1.The Effect of the Walking Exercise on Physiological index, Physical Fitness, Self Esteem, Depression and Life Satisfaction in the Institutionalized Elderly Women.

PURPOSE: The purpose of this study was to show the effect of walking exercise on physiological index, physical fitness, self-esteem. depression. and life satisfaction in institutionalized elderly women aged over 70. SAMPLE AND METHOD: Data were collected from subjects who consented to participate in this program for eight weeks from the 2nd of July to the 27th of August 2005. The subjects were divided into two groups, 26 subjects in the experimental group who had walking exercise and 30 in the control group who did not have walking exercise. The intensity of the walking exercise allowed 50-65% of the maximum heartbeat. RESULTS: The results of the experiment supported the hypothesis that the experimental group would have higher flexibility, left grip strength, sense of equilibrium, self-esteem and life satisfaction and lower depression than the control group. CONCLUSION: Walking exercise has the effect of decreasing diastolic blood pressure together with improvement in flexibility, left grip strength, sense of equilibrium, self-esteem, depression and life satisfaction for institutionalized elderly women aged over 70. According to the results presented above, walking exercise can be strongly recommended for improving the health of institutionalized elderly women.

2.Dermographism ( III ): Dermographism in Acute and Chronic Urticaria.

The prevalence of dermographism in 96 patients of acute or chronic urticaria was studied using dermographic tester. The results were summarized as follows : 1. The prevalence rate of dermographism in acute or chronic urticaria was 26.0% (25/96). 2. Dermographism was more common in acute or chronic urticaria than in the general population(p<0.005). 3. There was no significant difference in the prevalence rate of dermographiam between acute and chronic urticaria. 4. The prevalence rate of dermographism was not significantly correlated with the level of serum IgE.

3.High-Grade Myxofibrosarcoma Showing Pleomorphic Hyalinizing Angiectatic Tumor-like Appearance: A Case Report.

Myxofibrosarcomas (MFSs), which consist of multiple nodules with a variable cellular population in a myxoid matrix, are primarily located in subcutaneous tissue. Pleomorphic hyalinizing angiectatic tumors (PHATs) are rare soft-tissue tumors characterized by a proliferation of highly pleomorphic spindle or polygonal cells and abundant ectatic blood vessels in cellular or myxoid stroma. We present here an unusual case of a high-grade MFS with a PHAT-like appearance. A 67-year-old man presented with an asymptomatic subcutaneous mass in the right forearm. The tumor had myxoid, hypo-, and hypercellular areas with highly pleomorphic spindle or polygonal tumor cells that showed frequent mitoses and nuclear pseudoinclusions. Foci of punctuate necrosis and inflammatory infiltration were present throughout the tumor, and abundant ectatic, thick-walled vessels containing blood clots were noted. The tumor cells were immunohistochemically positive for vimentin but negative for CD34, S-100 protein, smooth muscle actin, desmin, and bcl-2.

4.A Case of Hereditary Trichilemmal Cyst with Proliferating Trichilemmal Tumor.

We report a case of hereditary trichilemmal cyst with proliferating trichilemmal tumor in a 64-year-oid man, who has generalized 157, variable sized firm cutaneous nodules for 40 years. The cystic nodules are chiefly located on the scalp and trunk which showed characteristic histopathologieal findings of trichilemmal cyst and proliferating trichilemmal tumors. His son also has multiple trichilemmal cysts and his mother seems to have had clinically similar lesions. The patient and his son were treated with surgical excision and therapeutic trial of oral isotretinoin for 2 months. Isotretionoin was ineffective in the patient, but the lesions of his son were decreased in size.

Acquired pulmonary artery stenosis which is secondary to tuberculosis is so rare that only a few scattered cases have been reported. We report one case of pulmonary stenosis caused by pulmonary tuberculosis.l A 50 year old man who gradually developed dyspnea was diagnosed as bilateral pulmonary stenosis, he underwent bypass surgery between the main diagnosed as bilateral pulmonary stenosis. he underwent bypass surgery between the main pulmonary artery and the right pulomonary artery with a 13mm Gortex ringed straight graft. The left pulmonary artery was too small to restore the perfusion. The patient was discharged on the 33rd day after the operation. Acquired pulmonary stenosis could be treated successfully with one-side pulmonary arery reconstruction.

6.High-dose Sulbactam Treatment for Ventilator-Associated Pneumonia Caused by Carbapenem-Resistant Acinetobacter Baumannii.

BACKGROUND: Several antibiotics can be used to treat ventilator-associated pneumonia caused by carbapenem-resistant A. baumannii (CRAB-VAP) including high-dose sulbactam. However, the effectiveness of high-dose sulbactam therapy is not well known. We report our experience with high-dose sulbactam for treatment of CRAB-VAP. METHODS: Medical records of patients with CRAB-VAP who were given high-dose sulbactam between May 2013 and June 2015 were reviewed. RESULTS: Fifty-eight patients with CRAB-VAP were treated with high-dose sulbactam. The mean age was 72.0 ± 15.2 years, and the acute physiology and chronic health evaluation II (APACHE II) score was 15.1 ± 5.10 at the time of CRAB-VAP diagnosis. Early clinical improvement was observed in 65.5% of patients, and 30-day mortality was 29.3%. Early clinical failure (odds ratio [OR]: 8.720, confidence interval [CI]: 1.346-56.484; p = 0.023) and APACHE II score ≥ 14 at CRAB-VAP diagnosis (OR: 10.934, CI: 1.047-114.148; p = 0.046) were associated with 30-day mortality. CONCLUSIONS: High-dose sulbactam therapy may be effective for the treatment of CRAB-VAP. However, early clinical failure was observed in 35% of patients and was associated with poor outcome.

