The Purpose of this study was to obtain the basic data to identify effective nursing methods for intravenous therapy patients by means of investigating the incidence of phlebitis and identifying the risk factors for intravenous-related phlebitis which cared by the IV team nurses. The data collected from August, 30, 1995 to October, 2, 1995. The subjects of this study were 495 IV catheters of 270 patients hospitalized to the 3 internal medicine and the 3 surgical wards of large general hospital who received the continuous IV therapy. In order to investigate the incidence of phlebitis, investigator developed the criteria for judging phlebitis and assessment records concerning the phlebitis. Catheter sites was inspected on a daily basis by IV team nurses, and development of phlebitis was graded and documented. The data were analyzed using the PC-SAS program : Percentages and Chi-Square test were used. The results were summarized as follows ; 1. Over-all rate of incidence of intravenous-related phlebitis was 8.7%(Grade III 8.5%, Grade IV 0.2%). 2. There was significant difference in the incidence of phlebitis by sex. It was 12.00% in female as compared to 6.44% in male. 3. There was no significant difference in the incidence of phlebitis by age. 4. There was significant difference in the incidence of phlebitis according to duration of catheter placement. The incidence of phlebitis was the highest within 24 hour after insertion. 5. There was significant difference in the incidence of phlebitis according to the location of the insertion site. Lower extremities were 22.22% as compared to 8.18% in upper extremities. But, there was no difference in the incidence of phlebitis according to the location of the insertion site among upper extremities catheters. 6. There was significant difference in the incidence of phlebitis by the size of cannula 22G was 11.08% as compared to 4.14% in 18G. 7. There was no significant difference in the incidence of phlebitis between surgical and medical wards. It was 10.96% in surgical wards as compared to 6.88% in medical wards. In conclusion, the following risk factors can be attributed to the incidence of phlebitis : sex, the duration of catheter placement, the location of the insertion site and the size of cannula inserted. Consequently, in order to prevent and detect the intravenous-related phlebitis early, a consistent, organized assessment and evaluation of the above factors during the intravenous therapy.

BACKGROUND: Modifying risk factors and improving stroke awareness in the general community have been emphasized for preventing stroke. This study aimed to explore modifiable lifestyle risk factors and the awareness of stroke and to compare scores for stroke knowledge according to lifestyle risk factors and recognition of important indicators among adults in urban areas. METHODS: A cross-sectional study was conducted with a total of 196 participants aged 40 to 65 selected by a convenient sampling of community cultural centers, churches, and recreational groups. Participants completed a structured questionnaire including general characteristics, risk factors of stroke, and awareness of stroke including general knowledge, risk factors, reactions to warning symptoms, and knowledge on personal risk factors of stroke such as body weight, blood pressure, blood sugar, and cholesterol level. RESULTS: Less than 30% of the participants engaged in regular physical activity and low fat diet and 33% of the participants were classified as having 'higher' level of knowledge on stroke. Knowledge on their personal risk factors of stroke were generally poor. Scores for knowledge on stroke were not different for lifestyle risk factors and knowing whether their body weight and blood pressure level were within normal limits. Meanwhile, scores for knowledge on stroke were significantly higher among those knowing whether their blood sugar and cholesterol level were within normal limits. CONCLUSIONS: There is a need to improve stroke awareness among community adults by providing more targeted information on warning symptoms, risk factors of stroke, and important indicators of major conditions related to stroke and their contribution to stroke prevention.

A case of 17 years old male with KPP of Tbost-Unna Type is reported. The akin lesions were characterized by the presence of confluent, sharply demarcated hyperk eratotic plaques on the palmar and plantar surfaces. Hypercontraction of palms and soles and hyperhidro is of scalp, face, palms and soles were also noted. Roentogenologic study revealed secondary changes of osteoporosis, flexion deformity and osteoarthritis due to soft tissue contracture and phalangeal and metatarsal bones of hands and feet showed bony resorption with tapering appearance. Histalogic findings of palmar lesion showed excessive hyperkeratosis, parakeratosis and acantosis in the epidermis and no pathology was noted in the dermis and skin appendages, He was treated with hot water compress, 5% Salicylic acid ointment and vit. A and D for 2 months and favorable effect was noted. Authors reviewed relevant literature and made discussions also.

