1.Relationships between Job Satisfaction and Burnout Experience among Nephrology Nurses.

The purpose of this study was to determine relationships between job satisfaction and burnout experience. The subjects were 225 nephrology nurses in Pusan, and Kyung Sang Namdo and Kyung Sang Bukdo. The data were collected from Nov. 20 to Dec. 3, 1996 using questionnaires method. Job satisfaction measured job satisfaction tool by Slavitt et al, and burnout experience measured burnout experience scale by Pines et al. The questionnaire consisted of question regarding job satisfaction scale(44 items 5 point scale) and burnout experience scale(21 items 7 point scale). The reliability of this instrument was that the hob satisfaction was Cronbach's alpha=0.8298 and the burnout experience was Cronbach's alpha=0.8960. The data were analyzed with the SPSS program using mean, standard deviation, frequency and percentage, t-test, ANOVA and Pearson's Correlation Coefficient. The results of this study were as follows : 1. In the demosociographic characteristics showed the highest level was as follows : 26-30 years old group(40.2%), married(56.4%), graduated junior college of nursing(87.6%), non the religious(35.6%), the effect of religion upon life is not effected(35.6%). In the characteristics related to nursing profession showed the highest level was as follows : Hospital style is secondary hospital(that have above 450 beds) (53.3%), staff nurse(72.9%), the length of clinical experience at hemodialysis room is less than 2 years(39.1%), number of patient was assigned a nephrology nurses is 5(40.4%), work in two shift(55.6%), the nurses professional motivation is family recommended(33.8%), the nurses intention to stay is until for needed(58.2%), the chance for professional growth is not enough(44.9%), degree of satisfaction with nursing is moderate(43.2%). 2. The mean score of the total hob satisfaction is 3.06 of 5 point Likert-type scale. Task requirements(3.51) among the component factors of the job satisfaction was the highest value and then the interaction among fellow nurses(3.34), job prestige/status(3.33), autonomy(3.27), organizational requirement(2.55), and pay(2.39) was the lowest 3. The mean score of the total burnout experience is 3.20 of 7 point Likert-type scale. Physical exhaustion(3.36) among the component factors of the burnout experience was the highest value and then emotional exhaustion(3.20), and mental exhaustion(2.95) was the lowest. 4. Job satisfaction according to demosociographic characteristics of the subjects showed significant differences in the effect of religion upon one's life(F=3.268, p=0.013). Job satisfaction according to characteristics related to nursing profession of the subjects showed significant differences in the hospital type(F=3.479, p=0.033), position(F=3.165, p=0.044), number of patient was assigned a nephrology nurses(F=2.552, p=0.040), nurses intention to stay(F=7.153, p=0.001), the chance for professional nursing growth(F=3.735, p=0.006), the degree of satisfaction with nursing(F=12.680,p=0.000). Burnout experience according to characteristics related to nursing profession of the subjects showed significant differences in the position(F=3.247, p=0.041), number of patient was assigned nephrology nurses(F=4.220, p=0.003), shift(F=3.148, p=0.045), nurses intention to stay(F=9.911, p=0.000), the degree of satisfaction with nursing(F=13.234, p=0.000). 5. Job satisfaction and burnout experience was signigicant negative correlation(r=-.5466, p<.001).

2.A Case of Sudden Hypotension Following Intraoperative Intercostal Nerve Block.

Postoperative pain may be associated with shallow breathing, inability to cough, and reduction in spirometric values which lead to restrictive pattern of ventilation with hypoxemia and/or hypercarbia. Therefore, postoperative pain should be managed with appropriate methods. The authors have usually performed intraoperative intercostal nerve block with 0.25% bupivacaine hydrochloride to alleviate postoperative thoracotomy pain. A 21 years old male patient developed sudden hypotension and severe bradycardia 1 minute following the intraoperative 4th, 5th, 6th intercostal nerve block with a total of 10 ml of 0.25% bupivacaine hydrochloride, who was treated by ephedrine with success. The authors discuss the possible causes of sudden hypotension and severe bradycardia in detail.