7.A Case of Dermatofibroma with Granular Cells.

Dermatofibroma with granular cells is composed mainly of spindle shaped cells mixed with in-conspicuous foamy cells that contain granular cytoplasm. We present a case of dermatofibroma with granular cells in a 36-year-old male who visited our clinic to evaluate a slow-growing nodule on the medial side of the left lawer leg. He have had the nodule for five years and sometimes felt an itching sensation. Biopsy of the nodule identified a dermal nodular tumor showing cellular spindle cell proliferation admixed with peripheral foamy histiocytes. With the immunohistochemical staining, the granular cell was positive for alpha-1-antitrypsin and CD68, and negative for S-100. A few electron-dense granules were seen in the cytoplasm on the electronmicroscopic study. These results suggested that the granular cells originated from histiocytes.

8.The Relationship of Uncertainty, Hope and Quality of Life in Patients with Breast Cancer.

PURPOSE: The purpose of this study was to explore the relationship of uncertainty, hope and quality of life in patients with breast cancer. METHOD: The subjects of the study were 113 breast cancer patients treated at 3 university hospitals in Daegu and Busan from June 1 to September 30, 2003. Uncertainty was measured by the Mishel Uncertainty in Illness Scale (MUIS), Hope was measured by Nowotny's Hope Scale, and quality of life by Ro's Korean Quality of Life(QOL). Data was analyzed with the SPSS program by t-test, ANOVA, Spearman's rank correlation and Multiple Regression Analysis. RESULT: The scores of uncertainty were negatively correlated with hope and QOL(all p's<0.05). Uncertainty revealed that there was a negative correlation among subitems of hope such as confidence, relationship with others, possibility of future, and inner motivation(r=-.333, p=.000; r=-.230, p=.015; r=-.260, p=.006; r=-.291, p=.003). Uncertainty also showed a negative correlation among subitems of QOL such as emotional status, self esteem, and relationship with family(r=-.29, p=.004; r=-.326, p=.000; r=-.197, p=.044), whereas hope revealed a positive correlation among subitems of QOL such as emotional status, self esteem, relationship with neighbor, and relationship with family(r=.243, p=.011; r=.487, p=.000; r=.29, p=.001; r=.29, p=.001). The score of uncertainty was low in graduate school students and subjects using problem solving as a coping method. However, the score of hope was high in college students and subjects using a problem solving method, and the score of QOL was high in professionals. CONCLUSION: Based on the study results, breast cancer patients experienced living with uncertainty and maintaining hope. Thus the study's findings can be useful in directing a comprehensive nursing care plan to improve QOL of breast cancer patients.

9.A Case of Congenital Cutaneous Candidiasis.

We report, a 1-day-old male with congenital cutaneous candidiasis who had generalized erythematous papules and pustules on the whole body including palms and soles from birth. At 5th month of pregnancy, his mother had a history of vaginal candidiasis for a month KOH mount of pus from pustules of several different sites showed budding spores and pseudohrphae and fungus culture revealed the growth of Candida albicans We believe this to he the first documented case of congenital cutaneous candidiasis in Korean literature.

10.Polymorphisms Linked to Factor IX Gene in Korean Population.

OBJECTIVE: Linkage analysis is a very useful method for prenatal diagnosis of Hemophilia B, especially when a mutation was not identified. Seven polymorphic markers were studied in Korean populations to evaluate the efficiency for prenatal and carrier diagnosis. METHODS: Subjects of this study was 100 healthy Korean women (200 X-chromosomes). Polymerase chain reacton-restriction fragment length polymorphism (PCR-RFLP) method was used to detect SalI, MseI, NruI, DdeI, XmnI, TaqI and HhaI polymorphisms. RESULTS: SalI (-) allele showed the frequency of 0.355 and SalI(+) allele 0.645. MseI(-) allele was 0.645 in frequency and MseI(+) allele was 0.355. SalI and MseI polymorphisms were in complete linkage disequilibrium. And no increase was expected in overall heterozygosity with these two polymorphisms. NruI(-) allele frequency was 0.855 and NruI(+) was 0.145. There was no polymorphism of DdeI, XmnI and TaqI marker systems in Korean population. In HhaI polymorphism, allele frequencies were estimated that HhaI(-) is 0.82 and HhaI(+) is 0.18. CONCLUSION: Only SalI, NruI and HhaI polymorphisms are useful for the diagnosis of hemophilia B in Korean population. Expected heterozygosity for above 3 poylmorphic markers was estimated to be 0.723, and 71 of 100 female subjects were heterozygous for at least one marker system. Korean population showed relatively low extent of polymorphisms compared to Caucasians, Blacks and Japanese. For the effective prenatal diagnosis of hemophilia B with linkage analysis, other polymorphic markers should be evaluated.
