1.The sensitivity of N. gonorrhoeae to several antibodies.

Decreased sensitivity of N. gonorrhoeae to many existing antibiotics and the emergence and increase of penicillinase producing and of spectinomycin resistant N. gonorrhoeae necessitate intensive efforts to study on resistance. Male patients with uncomplicated gonococcal urethritis treated during the year 1990 at the VD clinic of Choong-ku Public Health Center in Seoul were included in this study. A total of 85 strains, 44 strains of non-PPNG and 41 strains of PPNG isolated from the patients were studied to evaluate their sensitivities to ampicillin, penicillin, cefoxitin, kanamycin, tetracycline, erythromycin, spectinomycin and cotrimazole. The results are summerized as follw: In non-PPNG strains, the range of MICs. MIC50 and MIC90 for both ampicillin and penicillin were 0.125-32, 2 and 16 mcg/ml respectively. For cefoxitin, they were 0.125-8, s and 16 mcg/ml respectively, and for kanamycin, 8--<64, 32 and 64 mcg/ml, respectively. For tetracycline, they were 1-64, 8 and 32 mcg/ml, while for erythromycin, 0.06-16, 2 and 8 mcg/ml respectively. For spectinomycin, they were 4--<64, 32 and 64 mcg/ml and for cotrimazole, 5--<160, 40 and 160 mcg/ml respectively. In the case of PPNG, the range of MICS, MIC50 and MIC90 for ampicillin were 8--<128, 32 and 128 mcg/ml respectively but for penicillin, 8--<128, 64 and 128 mcg/ml respectively. And for cefoxitin, they were 0.5-32, 4 and 16 mcg/ml while for kanamycin, 16--<64, 64 and >64 mcg/ml respectively. For tetracycline, the values were 2-64, 16 and 32 mcg/ml, and for erythromycin, they were 0.5-16, 4 and 8 mcg/ml respectively. for spectinomycin, they were 8--<64, 32, and 64 mcg/ml and finally for cotrimazole, 5--<160, 80 and 160 mcg/ml respectively. From these results, it is concluded that the MICs of both PPNG and non-PPNG were about 2 folds higher than the results of sensitivity tests at the same instituition in 1985. 2) The prevalence of PPNG among 35 gonorrhoeae patients who received treatment before the visit was 60% (21/35), while that among the patients without previous treatment was 40% (20/50). 3) In the cases of ampicillin, penicillin and cotrimazole, their MIC values were found to be significantly higher for the strains isolated from the patients with previous treatment history than those without previous treatment (p<0.05). 4) In non-PPNG strains, a significant positive correlation is found between the sensitivities to most pairs of ampicillin, penicillin, cefoxitin, kanamycin, tetracycline, erythromycin and spectinomycin. However this excludes some pairs such as : tetracycline-cefoxitin, erythromycin-penicillin, erythromycin-cefoxitin, spectinomycin-penicillin, and spectinomycincefoxitin (p<0.05). 5) In PPNG strains, a significant positive correlation is also found between the sensitivities to most pairs of ampicillin, penicillin, cefoxitin, tetracycline, erythromycin and spectinomycin. However, the exceptional cases include tetracycline-cefoxitin, erythromyoinpenicillin, erythromycin-cefoxitin, spectinomycin-penicillin, and spectinomycin-cefoxitin pairs. In addition, the sensitivity to kanamycin was found to be strongly correlated with that to tetracycline, erytheromycin and spectinomycin (p<0.05). Therefore it is essential for the management fo gonorrhoeae, together with a correct diagnosis, to use currently most effective treatment regimens which would also prevent the emergence of resistant strains. It is also suggested that when a treatment is failed, to use the durg which do not show correlation in sensitivities on re-treatment.

2.Infantile Fibrosarcoma in Neonate.

Infantile fibrosarcoma is a rare malignant soft tissue tumor occurring especially in newborn and young children under 2 years. We experienced three cases of infantile fibrosarcoma presenting in the neonatal period. Case 1 presented with a multiseptated cystic mass on his left thigh at birth that was diagnosed as lymphangioma. After picibanil injection, we noted the size of the mass doubled and a solid lesion was prominent in the magnetic resonance image. Case 2 was found to have a reddish mass on his lower back mimicking hemangioma. Over 2 weeks, the mass grew rapidly with internal hemorrhaging. Case 3 was noted to have an encircling mass around the splenic flexure, which developed into congenital bowel obstruction. All of the tumors were resected completely, but microscopic resection margin was not clear in two patients. The two patients received adjuvant chemotherapy and all patients are well without evidence of recurrence.

