Juvenile systemic sclerosis (SSc) is a chronic connective tissue disease characterized by symmetrical thickening and hardening of the skin,associated with fibrous changes in internal organs.In the last years,several therapies have showed promising beneficial effects in the treatment of SSc.We summarized the recent advances in the anti-inflammation and immunosuppressive therapy,treatment of SSc related vascular diseases,anti-fibrosis therapy as well as prospective treatments.
4.Effect of osteoprotegerin on osteoclasts

Objective To analyse the effects of OPG (osteoprotegerin, OPG) at different concentrations on activity of osteoclasts (osteoclasts, OC) , and the relationship between them in physiological activity and function.Methods The bone marrow cells of six weeks old female mice were collected and cultured in vitro on the basis of adding different levels of OPG(10, 20, 50, 100 μg/L).The relationship between OPG and OC were analysed by tartrate resistant acid phosphatase ( TRAP) staining, F-actin cytoskeleton staining and bone resorption pits.Results OC showed adherent growth within 1h cultured, and there were no large multicore OC.Cultured for 5 days, the smaller monocytes gradually began to fusion with each other to form multinucleated cells and showed distinctive characteristics.After three days OPG treatment, the number of multicore cells gradually reduced with OPG concentration increaseing in experimental group, and showing a negative correlation between them(r=-0.516,P<0.05).And there were only a small amount of multinucleated cells at OPG concentration of 50 μg/L.At 20,50 μg/L of OPG concentration, OC number significantly decreased compared with control group (P<0.05), and when the concentration of OPG was 100μg/L, OC number showed significant reduction (P<0 01).the experimental group OPG bone resorption lacunae area, density and decrease the degree were positively correlated with concentration of OPG (r=0.459;r=0.426;r=0.389,P<0.05), and the area had a significant difference when two groups compared.Conclusion The inhibitive effects of OPG on OC function and activation increase with concentration increasing.

6.New insights into facial flushing and erythema in rosacea

Persistent centrofacial erythema with transient flushing is the most common clinical manifestation of rosacea. However, its pathological mechanisms are poorly understood, and there is a lack of high-level evidence from clinical studies. Based on research progress and clinical experience in China and other countries, the authors emphasize the necessity that facial flushing with erythema should be investigated as an independent clinical phenotype during the study. It is believed that mental stress, anxiety and/or depression may be driving factors in the pathogenesis of facial flushing with erythema. Based on mental factors, it is of potential clinical significance to investigate the relationship of neuroinflammation involved in facial flushing and erythema with skin immunity and blood vessels in rosacea.
8.Analysis of Electroneurophysiological Detection on Children with Progressive Muscular Dystrophy

Objective To explore the value of electroneurophysiological detection in diagnosis of children progressive muscular dystrophy (PMD).Methods The clinical features and laboratory data were analyzed in 32 children with PMD ,and electromyography(EMG) and nerve conduction velocity(NCV) were performed.Parameters studied included spontaneous activity , duration and amplitude of motor unit potential(MUP),pattern of recruitment as strong contracting,sensory conduction velocity(SCV),motor conduction velocity(MCV), distal latency and distal amplitude. 9.Use and short-term outcomes of intra-aortic balloon counterpulsation in acute myocardial infarction

Objective To examine current use, associated complications, and short-term outcomes in patients with acute myocardial infarction (AMI) requiring intra-aortic balloon pump (IABP) counterpulsation. Methods A single-centre registry was conducted between October 2004 and July 2008. Data were collected from 121 consecutive AMI patients treated with IABP counterpulsation at the 28th Division of Beijing Anzhen Hospital. Results Among 806 patients with AMI, IABP counterpulsation was employed in 121 patients. Placement of an IABP was indicated for hemodynamic support during high-risk catheterization and/or angioplasty (66.9%) and cardiogenic shock (27.3%). Balloon insertions were successful in 98.3% of patients and duration of mean use was 34.2?16.2 hours. No major IABP complications occurred in all the patients. Diagnostic catheterization was performed in all the patients inserted with IABP, and 93.4% underwent coronary revascularization before hospital discharge. The total in-hospital mortality was 13.2% (34.4% in patients with cardiogenic shock) and varied markedly by indication and use of revascularization procedures. Conclusion IABP counterpulsation may be successfully employed for a wide variety of conditions in the AMI setting, providing significant hemodynamic support with rare major complications in a high-risk patient population. IABP counterpulsation may reduce mortality in patients with AMI undergoing reperfusion therapy.
10.Biological significance of glucocorticoid receptor beta

Alternative splicing of the human glucocorticoid receptor (hGR) primary transcript produces two receptor isoforms, termed hGRα and hGRβ. hGRα is a ligand-activated transcription factor which, in the hormone-bound state, modulates the expression of glucocorticoid-responsive genes by binding to specific glucocorticoid response element (GRE) DNA sequences. In contrast, hGRβ dose not bind glucocorticoids and is transcriptionally inactive. We demonstrate here that hGRβ inhibits the hormone-induced, hGRα-mediated stimulations of gene expression, including glucocorticoid-responsive reporter gene (cat) and endogenous p21 gene. We also demonstrate that hGRβ can inhibit hGRα-mediated regulation of proliferation and differentiation of a human osteosarcoma cell line (HOS-8603). Our studies on the expression of hGR mRNA in nephrotic syndrome patients indicate that the hGRα/hGRβ mRNA ratio in peripheral white blood cell of hormone-resistant patients is lower than that of hormone-sensitive patients and health volunteers. These results indicate that hGRβ may be a physiologically and pathophysiologically relevant endogenous inhibitor of hGRα
