1.Malignant Fibrous Histiocytoma of the Liver.

Primary sarcomas of the liver are rare. A case of primary malignant fibrous histiocytoma of the liver is reported. A 55-year-old male was admitted with epigastric pain. An abdominal computed tomographic scan disclosed a 10cm, low-density area in the left lobe of the liver. Histological examination of the resected tumor showed bundles of spindle cells arranged in a storiform pattern. In some areas, many bizarre giant cells were scattered. Immunohistochemically, tumor cells were positive for alpha 1-antitrypsin and alpha 1-antichymotrypsin, and weakly positive for vimentin. The tumor cells did not express cytokeratin, desmin or alpha fetoprotein.

2.Effects of Robot Rehabilitation for Range of Motion and Balance in Ankle Sprain Patient: A Single Case Study

Purpose:
Recently, many studies on robotic rehabilitation have been conducted, but such studies on patients with ankle sprains are lacking. This study aimed to investigate the effects of robot-assisted rehabilitation on the range of motion of the ankle and balance in patients with ankle sprain .
METHODS:
This study used the A-B-A’ design and was conducted for a total of fifteen days. The subjects performed general physical therapy for five days each, during the baseline A and A` periods. In period B, robot rehabilitation was performed for five days, along with general physical therapy. The subjects were evaluated based on weight-bearing lunge test (WBLT), single leg stance (SLS), and functional reach test (FRT).
RESULTS:
The WBLT, STS, and FRT showed significant improvement in periods B and A’ compared to period A, but there was no significant improvement in period A’ compared to period B.
Conclusion
This study confirmed that robot-assisted rehabilitation was an effective intervention for improving the function of patients with ankle sprain. In the future, a study with a control group comparison should be performed.
3.MR Imaging of Intracranial Dural Arteriovenous Fistulae (Dural AVF).

PURPOSE: To describe the magnetic resonance (MR) findings of intracranial dural arteriovenous fistulae (DAVF) and associated venous hypertensive diseases, and to determine their diagnostic value. MATERIALS AND METHODS: MR Imagings of twelve cases of angiographically-proven intracranial DAVF were analyzed. The presence of signal voids, dilatation of involved dural sinus, the dilatation of the extra/intradural venous system, high signal intensity on T2 weighted image, cerebral edema, intracranial hemorrhage, and hydrocephalus were retrospectively analyzed. MR findings and angiographic classification were compared. RESULTS: In 11 of 12 patients (92%), MR revealed the location of DAVF. In five cases, these were cavernous, and in six, they were extracavernous; the locations of the latter were as follows: superior sagittal sinus (SSS), 4: torcula, 1; lateral sinus & SSS, 1. Except for the lateral sinus lesion, all the DAVF seen in thses 11 patients showed the presence of signal voids and dilatation of the involved dural sinus. Findings of associated venous hypertensive disease, namely dilatation of the superior ophthalmic vein and cortical venous system, cerebral edema, and high signal intensity on T2-weighted images were observed in three cavernous lesions (50%), but all extracavernous DAVF (100%) showed associated venous hypertensive disease. When the grade of angiographic classification was high and reflux of sinus blood to the cortical vein was seen on angiography, the MR findings of intracranial venous hypertension showed good correlation. CONCLUSION: The MR findings of DAVF include dilatation of the dural sinus and signal voids of feeding arteries and draining vein. Other findings which suggest intracranial venohypertensive disease are dilatation of the cortical and medullary vein, hydrocephallus, cerebral edema and intracranial hamorrhage. These and the angiographic findings correlate well and suggest that in the diagnosis of DAVF and the detection of intracanial venohypertension, MR is a non-invasivse technique which can be use be usefully employed before final diagnosis by angiography.

4.A Case of Generalized Granuloma Annulare with Perforating and Subcutaneous Granuloma Annulare.

We present a case of generalized grannuloma annulare with perforating and subcutaneous forms which occurred in a 15-year-old girl. Two years ago, multiple umbilicated, match head to pea sized, erythematous papules were noted on the upper extremities. Then the patient also experienced crusted or ulcerated erythematous papular lesions on the lower legs. Recently, tender, subcutaneous nodules appeared on both soles. The biopsy specimens from the skin lesions of the hand and the sole showed typical findings of granuloma annulare in the dermis and the subcutis, respectively. On the histopathological examination of the calf lesion, palisading granuloma as well as transepidermally eliminating dermal collagenous and necrotic material were seen. The skin lesions were almost completely improved by treatment with dapsone, 100mg a day, for 8 weeks orally.

5.Neonatal Spontaneous Gastric Perforation Caused by Group B Streptococcus (GBS) Chorioamnionitis.

We describe a case of spontaneous neonatal gastric perforation in a premature infant girl of 27(+1)- week gestational age with respiratory distress syndrome baby. The patient presented within postnatal 36 hours with abdominal distension, respiratory distress, and massive pneumoperitoneum on radiography. Ischemic perforation of the stomach tissues caused by an intrauterine infection from group B streptococcus chorioamnionitis was noted. This is the first report of a spontaneous neonatal gastric perforation caused by group B streptococcus chorioamnionitis in the literature.

7.Expression of p53 Protein in Gastric Adenoma and Carcinom.

