site B of HA protein. Nine potential glycosylation sites (27, 28, 40, 71, 151, 176, 303, 497, 536) in the HA sequence of A/Yuelu/314/2009 is the same with A/Brisbane/59/2007, and the sequences of potential glycosylation sites were conserved. In this study, laboratory evidence diagnosed seasonal influenza A virus (H1N1) as the etiologic agent of the outbreak. The virus isolated (A/Yuelu/314/2009) strain of H1N1 subtype is not a new variant in Changsha in 2009 compare with the vaccine strain (A/Brisbane/59/2007), the outbreak of influenza A virus (H1N1) from Changsha Foothill Mountain International School maybe are caused by the change in genetic characteristics between vaccine strains and the decreased of immunity to influenza A virus (H1N1) in the crowd.
5.Meta-analysis for the clinical efficacy of Kangai injection combined with transcatheter arterial chemoembolization for treatment of hepatic cancer

Objective To evaluate the clinical effect of Kangai injection combined with transcatheter arterial chemoembolization (TACE) in the treatment of hepatic cancer.Methods We searched CBM,CNKI,WanFang and VIP,clinical studies of Kangai injection combined with TACE in the treatment of hepatic cancer were included and compared.The methodological quality of included studies was assessed and Meta analysis was performed by Rev Man 5.2 software.Results 9 studies involving 595 patients were included.Meta analysis results indicated that the difference of effective rate was significant [RR =1.32,95 % CI(1.10,1.58),P =0.003] ; The quality of life improvement rate also had significant difference[RR =1.85,95% CI(1.42,2.41),P < 0.000 01].Conclusion Kangai injection combined with TACE in the treatment of hepatic cancer can get extra benefits compared with TACE alone.It can improve the curative effect,significantly improve the quality of life.
6.Effect of small molecule hydrogels on proliferation, apoptosis and myocardial differentiation of bone marrow mesenchymal stem cells

BACKGROUND:A short-peptide small molecule hydrogel (SMH) developed in the previous study has more obvious advantages than other hydrogels to improve local microenvironment, carry bioactive substances and interfere with stem cell signal transduction pathways. OBJECTIVE:To explore the effect of SMHs on bone marrow mesenchymal stem cells (BMSCs) proliferation, apoptosis and differentiation into myocardial cells. METHODS: (1) Passage 9 rat BMSCs in vitro were divided into control group and experimental group, followed by routine culture and culture in SMHs, respectively. At 7 days of culture, cell proliferation and apoptosis were detected. Cells in the two groups were exposed to anaerobic environment for 12 hours, and expression levels of Bcl-2, Bax and Caspase-3 in BMSCs were detected. (2) Passage 9 BMSCs were divided into four groups and then cultured in 5-azacytidine, SMHs, SMHs+5-azacytidine, and L-DMEM (normal control), respectively. After 4 weeks of induction, expression of CTnT, desmin and Cx-43 proteins was detected and expression levels of early cardiac transcription factors, NKX2.5 and GATA-4, were also measured. RESULTS AND CONCLUSION: (1) Compared with the control group, better proliferation and lower apoptosis of BMSCs were found in the experimental group. Under anaerobic conditions, the number of survival cells was reduced in both groups, but less apoptosis or necrosis was found in the experimental group than the control group (P < 0.05). Moreover, the level of Bcl-2 was higher in the experimental group than the control group (P < 0.01), while the levels of Bax and Caspases-3 protiens were lower in the experimental group than the control group (P < 0.01). (2) NKx2.5 and GATA-4 mRNA expression was found in both 5-azacytidine and SMHs+5-azacytidine groups, and moreover, the mRNA levels of early cardiac transcription factors were significantly higher in the SMHs+5-azacytidine group than in the 5-azacytidine group (P < 0.05). In the normal control group, cTnT expressed negatively, and desmin and Cx-43 expressed weakly. The expression of cTnT, desmin and Cx-43 proteins was higher in the SMHs+5-azacytidine group than in the 5-azacytidine and SMHs groups, while there was no significant difference between the latter two groups. To conclude, SMHs as a culture medium is conducive to the proliferation of BMSCs, reduces cell apoptosis, and promotes myocardial differentiation of BMSCs.
7.Application of Dexmedetomidine and Midazolam in Herniorrhaphy Surgery for Older Patients

