1.The Impact of COVID-19 Stress, Interpersonal Relations, and Information Literacy on the Adaptation of Nursing Students to College Life

Purpose:
Rapid changes have occurred in the educational environment of colleges since the outbreak of COVID-19. This study was conducted to determine the factors influencing college life adaptation for nursing students.
Methods:
The participants included 124 nursing students. A self-administered online questionnaire, available from March 30 to April 10, 2022, was used for data collection. The online questionnaires included COVID-19 stress, interpersonal relations, information literacy, and college life adaptation. Statistical analysis was performed using t-test, ANOVA, Pearson's correlation coefficient, and multiple regression analysis.
Results:
The average score for adapting to college life was 3.32±0.58. Significant differences in college life adaptation were found based on the number of exercise days per week (F=3.62, p=.015), regular daily routine (t=-3.41, p<.001), amount of sleep (t=-3.61, p<.001), sleep quality (t=-3.90, p<.001), and diagnostic self evaluation (F=19.55, p<.001). Factors that influenced college life adaptation included interpersonal relations (β=.36, p<.001), diagnostic self evaluation (good) (β=.48, p<.001), diagnostic self evaluation (fair) (β=.38, p=.005), and COVID-19 stress (β=-.11, p=.027), which explained 38.0% of the college life adaptation.
Conclusion
Development of a program that integrates interpersonal relations, diagnostic self- evaluation, and COVID-19 stress for nursing students is recommended to improve college life adaptation.
2.The Characteristics of Fetus with Neural Tube Defect Accompanied by Club Foot in Prenatal Ultrasonography.

OBJECTIVE: The objective of this study is to determine whether the club foot of the affected fetus is related to the location, size, and rupture state of the spinal neural tube defect lesion. METHODS: Ultrasound and medical records of 21 fetuses with spinal neural tube defect were reviewed. At the time of diagnosis 7 fetuses had clubfoot and 14 fetuses didn't have. All the fetuses with spinal neural tube defect were divided into two groups, one with clubfoot fetuses, and the others without clubfoot. RESULTS: The mean lesion size of the clubfoot group was significantly larger than normal foot group (4.1+/-1.22 cm vs 2.1+/-1.59 cm). And the level of lesion is significantly higher in club foot group. But the rate of sac rupture is lower in club foot group. CONCLUSION: The fetuses with clubfoot detected at the time of diagnosis had larger size and higher level of lesion.

3.Effects of Adolescent Temperament and Parent-child Attachment on Depression.

PURPOSE: This study was done to examine levels of temperament, parent-child attachment and depression of adolescents and verify its effects. METHODS: Surveys were conducted with 500 students from two middle schools, one located in Seoul and one in Gyeonggi Province. Adolescent temperament was measured using the Junior Temperament and Character Inventory, parent-child attachment using the Revised Inventory of Parent and Peer Attachment and depression using the Children's Depression Inventory. Cluster, t-test, correlation and logistic regression were used for data analysis. RESULTS: Characteristics of temperament were classified into 2 groups. The 'Adaptation vulnerable group' showed high Harm Avoidance and the 'Adaptation protective group' showed high Reward Dependence, and Patience. The 'Adaptation vulnerable group' showed lower attachment and higher depression than the 'Adaptation protective group'. Novelty Seeking and Harm Avoidance correlated positively with depression and negatively with attachment. Students with higher levels of attachment reported lower levels of depression. The logistic regression analysis showed that the 'Adaptation vulnerable group' was 2.16 times more likely to be affected by depression than 'Adaptation protective group'. CONCLUSION: Results of this study can be used to develop depression intervention programs for adolescent psychological health and provide encouragement in the development of parent-child attachment.

4.Reliability and Validity of the Korean Version of the Helicopter Parenting Scale.

