1.A Study on the Relationship between the Awareness of Blood Program and the Blood Donation Experience.

In this study, the relationship between the awareness of blood program and the blood donation experience has been studied to know whether they corelate or not. The awareness of blood program was divided into 4 areas of knowledge and measured: the eligibilityt of blood donation, knowledge about blood, blood donation program, and general facts about blood donation. The experience of blood donation was divided into 3 groups: the registered donor, the non-registered donor, and the nondonor. The result in this study shows that the awareness of blood program is related with 3 factors, such as the eligibility of blood donation, blood donation program, and general facts about blnation. The difference in this case is significant at 0.05 level. However the knowledge about the blood is not corelated with blood donation experience, but only with sex(P<0.05). These results suggest that stable blood supply should be realized by securing new donors and by estsablishing of fdonor registration system. This is realized by the stir up the level of awareness and interest of blood program through strengthening public relations.

3.A Case of Microscopic Polyangiitis with Acute Myocardial Infarction.

Microscopic polyangiitis is a systemic small-vessel vasculitis, which may involve multiple organs, but cardiac involvement is relatively rare. We report a case of microscopic polyangiitis with multiple organ involvement, in which myocardial infarction was the early manifestation of the disease. A 53-year-old man presented with sudden papulovesicular eruptions and swellings on the face, posterior neck, dorsa of both hands and fingers, and with diffuse erythematous patches on the back. He had suffered from renal dysfunction, arthralgia, and hypertension for more than 8 years. He had been admitted to the department of cardiology for acute myocardial infarction and had suffered from recurrent gastrointestinal bleeding, renal failure, acute pancreatitis and sepsis during the admission. Histopathologically, small-vessel leukocytoclastic vasculitis with out granuloma was seen. Direct immunofluorescence showed no immune deposite. A high serum level of P-ANCA was detected by ELISA.

4.Classification of the Calcaneal Fracture Based on Computed Tomography

The os calcis is most frequently fractured than any other tarsal bone and the displaced intraarticular fractures account for 60-75% of them. Because of complex contour of calcaneous, it is difficult to evaluate the pattern of fracture exactly by conventional roentgenograms. But recently, more detailed understanding of fracture pattern can be possible with CT scan, which is thought to be one of the most important factor in deciding method of treatment and predicting prognosis. From Jan. 1992 to Dec. 1992, intraarticular calcaneal fractures of 31 cases treated in Kyungpook university hospital were analysed preoperatively with CT scan and classified by Sanders classification system. The results were obtained as follows: 16 cases of 31 cases were classified as type II and this type were sudivided as IIA in 7 cases, IIb in 4 cases, IIC in 5 cases. Type III fractures were found in 9 cases and subdivided as III AB in 3 cases, III BC in 4 cases IIIAC in 2 cases. Type IV fractures were found in 6 cases.

Intradural schwannoma accompanied with torsion is rare. Intradural lumbar schwannoma was found in a 63-year-old man presenting with right sacral pain radiating to the lower extremity. This mass showed minimal enhancement on MRI. The tumor's location changed each time on MRI and CT-myelography. The patient underwent surgical resection and the cauda equina, linked to this schwannoma, was severely twisted.

7.Epidural Butorphanol Reduces the Side Effects from Epidural Morphine after Cesarean Section.

BACKGROUND: Epidural morphine has side effects, especially pruritus, nausea, and vomiting. Butorphanol has been added in studies to reduce these side effects in post cesarean patients. The purpose of this study was to evaluate the side effects and analgesic efficacy when a combination of epidural morphine and butorphanol was administered in patients having combined local anesthetic and opioid epidural infusion. METHODS: Sixty patients having epidural anesthesia for cesarean section were randomly divided into two groups. Group M (n=30) received a bolus of 0.25% bupivacaine 4 ml, morphine 2 mg, and saline 0.75 ml, whereas group B (n=30) received a bolus of 0.25% bupivacaine 4 ml, morphine 2 mg, and butorphanol 1.5 mg (0.75 ml). Continuous epidural infusion was done by Two-day Infusor containing either 0.25% bupivacaine 75 ml, morphine 5 mg, and saline 20 ml in group M or 0.25% bupivacaine 75 ml, morphine 5 mg, butorphanol 4 mg (2 ml), and saline 18 ml in group B. We compared the side effect and analgesic effect of group M to those of group B for 2 days. RESULTS: The incidence of pruritus and vomiting were reduced significantly in group B (p<0.05). There were no significant differences between both groups in the incidence of nausea or other side effects as well as no differences in analgesic effect. CONCLUSIONS: We conclude that the addition of butorphanol to morphine in combined local anesthetic and opioid epidural infusion for postoperative analgesia decreases the occurrence of pruritus and vomiting without significant increase of other side effects and adverse effect on analgesia.

8.Changes in ventricular repolarization after balloon valvuloplasty in patients with pulmonary stenosis.

BACKGROUND: Alterations in ventricular loading conditions lead to changes in action potential duration via mechanoelectrical feedback. A decrease in load immediately leads to prolongation of repolarization. QT interval and QT dispersion were measured to determine the long-term effect of changes in ventricular systolic load on the ventricular repolarization. METHOD: Corrected QT interval and QT dispersion were measured in 26 patients before and 3-6 months after valvuloplasty for pulmonary stenosis. To determine the effect of ventricular load on ventricular repolarization, patients were divided in 2 groups; Group 1 was those patients with a greater than 30 mmHg and Group 2 was those patients with a less than 30 mmHg decrease in right ventricular systolic pressure. RESULTS: Corrected QT interval (412.6+/-14.5 msec vs 426.4+/-16.8 msec, p < 0.05) and QT dispersion (35.0+/-7.3 msec vs 45.7+/-14..1 msec, p < 0.05)were increased significantly only in Group 1 after vlavuloplasty. CONCLUSION: It suggested that mechanoelectrical interactions are operative for long duration in humans that changes in ventricular load after successful pulmonary valvuloplasty showed long-term effect on the ventricular repolarization.

9.Legislation Direction for Health Information Privacy in the Telemedicine Era.

The Korean law on medicine was amended in 2003 to introduce new provisions for telemedicine, electronic medical record, and electronic prescription. However, this has not satisfied the realities of the increased demand on medical services and related technology development, resulting in calls for a legislative amendment. In this regard, recent active promotion projects and the advance notice proposing a new amendment by the Ministry of Health, Welfare, and Family Affairs have raised the need for active discussions on telemedicine because the current law on medicine and its amendment do not adequately address the individual privacy protection aspect in the telemedicine environment. In this regard, this study examines current domestic and foreign legal systems on telemedicine and privacy protection, drawing and reviewing subjects to be discussed for individual privacy protection in telemedicine, and proposes plans that may improve Korea's legal system. The domestic and foreign literature on telemedicine and privacy protection was reviewed, recent legislations on telemedicine and views of interest groups were considered, and expert opinions were collected. In addition, the main discussions on privacy protection in telemedicine were identified and reviewed, including information ownership, the scope of privacy protection, the right to review and request correction, and privacy protection matters related to foreign patients. The recent amendment to tele medicine contains a provision only on penalties for privacy protection violations. The main discussions in this study on privacy protection are expected to be reflected in future amendments to enforcement rules and sub-ordinances such as the enforcement ordinance.
