2.Bipolar Spectrum Disorder.

The concept of bipolar disorder is an ongoing process, still in evolution, although its roots can be found from ancient Greek. Until recently, it was believed that no more than 1% of the general population has bipolar disorder. But literature on the lifetime prevalence of the bipolar spectrum disorder suggests rates of 4-5%. Bipolar spectrum disorder is a longitudinal diagnosis characterized by abnormal mood swings comprising some of the following cross-sectional clinical states: mania, hypomania, mixed states, hyperthymic temperament, major depressive episode, and depressive mixed state. Failure to recognize subthreshold expressions of mania, such as hypomania, cyclothymia, and hyperthymia, contributes to the frequent under-diagnosis of bipolar disorder. There are several reasons for the lower rate of recognition of subthreshold manic symptoms, when compared to the analogous pure depressive one. These include the lack of subjective suffering, enhanced productivity, egosyntonicity, diurnal and seasonal rhythmicity associated with many of the manic and hypomanic symptoms, and the psychiatrists' difficulty in differentiating with personality disorders. In addition, most patients with bipolar spectrum disorder seek treatment for depression, rather than mania or hypomania. Therefore clinicians often miss the diagnosis of bipolar spectrum disorder. The recent 10 years of researches have suggested that 30-55% of patients with major depressive disorder are actually identified with broader spectrum of bipolar disorders. However, 48% of patients of bipolar disorder consulted 3 or more professionals before receiving a correct diagnosis, and 10% consulted 7 or more for their first diagnosis of bipolar disorder. Several studies have documented that patients often wait as long as 10 years for the correct diagnosis of bipolar spectrum disorder. This delay in diagnosis often has substantial adverse results. Patients do not get the appropriate treatment to alleviate their symptoms. They may even get treatments that exacerbate their symptoms, such as prescription of antidepressants precipitating mania and producing rapid cycling. The concepts of hypomania, cyclothymia, mixed state, depressive mixed states, hyperthymic temperament are the new areas of studying mood disorders in recent 20 years. The authors will review the studies on various subtypes of bipolar spectrum disorder with their historic aspects, and introduce the suggested screening tests for bipolar spectrum disorder in clinical practice.

3.Clear Cell Sarcoma of the Kidney: Report of two cases.

Clear cell sarcoma is a rare malignant rumor of the kidney which occurs in children and is differentiated from Wilms' tumor by its different clinicopathologic features and natural history. Previous studies indicate that this tumor may be of mesenchymal cell origin; however, this has not been proven conclusively. Further accumulation and study need to be conducted in order to clarify the histogenesis of this tumor. We report two cases of clear cell sarcoma of the kidney which occurred in a 2 and a half-year old and a 2-year old boy. This report places special emphasis on the clinicopathologic characteristics of these two cases including electron microscopic and immunohistochemical findings. Attempts were also made to differentiate the clinicopathologic aspects of clear cell sarcoma from Wilms' tumor and speculate on the histogenesis of this rumor.

5.Reconstruction Of Achilles Tendon Using Fascia Using Fascia Lata Allogrft: A Case Report

We had a female patient with soft tissue tumor involving large portion of Achilles tendon. After resection of tumor, Achilles tendon was reconstructed using an allogenic human fascia lata manufactured by Pfrimmer-Viggo. EMG findings on postoperative 8 months show marked improvement of function of medial and lateral gastrocnemius muscles. Biopsy specimen of fascia lata allograft, which was acquired during TAL 8 months postoperatively, shows myxoid degeneration, a few lymphocytic infiltration and some foreign body giant cells. Functional, histologic and EMG findings suggest that human fascia lata allograft could be used to bridge defect of Achilles tendon being resected during tumor surgery.

6.Analysis of reflective essays on interprofessional education experiences of college student in the health department: Applying topic modeling

Purpose:
: The effectiveness of the interprofessional education(IPE) program has been evaluated with quantitative data. The purpose of this study is to analyze the effectiveness of IPE using topic modeling technique in text network analysis tool as one of the qualitative data analysis methods.
Methods:
: In this study, 58 fourth-year students from medical, pharmacy, and nursing school participated in the IPE program. The IPE was conducted as a half day program centered on student activities of small group activities and role play. The role play scenario is about medical errors caused by health professional’s negligence. Fifty-eight reflective essays were analyzed as qualitative data. topic modeling technique was used for the analysis.
Results:
: Four topics were derived from this study: experience of the importance of collaboration among healthcare professionals (Topic 1), conflict situations and resolution among healthcare professionals in hospitals (Topic 2), empathize with the difficulties experienced by healthcare professionals through role play (Topic 3), and recognition of doctors’ responsibilities in emergency situations (Topic 4). Topic 3 had the highest share of topics at 33.3%.
Conclusion
: We found that IPE was an effective educational strategy for interprofessional collaboration. In addition, qualitative data such as reflective essays can be utilized to analyze the effectiveness of IPE.
7.Microscopic Anatomy of the Lower Eyelid in Koreans.