3.The Alcohol Drinking Pattern and the Related Factors in Problem Drinking among Adolescent's Parents in Korea.

PURPOSE: This study aimed to identify the alcohol drinking pattern of parents and to analyze drinking related factors in problem drinking among adolescent's parents. METHOD: This study was conducted through a structured questioning from November 5th to 22th in 2002. Those are 2,522 parents of the students of the 1st and 2nd grade who were selected randomly among 26 middle and high schools located in Socho-gu, Seoul. Data was analyzed using SAS program that included descriptive statistics, Chi-square test, and multiple logistic regression analysis. RESULT: 1. For frequency of drinking, 33.1% of the subjects responded to drink two-four times a month, 26.0% drink more than two times a week. 2. For the amount of drinking, 29.6% drink one or two glasses while 9.4% drink over ten glasses at a time. 3. The mean AUDIT score was 8.27+/-7.02(score of range: 0-40). 4. Based on AUDIT score, drinkers with less than eight point from AUDIT(normal group) were 55.1%, from more than eight point to less than twelve point from AUDIT(problematic drinker) were 16.3%, and more than twelve point from AUDIT(alcohol abuse and dependency) were 28.6%. 5. Based on more than twelve point from AUDIT, female(mother), high school and, have religion, housekeeper were the significantly higher score than counter parts. CONCLUSION: Alcohol drinking pattern and level of drinking of their parents links to their children's drinking and results in family, social, and national loss. Active prevention is needed. Specially, pertinent education about drinking and public education for mothers who are housekeepers should be carried out with concentrated intervention programs for the problematic drinker so alcohol abuse and dependency can be reduced.

4.The Changes of the Bone Mineral Density by Treatment Modality in Patients with Turner Syndrome.

PURPOSE:Decreased bone mineral density(BMD) has been reported in girls with Turner syndrome. Estrogen therapy is recommanded to improve sexual infantilism and decreased BMD. Short stature is also characteristic finding in patients with Turner syndrome. Treatment modality for short stature has included estrogen, anabolic steroids and growth hormone(GH). Recently GH therapy in GH deficient children could increase BMD in addition to improve short stature. We observed the treatment effects on bone mineral density in patients with Turner syndrome. METHODS: Bone Mineral Density in second to fourth lumbar spine area were measured by dual energy X-ray absorptiometry in 56 girls with Turner syndrome, before and after growth hormone and/or estrogen. All Turner girl was confirmed by clinical and chromosomal examination. RESULTS: 1) There was no significant difference in BMD according to karyotype. 2) The mean BMD of untreated Turner syndrome was 0.752+/-0.122g/cm2. 3) The mean BMD before and after GH treatment were 0.620+/-0.028g/cm2, 0.793+/-0.093g/cm2 respectively. The mean BMD before and after estrogen treatment were 0.761+/-0.125g/cm2, 0.918+/-0.141g/cm2 respectively. In combined group, the BMD were 0.752+/-0.087g/cm2 and 0.939+/-0.134g/cm2. Growth hormone was also effective to improve BMD as well as estrogen. But the changes of BMD were more significant in estrogen and combined group(p<0.05). 4) A significant positive correlation was found between age and BMD(p<0.05). CONCLUSIONS:Estrogen therapy can accelerate epiphysial maturation and compromise final height. Growth hormone therapy in Turner girls was effective for improvement bone mineral density as well as growth improvement. But growth hormone and estrogen combined therapy or Estrogen therapy is more effective to improve bone mineral density in Turner syndrome. Estrogen replacement can be delayed for a while on growth hormone treatment and the appropriated time of estrogen therapy should be elucidated.

5.Pemphigus Foliaceus: Report of 5 Cases.

Pemphigua foliaceus is a rare, relatively mild and chronic variety of pemphigus which is characterized by superficial flaccid bullae which show acantholytic changes in the upper part of the epidermis histopathologically. We report herein five cases of pemphigus foliaceus. The patients had erythematous and crusted patches chiefly on face, chest and back areas, except for case 2 who had flaccid bullae on the upper extrimities. The diagnosis of all these patients were confirmed by immunofluorescence studies. Among these five patients, one was recognized as a rifampicin induced pemphigus foliaceus, and the other one revealed concurrent lesions of psoriasis.

