Decreased sensitivity of N. gonorrhoeae to many existing antibiotics and the emergence and increase of penicillinase producing and of spectinomycin resistant N. gonorrhoeae necessitate intensive efforts to study on resistance. Male patients with uncomplicated gonococcal urethritis treated during the year 1990 at the VD clinic of Choong-ku Public Health Center in Seoul were included in this study. A total of 85 strains, 44 strains of non-PPNG and 41 strains of PPNG isolated from the patients were studied to evaluate their sensitivities to ampicillin, penicillin, cefoxitin, kanamycin, tetracycline, erythromycin, spectinomycin and cotrimazole. The results are summerized as follw: In non-PPNG strains, the range of MICs. MIC50 and MIC90 for both ampicillin and penicillin were 0.125-32, 2 and 16 mcg/ml respectively. For cefoxitin, they were 0.125-8, s and 16 mcg/ml respectively, and for kanamycin, 8--<64, 32 and 64 mcg/ml, respectively. For tetracycline, they were 1-64, 8 and 32 mcg/ml, while for erythromycin, 0.06-16, 2 and 8 mcg/ml respectively. For spectinomycin, they were 4--<64, 32 and 64 mcg/ml and for cotrimazole, 5--<160, 40 and 160 mcg/ml respectively. In the case of PPNG, the range of MICS, MIC50 and MIC90 for ampicillin were 8--<128, 32 and 128 mcg/ml respectively but for penicillin, 8--<128, 64 and 128 mcg/ml respectively. And for cefoxitin, they were 0.5-32, 4 and 16 mcg/ml while for kanamycin, 16--<64, 64 and >64 mcg/ml respectively. For tetracycline, the values were 2-64, 16 and 32 mcg/ml, and for erythromycin, they were 0.5-16, 4 and 8 mcg/ml respectively. for spectinomycin, they were 8--<64, 32, and 64 mcg/ml and finally for cotrimazole, 5--<160, 80 and 160 mcg/ml respectively. From these results, it is concluded that the MICs of both PPNG and non-PPNG were about 2 folds higher than the results of sensitivity tests at the same instituition in 1985. 2) The prevalence of PPNG among 35 gonorrhoeae patients who received treatment before the visit was 60% (21/35), while that among the patients without previous treatment was 40% (20/50). 3) In the cases of ampicillin, penicillin and cotrimazole, their MIC values were found to be significantly higher for the strains isolated from the patients with previous treatment history than those without previous treatment (p<0.05). 4) In non-PPNG strains, a significant positive correlation is found between the sensitivities to most pairs of ampicillin, penicillin, cefoxitin, kanamycin, tetracycline, erythromycin and spectinomycin. However this excludes some pairs such as : tetracycline-cefoxitin, erythromycin-penicillin, erythromycin-cefoxitin, spectinomycin-penicillin, and spectinomycincefoxitin (p<0.05). 5) In PPNG strains, a significant positive correlation is also found between the sensitivities to most pairs of ampicillin, penicillin, cefoxitin, tetracycline, erythromycin and spectinomycin. However, the exceptional cases include tetracycline-cefoxitin, erythromyoinpenicillin, erythromycin-cefoxitin, spectinomycin-penicillin, and spectinomycin-cefoxitin pairs. In addition, the sensitivity to kanamycin was found to be strongly correlated with that to tetracycline, erytheromycin and spectinomycin (p<0.05). Therefore it is essential for the management fo gonorrhoeae, together with a correct diagnosis, to use currently most effective treatment regimens which would also prevent the emergence of resistant strains. It is also suggested that when a treatment is failed, to use the durg which do not show correlation in sensitivities on re-treatment.

PURPOSE: This study aimed to identify the alcohol drinking pattern of parents and to analyze drinking related factors in problem drinking among adolescent's parents. METHOD: This study was conducted through a structured questioning from November 5th to 22th in 2002. Those are 2,522 parents of the students of the 1st and 2nd grade who were selected randomly among 26 middle and high schools located in Socho-gu, Seoul. Data was analyzed using SAS program that included descriptive statistics, Chi-square test, and multiple logistic regression analysis. RESULT: 1. For frequency of drinking, 33.1% of the subjects responded to drink two-four times a month, 26.0% drink more than two times a week. 2. For the amount of drinking, 29.6% drink one or two glasses while 9.4% drink over ten glasses at a time. 3. The mean AUDIT score was 8.27+/-7.02(score of range: 0-40). 4. Based on AUDIT score, drinkers with less than eight point from AUDIT(normal group) were 55.1%, from more than eight point to less than twelve point from AUDIT(problematic drinker) were 16.3%, and more than twelve point from AUDIT(alcohol abuse and dependency) were 28.6%. 5. Based on more than twelve point from AUDIT, female(mother), high school and, have religion, housekeeper were the significantly higher score than counter parts. CONCLUSION: Alcohol drinking pattern and level of drinking of their parents links to their children's drinking and results in family, social, and national loss. Active prevention is needed. Specially, pertinent education about drinking and public education for mothers who are housekeepers should be carried out with concentrated intervention programs for the problematic drinker so alcohol abuse and dependency can be reduced.

