2.A case of solitary fibrous tumor arising from the vagina.

Solitary fibrous tumor (SFT) arising from the vagina is a very rare benign stromal tumor, which has a well-demarcated margin and proliferation of spindle cells with the absence of any atypical features. Immunohistochemically, the cells are strongly positive for CD34. Surgical resection of the tumor is recommended as a definite treatment. Clinical courses of SFT are almost benign but long-term follow-up of SFT of the vagina have not been reported yet. We have experienced a case of solitary fibrous tumor arising from the vagina and report this case with a brief of literature.

A 47-year-old lady, presented with progressive proptosis of left eye with deterioration of vision. She had a history of
left solitary fibrous tumour and had undergone left frontal craniotomy and orbitotomy in 2004. Surveillance
Magnetic resonance imaging (MRI) six years later showed tumour recurrence with intracranial extension. However,
she did not follow-up and only presented again 3 years, later. Tumour resection and left exenteration was performed.
Histology showed ‘patternless’ pattern of neoplastic cells, and CD34 staining was diffusely positive. Diagnosis of
recurrent solitary fibrous tumour with intracranial extension was made.

Solitary fibrous tumors (SFTs) are unique soft-tissue tumors of submesothelial origin. These tumors are mainly located in the pleural space but they can be originated within a variety of sites, including the abdomen, the pelvis, the soft tissues and the retroperitoneum. SFTs from all sites are usually benign, and the surgical resection is curative in almost all cases. According to the review of literatures, during the surgical resection, massive hemorrhage could occur due to the hypervascular nature of SFTs. This is a case report on SFT in the pelvis presenting great vessel injury, which resulted in life threatening hemorrhage during the resection of tumor. We wish this paper alerts gynecologists about the risk of massive bleeding during the resection of tumor located at adjacent to great vessels in the pelvis.

Solitary fibrous tumor (SFT) is a rare neoplasm, which is usually presented as a pleural based mass, but can also occur in unusual locations based on its mesenchymal origin. However, the radiologic features of SFT occurred in the ischiorectal fossa have been rarely reported. In this case, we describe the MRI findings in a case of a SFT involving the ischiorectal fossa of a 36-year-old man. The tumor appeared as homogeneous iso-signal intensity relative to the adjacent muscle on T1 weighted images, a mixed high signal intensity on the T2 weighted images, and heterogeneous enhancement following the administration of the contrast material.
