2.Study on situation and clinical characters of leukoderma during 1/2000-1/2005

A prospective and cross-sectional study on 135 patients with leukoderma at the Central Military Hospital N0 108 and the Military Hospital N0 105 showed that: the disease developed at age of 10-19 (26.67%), mainly in patients under 40 year of age (82.98%), females more than males. 16.3% of patients have family history with leukoderma, mainly siblings (45.45%), parents (22.72%). Psychological trauma was observed in 18.52% of patients, 30.37 % of patient have other diseases, mainly gastro-duodenal ulcer (24.44%). The lesions developed in head, neck and face area (55.56%), four limbs (28.14%); pervasive form (60.74%), localized form (11.85%). The disease at moderate level (39.26%), serious level (31.11%); mild level (29.63%). The disease is the most common at progressive stage (75.56%), stable stage (24.44%), and period of disease less than 5 years accounted for 65.19%.

4.A Web-Based Bulletin Board System for Treatment of Skin Disease.

BACKGROUND: Electronic bulletin board systems have been used for education, information exchange, and medical consultation for a number of years. However, most of these systems have been text-based. The field of dermatology has never had a system that allowed a quick exchange of visual information. OBJECTIVE: Dermatology is a field of medicine which especially requires a visual image for an accurate diagnosis, and subsequent effective treatment. We have developed a web-based bulletin board system which can be used by any clinician with access to the internet to view clinical images of skin diseases with accompanying clinical findings. METHODS: A web-based bulletin board system was developed for the internet. The system is designed to operate independently of client hardware and software using hyper text markup language so participants using different platforms can access and use the site effectively. RESULTS: Any participant can easily upload a diagnostic image to the board along with relevant clinical information. Other participants can make comments regarding the presented case which may prove useful to the presenter in diagnosis and development of an effective treatment modality. CONCLUSION: Our web-based bulletin board system for discussion of skin diseases has the potential to help in the diagnosis and treatment of various skin diseases. Dissemination of information relating to clinical cases and participation of a widespread audience of experienced clinicians will allow better practice of dermatology and aid in further training of inexperienced clinicians.

BACKGROUND: Skin laser treatment has improved significantly and has become an effective treatment approach for many skin diseases while also having applications for beauty treatments. However, since skin laser transfers energy directly to the skin, the misuse of such treatment may result in permanent damage to skin tissues. OBJECTIVE: This survey of Korean adults, conducted to obtain their perspectives on and their treatment experience with skin laser treatment, will be used to determine the current status of skin laser treatment and to identify necessary changes to ensure proper and safe conduct of skin laser treatment. METHODS: From April 5th to April 12th, 2016, a survey was conducted to obtain information regarding the perspectives and the treatment experience of adults aged 20~59 years. RESULTS: Approximately 50% of the participants had experience with skin laser treatment, and among these, 24.7% had not received treatment at a dermatology clinic. Compared to treatment at a dermatology clinic, the danger of side effects was 1.7 times higher at a non-dermatology clinic, 2 times higher at a skin care shop, and 5.3 times higher at an Oriental medical clinic. Among patients who received skin laser treatment, 16.1% experienced side effects, and among these, 1 out of 4 patients visited a non-dermatologist for treatment of these side effects. CONCLUSION: The results of the survey showed that in order for the public to receive safe and effective skin laser treatment based on professional diagnosis, there is a definitive need to provide correct information to the public and to implement changes to ensure proper understanding of skin laser treatment among this population.

6.The value of direct immunofluorescence technique in diagnosis of blistering dermatoses

21 patients aged 2-76 years old (7 males, 14 females) with blistering dermatosis were diagnosed and treated in HCM city- dermatovenerology Hospital from May 1996 to May 1997- control group included subjects without blister. Primary diagnosis, routine tests, biopsy of blister, patho-anotomical examination and direct immunofluorescence assays, antibodies were conducted on the skin areas near by the blisters. Results showed that by direct IF assays, antibodies were detects as immunoflobulines and complements adhered on the skin, which got diagnostic values, to help to defimite and to differenciate in blistering dermatoses, dpecially in difficult cases.

7.Epidemiologic Study on Animals Possibly Causing Skin Diseases in Korea.