Background: Recent studies have shown that oral administration of probiotics may improve the immune imbalance caused by dysbiosis of the gut microbiome in atopic dermatitis (AD). This study aimed to investigate the clinical and immunological effects of Lactobacillus pentosus in children with mild to moderate AD. Methods: Children aged 2–13 years with AD were randomized to receive either 1.0 × 1010 colony-forming units of L. pentosus or placebo, daily, for 12 weeks. The clinical severity of AD and transepidermal water loss were evaluated. Blood eosinophil counts, serum total immunoglobulin E (IgE), and cytokine levels were measured. The diversity and composition of the gut microbiota were also analyzed. Results: Eighty-two children were recruited, and 41 were assigned to the probiotics intervention group. The mean scoring of atopic dermatitis (SCORAD) indices at baseline were 30.4 and 34.3 for the probiotics and placebo groups, respectively. At week 12, the mean indices were 23.6 and 23.1 for the probiotics and placebo groups, respectively. Clinical severity decreased significantly over time in both groups, with no significant difference between the two groups. In both groups, there were no significant differences in cytokine levels, microbial diversity, or the relative abundance of the gut microbiota at week 12 compared with the corresponding baseline values. The mean subjective scores of SCORAD indices after intervention for the probiotics group were significantly lower than those for the placebo group in IgE sensitized AD (P = 0.019). Conclusion Our results show improved symptoms in the probiotics and placebo groups, and we could not find additional effects of L. pentosus in AD. However, the mean subjective scores of SCORAD indices for the probiotics group are significantly improved compared with those for the placebo group in allergen-sensitized AD.

PURPOSE: To develop a Critical Thinking Skill Test for Nursing Students. METHODS: The construct concepts were drawn from a literature review and in-depth interviews with hospital nurses and surveys were conducted among students (n=607) from nursing colleges. The data were collected from September 13 to November 23, 2012 and analyzed using the SAS program, 9.2 version. The KR 20 coefficient for reliability, difficulty index, discrimination index, item-total correlation and known group technique for validity were performed. RESULTS: Four domains and 27 skills were identified and 35 multiple choice items were developed. Thirty multiple choice items which had scores higher than .80 on the content validity index were selected for the pre test. From the analysis of the pre test data, a modified 30 items were selected for the main test. In the main test, the KR 20 coefficient was .70 and Corrected Item-Total Correlations range was .11-.38. There was a statistically significant difference between two academic systems (p=.001). CONCLUSION: The developed instrument is the first critical thinking skill test reflecting nursing perspectives in hospital settings and is expected to be utilized as a tool which contributes to improvement of the critical thinking ability of nursing students.

Many displaced North Koreans (NK) are living in South Korea (SK); however nutrition research with the displaced NK is limited. This study examined food and nutrient consumption status of displaced NK children (6-18 year-old) currently living in SK. A total of 154 children were recruited, and a pre-tested dietary behavior questionnaire, food frequency questionnaire, 24-hr recall method were used. Sex- and age-matched SK children (n = 462) randomly selected from 2009-2010 Korea National Health and Nutrition Examination Survey were used as the comparison group. This study found that more NK children skipped breakfast (37%) and dinner (11%), and ate breakfast (38.8%) and dinner (18.2%) without family members than SK children. Many NK children reported that they rarely ate bread, rice cake, hamburger, pizza, fried food, candy. NK children consumed significantly less energy and nutrients (except calcium) and obtained more energy from fat and protein than SK children. Overall index of nutrient quality in NK children, however, was generally good. Length of stay in SK and breakfast skipping rates were significantly associated with lower diet quality. Therefore, nutrition education with displaced NK children should target those who recently came to SK. How to incorporate "new" foods, generally high in energy, sugar, or fat, in healthy ways and importance of breakfast should be emphasized. The growth patterns of the displaced NK children who were born and raised in food-deprived environments and will grow in food-affluent environments of SK should be monitored for health promotion of the NK children and for nutrition policy of the future united Korea.

