1.CT Findings of the Brain Damages Resulting from the High Voltage Electric Injuries.

PURPOSE: The purpose of this study is to evaluate the CT features and pathogenesis of the electric brain injuries. MATERIALS AND METHODS: We reviewed the CT scans of 3 patients injured by high-voltage electricity. We evaluated the findings early and delayed periods in each patients. RESULTS: The early CT findings were diffuse brain edema, scalp swelling, and focal hemorrhagic contusion. The findings of delayed period were cerebral infarction, pneumocephalus, brain abscess, and pneumatocele. CONCLUSION: CT was useful to correlate the pathogenesis and variable features of electric brain injuries.

2.Pemphigus Foliaceus: Report of 5 Cases.

Pemphigua foliaceus is a rare, relatively mild and chronic variety of pemphigus which is characterized by superficial flaccid bullae which show acantholytic changes in the upper part of the epidermis histopathologically. We report herein five cases of pemphigus foliaceus. The patients had erythematous and crusted patches chiefly on face, chest and back areas, except for case 2 who had flaccid bullae on the upper extrimities. The diagnosis of all these patients were confirmed by immunofluorescence studies. Among these five patients, one was recognized as a rifampicin induced pemphigus foliaceus, and the other one revealed concurrent lesions of psoriasis.

3.Classification of the Calcaneal Fracture Based on Computed Tomography

The os calcis is most frequently fractured than any other tarsal bone and the displaced intraarticular fractures account for 60-75% of them. Because of complex contour of calcaneous, it is difficult to evaluate the pattern of fracture exactly by conventional roentgenograms. But recently, more detailed understanding of fracture pattern can be possible with CT scan, which is thought to be one of the most important factor in deciding method of treatment and predicting prognosis. From Jan. 1992 to Dec. 1992, intraarticular calcaneal fractures of 31 cases treated in Kyungpook university hospital were analysed preoperatively with CT scan and classified by Sanders classification system. The results were obtained as follows: 16 cases of 31 cases were classified as type II and this type were sudivided as IIA in 7 cases, IIb in 4 cases, IIC in 5 cases. Type III fractures were found in 9 cases and subdivided as III AB in 3 cases, III BC in 4 cases IIIAC in 2 cases. Type IV fractures were found in 6 cases.

4.Balloon Catheter Dilatation in Esophageal Achalasia: Long Term Follow-Up.

PURPOSE: To evaluate the clinical efficacy of balloon catheter dilatation in the treatment of esophageal achalasia. MATERIALS AND METHODS: Seven patients(three males and four females) with esopha-geal achalasia were treated with balloon catheter dilatation. Balloon catheters of variable sizes were used depending on patient's conditions. The patients were followed up over a period of 12-39months. RESULTS: Balloon catheter dilatation in esophageal achalasia was successful in all patients without esophageal perforation. All patients were relieved from dysphagia. Recurrence was not found in 5 patients on long term follow-up study, but was seen in 2 patients after 18 and 21 months, respectively. CONCLUSION: Balloon catheter dilatation was a safe and effective method in the treatment of esophageal achalasia with low recurrence rate of 29% on follow-up study.

5.Deleterious consequence of acupuncture: case report of acute cervical epidural hematoma

Acupuncture, one of oriental medicine’s crucial treatment methods, is a popular practice around the world. Although its mechanism of effect remains uncertain and unknown, many people have confidence in this treatment method. Ironically, due to its rise in popularity, there are increased reports of treatment complications. We present here a case where a 75- year-old patient suffered acute cervical epidural hemorrhage mimicking symptoms of acute disc rupture, immediately after being administered acupuncture on the neck. Fortunately, the patient required no surgical intervention since his motor symptoms spontaneously improved with only pain management. After a close observation period of 10 days, the patient was discharged with mild radiculopathy. This case was unique since it mimicked the symptoms of acute disc rupture or severe stenosis. Thus, to successfully manage patients, the initial history taking at the emergency room is crucial. Patients should always consider that even though the reported complication rate for acupuncture is very low, there is a possibility of severe adverse effects, such as the consequences seen in this case.
6.Virtual Reality for Obsessive-Compulsive Disorder: Past and the Future.