We have encountered many patients with dermatologic problems caused by animals, but there are not so many systematized literature about them in Korea. Since these causative animals are different from foreign animals in their distribution and habitat, there have been much difficulties in making diagnosis and in studying them based on foreign literature. We have undertaken this study to get the basic material for the possible causative animals in Korea by comparing with foreign literature, searching out the books of zoological claasification, and through personal contacts. One hundred and five species are found to be identical with the species reported in foreign literature, but 324 species are not, although they could be placed in the same genus.

8.A Statistical Study on Histopathologic Diagnoses of Skin Diseases.

BACKGROUND: Skin biopsy is an effective diagnostic tool commonly used in dermatology practice. Clinicopathologic correlation is important for diagnostic accuracy. OBJECTIVE: The purpose of this study was to investigate the distribution of skin biopsy specimens, and compare the pathologic diagnosis with the final dermatologic diagnosis. METHODS: This study included 4,018 skin biopsy specimens taken during a period of 10 years from June 1995 to May 2005 at the Department of Dermatology, Korea University Anam Hospital. RESULTS & CONCLUSION: Of all biopsy specimens, 61.49% were diagnosed as tumors, and 38.51% were diagnosed as non-tumors. For the tumors, 87.15% were benign, 4.23% were premalignant and 8.62% were malignant. The rate of concordance between pathologic diagnosis and final dermatologic diagnosis was 76.69%, and the rate of discordance between them was 4.18%. The rate of description without a pathologic diagnosis in pathologic reports was 18.94%. The rate of concordance was higher in tumors (89.65%) than in non-tumors (56.00%), and the rate of description in pathologic reports was higher in non-tumors (39.08%) than in tumors (6.32%). There was no significant difference in the rates of discordance between tumors and non-tumors. The rate of concordance was higher in malignant tumors (95.72%) than in benign tumors (89.77%) or premalignant tumors (74.76%). The rate of description in pathologic reports was higher in premalignant tumors (18.45%) than in benign tumors (6.27%) or malignant tumors (0.95%). There was no significant difference in the rates of discordance between the three groups.

9.A New Method for Diagnosing Erythemato-squamous Diseases Based on Virtual Coding and Multinomial Logistic Regression Penalized via Elastic Net.

Erythemato-squamous diseases are a general designation of six common skin diseases, of which the differential diagnosis is a difficult problem in dermatology. This paper presents a new method based on virtual coding for qualitative variables and multinomial logistic regression penalized via elastic net. Considering the attributes of variables, a virtual coding is applied and contributes to avoid the irrationality of calculating nominal values directly. Multinomial logistic regression model penalized via elastic net is thence used to fit the correlation between the features and classification of diseases. At last, parameter estimations can be attained through coordinate descent. This method reached accuracy rate of 98.34% +/- 0.0027% using 10-fold cross validation in the experiments. Our method attained equivalent accuracy rate compared to the results of other methods, but steps are simpler and stability is higher.

10.Concordance Rate Between Clinical Impression and Pathologic Diagnosis in Dermatologic Skin Biopsy.

BACKGROUND: Skin Biopsy is a equently used diagnostic tool in dermatology and clinical impressions before biopsy may be different with pathologic diagnosis. OBJECTIVES: The purpose of this study was to investigate the concordance rate of clinical impression and pathologic diagnosis. METHODS: Clinical and Pathological studies were done on 559 biopsied skin diseases among 16, 944 out-patients in Dermatologic department of Chungnam University Hospital during the period of 1 year from March, 2002 to February, 2003. RESULTS AND CONCLUSION: The study results are summarized as follows: 1. The most frequently biopsied skin disease group was non-tumor, noninfectious disease (40.8%), followed by benign tumor (37.4%), infectious disease (11.8%), malignant tumor (10.0%). 2. The concordance rate between clinical impression and pathologic diagnosis showed 68.9% in complete agreement and 79.6% in complete and partial agreement. 3. The concordance rate of professor committed cases was 68.8% and that of residents committed cases was 69.1%. 4. The concordance rates among disease groups were highest in malignant tumor (74.6%), then benign tumor (70.8%), non-tumor infectious disease (67.4%) and non-tumor noninfectious disease (65.2%).
