3.A STUDY ON THE STAIN RESISTANCE OF DENTAL CERAMICS FOR ALL CERAMIC CROWN.

Simulation of naturally appearing enamel in fixed prosthodontics could be achieved with all ceramics. The purpose of this study was to investigate the stain resistance of 4 different all ceramics materials (In-ceram, IPS Empress, Celay, Vintage). Forty specimens were prepared using manifacture's instruction and the samples were divided into two groups. The specimens for one group were glazing and those for other group wee ground to remove the glazing layer. All specimens were immersed in methylene-blue for 24hours. The color values for each specimen was measured with colorimeter (Mode Tc-6Fx, Tokyo Denshoku Co.) prior to and after immersion in methylene-blue. The following conclusions were drawn from this study. 1. The stain resistance of glazing ceramics was higher than that of unglazing ceramics. 2. The changes of E* and L* values were revealed in Vintage and unglazing Celay. 3. The changes of a* values were revealed in unglazing vintage, Celay, In-cream. 4. The changes of b* value were revealed in Vintage, IPS Empress and unglazing Celay.

4.Detection and characterization of excretory/secretory proteins from Toxoplasma gondii by monoclonal antibodies.

Excretory/secretory proteins (ESP) from Toxoplasma gondii were analyzed to define the function in the penetration process into host cells. Whole ESP obtained at 37 degrees C were composed of 15 bands with molecular mass of 110, 97, 86, 80, 70, 60, 54, 42, 40, 36, 30, 28, 26, 22, and 19 kDa. Five ESP of 86, 80, 42, 36, and 28 kDa were reacted with monoclonal antibodies (mAb), named as Tg386 (microneme), Tg485 (surface membrane), Tg786 (rhoptry), Tg378, and Tg556 (both dense granules), respectively. The ESP was released by a temperature-dependent/-independent manner and all at once whenever ready to pour out except Tg786. Each ESP was not exhausted within the parasite but the amount was limited. Tg786 was released continuously with increment, whereas Tg378 and Tg556 were ceased to release after 3 and 4 hr. Dense granular Tg378 and Tg556 were released spontaneously and constitutively before the entry into host cells also. The entry of T. gondii was inhibited by all the mAbs differentially. And the parasite deprived of ESP was inhibited to enter exponentially up to 90.1%. It is suggested that ESP play an essential function to provide appropriate environment for the entry of the parasite into host cells.

6.New DNA Extraction Method for Diagnosis of Tuberculosis by Polymerase Chain Reaction.

BACKGROUND: To assess the clinical utility of new DNA extraction method, the authors attempted PCR using mycobacterial DNA extracted by Chelex 100 ion exchange resin method for 63 clinical samples in patients with pulmonary tuberculosis and compared with proteinase K method, simultaneously. METHODS: We used Chelex 100 ion exchange resin for preparation of DNA. Decontaminated sputums were mixed with resin and incubated at 56degrees C and 100degrees C without opening tube. After centrifugation, supernatants were used directly as template for PCR. 245 bps in primary PCR and 188 bps in nested PCR were amplified and analysed by agarose gel electrophoresis EtBr staining. RESULTS: Chelex 100 ion exchange resin method is more simple, rapid and reliable than proteinase K method, and during sample preparation, carry-over contamination loss of amplificated DNA, influence of organic solvents and cross-contamination are diminished. The results of PCR products are interpreted more distinctively in Chelex 100 ion exchange resin method than proteinase K method. CONCLUSIONS: In the basis of the results, it could be suggested that extraction of mycobacterial DNA by Chelex 100 ion exchange resin is more simple, rapid reliable method than that of conventional method for detection of mycobacterial DNA in patients with tuberculosis by polymerase chain reaction.

7.CT and MR Findings of Kimura's Disease in the Head and Neck.

PURPOSE: Kimura's disease is an inflammatory condition of unknown etiology which usually occurs in salivary glands and cervical lymph nodes. This study was undertaken to evaluate the radiologic characteristics of Kimura's disease using CT and MRI. MATERIALS & METHODS: We evaluated CT & MR of 10 patients with histologically proved Kimura's disease in the head and neck. Six males and four females between 16 and 58 years old (mean age, 37.6 years) were included. Contrast enhanced CT scan was performed in all, and spin-echo MRI was performed in 2 patients. RESULTS: Both CT and MRI demonstrated the distinctive sites of involvement that were major salivary glands (4 cases) and lacrimal gland (1 case), periglandular soft tissue (8 cases), and cervical lymph nodes (8 cases). Majority of soft tissue lesions were ill-defined with infiltration to adjacent fascial planes, but nodal lesions were relatively well-defined and homogenous. The lesions were iso-intense on Tl-weighted image and hyper-intense on T2-weighted image. All but a few nodal lesions showed variable degree of contrast enhancement on CT and MRI. CONCLUSION: In diagnosis of Kimura's disease, CT & MRI of head & neck, especially salivary glands, are useful. In cases of recurrent periglandular soft tissue mass with contrast-enhancing cervical lymphadenopathy in adult patients, Kimura's disease should be included in the differential diagnosis.

8.Repair of Posttraumatic Tricuspid Regurgitation Using Artificial Chordae and an Annuloplasty Ring.

A 52-year-old man was taken to the emergency room following a motor vehicle accident. An echocardiogram showed moderate to severe tricuspid regurgitation due to rupture of the anterior chordae. An operation to repair the traumatic tricuspid regurgitation was recommended; however, the patient refused because he was asymptomatic. Two years later, he developed mild generalized edema and dyspnea. The echocardiogram revealed progressive severe tricuspid regurgitation and annular dilatation. We treated the posttraumatic tricuspid regurgitation successfully using artificial chordae and ring annuloplasty.

9.Complicated Cataract Following Radial Keratotomy.

For correction of myopia, we had performed 247 cases of 8-incision radial keratotomy from July 1986 to Dec. 1987. Among them, we experienced one case of complicated cataract in 23 year old man. During the surgery, there were no surgical complications, including microperforation. Postoperative uncorrected V.A. was 0.9 at 1 day after surgery. At 2 days after surgery, V.A. decreased abruptly, and corneal edema, hypotony, severe iritis were noted. Although corneal edema and iritis were improved 10 days later, decreased V.A. and hypotony persisted. Posterior synechia, iris atrophy and posterior subcapsular cataract could be seen on follow-up examination. An intumescent cataract developed about 6 weeks after the surgery. Successful extracapsular cataract extraction was done about 11 months after the original radial keratotomy. Postoperative corrected V.A. was excellent.