The use of computers, especially for virtual reality (VR), to understand, assess, and treat various mental health problems has been developed for the last decade, including application for phobia, post-traumatic stress disorder, attention deficits, and schizophrenia. However, the number of VR tools addressing obsessive-compulsive disorder (OCD) is still lacking due to the heterogeneous symptoms of OCD and poor understanding of the relationship between VR and OCD. This article reviews the empirical literatures for VR tools in the future, which involve applications for both clinical work and experimental research in this area, including examining symptoms using VR according to OCD patients' individual symptoms, extending OCD research in the VR setting to also study behavioral and physiological correlations of the symptoms, and expanding the use of VR for OCD to cognitive-behavioral intervention.

7.Presentation Time of Necrotizing Enterocolitis Diagnosed by Sonography according to Gestational Age.

PURPOSE: In this single center study, we assessed the timing of presentation of necrotizing enterocolitis (NEC) diagnosed by sonography according to the gestational age. METHODS: We retrospectively reviewed the medical records of 49 newborn patients who were diagnosed with NEC (modified Bell's stage II and higher according to abdominal sonography and simple abdominal radiography) and were admitted to the neonatal intensive care unit of St. Mary's Hospital between January 2008 and December 2012. Infants were grouped according to their gestational age (GA): Group I (GA <28 weeks), Group II (GA, 28-32 weeks), Group III (GA, 33-36 weeks), and Group IV (GA > or =37 weeks); early-onset NEC was considered when NEC developed at <14 days of age and late-onset NEC was considered when NEC developed at > or =14 days of age. RESULTS: The number of infants in each group were: Group I (n = 16), Group II (n = 20), Group III (n = 11), and Group IV (n = 2). The mean age at diagnosis of NEC was: Group I (31.6 days), Group II (15.9 days), Group III (11.0 days), and Group IV (2.0 days). Early-onset NEC developed at a mean of 5.5 days of age, whereas late-onset NEC developed at a mean of 36.1 days of age. CONCLUSION: Based on early sonographic diagnosis, infants with lower gestational ages developed NEC at a more advanced postnatal age as compared to more mature infants. However, further studies are needed to understand the etiology of this disease process.

8.A Case of Vesicourachal Diverticulum.

The urachus is a vestigial structure extending from the apex of the bladder to the umbilicus. Embryololigically, the urachus becomes obliterated during the third month of fetal life, but incomplete regression of the urachus results in anomalies. Urachal anomalies are very rare and present with varied appearances, one of which is vesicourachal diverticulum. Recently, we experienced a case of vesicourachal diverticulum complained of recurrent urinary tract infection in 13 month old male. Embryology, classification, clinical presentation, diagnostic modalities and treatment of urachal anomalies are reviewed.

10.Analysis of Current Status of Chronic Red Blood Cell Transfusion.

BACKGROUND: Chronic red blood cell (RBC) transfusion is widely practiced as part of supportive care in patients with chronic anemia. However, there were few reports on the actual situation in Korea. Therefore, we studied the current status of chronic RBC transfusion. METHODS: Among patients who received RBC transfusions at a regional hospital from January 2011 to December 2015, patients who were consistently transfused with more than 15 units of RBC for more than one year were included. We retrospectively analyzed the status of RBC transfusion, including the laboratory findings related to transfusion using medical records. RESULTS: A total of 58 patients were included in the study. The mean age of patients was 66.4 years, and there were 24 hematologic malignancies (41.4%), 20 malignant tumors (34.5%), and 14 chronic diseases (24.1%). The mean hemoglobin level at the beginning of transfusion was 7.0 g/dL, and eight patients (13.8%) showed RBC alloimmunization as the transfusion proceeded. The mean serum ferritin level at the initial phase of transfusion was 475.3 ng/mL and increased to 1,462 ng/mL during the late phase of transfusion. CONCLUSION: Since the number of elderly patients treated with chronic diseases including tumors is expected to increase significantly, patients with chronic transfusions are also expected to increase. It is necessary to research and prepare measures such as establishment of guidelines.
