1.A Study on the Relationship between the Awareness of Blood Program and the Blood Donation Experience.

In this study, the relationship between the awareness of blood program and the blood donation experience has been studied to know whether they corelate or not. The awareness of blood program was divided into 4 areas of knowledge and measured: the eligibilityt of blood donation, knowledge about blood, blood donation program, and general facts about blood donation. The experience of blood donation was divided into 3 groups: the registered donor, the non-registered donor, and the nondonor. The result in this study shows that the awareness of blood program is related with 3 factors, such as the eligibility of blood donation, blood donation program, and general facts about blnation. The difference in this case is significant at 0.05 level. However the knowledge about the blood is not corelated with blood donation experience, but only with sex(P<0.05). These results suggest that stable blood supply should be realized by securing new donors and by estsablishing of fdonor registration system. This is realized by the stir up the level of awareness and interest of blood program through strengthening public relations.

3.Malignant Fibrous Histiocytoma of the Liver.

Primary sarcomas of the liver are rare. A case of primary malignant fibrous histiocytoma of the liver is reported. A 55-year-old male was admitted with epigastric pain. An abdominal computed tomographic scan disclosed a 10cm, low-density area in the left lobe of the liver. Histological examination of the resected tumor showed bundles of spindle cells arranged in a storiform pattern. In some areas, many bizarre giant cells were scattered. Immunohistochemically, tumor cells were positive for alpha 1-antitrypsin and alpha 1-antichymotrypsin, and weakly positive for vimentin. The tumor cells did not express cytokeratin, desmin or alpha fetoprotein.

5.Pneumocystis carinii Pneumonia Presented as Diffuse Alveolar Damage: Report of a case.

Pneumocystis carinii is the most common cause of diffuse pulmonary infiltrates in the immunocompromised patients. Microscopically, Pneumocystis carinii pneumonia(PCP) shows characteristic frothy intraalveolar exudate and interstitial lymphocytic and plasma cell infiltrate. However, sometimes the only histologic finding of PCP on routine hematoxylin-eosin stain is that of diffuse alveolar damage(DAD), when we can miss the diagnosis without aid of special stains. We report a case of Pneumocystis carinii pneumonia presenting as DAD in a 50-year old man after chemotherapy due to malignant lymphoma. Open lung biopsy specimen reveals the early stage of DAD without any characteristic findings, such as foamy exudate. However many cysts of Pneumocystis carinii were found on Gomori's methenamine silver(GMS) stain. Therefore, GMS stain should be routinely performed on all biopsy specimens obtained from immunocompromised patients.

6.Analysis of Cell Proliferative Activity, p53 Protein Overexpression and Apoptosis in Hepatocellular Carcinoma and Surrounding Nontumorous Liver.

BACKGROUND/AIMS: Although the mechanism of hepatocellular carcinogenesis still remains to be clarified, it has been suggested that persistent hepatic necrosis and resultant irregular regeneration might cause genetic mutations, such as activation of protooncogenes, inactivation of tumor suppressor genes and modulation of apoptosis-related genes, finally leading to hepatocellular carcinoma (HCC). To elucidate the role of cell proliferative activity and apoptosis, a major mechanism of cell death, in hepatocellular carcinogenesis, we analyzed expression of proliferation cell nuclear antigen (PCNA), p53 protein and apoptotic cells in HCC and surrounding nonneoplastic hepatic parenchyma. METHODS: We performed immunohistochemical staining to detect P CAN, p53 protein, and terminal deoxynucleotidyl transferase-mediated dUTP-biotin nick end labelling (TUNEL) method for the detection of apoptosis in 52 hepatocellular carcinomas and their adjacent nonneoplastic liver. We scored the expression of PCNA and p53, and apoptotic index by a 5 point scale' 0, 0%,1, 1-25%', 2, 25-50%, 3, 50-75%', 4, >76%, and analyzed the results with other clinicopathologic characteristics. RESULTS: p53 protein was expressed in 42.3% of the HCC, but was not evident in nonneoplstic liver. P53 overexpression was correlated with the histologic grade of HCC (p<0.05). PCNA labelling indices (LI) of HCC were correlated with those of liver cell dysplasia and normal liver (p<0.05). Leading edges of HCCs showed higher proliferative activity than the central part of HCC. Four cases of HCCs with high TUNEL also showed high proliferative activity. There was no difference of the TUNEL between HCC and surrounding nonneoplastic liver. Expression of p53, PCNA LI and TUNEL had no relationship with clinicopathologic parameters including viral markers, aFP elevation, tumor size and underlying cirrhosis. CONCLUSION: p53 overexpression in HCC and absence of p53 mutation in nonneoplastic liver indicates the active participation of p53 in hepatocellular carcinogenesis. Invasiveness and metastatic potential appear to be related with the strong expression of PCNA, but apoptosis in HCC has no direct implication in hepatocellular carinogensis.

7.Effects of a Clonorchiasis Prevention Education Program for Clonorchiasis Prevention Lecturers.

