1.A Rapid Simple Stain for the Diagnosis of Superfical Mycosis with Mixed Chlorazol Black E Solution.

We compared the sensitivity and practicability for the direct examination of fungal hypae between the specimens treated with mixed Chlorazol black E solution and simple KOH(potassium hydroxide) solution in 118 cases of superficial myosis. The results were as follows: Higher positivity of fungal hypae in the specimens treated with mixed Chlorazol black E solution(99. 2%) than simple KOH solutiion(87. 3%.). 2, It was time-saving and easy to detect fungal hypae in the specimens treated with mixed Chlorazol black E solution than simple K(3H solution.

2.The risk factors of blood pressure in primary hypertension patients.

The purpose of this study was to investigate the relation between hypertension and risk factors. A positive association between obesity, age, serum lipid and such life style factors as smoking, exercise, alcoholic beverage use with blood pressure problems and the prevalence of hypertension has been described in many studies. Therefore to identify and evaluate the effects of variables, which were known to be related to hypertension, multiple regression analysis was performed. We studied 110 subjects. The participants were 45 men and 65 women who visited one university hospital located in Seoul from January to December 1998. All 110 persons were identified as having primary hypertension by their physicians. The significant risk factors identified for hypertension were Obesity, BMI, length of time with hypertension. The results were as follows; 1. The systolic blood pressure average in 110 subjects was 156.65mmHg +/- 14.08mmHg, the diastolic blood pressure average was 100.73mmHg +/- 5.64mmHg. 18.2% of the subjects were smokers, 26.4% of the subjects were drinkers, and 33.6% of the subjects exercised regularly. 13.6% had a family history of hypertension. The average mean pressure was 119.37 +/- 7.40mmHg. The average history of hypertension was 28.36 +/- 34.56months. The average body weight of subjective was 65.25kg, and their average BMI was 24.54kg/m2. The average serum cholesterol of the subjects was 204.54mg/dl, HDL was 50.93mg/dl. 2. The result of this study were that variables which showed significance rationale for high systolic blood pressure in hypertension were mean arterial pressure(R2=0.808), and age(R2=0.032). And the variables which showed significance rationale for diastolic blood pressure in hypertension were mean arterial pressure(R2=0.697), age(R2=0.051), sex(R2=0.014), and somking(R2=0.010). And the best variable for prediction of mean blood pressure was the length of history of the disease. 3. According to multiple regression analysis by demographic variables, age and the length of history of hypertension were predictable variables for hypertension. And mean the blood pressure was identified best explaining variable of hypertension by biophysical variables. Therefore, life style modification for hypertension patients as a primary regimen is less important than for normal blood pressure patients. Life style modification is important intervention for normal blood pressure subjects, on the other hand drug therapy and its compliance is the most important intervention for hypertensive subjects. So to regulate blood pressure and prevent the complication of hypertension, the first regime of all should pharmacologic therapy. There is a need to develop nursing intervention to improve pharmacologic compliance for primary hypertension patients.

3.Effects of Letter on the Family Functioning, Fear of Death, and Depression among the Terminally Ill Cancer Patients.

PURPOSE: To improve the family functioning, fear of death and depression among the terminally ill cancer patients, the effects of letter as an independent nursing intervention were identified. METHODS: Non-equivalent control group pre and post test design was used. Subjects were 60 patients and their family members who were hospitalized at hospice units of an university-affiliated hospital. Patients and their family members were encouraged to write a letter to each other at least 4 times for 2 weeks. RESULTS: Family functioning, fear of death and depression in the experimental group were significantly more improved after intervention than those of control group, even depression of experimental group before intervention was worse than that of control group. CONCLUSION: Writing a letter to family members is worth to use as an independent nursing intervention for terminally ill patients. It is recommended that further study to identify the potential of letters as an independent nursing intervention is implemented for various patients.

5.Outpatient General Anesthesia for Mentally and Physically Handicapped Children Undergoing Extensive Dental Treatment.

