1.A Case of Meibomian Gland Adenocarcinoma.

The clinical resemblance of carcinoma of the Meibomian gland to the relatively frequent chaiazion should be kept in mind by anyone performing eye lid surgery. A 46-year-oId Korean house wife was first visited here in May 2nd '69 with complaints of localized swelling, ulceration and dull pain on the left upper lid. She had been taken the operation for chaiazion on the same side of the left upper eye lid a year ago. Examination revealed a reddened hard, tender lesion elevating the skin on the medial one third of the left upper lid with ulceration at its center. The tarsal conjunctiva was noted yellowish patches with ulceraion extended to its margin. Local resection with full thickness containing normal tissue around he lesion of upper lid was performed and graft was made in lower lid and rotatory transplantation was given, and pathologic examination revealed adeno-carcinoma of Meibimian gland. There was no evidence of recurrence and metastasis to remote site a year after surgery.

2.Seroepidemiologic Analysis of Acute Febrile Illness from Korea in 1996.

Hemorrhagic fever with renal syndrome (HIFRS), scrub typhus, murine typhus and leptospirosis have been the principal acute febrile diseases in Korea. To evaluate the seroepidemiologic patterns of acute febrile illness, sera collected from 2,423 patients in 1996 were examined for antibodies against Hantaan virus, Orientia tsutsugamushi, Rickettsia typhi, and Borrelia burgdorferi by indirect immunofluorescent antibody technique (IFA) and macroscopic agglutination test for Leptospira interogans. Seropositive cases against O. tsutsugamushi, Rickettsia typhi, Leptispira interogans and Hantaan virus were 192 (7.9%), 193 (8.0%), 12 (0.5%) and 324 (13.4%), respectively. Male was more affected in HFRS and murine typhus contrasting to scrub typhus and leptospirosis in female. Most positive cases occurred during October and November for scrub typhus, and during November and December for HFRS. These results showed similar patterns with previous epidemical data for recent couple of years, and possibly implied no significant changes occurred in ecologic situations for acute febrile diseases in Korea.

3.The Effects of Foot Reflexologic Massage on Blood Pressure and Sleep of the Elderly With Essential Hypertension.

PURPOSE: This study was conducted to evaluate the effects of foot reflexologic massage on blood pressure and sleep of elderly with essential hypertension. METHOD: This study was employed non-equivalent control group pretest-posttest quasi-experimental design. The subjects were divided into two groups: 20 elders for experimental group and 18 elders for control group. For experimental group, foot reflexologic massage was done three times at three-day intervals. The data analyzed by SPSS version 10.0 program. RESULT: The systolic and diastolic blood pressure after foot reflexologic massage did not show a significant difference between the two groups. In experimental group, there was a significant difference in the systolic(t=3.559, p=.001) and the diastolic blood pressure(t= 3.048, p=.004) that measured after foot reflexologic massage. There was a significant difference(t=2.665, p=.011) in sleep between the two groups. CONCLUSIONS: The results suggest that foot reflexologic massage was partially effective to reduce the degree of blood pressure and improve sleep of the elderly with essential hypertension. A follow up research is needed to compare and confirm its long-term effects.

6.Evaluation of ES-300 for the Detection of Anti-HCV Antibody.

BACKGROUND: A fully automated enzyme-immunoassay (EIA) analyzer, Enzymun System, ES-300 (Boehringer Mannheim, Germany) uses streptavidin technology and performs single test or panels of up to 12 tests per run. We evaluated the results of ES-300 for anti-HCV by comparing the results with microplate-EIA, radioimmunoassay (RIA), and confirmatory test. METHODS: Total 79 sera (51 positive, 24 negative, 4 indeterminate results confirmed by Lucky HCD Confirm) were analysed. ES-300 with Enzymun-Test(R) Anti-HCV (Boehringer Mannheim, Germany) and microplate-EIA (Green Cross Center Innotest HCV 3.0(R)) were used. Fifty one sera were examined additionally by 2nd-generation RIA method, NANBDINE 125C(General Biologicals Corp., R.O.C.). And all results were compared to the results of Lucky HCD Confirm. RESULTS: The overall concordance rate of ES-300 and Innotest(R) was 72/79 (91.1%). The results of Lucky HCD Confirm on seven discrepant samples were five negative and two indeterminate. The results of ES-300 and NANBDINE 125C showed concordance rate of 90.2%. The sensitivity and specificity of ES-300 with regard to Lucky HCD Confirm were 94.5%, and 87.5%, respectively, and that of Innotest(R) were 98.2% and 66.7%, respectively. Clear distinction of positive and negative results by signal/cut off ratio was available in both EIAs. The positive predictive values of ES-300 and Innotest(R) were 94.5%, and 87.1%, respectively. CONCLUSIONS: ES-300 showed relatively good results in sensitivity and positive predictive value with regard to confirmatory test. In EIA-positive persons, however, follow-up study would be necessary for reliable evaluation of HCV infection.

7.The Detection of the Cystoid Macular Edema after Cataract Extraction with Oral Fluorescein Angiography.

Cystoid macular edema (CME) is a relatively common cause of temporary or permanent visual impairment after cataract extraction. Fluorescein angiography is indispensable to diagnose the CME. Intravenous fluorescein angiography is asscciated with a high incidene of minor adverse effects but a very low incidence of life-threatening serious reactions. There are no reports of oral fluorescein causing a senous reaction, and minor adverse effects are uncommon. We had Performed the oral fluorescein angiography in 30 consecutive cases (31 eyes) to detect the CME after cataract extraction. Two eyes (6.45%) had definite evidence of CME on the fluorograms. The typical angiogram of CME were obtained at 30 or 40 minutes after they had taken the fluorescein sodium. A dose of 20mg/Kg body weight produced good quality angiograms in 71% of the patients. There was no serious reaction but a mild pruritus was noted in one patient(3.3%). Therefore, we have concluded that the oral fluorescem agniography is very useful method because the oral fluorogram may avoid the adverse effect that could occur in intravenous fluorescein injection and be comparable in quality with those produced by intrvenous fluorescein.

10.Bladder Dysfunction Caused by Herniated Lumbar Disc.

The cystometry had been performed in 28 cases of H.L.D. and 1 case of unstable lumbar spine due to spondylolysis from January 1974 to September. Among them, 24 cases were checked cystometry again after one week of postoperative period. We obtained the results as follows: 1. Preoperative cystometrograms were revealed that 21 cases were bladder hypofunction,3 cases were bladder irritability, and 5 cases were normal. 2. The postoperative cystometrograms were revealed that were improved in 3 cases, unchanged in 19 cases, and worsened in 2 cases. 3. It is likely that the bladder hypofunction is related to the severity of radiating pain rather than the severity of myelographic findings, the direction and the level of disc herniation.
