1.A Case of Scrotal Calcinosis Associated with Epidermal Cysts.

Scrotal calcinosis, consisting of solitary or multiple circumscribed calcium deposits within the dermis of the scrotum, is generally assumed to be one the idiopathic type of calcinosis. We present herein a case of serotal calcinosis associated with scrotal epidermal cysts in a 29-year-old male patient who had multiple, 2 to 6mm sized, yellowish or flesh colored nodules in the dermis, Histopathologically multiple ncdules showed intact epidermal cyzts, an epidermal cyst with a partialIy degenerated wall and surrounding foreign body reaction, and calcium deposits with foreign body reaction in the dermis without an epithelial lining around calcium deposit. Therefore, idiopathic scrotal calcinosis may repreeent the end stage of dystrophic calcificati.on associated with inflammation of the scrotal epidermal cysts.

2.Diffuse Neurofibromas: Clinicopathologic Analysis of 11 cases.

We reviewed surgical specimens from 11 patients with diffuse neurofibroma to define the specific clinicopathologic characteristics. Ten cases were cutaneous neurofibromas and one case was an uncommon gastrointestinal neurofibroma involving the rectum. The most frequent sites of involvement were the head and neck, especially the eyelids and the periorbital areas. They usually presented as a plaque-like elevation of the skin. They primarily occured in children and young adults and positive family histories of von Recklinghausen's neurofibromatosis were obtained in 45.4%. Pathologically, the involved skin & rectum were diffusely thickened by an infiltrative growing mass, showing proliferation of short fusiform cells in the uniform matrix of fine fibrillary collagen. The characteristic prominence of Wagner-Meissner bodies (45.4%) suggests they could be associated with pathogenesis of diffuse neurofibroma. On the basis of these findings, we could confirm diffuse neurofibroma to be a distinct form of neurofibroma.

4.Two Cases of Recurrent Herpes Zoster : Observation of the Herpes Virus by Electron Microscopy.

Herpes zoster is known to recur only rarely. However reports on the recurrence rates by various authors ranges from 0.l% to 8%. We have experienced 2 cases of recurrent herpes zoster in a 63-year-old female and a 62-year-old male who presented with typical clinical features and previous history of herpes zoster. They were suffering from breast cancer and Hodgkin's disease, respectively. By employing negative and positive staining methods of electron microscopy, typical herpes viral particles were demonstrated from the vesicular fluid.

6.Various clinical applications of the continuous buried suture method: non-incision double eyelid operation.

The double eyelid operation is one of the most common cosmetic surgery performed in orientals, and there are numerous studies about surgical procedures of that. In the case of appropriate candidates, the non-incision technique makes excellent cosmetic results, with the advantages of shorter operating time, less morbidity, less pain, no scarring and easy revision of shape. But, usually the non-incision technique is recommended in the cases with thin upper eyelid, small amount of orbital fat, less redundant skin of upper eyelid and in whom a wide fold is not desired so on. Therefore, the disadvantages of this method are that it cannot be applied to all patients. The postoperative complications are disappearance or faded fold, formation of cyst or lumps, asymmetry etc. and the incidence of these complications is slightly higher than those of the incisional method. In this study, the double eyelid operation using the continuous buried suture method was performed in 210 patients for formation of double eyelid from Jan. 1993 to Dec. 1995. In conclusion very good results can be obtained by the continuous buried suture method even in the cases of puffy upper eyelid. This method can make a double eyelid shape in the fashion the patient wants and can be a new indication for correction of unilateral double eyelid. This method is safe, satisfactory and take shorter postoperative recovery time and it can reduce the incidence of postoperative complications.

7.Electron Microscopic Observation of Treponema pallidum in Syphilitic Chancres and Syphilids.

Electron mlactimpic studies were performed on three primary and two early secondary syphilitic lesions from three patients. AZ of these patients had both secondary syphilitic lesions and chancres. There was no difference in the morphology and location of treponemes in these patients as compared to those who had only a chancre or only a syphilid reported earlier. This study revealed that Treponema pallida were principally located in the intercellular space of keratinocytes and exhacellular space of the dermis. Furthermore, these organisms were observed within histiocytes, endothelial cells, fibroblasts, lymphocytes and even plasma cells. The plasma cells had well developed endoplasmic reticulum, and in some, dilatation of the endoplasmic reticulum was noted. Many t mponemes were demonstrated in and around blood vessels. The vascular endothelial cells were proliferating and the basement membranes were thick.