Objective To compare the feasibility and efficacy between dexmedetomidine and midazolam in herniorrhaphy surgery for older patients. Methods Sixty American Society of Anesthesiology (ASA) gradeⅠ~Ⅱpatients, treated by herniorrhaphy surgery under local anesthesia,were randomly divided into dexmedetomidine group (n=30) and midazolam group (n=30) .Patients in dexmedetomidine group were given dexmedetomidine at a loading dose of 1μg/kg for 10 min,then they were injected continuously by 0.4μg/(kg·h),whereas midazolam group were given midazolam at a loading dose of 0.06 mg/kg, then 0.04 mg/(kg·h) injected continuously.The mean blood pressure (MAP) and heart rate (HR) were recorded before infusion (T0),incision of skin (T1),15min (T2) and 30 min (T3) after administration and when sutured skin (T4), adverse reaction were also assessed. Results The difference of sedation level was not significant between the two groups (P>0.05) .Compared with T0 , the decrease of HR was significantly more in dexmedetomidine group from T1 to T4 (P<0.05) . Compared with midazolam group , the decrease of HR was significantly more in dexmedetomidine group from T1 to T4 (P<0.05) . Compared with dexmedetomidine group, the rate of respiratory depression and restlessness were more in midazolam group, but bradycardia was lower (P<0.05) .Conclusions Dexmedetomidine is a comparable alternative to midazolam for sedation in herniorrhaphy surgery under local anesthesia. It is associated with better respiration and lower restlessness but with a high incidence of bradycardia.
8.Clinical Application of InstaTrak3500 Plus with Fluoro Trak Electric-magnetic Guided Imaging System

Objective To explore the clinical efficacy of lnstaTrak3500 Plus with Fluoro Trak magnetic imaging guidance for spine surgery, Methods The construction and operation progress of the system were applied in thoracic-lumbar spine surgery, Results The electric-magnetic guided imaging system can improve success rate in thoracic-lumbar spine surgery,and all operative pedicle screw were inserted accurately with no related complications error. Conclusion The electric-magnetic imaging system can improve the accuracy of operations and guide mini-invasive surgery, which can significantly decrease X-ray irradiation dosage.
9.Clinical observation on triple-therapy for terminating ectopic pregnancy.

OBJECTIVETo select a safe, quick and effective method for terminating ectopic pregnancy (EP) with few adverse effects.
METHODSPatients were divided into 2 groups. The observed group was treated with methotrexate (MTX, 50 mg/m2 for single dose intramuscular injection) plus RU 486 (600 mg taken orally in the morning with empty stomach, followed with fasting for 2 hrs) and Waiyun Zhuyu decoction (WZD, one dose a day for 7-10 days). The control group was only treated with MTX, with the same regimen as used in the observed group.
RESULTSThe curative rate, time for blood beta-HCG recovering, lump absorption time, and tube recanalization rate in the observed group were better than those in the control group (P < 0.01). CONCLUSION; The key links for successfully treating EP with conservative drug therpy are to diagnose the disease early and clearly, and to select indicative subject strictly. The scheme of single dose administration of MTX plus RU486 combined with WZD, with its high efficacy and few adverse reaction, may be used as the first choice for referential clinical drugs administration.

10.Clinical application of maxillary endossenous implant with edentulous ridge expansion technique.

OBJECTIVETo evaluate the application and the effect of edentulous ridge expansion(ERE) technique in maxillary endossenous implant placement.
METHODS49 patients with maxillary alveolar ridge atrophy received edentulous ridge expansion using condenser. In order to be similar to natural root, dental implants were selected and placed to tooth missed sites according to the requirements of aesthetics, function and dimension.
RESULTS49 patients with atrophied alveolar ridge received 86 implants. The labio-lingual width augmented from 3.3 to 5.4 mm and the alveolar ridge height from 2 to 7 mm 6 months after operation. The implants osseintergrated tightly with alveolar bone and second-step prosthesis was performed 6 months after implant placement.
CONCLUSIONThe edentulous ridge expansion technique can meet the requirements of aesthetics and function and is applicable to endossenous implant placement in maxilla. The method is simple and valuable to clinical application.
