4.Two Histologic Variants of Giant Cell Carcinoma of the Pancreas.

Two cases of unusual giant carcinoma of the pancreas are presented. Each presented with a large tumor mass with or without cystic change, either in the head or tail portion of the pancreas. A pleomorphic gaint cell carcinoma variant (case 1) differed from an epulis type (case 2) with nuclear atypism of giant cells, but widespread metastasis in both cases reflected the accountability of spindle cell element as an important parameter for the determination of their biological behavior.

5.Displacement of a lower third molar into the lateral pharyngeal space.

The displacement of an entire tooth into the adjacent anatomical area is an uncommon complication of a tooth extraction. We encountered a 23-year-old woman who had previously undergone surgery under local anesthesia to remove the lower third molar about 12 weeks prior and the upper third molar was extracted 2 days prior to visiting this hospital. Upon admission, she complained of a swallowing discomfort and a mouth opening limitation. Panoramic radiograph and a CT scan revealed a displacement of the entire tooth into the lateral pharyngeal space. The tooth was retrieved via the transoral approach under general anesthesia. The removed tooth had an indentation formed by a dental bur. Therefore, it was concluded that the tooth displaced into the lateral pharyngeal space was the lower third molar. This report describes an unusual case of a third molar that was displaced into the lateral pharyngeal space with a review of the relevant literature.

7.Characteristics of MMPI Finding in Low Back Pain Patients.

OBJECTIVE: The goal of this study was to examine whether MMPI scores could discriminate between normal group and exaggerated pain group and predict treatment prognosis in exaggerated pain group. METHODS: For psychological evaluation, MMPI was administered to 37 low back pain patients and 36 normal controls. MMPI findings in two groups were compared, using t-test and discriminant function analysis. Nineteen patients of exaggerated low back pain group contacted by telephone were questioned about treatment satisfaction degree. And satisfaction group(9 patients)nd dissatisfaction group(6 patients)s MMPI findings were compared. RESULT: Hy, Hs, D scale scores in exaggerated pain group were higher than in control group. In discriminant function analysis by Hs, Hy, and D, percent of correctly classified 'gouped' cases was 74%. Discriminant function of Hy scale was the highest and that of D scale was the lowest. In satisfaction degree study, no finding was significant. CONCLUSION: MMPI findings were effective in discriminating between exaggerated pain group and normal control group. Further researches will need more structured and comprehensive approach.

8.Prevalence and Risk Factors of Orthostatic Hypotension among the Community-Dwelling Aged.

PURPOSE: This study was to identify the prevalence of orthostatic hypotension and its association with risk factors of orthostatic hypotension aged over 60 in Seoul and Chungju, Korea. METHOD: The data were collected from the 22th of August, 2000 to the 7th May 2001. The participants were 74 community-dwelling aged who could stand up from sitting position without assistance. Subjects were interviewed with structured questionnaire in order to ask experience of previous falls, hours in per day, symptoms related orthostatic hypotension and demographic characteristics. Orthostatic hypotension was assessed at 1 minute after the subjects standing from sitting position and defined as 20mmHg or greater decrease in systolic blood pressure after standing. RESULT: The prevalence of orthostatic hypotension was 17.1%. The mean drop of systolic blood pressure was 27.46mmHg among orthostatic hypotension subjects. The significant variables which explain the occurrence of orthostatic hypotension was the basal systolic blood pressure, the hit ratio of discriminant function with basal systolic blood pressure was 69.7%. CONCLUSION: Finding indicate that this study will contribute to develop nursing strategies to identify risk factors and to prevent orthostatic hypotension for the aged.
9.A Clinical Survey of 18,413 Cases of Anesthesia .

A total of 18,413 cases had anesthesia at Maryknoll Hospital in Pusan from January 1970 to December 1979. These data were analyzed statistically and the following results were obtained. 1) The number of general anesthesia cases steadily increased while those of local anesthesia decreased. 2) Of all departments, general surgery had the largest number of cases. 3) Sex ratio was equal when the OB-GYN cases were included. 4) The 20-30yr age group numbered nearly the total number of cases. 5) Geriatric and pediatric cases increased progressively. 6) Among anesthetic agents used, ether was used mostly, but halothane was used with increasing frequency. 7) Of the anesthetic techniques used in general anesthesia, the circle system was mostly used and the non-rebreathing system was used for all pediatric cases, which also have increased in number. 8) For induction of anesthesia, thiopental sodium was mainly used. Since 1975, pancuronium was mainly used in addition to S.C.C. fro muscle relaxation.

10.Two Case Report on Anesthesia for Pheochromocytoma .

The following is a report of the anesthetic experience in the surgical management of two cases of surgery for pheochromocytoma performed at Maryknoll Hospital, Busan, between the dates of June and July, 1982, The report covers the pre-surgical tests and the laboratory results on the two cases, which was similar, and the anesthetic agents halothane and enflurane, was used on the cases, respectively. During surgery using the anesthetic agent halothane, which was chosen because of the high blood pressure of the patient, and our effect to control it, we encountered dangerous arrythmia. In the case where enflurane was the anesthetic agent uaed, it was found that while the blood pressure of the patient was more difficult to control, the incidence of arrythmias was much less frequent. In one of the cases, becsuse of hemorrhage from the operative site during the immediate surgical period, re-exploration was done with adequate replacement of whole blood and catecholamines administered as needed during the anesthetic management. In comparing the two cases, the one case in which alpha-sdrenergic blockers were not administered during the pre-surgical period, when compared with the case in which the the blockers were used, showed, after removal ef the tumor, a decrease in the need for alpha-adrenergic stimulators.
