1.Evaluate the results of the \u201cFamily doctor\u201d model in health care for people in Minh Lap commune, Dong Hy district, Thai Nguyen province

Background: Minh Lap is a mountainous commune of Dong Hy district, Thai Nguyen province with a high number of ethnic minorities. Social \u2013 economic conditions as well as heath care services are poor. Objective: To evaluate the results of implementation of the \u201cFamily doctor\u201d model in health care for people in this area. Subject and method: The study was carried out on people in Minh Lap commune, Dong Hy district of Thai Nguyen province from May 2007 to December 2007 and the researchers used the community intervention method. Result and conclusion:9.9% of households had their medical records completed and 54.05% of pregnant women giving birth in this year were set up with medical records and followed up. 60 cases of pregnant women were consulted with a total of 141 times, 11 cases with abnormal symptoms were diagnosed and treated in the following process. The rate of examination for women in the first week after delivery accounted for 61.26%. In this period of 8 months, the mean follow up time was 1.7 times and 2.9 times for children under 36 months and children under 12 months, respectively. The results demonstrated that the \u201cFamily doctor\u201d is a good way to improve primary health care for people.

2.Posterior fossa extradural hematoma due to closed head injuries

Extradural hematoma of posterior cranial fossa that related to closed head injuries was rare and difficult diagnosis. The study involved 45 patients. 40/45 patients had signs of hematoma in occipital region, 34/45 patients had headache, 28/45 patients had vomit, 22/45 had dizziness, 30/45 patients had sleepy. 23 patients received X-ray imaging, in which, 11 patients showed the rupture of occipital bone. On CT images, rate of partial ventricular collapse and displacement was 95.6%. 28/45 patients with posterior fossa extradural hematoma had conscious duration. And in 18 patients this duration longed for more than 1 days. Perception worsened rapidly is a common sign of posterior fossa extradural hematoma, especially in acute hematoma. Mortality of posterior fossa extradural hematoma was 20%. Rate of patient without post-operative complication was 71.1%. Rate of patient with post-operative complication was 8.9%.

3.Peritoneal lavage in the diagnosis of patients with the closed abdominal trauma in Phu Tho Provincial Hospital

The usefulness of peritoneal lavage in the identification of patients who require laparotomy after close abdominal trauma is not controversial. Peritoneal lavage should be seen as an adjunct to clinical presentation poses unusual difficulty, for example, when consciousness in impaired or when diagnostic doubt exists... The diagnostic accuracy of peritoneal lavage in blunt abdominal trauma has been shown of 100%. In this collected of 17 patients, the false positive rate was 0%.

4.Some primarily opinions of diagnostic ultrasound of appendicitis

Study on 35 patients with ages of 10 suspected with the appendicitis diagnosed by ultrasound. The results have shown that the appendicitis most frequently occurred in persons with ages of 21-60. The diameter of appendicitis was about 8-10 mm by ultrasound. There was no suitability between ultrasound and operation for symptoms of fluid in the terminal bag of Douglas and pelvic cavity.

5.On the habit of smoking at some enterprises with high levels of harmful factors

100 subjects were enrolled in a randomized epidemiological survey on smoking habit. Results show a high rate of users of tobacco (49% subjects) despite of the large awareness (95.7%) of the harm of tobacco. The behavior of smoking in public environment, the intention of smoking cease is varied widely among the studied groups.

6.Effects of the multiplex PCR kit for diagnosis of tuberculosis

Background: Facing urgent requirements of the rapid diagnosis of tuberculosis nowadays, our laboratory have improved the multiple PCR for 3 target genes spontaneously (IS 1081, IS 6110, 23S rDNA) and developed into the multiplex PCR kit to improve the effects of diagnosis of tuberculosis. Objective: To evaluate the multiplex PCR kit for the 3 target genes (IS 1081, IS 6110, 23S rDNA) for the diagnosis of tuberculosis on standard panel and clinical specimens. Subjects and method: DNA extracted from M. tuberculosis strains and clinical specimens were amplified by multiplex PCR kit, comparison with singleplex PCR for each target gene. Results: On standard panel, sensitivity and specificity reached 100% and detected limitation was 100 fg DNA template (about 3 M. tuberculosis cells). Multiplex PCR kit detected 195/307 (63.52%) suspected clinical specimens compared with singleplex PCR which detected 106/307 (34.53%) for IS 1081, 182/307 (59.82%) for IS 6110 and 136/307 (44.30%) for 23S rDNA. Conclusions: PCR amplification with one target gene was insufficient for diagnosis of the tuberculosis strains in Viet Nam. Multiplex PCR kit for 3 target genes simultaneously (IS 1081, IS 6110, 23S rDNA) could detect all tuberculosis strains.