Twenty-two cases of idiopathic myocardiopathy were observed during the period of 1962 to 1971 and follow up clinical study was made in 5 cases. The criteria of diagnosis was based mainly on exclusive diagnosis in etiology unknown marked cardiomegaly. 1) Male and female ratio of idiopathic myocardiopathy was 1.1:1 and age of onset were distributed from first decade to fifth decade with similar number of cases. 2) Cardinal symptoms were dyspnea, palpitation, chest pain and cough. Common physical findings were protodiastolic gallop (in 2 cases), moist rales (in 3 cases) and hepatomegaly(in 3 cases). 3) Laboratory findings were normal except elevated T.T.T. in one case. 4) Electrocardiogram were abnormal in all cases. There were 3 cases of left ventricular hypertrophy, 3 cases of non-specific ST-T changes and one case of first degree A-V block, intraventricular conduction defect, abnormal Q wave and low valtage, respectively. 5) After medical treatment, symptoms were improved in 4 cases but heart size was reduced in only one case and E.C.G. abnormalities were unchanged in all cases.

Authors observed the antihypertensive effect of debrisoquin sulfate in 35 cases of essential hypertension and following results were obtained. 1) Results of short term control of blood pressure with debrisquin in 35 hypertensive patients revealed good control in 74.3%, fair in 14.3%, poor in 5.7% and failure in 5.7% of cases. Significant reduction of blood pressure was achieved in 7 of 9 cases of hypertension with addition of hydrochlorthiazide to debrisoquin. 2) During long term therapy with debrisoquin (duration of average 3.9 months), good control cases were slightly reduced(good in 58.6%, fair in 24.1%, poor in 6.9%, failure in 10.3%). But average effective dose of debriioquin was not increased significantly. 3) Antihypertensive effect of debrisoquin was slightly reduced in the cases of severe hypertension, hypertension of over 10 years duration and complication of cerebrovascular accident. 4) The side effects during treatment with debrisoquin were dizziness in 31.4%, headache in 11.4%, dry mouth, blurred vision, general weakness, insomnia in 8.6% respectively. There were no side effects in 48.6% of cases.

PURPOSE: In the evaluation of visual field defect, Goldmann kinetic perimetry is the preferred method. However, in many cases, Humphrey static perimetry is performed for visual disability evaluation in Korea. In the present study we investigated the correlation between Goldmann kinetic perimetry and Humphrey static perimetry for disability evaluation using visual field score (VFS). METHODS: This study included 126 eyes, classified into the following groups: 60 eyes, normal group; 11 eyes, contraction of central visual field group; 42 eyes, irregular visual field group; 13 eyes, hemianopsia group. All subjects were examined with Goldmann kinetic perimetry and Humphrey static perimetry. We studied the correlation of the VFS between Goldmann kinetic perimetry and Humphrey static perimetry according to the Korean Academy of Medical Science Guides for Impairment Evaluation (KAMS Guides) and American Medical Association Guides for the Evaluation of Permanent Impairment (AMA Guides). RESULTS: Regarding contraction of central visual field group, Goldmann VFS, Humphrey VFS and extended Humphery VFS showed no statistical significance (AMA: p = 0.50, p = 0.30, KMAS: p = 0.36, p = 0.18. respectively). In the irregular visual field and hemianopsia groups, Goldmann VFS and Humphrey VFS showed statistical significance (AMA: p = 0.00, p = 0.00, KMAS: p = 0.00, p = 0.00. respectively). Goldmann VFS and extended Humphrey VFS showed no statistical significance (AMA: p = 0.13, p = 0.12, KMAS: p = 0.08, p = 0.99. respectively). CONCLUSIONS: The contraction of central visual field based on Humphrey static perimetry can be applied to visual disability evaluation. However, in the majority of cases, there is a difference between the two tests and Goldmann kinetic perimetry should be used first in the evaluation of visual field disability evaluation.
