Ascariasis is probably the most common helminthic infestation of man, but it seldom causes severe illness. Pathologic conditions of Ascaris may be caused by adult worms, eggs or larvae. We describe a case of Ascaris egg granulomas that were found incidentally on the surface of the liver in a 75-year-old woman who had undergone a segmentectomy for an intrahepatic stone. Grossly, there were several yellowish calcific nodules of 0.4 cm in diameter on the lateral surface of the left lobe of the liver. Microscopically, the lesions were located in the hepatic capsule and consisted of fibrocalific nodules with many eggs. The eggs were round to oval, thick-shelled and measured 50~75x30~50 um. Most of the morphologically preserved eggs were fertilized eggs, but they had smooth shells without external protein coats. This case is of interest for the unusual location of the lesion, the presence of eggs without mammillation, and the association with the intrahepatic stone.

Brenner tumors constitute about l.5~2.5% of all primary ovarian neoplasms and are almost always benign. It appears to derive from the surface epithelium of the ovary which undergoes metap1asia to form the urothelial-like components. we experienced a case of malignant Brenner tumor with adenocarcinoma and squamous cell carcinoma patterns in a 57-year-old woman. It was partly cystic tumor and contained a 4cm-sized gray yellow, lobulated or papillary solid mass, projecting from the cystic wall. Ultrastructurally, the solid mass was composed of malignant urothelial-like cells with focal glandular differentiation.

We report a case of angiosarcoma of the jejunum in a 65-year-old female. This angiosarcoma developed 20 years after postoperative irradiation for cervical carcinoma of the uterus. Grossly, the resected jejunum showed a 8 cm long segmental hemorrhagic lesion with multiple, small hemorrhagic nodules or cysts on its serosa and mesentery. Microscopically, the wall of jejunum was infiltrated by epithelioid or polygonal tumor cells arranged in solid nests or lining irregular vascular spaces. The case is of interest that the tumor occurred in the very unusal site for angiosarcoma and the patient had a past history of irradiation for uterine cervical carcinoma 20 years ago. In addition, the resected intestine showed histologic changes of chronic irradiation effect. Therefore, this case supports the view that there is cause and effect relationship between irradiation adn angiosarcoma.

Dental porcelain should have adequate strength and similar esthetics as natural teeth. Recently esthetics has become a high priority in clinical dentistry. Thus the evaluation of color stability of dental porcelain has become an important part in dental research. The purpose of this study was to evaluate the difference in color stability of 3 types of porcelain (Vintage, VMK 95, CERAMCO II). The porcelain were divided into groups by 2 types of treatment (Glazing or non-glazing and thermocycling or non-thermocycling). The porcelain wee dyed with methylene blue and distilled water, and then the color stability was evaluated with a spectrophotometer. The results were as follow ; 1. The color change of porcelain increased in the order of VMK 95, Vintage, CERAMCO II. But there was no significant difference. 2. The color change decreased in the glazing group compared to the non-glazing group(p<0.05). 3. The color change increased after thermocycling But there was no significant difference. 4. The color change according to dye method was significantly larger with methylene blue copared to distilled water(P<0.05).

Non-melanoma skin cancer in a psoriasis patient is known to be associated with the treatments for psoriasis such as long-term use of PUVA and cyclosporine rather than the psoriasis itself. We report here on a case of double primary squamous cell carcinoma and acanthosis on a patient who had no history of oral steroid and cyclosporine management. A 44 years old male patient presented for ulcerative skin lesion on both soles. He underwent an operation of wide excision and reconstruction for two separated malignant lesions with using a free flap and skin graft, respectively. Another skin lesion was found on his finger during the follow-up period and excisional biopsy was performed. No evidence of recurrence or metastasis of the squamous carcinoma was found during 2 years follow-up. The excisional biopsy done on his finger was revealed to be acanthosis and no further surgical intervention was needed. This case has an implication that physicians should consider the possibility of multiple skin cancers in a psoriasis patient irrespective of having received immunosuppression therapy, and physicians should try to make an early diagnosis to minimize the patient's morbidity and mortality.

Background: Recent studies have shown that oral administration of probiotics may improve the immune imbalance caused by dysbiosis of the gut microbiome in atopic dermatitis (AD). This study aimed to investigate the clinical and immunological effects of Lactobacillus pentosus in children with mild to moderate AD. Methods: Children aged 2–13 years with AD were randomized to receive either 1.0 × 1010 colony-forming units of L. pentosus or placebo, daily, for 12 weeks. The clinical severity of AD and transepidermal water loss were evaluated. Blood eosinophil counts, serum total immunoglobulin E (IgE), and cytokine levels were measured. The diversity and composition of the gut microbiota were also analyzed. Results: Eighty-two children were recruited, and 41 were assigned to the probiotics intervention group. The mean scoring of atopic dermatitis (SCORAD) indices at baseline were 30.4 and 34.3 for the probiotics and placebo groups, respectively. At week 12, the mean indices were 23.6 and 23.1 for the probiotics and placebo groups, respectively. Clinical severity decreased significantly over time in both groups, with no significant difference between the two groups. In both groups, there were no significant differences in cytokine levels, microbial diversity, or the relative abundance of the gut microbiota at week 12 compared with the corresponding baseline values. The mean subjective scores of SCORAD indices after intervention for the probiotics group were significantly lower than those for the placebo group in IgE sensitized AD (P = 0.019). Conclusion Our results show improved symptoms in the probiotics and placebo groups, and we could not find additional effects of L. pentosus in AD. However, the mean subjective scores of SCORAD indices for the probiotics group are significantly improved compared with those for the placebo group in allergen-sensitized AD.

BACKGROUND: Lidocaine sprayed down the endotracheal tube (ETT) before extubation and during ETT removal was reported to prevent increases in blood pressure and heart rate during and after extubation. We hypothesized that lidocaine administered via this route would suppress the cough reflex associated with tracheal extubation. METHODS: Eighty-five patients requiring intubation in the supine position were studied. All patients received a standardized anesthetic protocol. At the end of surgery, the patients were randomly divided into three groups and given no drug (group 1-control), given 1 mg/kg 2% lidocaine sprayed down the ETT 5 minutes prior to extubation (group 2), or given intravenous lidocaine (IVL) 1 mg/kg 3 minutes prior to extubation (group 3). Extubation was performed when the patients met the criteria of extubation. Number of coughs was recorded from 5 minutes before until 5 minutes after extubation. Each value was compared among the three groups. RESULTS: Seventy-five patients completed this study (n = 25 for each group). The incidence of patients experiencing coughing was decreased in group 2 (P = 0.03), compared with group 3 or the control. Number of coughs per patient was significantly less in group 2 (P = 0.00). IVL did not significantly diminish the incidence or the amount of coughing compared with the control. CONCLUSIONS: Lidocaine sprayed down the ETT suppresses cough reflex and is more effective than IVL in blunting the cough reflex. This study indicates that lidocaine sprayed down the ETT has a topical anesthetic effect suppressing the cough reflex.