PURPOSE: The study was conducted to clarify effects of a clonorchiasis prevention education program for clonorchiasis prevention lecturers. METHODS: The research adopted a single group pretest-posttest design to see effects of the educational program to prevent Clonorchis sinensis infection. The subjects of this study were 74 clonorchiasis prevention lecturers from primary health care facilities. The pretest was conducted before the clonorchiasis prevention education program and the post test was done after the 2-day program in August, 2011. Descriptive statistics, t-test, and ANOVA were conducted to analyze the data. RESULTS: The confidence level in Clonorchiasis management activities was improved significantly from 4.1+/-0.53 points before the education to 4.4+/-0.46 points after the education (t=-5.117, p<.001). The knowledge level about prevention of Clonorchis sinensis was improved significantly from 16.1+/-2.72 points before the education to 18.3+/-1.14 points after the education (t=-6.629, p<.001). CONCLUSION: The results suggest that the education program was effective in improving the confidence and knowledge levels in Clonorchiasis management activities for the clonorchiasis prevention lecturers. Based on the results of this study, continuous research on how the increased knowledge and confidence levels of Clonorchis sinensis prevention affect the prevalence of Clonorchis sinensis infection.

8.Legislation Direction for Health Information Privacy in the Telemedicine Era.

The Korean law on medicine was amended in 2003 to introduce new provisions for telemedicine, electronic medical record, and electronic prescription. However, this has not satisfied the realities of the increased demand on medical services and related technology development, resulting in calls for a legislative amendment. In this regard, recent active promotion projects and the advance notice proposing a new amendment by the Ministry of Health, Welfare, and Family Affairs have raised the need for active discussions on telemedicine because the current law on medicine and its amendment do not adequately address the individual privacy protection aspect in the telemedicine environment. In this regard, this study examines current domestic and foreign legal systems on telemedicine and privacy protection, drawing and reviewing subjects to be discussed for individual privacy protection in telemedicine, and proposes plans that may improve Korea's legal system. The domestic and foreign literature on telemedicine and privacy protection was reviewed, recent legislations on telemedicine and views of interest groups were considered, and expert opinions were collected. In addition, the main discussions on privacy protection in telemedicine were identified and reviewed, including information ownership, the scope of privacy protection, the right to review and request correction, and privacy protection matters related to foreign patients. The recent amendment to tele medicine contains a provision only on penalties for privacy protection violations. The main discussions in this study on privacy protection are expected to be reflected in future amendments to enforcement rules and sub-ordinances such as the enforcement ordinance.

9.A Study on the Prevalence of Clonorchis Sinensis and the Effects of Educational Program among Residents in the Basin of the Youngsan River, Korea.

PURPOSE: The goal of this study was to analyze the prevalence of Clonorchis sinensis infection in people living within 5 km of the Youngsan River basin, to develop an educational program to prevent the infection, and to examine the effects of the educational program. METHODS: This study employed a one group pretest-posttest design, the subjects were 384. This study was conducted from November 20, 2008 to June 16, 2009. The results were analyzed with the SPSS/WIN 18.0 program. RESULTS: Of the residents, 5.7% were infected with Clonorchis sinensis and the knowledge level about the prevention of Clonorchis sinensis was improved significantly from 9.75+/-5.12 points before the education to 15.05+/-2.43 points after the education. One of the important effects was that the willingness to eat raw fresh-water fish was reduced after the education. CONCLUSION: Based on the results of this study, a continuous research needs to be conducted on how education and knowledge level-up change people's living attitudes for the prevention of Clonorchis sinensis infection and the reduction of the infection rate in the future.

10.Vaginal Birth after Cesarean Section.

Repeat cesarean section is one of the leading causes to make increasing a rate of cesarean section. Trial of labor TOL after cesarean section is attempt to reduce the rate of repeat cesarean section. Trial of labor has been well established as a safe alternative in carefully selected women with transverse scars in the lower uterine segment. This study was based on 104 cases of delivery with prior cesarean birth at the Chonbuk National University Hospital from January, 1991 to June, 1997. The results were as follows: 1) Among 1400 cases with previous cesarean delivery, trial of labor was done in 104 cases (7.4%). Among 104 cases, vaginal birth was successfully completed in 96 cases (92.3%). 2) Among 28 cases with PGE2, vaginal tablet, vaginal delivery was done in 23 cases (82.1%). 3) Among 22 cases with more than 4 of Bishop score, vaginal delivery was done in 21 cases (95.5%), and among 6 cases with less than 3 of Bishop score, vaginal delivery was done only 2 cases (33.3%). 4) There was no matemal death or uterine rupture in the cases of trial of labor, But in 2 cases, uterine dehiscence was observed in 4 cases and postpartal bleeding (more than 400ml) was developed. 5) In the cases of cesarean section, mean of hospital day was 7 days and medical fee was about 400,000 won. In the cases of vaginal birth after cesarean section, mean of hospital day was 2.6 days and medical fee was about 100,000 won. In condusion, vaginal birth after cesarean section is safe and effective alternative to elective repeat cesarean section and also the use of PGE, vaginal tablet is so effective to increase success rate of vaginal delivery. After all, positive these trials might decrease cesarean rate and increase maternal health and quality of medical care.