4.Is There a Role of RigiScan(R) in the Measurement of Rigidity after Intracorporeal Injection of Prostaglandin E1?.

PURPOSE: To evaluate whether radial rigidity measured using RigiScan(R) represents the intracorporeal pressure effectively. MATERIALS AND METHODS: From January 1998 to May 1999, total of 23 patients with erectile dysfunction were evaluated by RigiScan(R) and duplex ultrasonography after the intracorporeal injection of prostaglandin E1. Peak systolic velocity and end diastolic velocity were measured by duplex ultrasonography and then the resistance index was calculated as (peak systolic velocity-end diastolic velocity)/(peak systolic velocity). Radial rigidity of penile tip and base was measured by RigiScanR . The results were analyzed statistically by PC-SPSS version 7.5. RESULTS: There were statistically significant correlations between radial rigidity of penile tip and base and the resistance index by Spearman's correlation analysis, respectively (r=0.680, p<0.001)(r=0.703, p<0.001). When radial rigidity of penile tip and base exceeded 60% of maximum, radial rigidity of penile tip and base again correlated well with the resistance index, respectively (r=0.659, p=0.020)(r=0.759, p=0.011). Based on clinically determined degree of erection, radial rigidity of penile tip and base represented the intracorporeal pressure effectively. CONCLUSIONS: Radial rigidity measured by RigiScan(R) represents the intracorporeal pressure effectively.

5.Ocular injudes, analytical view of 340 cases.

The authors analysed 340 cases of the ocular injuries among 7633 out and in-patients who visited to the clinic of P.N.U. hospital during 5 years, from Jan. 1964 to Dec. 1968. Followings were discussed. 1. We found that 4.6% of all cases attending our eyes clinic was victims of ocular injuries. 2. The sex incidence showed that 78.9% of such ocular injuries occured in male and 21.1% in female. 3. The incidence was prevalent in young adult 21-30 years of age, and their large number of cases were injured from blowing and industrial accidents, while in children under 10 years of age sharp-pointed toys were more frequent causes. 4. The subconjunctival hemorrhage was the most frequent disturbance of ocular injuries and then came traumatic cataract and corneal foreign bodies in that order. 5. The prognosis of ocular injuries was generally poor, especially, poor, especially in the perforating and penetrating wound of the whether the foreign body was retained or not.

6.A case of preoperative angiographic uterine artery embolization for the conservative treatment of cervical pregnancy.

A 35 year old woman, gravid 5, multiparous, was admitted to our department at 6 weeks and 5 days of gestation after being diagnosed with cervical pregnancy. Before the evacuation, the uterine arteries were embolized using angiographic techniques. As a result, dilatation and curettage were performed with minimal hemorrhage. We report one case of cervical pregnancy managed, successfully with preoperative uterine artery embolization and evacuation.

8.Hailey-Hailey Disease with a Family Histroy and Unique Nail Lesions.

Hailey-Hailey disease(benign familial chronic pemphigus) is a rare autosomal dominant disorder characterized by blisters at sites of friction such as the neck, axillae and groin which are caused by suprabasal epidermal acantholysis. We report two cases of Hailey-Hailey disease in the one family. One of the two cases has asymptomatic multiple longitudinal white bands in the fingernails associated with typical skin lesions. The nail lesions have not been described until reported by Burge in 1992 and it may be a characteristic finding in Hailey-Hailey disease.

9.A Case of Diktyoma.