6.Treatment of Uncomplicated Male Gonococcal Urethritis with Ceftriaxone.

Ceftriaxone is a new semi-synthetic, broad spectrum third generation cephalosporin. It has a high degree of stability in the presence of beta-lactamase and its half-life averages about 8 hours. A total of 109 patients with uncomplicated gonococcal urethritis seen at the VD Clinic, Choong-Ku Public Health Center, in Seoul in the periods of March April 1986, and April-May 1987, were subjected to this study. The diagnosis of gonococcal infection and the test. of cure were made on the basis of culture and Gram staining of urethral smear. The patients were divided randomly into two groups consisting of 49 and 60, and treated with ceftriaxone, 250mg, IM and 500mg, IM respectively. No failure case was seen in both groups. lt is suggested that ceftriaxone, 250mg, IM regimen may be used as the first line treatment for uncomplicated gonococcal urethritis.

7.A Study on Attitude to Euthanasia by Student Nurses.

The purpose of this study was to explore the attitude of student nurses to euthanasia. The convenient sample for this study consisted of 293 nursing students at a nursing college in Kyeongi Province. The data were collected from Oct. 15, 2002 to Oct. 30, 2002 and were analyzed using frequency, percentage, and X2-test with the SAS package. The results of this study are as follow :77.1% of students agreed to passive euthanasia and 70.6% subjects agreed to legalization of passive euthanasia.78.5% of students can given an exact explanation of the concept of hospice. They obtained information about hospice from the mass media (1.1%), books (30.0%), friends and neighbors (3.4%), and religious groups (2.4%). The preferred place for death was reported to be home (75.4%), hospital (9.4%) or a peaceful place (6.3%).Fears of facing death were suffering in death (23.4%), grief of remaining family (21.2%), unknown about the other world (10.0%), separation from people who are loved (9.7%), forgotten from the people's memory(6.3%). In conclusion, the attitude of student nurses toward passive euthanasia approached a positive direction. But euthanasia was seen as dangerous and having multiple problems. Therefore the training for student nurses as health professionals should include content on dealing with hospice care and euthanasia.

8.Development and Effects of a Community-Based Death Education Program for Older Adults

Purpose:
This study was conducted to investigate the effects of a community-based death education program for older adults.
Methods:
The study was conducted as a quasi-experimental study with a non-equivalent control group and pretest-posttest design. The subjects were community elders aged over 65 registered at a community health center and were convenience sampled. The experimental group consisted of 33 participants and the control group consisted of 32 participants. Experiments are conducted from June 18 to July 24, 2020. We tested our hypothesis using an independent t-test, and paired t-test.
Results:
The experimental group had significantly higher scores for psychological well-being than the control group after treatment (t=2.24, p=.028). In general attitude toward the use of life-sustaining technology, however, only the experimental group had a significant difference before and after the experiment with lower scores compared to the control group (t=-5.41, p<.001).
Conclusion
We found that the community-based death education program developed in this study was partially effective in improving older adults’ psychological well-being and general attitude toward the use of life-sustaining technology.
9.The effects of changes in intracellular Ca2+ activity of osteoblast-like cell on fracture healing

Bone formation by osteoblast may be closely related to the increase in intracellular Ca2+ activity of osteoblast. In order to study the effects of changes in Ca2+ activity of osteoblast-like cell on fracture healing, we changed intracellular Ca2+ activity of osteoblast-like cells by vanadate and verapamil. And the process of fracture healing was observed after injection of the treatment osteoblast-like cells into the fracture site by hematoxylin-eosin (H-E) stain and bromodeoxyuridine (BrdU) stain. The results were as follow: 1) The most effective range of concentration which could facilitate bone formation was 10-6 to 10-5 M. 2) H-E stain showed more abundant osteoblast and osteoid tissues, more active mitotic division of osteoblast, and earlier appearance of chondroblast and chondroid tissue, making the maturation of woven bone faster in the vanadate-treated group than in the control group. The opposite was true in the verapamil-treated group compared with the control group. 3) BrdU labeling index showed more active osteoblastic proliferation in the vanadate-treated than in the control group. The opposite was observed in the verapamil-treated group compared with the control group. From these results, the fracture healing appears to be facilitated and decelerated by vanadate which apparently increase intracellular Ca2+ activity of osteoblast and verapamil which decreases it, repectively. Therefore the change of intracellular Ca2+ activity of osteoblast can be considered to be one of fracture healing mechanisms and expected to be applied for clinical purpose.