PURPOSE: This study was done to develop a Web-based preconception health promotion program to improve preconception health promotion awareness, perceived benefits, perceived self-efficacy and health behavior, and to reduce perceived barriers in couples about to be married. METHODS: Participants were assigned to the experimental group (n=26 couples) or control group (n=25 couples). This program was comprised of a six hour Web-based lecture series which was given to the experimental group over a four week period. RESULTS: Results for the two groups showed: 1) a significant difference in preconception health promotion awareness in men (self-perception: z=-3.62, p <.001 content: F=18.49, p <.001) and in women (self-perception: z=-2.89, p =.004 content F=36.93, p <.001), 2) a significant difference in perceived benefits in men (z=-3.09, p =.002), and in women (F=6.38, p =.004), 3) a significant difference in perceived barriers in men (F=3.64, p =.030), and in women (F=11.36, p <.001), 4) a significant difference in perceived self-efficacy in men (F=12.16, p <.001), but no significant difference in women (F=1.93, p =.166), 5) a significant difference in health promoting behavior in men (F=16.31, p <.001), and in women (F=10.99, p =.001). CONCLUSION: The results of this study show that the program is an effective nursing intervention in couples about to be married. Therefore, this program can be useful in encouraging health-promoting behavior for couples about to be married.

PURPOSE: The purpose of this study was 1) to examine levels of awareness on preconception care and pregnancy, and reproductive health promoting behavior among pre-honeymooners; and 2) to explore the relationship between awareness levels of preconception care and pregnancy, and reproductive health promoting behavior. METHODS: This study was a correlation design with a total of 134 participants (67 couples of pre-honeymooners). Data were collected with questionnaire. RESULTS: Mean score of self-perception of awareness of preconception care and pregnancy was 2.82 out of 4. Content awareness level was 6.83 out of 13, and reproductive health promoting behavior score was 3.02 out of 4, indicating low to mid-level. Women reported higher scores in all variables than those in men. Self-perception awareness and content awareness of care and pregnancy had a weak positive correlation (r=.18, p=.038). Reproductive health promoting behavior was positively related to self-perception awareness of care and pregnancy (r=.33, p<.001) and content awareness of care and pregnancy (r=.23, p=.008). CONCLUSION: Based on this results, a variety of nursing intervention strategies may need to promote reproductive health behavior such as raising awareness of the preconception care and pregnancy.

PURPOSE: The purpose of this study was to construct and test a structural equation modeling on the reproductive health behavior of single women with sexual experiences. This study employed Ajzen's Theory of Planned Behavior (TPB). METHODS: The data were collected after receipt of consent from 250 single women with sexual experiences, and analyzed using SPSS 18.0 and AMOS 18.0. RESULTS: Model fit indices for the hypothetical model were suitable for the recommended level: x²=362.407, RMR=0.065, RMSEA=0.070, GFI=0.867. TLI=0.927, CFI=0.938, IFI=0.939, and x²/dF=2.237. Intention showed direct effect with the biggest effect being on reproductive health behavior. Attitude, subjective norm and perceived behavioral control were found to have a direct effect on intention. Among them, perceived behavioral control revealed the largest influence. CONCLUSION: This study suggests that the TPB is a suitable model in explaining the reproductive health behavior of single women with sexual experience. Strategic plans for educational and intervention programs should be aimed to encourage single women to engage in reproductive health behavior.

Intravenous anesthesia, using sedative for the rapid loss of consciousness and analgesic for the potent and short-acting effect, has been widely done. So tbis study is aimed to see if propofol-fentanyl are effective enough for extracorporeal shock wave lithotripsy(ESWL) used for outpatient and how it will effects on cardiovascular system. Above all, 45 patients were selected randomly and their blood pressure(BP), heart rate(HR), respiratory rate(RR) and arterial oxygen saturation(SaO2) were measured shortly before giving propofol and fentanyl. Then, after giving propofol and fentanyl, BP,HR, RR and SaO2 were measured as the adequate depth of anesthesia. And after 5 to 10 minutes, ESWL was done. Involuntary movements and pain were checked as additional depth of anesthesia. With the time, HR and SaO2 showed statistically significant decrease with no clnical meanings. And there was no special treatment for involuntary movements founded in 8 patients but 13 patients suffering from pain were given additional fentanyl. In conclusion, intravenous anesthesia with propofol and fentanyl works effectively for ESWL. Therefore if during ESWL, cardiopulmonary functions are monitored carefully, intravenous anesthesia with propofol and fentanyl will be an effective and safe anesthesia.