A 12 years old Korean girl was seen on 10 Oct. 1969 with chief complaint of a large fleshy mass extruded out of the orbit associated with ocular pain of mild degree for several months. Her parents were first noticed that the girl's right eye had a peculiar reflex from the pupillary zone in 2 years of her age. She was advised by a local clinician to correct surgicaIly someday later in her childhood. At the age of four, the pupillary margin of the affected eye had become distorted and they became aware of the eyeball was a little protruded and the grade of protrusion had slowly advanced as she had grown up. About a year ago the patient suffered from sudden attack of severe ocular pain associated with vomiting. Having experienced the recurrences of similar attack several times, she at last in Sept. 1969 met a spontaneous outburst of the extruded mass from which the purulent and bloody contents flowed out. It was to be regretted that the history could not reveal when and how the globe had been disorganized as completely as such that no trace of configuration of the anterior segment could be recognized. Familial and past history were not contributory except measle with good recovery at 2 years of age. On examination, the left eye had good vision and no structural changes, wherease the right side showed a marked extrurusion of fleshy mass out of the lid aperture. The mass had a relatively smooth and congested coverage of the conjunctiva except it had the fistulae through which a small amount of the purulent and bloody discharge flowed out under the digital pressure. It was felt somewhat elastic and movable with a broad base into the orbit. On its surface no trace of the cornea or the other global structures could be found. Physical examinations of the whole body including regional lymphnodes revealed normal X-ray Was of chest, skuIl and optic foramina were normal. On 14 Oct. 1969 under general anesthesia an orbital exentration was undertaken because of either probability of a malignant tumor or a long standing intraocular inflammation. In the way of procedure necrotic contents bursted out by mechanical pressure through the weak point, i.e. the fistulae, and correspondingly the tumorous mass became softened. Confirming no hard mass palpable into the orbit, the operating procedure shifted to do partial exentration. The extirpated mass was subjected to the histological study. Grossly, the dissected area revealed that it contained a well preserved structure of sclera on the one side and completly necrotised material on the other, but it did not contain any remnants of the ordinary global structures such as cornea, iris, lens, vitreous and choroid. The hollowed space partially surrounded by scleral structure was filled with the dark and gelatinous materia. The mass was as a whole rather soft Histological examination showed numerous gland like proliferations composed of poorly differentiated and unpigment columner cells having relatively large nuclei, some of which showed a mitotic figure. These were conceivable to be of an embryonic retina. The lumina of the gland like structures with the various sizes and shapes were filled with a pink staining coagulum and an extremly delicate and regularly arranged fibrillar structure. In some parts the tumor consisted of solid cellular conglomeration with rosett like arrangement and in the other necrotic masses containing tumor cells in all stages of degeneration. For the most part and the surrounding structure of the tumor cells, the picture showed likely to be a diffuse inflammation with an infiltration of polymorphonuclear cells, partly vessels being mostly of capillaries and recent hemorrhages, but the cartilage was not found.

10.Symptom Management to Predict Quality of Life in Patients with Heart Failure: A Structural Equation Modeling Approach.

PURPOSE: The focus of this study was on symptom management to predict quality of life among individuals with heart failure. The theoretical model was constructed based on situation-specific theory of heart failure self-care and literature review. METHODS: For participants, 241 outpatients at a university hospital were invited to the study from May 19 to July 30, 2014. Data were collected with structured questionnaires and analyzed using SPSSWIN and AMOS 20.0. RESULTS: The goodness of fit index for the hypothetical model was .93, incremental fit index, .90, and comparative fit index, .90. As the outcomes satisfied the recommended level, the hypothetical model appeared to fit the data. Seven of the eight hypotheses selected for the hypothetical model were statistically significant. The predictors of symptom management, symptom management confidence and social support together explained 32% of the variance in quality of life. The 28% of variance in symptom management was explained by symptom recognition, heart failure knowledge and symptom management confidence. The 4% of variance in symptom management confidence was explained by social support. CONCLUSION: The hypothetical model of this study was confirmed to be adequate in explaining and predicting quality of life among patients with heart failure through symptom management. Effective strategies to improve quality of life among patients with heart failure should focus on symptom management. Symptom management can be enhanced by providing educational programs, encouraging social support and confidence, consequently improving quality of life among this population.