PURPOSE: Helicopter parenting is emerging in parenting as a way of rearing adolescents and adult children. The aims of this study were to develop a Korean version of the Helicopter Parenting and Autonomy Supportive Behaviors Scale (HPASB). METHODS: The HPASB questionnaire items were translated into Korean and reviewed by experts and Content Validity Index (CVI) in a preliminary study with 10 university students. During September and October, 2014 data were collected from 229 nursing students from five different universities in different locations. For data analysis, SPSS 21.0 statistics which included exploratory factor analysis, t-test, one-way ANOVA were utilized. RESULTS: Content validity was over CVI .8. The 6 factors of K-HPASB were extracted and accounted for 59.30% of variance. The Cronbach's alpha coefficient was .71 indicating high reliability. CONCLUSION: The Korean version of the HPASB was identified as a scale with a high degree of validity and reliability. The results of this study provide a valuable scale which can be useful in the study of parenting as a way of rearing adolescents and adult children in Korea. To enhance the positive aspect of helicopter parenting, we suggest the development of intervention programs on parenting.

5.An Evolutionary Concept Analysis of Helicopter Parenting.

PURPOSE: Helicopter parenting is an emerging concept as a way of rearing adolescents and adult children. However, helicopter parenting from a nursing perspective has not been elucidated. Therefore, we undertook a concept analysis to understand the attributes, antecedents and consequences of helicopter parenting in the context of nursing. METHODS: Using Rodgers' evolutionary concept analysis, we analyzed literature on helicopter parenting to discover critical attributes, antecedents, and consequences of this phenomenon. Data were collected from seven electronic search engines. Twelve studies matching inclusion criteria were reviewed RESULTS: Three core attributes of helicopter parenting were hovering, highly deep involvement, and proxy decision making. The antecedents and consequences were retrieved from three important domains including social, parent, and child aspects. Surrogate terms were black hawk, hummingbird, and hovercraft parenting, and related terms were stealth fighter and Kamikaze parenting. CONCLUSION: Based on the results of this study, helicopter parenting has both positive and negative effects on both children and parents. To enhance the positive effects, measurement tools for helicopter parenting and nursing interventions on parenting need to be developed.

6.The Efficacy of Ultrasound Diagnosis of a Partial Tendon Injury by Emergency Doctors: Using a Swine Model.

PURPOSE: To evaluate the efficacy of ultrasound for the detection of a partial tendon injury of the dorsum of the hand by emergency doctors using a swine model. METHODS: Fifteen swine feet were used for the study. Two tendons from each foot were examined using a 10 MHz hocky stick scan head. A 0%, 10%, 50% and 90% incision was made in each extensor tendon at the proximal site of the skin incision. Four emergency physicians and five senior emergency residents were blinded to the injuries and evaluated the samples for a partial tendon rupture. RESULTS: The difference between the results for the emergency physicians and the emergency senior residents were not statistically significant.The diagnostic sensitivity for all participants was more than 10%; more than a 50% tendon injury was identified in 69.2% and 77.0%, respectively. The specificity was 51.4% and 53.5% for each case. The sensitivity and specificity showed no significant differences. (sensitivity p=0.243, specificity p=0.992) CONCLUSION: The results of this study confirm that ultrasound was useful in the detection of a partial tendon rupture by emergency doctors. The findings of this study have implications for the training of emergency room doctors.

7.A Case of Evans Syndrome during Valproate Therapy.

We report the first case of immune mediated thrombocytopenia and concurrent hemolytic anemia (Evans syndrome) that developed during valproic acid (VPA) therapy. The 6 years-old girl had been stable with VPA therapy for seizure disorder for 5 years. She was admitted due to purpura and petechiae on both legs. Her blood test was only significant for thrombocytopenia with normal hemoglobin level. Her thrombocytopenia was not caused by VPA toxicity, because of positive anti-platelet antibody. Thrombocytopenia was improved after steroid and intravenous globulin. However, hemoglobin was shown gradual drop with increased reticulocytes counts, high level of lactic dehydrogenase, and a positive direct Coombs test. She had a complete clinical recovery with negative anti-platelet antibody and direct Coombs test after discontinuation of VPA. VPA has a lot of side effects including hematologic toxicity, although this drug is used for seizure disorders. This report is the first Evans syndrome due to VPA, even though VPA induced pure red cell aplasia with positive anti-nuclear antibody or thrombocytopenia by direct toxicity.