7.Soil transmitted helminth infection in Son La province

Background: Helminth infection cause bad impacts on human health, especially for children.Helminth infection directly related to poverty, bad hygiene, low quality water supply, people awareness... Objective: To investigate the current situation of soil transmitted helminth infection in Son La province. Subject and Method: A total sample of 7,150 fecal specimen collected from72 hamlets, 18 communes in 6 districts of Mai Son, Muong la, Thuan Chau, Song Ma, Quynh Nhai and Yen Chau were examined for soil-transmitted helminths. Result: A cumulative infection rate of 93.45% was found, of which Ascaris: 85.85%, Trichuris: 24.52%, and hookworm: 18.92%. No different' infection rate was found between males and females but it was higher with ages. The prevalence of Trichuris and hookworm was higher in adults against children and higher with the older ages. The infection of Trichuris and hookworm in adults was higher than that in children. The single helminth infection rate was 82.17%, and mixed infection of two species was 15.9% and three species 0.43%. The helminth infection among Sinh MUD ethnic group: 97.2%, Thai ethnic group: 96.6%, La Ha ethnic group: 85%, Kinh ethnic group: 80.62%, and H'mong ethnic group: 45.96%. The hookworm infection rate of Kinh and H'mong groups was found higher that of the others ethnic groups. KAP surveys were conducted with 2,188 households showing a high latrine coverage of 90.6%, of which 92.9% of latrines were self-made. The more was the rate of self-made latrine, the higher was the helminth infection. Conclusion: Low awareness and knowledge of the local people on helminth infection was found so as they do not know how to prevent from helminth.

8.Evaluating the acceptance of the community on field training of medical students

Background: Training community-oriented general practitioners with the aims to provide the medical knowledge and skills of working in the community for medical students is appropriate with the socioeconomic context in Vietnam. Objectives: To evaluate the acceptance of the community in two districts Kim Bang (Ha Nam province) and Dong Hy (Thai Nguyen province) on field training for medical students. Subjects and method: The cross - sectional study was carried out on 300 households that were chosen randomly from 2 rural districts on April 2006. The heads of households were interviewed by open - ended questionnaires. Another 120 key informants from these two districts were invited to participate in a focus group discussion. Results: 81% of households voluntarily participated in the organization of field training for the medical students. More than 97% households paid attention to the medical students\u2019 information about health care and environmental hygiene. With medical community-based education, the people from communities receives\u2019 improvement in their knowledge of child care and personal hygiene in order to prevent diseases. Conclusion: Being exposed to health problems in communities, medical students can enhance their working and communication skills. People in the communities had a chance to improve their knowledge of disease preventions.

9.Noise pollution, occupational failing hearing and occupational hearing loss among auto-engineering workers in Hue city

Background: Noise pollution has seriously negative effects on worker\u2019s health, especially their hearing ability. Occupational hearing loss is one of 21 insured occupational diseases. Occupational hearing loss is common in workers in transpostation, construction, energy heavy and light industry areas. Objectives: 1. To determine the degree of noise pollution in Thua Thien Hue Unity Auto Mechanical Joint Stock Company. 2. To evaluate occupational failing hearing and occupational hearing loss among workers in the company. 3. To promote preventive measures which imnimize occupational failing hearing and occupational hearing loss. Subjects and method:A cross-sectional, descriptive study was carried out in 254 employees in Thua Thien Hue Unity Auto Mechanical \r\n', u'Joint Stock Company. Results and Conclusion: 72.7% the employees of Thua Thien Hue Unity Auto Mechanical \r\n', u'Joint Stock Company have exposed to environmental noise at the level higher than the permitted one. The rate of occupational failing hearing among these people is rather high (33.1%), and occupational hearing loss rate is 11.8%. There is a increased rate of occupational hearing loss according to the employee\u2019s age and seniority. Proposed preventive measures should be immediately conducted (P < 0.05).\r\n', u'

10.Lead level in blood and urine of fishermen in Canh Duong village, Phu Loc, Thua Thien Hue province

Background: In Viet Nam, there were many researches on determining lead level in blood, food and environment. But until now, there were not researches on determining lead level in fishermen community. Objectives: Determine trace amount of lead by von-ampe dissolve anot using electrode BiFE. Analysis on trace amount of lead in blood and urine of fishermen in Canh Duong village. Propose solutions for preventing lead poisoning in studied community. Subjects and method: Fishermen in Canh Duong village, Phu Loc, Thua Thien Hue province. The method differential pulse-anodic stripping voltametry (DP-ASV) using Bismuth Film Electrodic in situ was successfully applied to measure lead levels in blood and 24h urine of fishermen in Canh Duong village. Results: The mean level of lead in blood of the suspected group (fishermen) in Canh Duong village (34,7 \xb5g/dL) was higher than that of the control group (16,3 \xb5g/dL). For both groups, 37 out of 40 blood samples (92,5%) had lead level higher than WHO\u2019s recommended level (10 \xb5g/dL). Especially, 6 of 40 blood samples (15%) had lead level higher than 50 \xb5g/dL. The lead level in 24h urine of suspected group had slightly higher (about 1,2 times) than that in the control group. The lead level of the two groups was much higher than the normal level. Conclusion: The method DP-ASV/BiFE in situ was applied successfully to determine lead level in blood and 24h urine. The investigative results of related information showed that: the risk of chronic lead poisoning in studied community very high.