BACKGROUND: Fear of dental treatment is a very real problem for many people. Very young patients or children that are mentally or physically handicapped have various problems that preclude routine dental treatment in the office and require general anesthesia for extensive dental restoration. In America, outpatient operations are performed in thousands of dental offices annually, but there is no report about outpatient general anesthesia in Korea. METHOD: A review of forty children treated under outpatient general anesthesia for extensive dental treatment between 1994 and 1996 inclusive was carried out to assess the patient selection, anesthetic method, recovery time and complication. RESULTS: The mean age was 8.4 years, and twenty-five percent of the patients were autism. The length of the postoperative observation period before discharge was 3.2 hours, and postoperative fever was major complication. CONCLUSION: For extensive dental treatment in handicapped children, we suggest that outpatient general anesthesia can provide reasonably safe treatment, while reducing its expense and requiring less hospital bed space.

7.In Situ Characterization of Immune Cells in the Annular Lesion of Leprosy.

To characterize the immunopathologic phenotype of the cells in the lesional ti.:sue and to further examine the mechanism of the development of the annular lesions in leprosy, we have studied immune cells (T lymphocyte and its subsets, Langerhans cells, and HLA-DR antigen expressing cells) at different anatomical sites inside, active border, and outside normal skin in the annular lesions of leprosy. We took biopsy specimens from 4 patients of BT type, then processed the specimens by the staining methods eif indirect immunoperoxidase with monoclonal antibodies. In the active border the number of T cell was over 50g of the total cells infiltrated in the dermis. Helper T cells were dominant in number, and about three fourths of the cells were positive for HLA-DR staining. In two patients they had expression of DR antigen on the surfaces of the keratinocytes in the epidermis, in contrast to that of the inside, even the intensities were not. strong. At the inside of the annular lesions T cells were about 40% and the ratio of helper/suppressor T cell was approxiinately 1: 1, However, HLA-DR positive immune cells were not more than 10g among the total infiltrates. Langerha,ns cells were increased in number and in size either in the border or at the inside of the annular lesions. With these results we presume that the T cell mediated imrnune responses against Mycobacterium leprae may play an important role in the formation and extension of the annular lesions in leprosy.

9.Nutritional and Health Status of Korean Elderly from Low-income, Urban Area and Improving Effect of Meal Service on Nutritional and Health Status - V. The Effect of Meal Service for One Year on Nutritional and Health Status.

This study was performed to assess the effect of one year's of meal service for home-staying urban elderly with low incole on their nutritional status. One hundred and eighty three subjects, who had already completed the first nutritional survey, were assigned to two group : meal served(served) and non-meal served(non-served). A meal containing approximately on half of the RDA for energy, protein, calcium and iron was served as lunch everyday to served group. After on year of meal service, follow-up-nutritional survey was done and changes of parameters were analyzed with paired t-test. Served female showed signficantly increased intake of riboflavin and calcium, while non-served female showed significantly decreased intake of calcium. Serum total protein, serum albumin and serum cholesterol were significantly increased in female regardless of meal service. Served remale was observed with significantly elevated LDL-cholesterol, whereas non-served female showed singnificantly lowered HDL-cholesterol. Significantly decreased serum iron, serum transferrin saturaion and significantly increased TIBC were observed for female regardless of meal service. But the proportion of anemic elderly according to Hb or serum iron was decreased more in served group. Female showed significantly increased serum zinc and copper regardless of meal service, whereas only served male showed significantly increased serum copper.

10.A Case of Symmetrical Peripheral Gangrene Associated with Disseminated Intravascular Coagulation.

We report a case of symmei.rical peripheral gangrene in a 22-day-old female associated with dissem-inated intravascular coagulation, which probably occured from septicmia of Enterobacter aerogenes. The skin lesions showed well-defined blackish gangrene surrounded by purpuric patches on the whole fingertips and toes except the loft thumb. Histopathologically, there were epidermal necrosis, diffuse extravasation of RBCs, mild petivascular inflammatory infiltrates and delated and RBC-filled vessels in the dermis. But, there was no definite evidence of vasculitis. In spit,e of aggressive antibictics therapy and other supportive measrres, she died at 39th day after birth.