8.Korean Pharmacological Treatment Guideline for Child-Adolescent Depression: The Efficacy and Choice of Antidepressant.

OBJECTIVE: The purpose of this guideline was to suggest recommendations for appropriate use of antidepressants in the child-adolescent depression. The differences of efficacy among antidepressants were evaluated. METHODS: Four kinds of reliable guidelines for the treatments of child-adolescent depression had been selected, and evidences and recommendations were extracted by the executive committee under the peer review. All the process was applied to the Manual for Guideline Adaptation version 2.0 by National Evidence-based Healthcare Collaborating Agency. RESULTS: Selective serotonin reuptake inhibitor (SSRI) could be considered for the treatments of moderate to severe child-adolescent depression. Among SSRIs, fluoxetine, sertraline, and escitalopram were recommended as having antidepressant efficacy compared with placebo, while paroxetine, venlafaxine, and tricyclic antidepressant were not recommended owing to lack of evidence. Another recommendation was to use combined treatment with cognitive behavioral therapy. CONCLUSION: This guideline, which was made through former and strict process of guideline adaptation, would contribute toward improving the quality of child-adolescent depression treatment by providing useful recommendations for the choice of antidepressant.

The aim of this study was to investigate the clinical characteristics of acute hepatitis A during a recent outbreak in Korea. Data of patients diagnosed with acute hepatitis A from 2007 to 2009 were collected from 21 tertiary hospitals retrospectively. Their demographic, clinical, and serological characteristics and their clinical outcomes were analyzed. A total of 4,218 patients (mean age 33.3 yr) were included. The median duration of admission was 9 days. The mean of the highest ALT level was 2,963 IU/L, total bilirubin was 7.3 mg/dL, prothrombin time INR was 1.3. HBsAg was positive in 3.7%, and anti-HCV positive in 0.7%. Renal insufficiency occurred in 2.7%, hepatic failure in 0.9%, relapsing hepatitis in 0.7%, and cholestatic hepatitis in 1.9% of the patients. Nineteen patients (0.45%) died or were transplanted. Complications of renal failure or prolonged cholestasis were more frequent in patients older than 30 yr. In conclusion, most patients with acute hepatitis A recover uneventfully, however, complication rates are higher in patients older than 30 yr than younger patients. Preventive strategies including universal vaccination in infants and active immunization of hepatitis A to adult population should be considered for prevention of community-wide outbreaks of hepatitis A in Korea.

BACKGROUND/AIMS: The meaning of specialized intestinal metaplasia (SIM) in the diagnosis of Barrett's esophagus (BE) is not clear. This study was designed to determine the clinical significance of SIM in the diagnosis of Barrett's esophagus. MATERIALS AND METHODS: Biopsies were taken from 601 subjects with endoscopically suspected columnar-lined esophagus. Under light microscopy with Alcian-blue stain, SIM was identified. Demographic characteristics, gastroesophageal (GE) reflux symptoms and endoscopic findings were compared between the SIM-present group and the SIM-absent group. RESULTS: Among 601 subjects, 184 (30.6%) were confirmed by pathology to have SIM. Age over 40 years (P<0.001) and a medication history of proton pump inhibitor or H2 blocker were found more frequently in the SIM-present group (P=0.01) than in the SIM-absent group. Any of 7 GE reflux symptoms (heartburn, acid regurgitation, chest pain, hoarseness, globus sensation, cough and epigastric soreness) were more frequent in the SIM-present group than SIM-absent group (P<0.001). Specifically, heartburn, chest pain and cough were significantly more common in the SIM-present group. There was no clinically significant difference associated with endoscopic findings or other clinical characteristics. CONCLUSIONS: When subjects with endoscopically suspected BE are analyzed based on the presence or absence of SIM, the SIM-present group was significantly associated with GE reflux symptoms suggestive of frequent GE reflux. However, the presence of SIM did not correlate with endoscopic findings.
