Wells' syndrome or eosinophilic cellulitis is an uncommon cutaneous disorder characterized by recurrent erythematous and edematous plaques that often resemble acute cellulitis. The characteristic microscopic findings are diffuse dermal eosinophilia with marked edema, and flame figures consisting of amorphous or granular eosinophilic material around collagen bundles of the dermis. Etiology and pathogenesis are unknown, but the disease has been found to be associated with infections, arthropod bites, drug administration, surgery and hematologic disorders. We report the case of a 55-year-old man with pancreatic carcinoma who was presented with an asymptomatic erythematous to brownish subcutaneous nodule on the left buttock of several days' duration. A biopsy specimen taken from the erythematous to brownish nodule showed many flame figures and diffuse infiltration of eosinophils in the dermis and subcutaneous fat tissue. The rapid resolution of the skin lesion with chemotherapy of the pancreatic carcinoma supports a relationship between the two conditions.

Churg-Strauss Syndrome*

For the reconstruction of skin defects after remoral of cutanous tumors in the nasal area, local flaps or full-thickness skin grafts can be utilized depending on the size and site of the defectg. Burows graft, which is, variant of the full-thickness skin graft, makes use of the skin nearby as the donor site, minimizes deformity of the recipient site, maximizes ireservation of skin, easily aligns relaxed skin tension line, and is thus one of the methods with a good esultant color and texture match. A pigmented basal cell carcinoma on the nasal bridge in a 51-year old male was remaved by 4 serial sections of Mohs micrograplic surgery and was then successfully enstructed using a Burows graft.

A 13-month-old boy had irregularly shaped, erythematous to dusky red, slightly raised patches and plaques on the left antecubital fossa and arm for 7 months. The lesions were firm and tender. Histopathologically the lesion consisted of small, circumscribed, angiomatous tufts and lobules with a cannonball appearance scattered throughout dermis, a characteristic feature of tufted angioma. For 15 weeks, interferon-a intralesional injections have been performed and the lesions showed an improvement.

Lanolin is a complex material consisting of a mixture of numerous esters and polyesters of high-moleculer-weight alcohols and fatty acids, Lanolin allergy is important because lanolin is widely used as an emulsifier in topical medicaments and in numerous cosmetic preparations. We report a case of allergic contact dermatitis in a 27-year-old female. An erythematous pruritic rash develgood around the eyelid developed after applying a new eye cream and was aggravated after applying Cambison ophthalmic ointment. Patch testing showed positive reactions to Cambison ophthalmic ointment, 30% wool alcohol, Amerchol L101 and Nivea cream. A use test with 100% Cambison ophthalmic ointment base was positive

For treatment of partially edentulous patients, a treatment using implant is widely used. Treatment method using implant are implant fixed prostheses and removable partial dentures, and for patients with severe bone resorption, removable implant overdenture with the effects of aesthetic and reducing cost can be used as treatment options. Specially, prosthesis with milled-bar and attachment has the effect of being splinted between implant fixtures, higher retention and stability than conventional removable partial denture. And it has the effect of improvement of aesthetic through lip support by denture base. In this case, the patient with severe alveolar bone resorption and partial edentulous maxilla and mandible was treated by implant-assisted removable partial denture using Milled-bar and ADD-TOC attachment. The esthetic was improved by removing the clasp because of effects of additional retention by using the attachment, and reducing palatal coverage of implant-assisted removable partial denture. The clinical results were satisfactory on the aspect of aesthetic and masticatory function.

The use of implants could improve the support, retention, and stability of removable prosthetic restoration for fully edentulous patients with severe alveolar bone resorption. When the prosthesis is manufactured without accurate diagnosis and evaluation, this may lead to unfavorable treatment result. The patient in this case had the treatment of implant overdentures at a private dental clinic, but visited the hospital for retreatment due to soft tissue pain and decreasing retention of existing dentures. The vertical dimension was raised compared to the existing dentures, and overdentures were manufactured using milled bars and additional attachments. The clinical results were satisfactory in the aspect of improved oral hygiene maintenance, function and esthetics.

BACKGROUND: Dermatologic practice occurs mainly in the outpatient setting. The reasons for, frequency, and impact of inpatient dermatologic consultation may be quite different from outpatient practice and there has been a lot of changes in dermatologic consultation recently. Several studies about characteristics of dermatologic consultation have been reported. OBJECTIVE: The purpose of this study was to examine the characteristics of inpatient dermatologic consultation in Ewha womans university hospital for 3 years and also to compare the result of the report in 1991. METHODS: During a period of 3 years (1996.2-1998.3), we prospectively recorded the demographics of the patients for whom consultation was requested, the final diagnosis of the dermatologic service, and the tests necessary to arrive at diagnosis, and reasons for dermatologic consultation. RESULTS: 1) The consulted patients were 1019 among 38,402 inpatients (2.7%). 2) The total number of male and female patients were 547(53.7%) and 472(46.3%), respectively (M:F = 1.2:1). 3) The most frequent age group was the 6th decade(18.4%) and the highest incidence of seasonal distribution was spring. 4) The patients in internal medicine were the most frequently consulted. 5) The most common dermatoses were eczema group, followed by drug eruption, fungal infection, viral infection, acne vulgaris, urticaria, etc. 6) Distribution of dermatoses according to seasons and individual departments was statistically different in various dermatoses. 7) Frequent causes of consultation were as follows : the dermatologic diseases or conditions 68.3%, the diseases related to treatment 12.3%, the skin lesions of systemic diseases 10.0%, the dermatologic tests 3.5%, and admission to other departments with dermatologic diseases 1.9%, respectively. 8) The rate of consultation for drug eruption was increased markedly compared to that of new patients at OPD in 1998. CONCLUSIONS: The characteristics of inpatient dermatologic consultation in our hospital have been changed and furthermore the characteristics were quite different from that of oupatient practice.

PURPOSE: The purpose of this study was to investigate the status of infection control in long-term care facilities in Korea and educational needs of nurses in charge of infection control. METHODS: This was a descriptive study. A self-reported questionnaire was provided to the nurses in charge of infection control in 250 hospitals with long-term care. 209 nurses answered to the questionnaire. Data were collected from September 30 to November 7, 2016. Data were analyzed using SPSS/WIN 18.0, and the educational needs were analyzed by applying the Borich Needs Assessment Model. RESULTS: Only 17.4% of the hospitals had infection control departments, and only 1.0% of the hospitals had nurses who were fully-in-charge of infection control. Regarding the educational needs on infection control, level of knowledge was statistically significantly lower in all 50 items compared to the importance. Specifically, educational demand on air and water quality management, construction and infection control, indicator management, and infectious disease management were also high. CONCLUSION: The results of this study showed that organization and manpower were needed for effective infection control of long-term care facilities in Korea. In addition, it was deemed necessary to develop and applicate infection control education programs as reflected on the scores obtained in the educational needs on infection control.