In the present study, immunohistochemical detection of p53 oncoprotein was performed to determine whether the grade of differentiation and the histologic type of gastric adenocarcinoma, and the degree of atypia accompanied with adenoma can be related to p53 mutation. Paraffin sections of 22 gastric adenomas and 56 gastric adenocarcinomas were examined for the overexpression of p53 oncoprotein with the avidin-biotin peroxidase complex staining procedure. The obtained results were as follows; 1. All the 22 cases of adenomas and 16 cases of well differentated adenocarcinomas showed uniformly negative staining. 2.Seven of 18 cases of moderately differentiated adenocarcinomas(39%), and five of 30 cases of poorly differentiated adenocarcinomas(17%) exhibited p53 protein expression. 3. Three of 29 cases of diffuse type (10%) and 9 of 19 cases of intestinal type(47%) exhibited p53 protein expression. These results suggest that p53 mutation is important in carcinogenesis of the intestinal type of gastric adenocarcinoma, and there is no correlation between the differentiation of gastric adenocarcinoma and the degree of p53 oncoprotein overexpression.

8.The Effects of Oral Cryotherapy on Nausea, Vomiting, Oral Mucositis, and Oral Pain in Breast Cancer Patients Undergoing Chemotherapy

Purpose:
The purpose of this study was to determine whether repeated oral cryotherapy was effective in reducing nausea, vomiting, oral mucositis, and oral pain in patients with breast cancer receiving chemotherapy during the 1 st , 2 nd , and 3 rd rounds of chemotherapy for the first time.
Methods:
A nonequivalent control group non-synchronized design was used. The experimental group (n=21) received oral cryotherapy that applied a 2×2×2 cm ice cube in the mouth for 30 minutes at each chemotherapy. Both experimental group and control group (n=20) received oral care educations. Nausea, vomiting, oral mucositis and oral pain were measured on 5 days, 14 days, and 21 days after each chemotherapy. Data were collected from July 2019 to April 2020 at a university hospital in Korea, and analyzed using the SPSS WIN 25.0 software package.
Results:
There was a significant difference between the experimental and control groups in the nausea·vomiting score, oral mucositis grade, and oral pain score. Oral cryotherapy was confirmed to be effective in delaying nausea and vomiting. It has been shown that stomatitis and oral pain are related.
Conclusion
For breast cancer patients receiving chemotherapy, it is recommended to apply oral cryotherapy as a nursing intervention to reduce nausea, vomiting, oral mucositis, and oral pain.
9.IKKγ Facilitates the Activation of NF-κB by Hepatitis C Virus Core Protein.

Hepatitis C virus (HCV) is a major cause of chronic hepatitis, liver cirrhosis and hepatocellular carcinoma. HCV core protein has been shown to modulate various cellular signaling pathways including the nuclear factor κB (NF-κB) pathway which is associated with inflammation, cell proliferation and apoptosis. However, there have been conflicting reports about the effect of HCV core protein on NF-κB pathway, and the mechanism by which the core protein affects NF-κB activity remains nuclear. In this study, the functional interaction of HCV core protein and IκB kinase γ (IKKγ) was investigated using the expression plasmids of core and the components of IKK complex. The data revealed that HCV core protein activates NF-κB. Also, HCV core protein up-regulated the phosphorylation and degradation of IκBα. The activating effect of HCV core protein on NF-κB was synergistically elevated by IKKγ. It was noticed that the N-terminal IKKβ binding site, C-terminal leucine zipper, and zinc finger domains of IKKγ are not necessary for its synergistic effect. HCV core protein and IKKγ appeared to activate NF-κB by up-regulating the IKKβ activity resulting in the degradation of IκBα. As expected, HCV core protein induced the expression of NF-κB-targeted pro-inflammatory genes such as iNOS, IL-1β and IL-6 in the transcription level. These results suggest that HCV core protein induces NF-κB through the interaction with IKKγ and may play a critical role in the development of inflammation and related liver diseases.

10.Factors of unmet dental care needs among elderly living alone

Objectives:
The purpose of the study was to investigate the factors affecting unmet dental care needs among elderly living alone.
Methods:
The study subjects were 19,019 elderly living by themselves, who participated in the Korea Community Health Survey 2018 (KCHS, 2018). The data were analyzed using frequency analysis, Rao-Scott chi-square test, t-test, and binary logistic regression analysis, using the SAS 9.4 software.
Results:
Results from the multiple logistic regression analysis revealed a higher odds ratio (OR) for unmet dental care needs among those with a monthly household income below KRW 1,000,000 (OR=1.64, 95% confidence interval, 95%CI=1.39-1.92), compared to those with an income of above KRW 2,000,000. With regards to stress levels, unmet dental care needs were more prevalent among participants in the “stress moderate” (OR=1.19, 95% CI=1.06-1.33) and “stress” groups (OR=1.68 95% CI=1.48-1.92), compared to the “no stress” group. Furthermore, the OR for unmet dental care needs was significantly higher among participants in the “depression” group (OR=1.84, 95% CI=1.61-2.11) than the “no depression” group.
Conclusions
Higher unmet dental care needs were found among participants in the low monthly household income, stress, and depression groups. Further study is needed to analyze the association between factors and needs related to unmet dental care.
