3.A Case of Behcet's disease.

The author presents a case of Behcet's disease which was developed in both eyes of a Korean male. Prednisolone and atropine was topically administered. Depomedrol was administered by subconjunctival and intramuscular injection. The visual acuity was not improved and finally decreased to blindness, but lesions in the skin and buccal mucosa were cleared.

5.Statistical Observation on Genitourinary Tract Injury due to Traffic Accident.

2,149 injury cases due to traffic accident who visited to emergency room of Woo Sok University Medical College Hospital during the period of 3 years and 8 months from Jan. 1967 to Aug. 1969, were Studied statistically and following results were obtained. 1) Traffic injury revealed increasing tendency every year. 2) Observing from the view point of seasonal distribution, traffic accident occurred more frequently in Autumn and in Spring. 3) The neurosurgical patients were the most in number, and Genitourinary tract injury were 27 cases, about 1.21% of total. 4) The age group ranging from 21 to 40 years of age (working age group) and children (0 ~ 10 year of age) were apt to get traffic injury. 5) From 1960 to 1963, most of injury cases were male but after 1963, female patients have been increasing gradually. Statistical and clinical observation for 53 inpatients with genitourinary tract injury due to traffic accident during the period of 9 years and 8 months from Jan. 1960 to Aug. 1969 was performed and following results were obtained. 1) In sex distribution, patients were almost male 2) Patients in age group ranging from 21 to 30 were predominant, and children were quite rare. 3) From the view point of organ distribution, urethral injury was the most in number (25 cases, 47%), bladder injury was the second (15 cases, 28.30%) and renal injury was the 3rd (5 cases, 8.43%).

6.Clinical Observation on Cystoscopic Removal of Ureteral Stone using Woven Loop Stone Dislodger.

The cystoscopic manipulation was applied in 89 cases of ureteral Stone who visited the Department of Urology, Woo Sok University medical College Hospital during the period of 1 year 8 months From Jan, 1st 1970 to Aug. 31st, 1971, using woven loop stone dislodger and following results were obtained. 1) In 66 cases, ureteral stone was extracted successfully. 2) Among the 66 successful cases, 50 ureteral stones were extracted immediately and in 16 cases, stone was delivered spontaneously after cystoscopic manipulation, among them, 14 cases Within 5 days, 1 case after 26 days and 1 case after 2 months. 3) 2 successful cases were upper ureteral stone and 4 were middle ureteral stone and all of this 6 ureteral stone were extracted immediately. 4) The largest stone extracted was 11 mm X 14 mm in size. 5) No specific complications were found during or after cystoscopic manipulation.

7.Comparison of Propofol with Enflurane Anesthesia in Laparoscopic Cholecystectomy for the Change of Liver Function.

BACKGROUND: Recently, using propofol as intravenous anesthetic agent is increasing. And it is known that propofol has little effects on liver function even after long operation such as plastic surgery. But its effect on liver functon after hepatobilliary operation which may damage liver was not studied. Thus, authors carried out this study to evaluate the effect of propofol on liver function by comparing with enflurane in the patients who had laparoscopic cholecystectomy. METHODS: Patients's anesthesia records and hospital charts from January 1994 to June 1996 were anlaysed retrospectively. Three hundred and thirty three patients who had normal liver function preoperatively and had no complications during and after operation were selected. They were divided into two groups ; propofol group (n=191) and enflurane group (n=142). The preoperative values of aspartate aminotransferase (AST), alanine aminotransferase (ALT) and alkaline phosphatase (ALP) were compared with those of 1 and 3 days after operation. RESULTS: One day after operation, both propofol and enflurane group showed significant increase in AST and ALT (p<0.05) and decrease in ALP (p<0.05). But there was no difference between two groups. CONCLUSION: Concerned to liver function, propofol is as useful as enflurane to the patients who scheduled for laparoscopic cholecystectomy.

8.A Clinical Study of Splanchnic Nerve Block.

BACKGROUND: It is difficult to manage intractable pain from advanced carcinoma of the upper abdomen. One method used to control pain associated with these malignancies is to block the splanchnic nerve. We investigated that VAS (visual analogue scale) difference before and after splanchnic nerve block (SNB) and pain relief day. Also we studied relationship between VAS before SNB and pain relief day. METHODS: A rewiew of 70 patients who took splanchnic nerve block (SNB) from September 1994 to February 1998 was carried out to assess age, sex, primary diseases, pain sites, VAS before and after SNB, date of diagnosis, date of SNB, date of death and pain relief day, etc. RESULTS: Of 70 patients, 44 were males and the remaining 26 were females. The causes of pain were stomach cancer 28 (40%), pancreatic cancer 18 (25%), gall bladder cancer 7 (10%), hepatoma 6 (8.6%) respcectively. Average day from diagnosis to SNB was 272 and average day from diagnosis to death was 341. So, patients died on the average 69 days after they took the splanchnic nerve block in pain clinic. VAS average before SNB was 8.01 and VAS average after SNB was 3.64. Patients felt pain relief during 35 days after SNB. Pain relief day of patients who had lower VAS before SNB was longer than that of patients who had higher VAS before SNB. CONCLUSION: Early application of splanchnic nerve block will make the patients endure the cancer pain more easily.

9.A Case of Congenital Patent Urachus.

Urachal anomalies are very rare. Since Cabrolius' report of patent urachus in 1550, only 315 cases have been collected in the literature. Failure of the urachal lumen to close and completely obliterate causes one of several anomalies. These have been classified into four distinct entities by Blichert-Tott and Neilsen, Who also tabulated the incidence of each variant in the 315 reported cases : patent urachus , 48 per cent ; urachal cyst , 31 percent ; urachal sinus or alternating sinus, 18 per cent , and vesicourachal diverticulum, per cent . A brief discussion of embryology, symptoms , histopathology , diagnosis and treatment is made and a case of congenital patent urachus is reported here at Korea General Hospital.

10.Two Cases of Endometriosis at the site of Episiotomy Scar and Abdominal Scar following Cesarean Section.

The most frequent ectopic locations of endometriosis include, in descending order of frequency, the ovaries, uterine ligaments, the rectovaginal septum, and peritoneum covering the pelvic organs, that is, uterus, fallopian tubes, rectum, sigmoid colon, and bladder. Endometriosis at the site of episiotomy scar and abdominal scar following cesarean section are very rare conditions among the extrapelvic endometriosis. We have experienced two cases of endometriosis. One occurred from episiotomy scar and the other occurred from abdominal scar following cesarean section. The diagnosis of scar endometriosis is suggested if the symptoms are cyclic or in association with the patient's menstrual cycle. We can use ultrasonography, computerized tomography, magnetic resonance imaging to differentiate from other tumors. Surgical excision is the method of choice for diagnosis and treatment. We report the two cases with review of rare extrapelvic endometriosis.
