1.To assess the correlation between the skin microvasculary damages and the reticular pathology due to diabetes

Results of the biopsies from 55 diabetes patients with damage of capillary system on the skin were classified by Larsson’s scale. The state was mainly 2nd grade (41.8%), 1st grade and 3rd grade 29.1%. No case of 4th grade damage. The capillary damage was strictly assosciated to diabetes retinian capillary damage. The damage of skin capillary developed earlier considerably than that of the retine. The damage of capillary system on skin can be considered golden criterion for diagnosis of early damage of the capillary vessel in diabetes.

2.Dermatologic manifestation of Behcet's disease.

Dermatologic lesions in Behcet's disease are regarded as important manifestation for diagnosis. Various kinds of cutaneous lesions appear in patients with Behcet's disease. They present as erythema nodosum-like lesion, papulopustular eruption, erythema multiforme-like lesion, thrombophlebitis, skin ulcer, Sweet's syndrome-like lesion, bullous necrotizing vasculitis, and pyoderma gangrenosum. The hyperreactivity of the skin to any intracutaneous injection or needle prick, which is known as pathergy, is one clinically-unique feature of the disease.

14 workers in the 1, 8-diaminonaphthalene workshop of a chemical company in Nantong City had symptoms or signs of varying degrees of pruritus and pigmentation of the face, neck and waist. Pathological examination of skin biopsies showed hyperkeratosis, the basal cells were liquefied and denatured. Seven workers were eventually diagnosed with occupational melanosis. To explore the causes of occupational melanosis caused by exposure to 1, 8-dinitronaphthalene and 1, 8-diaminonaphthalene, and to provide reference for the prevention and treatment of occupational melanosis in the future, this paper reported 14 cases of melanosis in the skin of workers in chemical industry.

7.Painless skin nodules and ecchymosis in a school-aged girl.

A 7-year-old girl was admitted to Xiangya Hospital due to systemic lymphadenectasis for 2 months and skin ecchymosis for 3 days. Nine months ago, the girl experienced painless nodules in the left lower extremity with no apparent causes. Three months later, dermatorrhagia and ecchymosis occurred in many regions such as the periocular areas, conjunctiva, oral mucosa, perineal area, and groin, with a "raccoon sign" in both eyes; superficial lymphadenectasis and hepatosplenomegaly were also observed in many regions. The pathological sections for the skin nodules showed malignant tumors in lymphohematopoietic cells, and in combination with clinical manifestations, immunohistochemistry, and positive results for CD4, CD56, and CD123 by bone marrow flow cytometry, the girl was diagnosed with blastic plasmacytoid dendritic cell neoplasm. Then high-risk ALL regimen was applied as the chemotherapy for this girl. At present, the girl has been followed up for 3 months; ecchymosis has disappeared, and the enlarged lymph nodes have shrunk. No abnormal cells have been found in bone marrow morphological examination, and bone marrow flow cytometry has shown that primitive precursor cells account for 1.5% and express CD33, CD34, CD123, and CD117.

8.Mast Cell Changes in Skin Diseases.

Mast cell changes, numbers and degranulations, of 264 cases of skin biopsy lesions were studied. An increase of mast cells was noted in congenital diseases; noninfectious erythematous, papullar, and squamous diseases; vascular diseases; bacterial diseases; fungal diseases; lipidoses; metabolic diseases; connective tissue diseases; tumors and cysts of the epidermis; tumors of epidermal appendages; tumors of fibrous tissue; tumors of vascular tissue; and benign tumors of melanocytes. The increase was noted mainly in the surrounding areas of the lesions rather than within the lesions. In only a few conditions; vascular diseases, connective tissue diseases, and tumors of vascular tissue, an increase of mast cells within the lesion was noted. With regard to the relation between mast cell changes and gross appearance of skin lesions, an increase of mast cells was observed in the surrounding areas of scally, vesicular, nodular or warty, and ulcerated lesions. Relationship between the degree of degranulation to the types of skin disease or gross appearance can not be clearly established. In all conditions, the increase of mast cells was intimately associated with formation of new fibrous connective tissue.