PURPOSE: To study the microscopic anatomic relation of capsulopalpebral fascia, orbital septum, and tarsus of the lower eyelid in Korean. METHODS: Foureteen normal Korean lower eyelids from 14 formalin-fixed cadavers (mean age of 66.5 years) were harvested and studied with light microscopy after staining with hematoxylin and eosin. Dimensions of the structures and distances between them was measured by analyzing the digital photographs. RESULTS: There was no fusion of the orbital septum and the capsulopalpebral fascia at the lower border of the tarsal plate in 10 specimens and limited fusion of these tissue in 4 specimens. The average distance from the inferior tarsal border to the first identifiable smooth muscle nuclei of the inferior tarsal muscle was 1.38 mm. The average tarsal plate thickness was 0.98 mm and the average tarsal plate height was 3.96 mm. CONCLUSIONS: The Korean lower lid anatomy differs from that of their non-Asian counter parts. Furthermore, there was a difference in tarsal plate thickness in comparison to Chinese lower lids. There was no consistent fusion between the capsulopalpebral fascia and the orbital septum. No extension of the capsulopalpebral fascia to the skin was observed.

8.Utilization of nutrition labels and related factors among patients with diabetes in Korea

BACKGROUND/OBJECTIVES:
The prevalence of diabetes has continued to increase globally.Changes in eating habits, lack of exercise, increased stress, and aging are major contributors.Glycemic control is the key strategy of diabetes management. The purpose of this study was to analyze the utilization of nutrition labels and related factors among patients with diabetes.MATERIALS/METHODS: Data from the 7th Korea National Health and Nutrition Examination Survey were used. General, health-related, diabetes-related characteristics from 1,587 adults with diabetes history were included. Nutrition label utilization was assessed with awareness and use of nutrition labels and effects on food choice. For statistical analyses, chi-square test and multiple logistic regression analysis were performed.
RESULTS:
The prevalence of awareness, use, and effects of nutrition labels on food choice among diabetic patients were 48.8%, 11.4%, and 9.6%, respectively. High monthly income, walking frequency, family history of diabetes, younger age at diagnosis, and shorter duration of diabetes were associated with higher nutrition label awareness. Nutrition label use and effect on food choice were higher in women, those with high monthly income, those diagnosed at younger than 45 yrs, those with diabetes for less than 10 yrs, those with meal therapy, or patients who had undergone a fundus examination.
CONCLUSIONS
Nutrition label utilization status was low in Korean patients with diabetes.Strategies are needed to promote nutrition label use as a diet management tool for patients with diabetes.
9.Defense Mechanism of Professors and Residents: Comparison between Medicine and Surgery Groups.

The purpose of this study was to explore the difference of defense mechanisms between medicine and surgery groups, and between residents and specialists who were professors of a medical school. The author evaluated the defense mechanisms by using Ewha Defense Mechanisms Test. The author compared the defense mechanisms of medicine and surgery specialist groups who are employed by Soonchunhyang university hospital as professors in 2000, and of medicine and surgery resident groups in 1999. First, there were significant differences in mean scores of the defense mechanisms ratings. In surgery specialist group, the rate of using projection and show-off were significantly higher than those of medicine group. In resident group, show-off, passive-aggression, dissociation, somatization, acting-out and regression were significantly higher than those of specialist group. In medicine group, residents were higher than specialists in show-off, passive-aggression, acting-out and regression. But for the specialist group, the score on anticipation was higher than in the resident group. In surgery group, residents were higher than specialists in identification, rationalization and regression. In the interaction, analysis by hierarchy and field anticipation and altruism have a significant interaction effect. Second, in comparison of maturity level between the field of speciality and level of hierarchies, surgery group used more neurotic and narcissistic defense than medicine group significantly. For the level of hierarchies, residents used more immature and neurotic defense than specialists. In medicine group, residents used immature defense more than specialists, but more mature defenses were used in specialists than residents significantly. In surgery group, residents used more immature and neurotic defenses than specialists. Mature defenses have more interaction effect. This results may reflect the fact that discipline in surgery have characteristic apprentices system and more dramatical therapeutic approach. The results seem to be useful in understanding the optimal character for each speciality. In the process of selecting spescialities evaluating one's defenses and matching with the characteristics and coping mechanisms of each specialty group can be helpful.

10.Attachment and Behavior Problems in Adolescents: Internalizing and Externalizing Problems.

OBJECTIVE: The purpose of this study was to examine the relationship between attachment and behavior problems in adolescents. METHOD: A sample of 792 high school students were examined. Revised Adult Attachment Scale, Self-report Attachment Style, Youth Self-Report Child Behavior Checklist (YSR) were used to measure the attachment quality, the attachment pattern and the behavior problems respectively. RESULTS: As to the attachment quality, high score in fear of rejection, low dependability and low intimacy were related to the development of adolescent's internalizing problems. As to the attachment pattern, preoccupied and fearful with attachment experience were linked to the internalizing behavior problems of higher level as well as 'Attention problems' and 'Aggressive behavior' sub-scales of externalizing ones on YSR. CONCLUSION: High fear of rejection, low dependability and low intimacy characterise the insecure attachment. So the insecure attachment might predict the development of all internalizing behavior problems, attention problems and aggressive behavior in adolescents. Also the result suggested that the adolescents classified as Dismissing attachment may have a positive view of the self and may minimize the self reported behavior problems.
