1.Quadrant System in lumbar disc herniation treatment of lumbar instability

BACKGROUND:There are stil about 10%-30%of patients presenting no obvious improvement of symptoms after lumbar disc herniation surgery, which are col ectively known as the lumbar spine post-surgery failure syndrome, and lumbar instability is one of the important reasons. OBJECTIVE:To evaluate the feasibility and efficacy of surgery including posterior lumbar spinal canal decompression, intervertebral disc resection, lumbar interbody fusion, and pedicle screw fixation on lumbar intervertebral disc protrusion concurring lumbar instability via MAST Quadrant retractor. METHODS:From December 2011 to October 2012, 62 cases of lumbar intervertebral disc protrusion concurring lumbar instability were treated with posterior lumbar spinal canal decompression, intervertebral disc resection, lumbar interbody fusion, and pedicle screw fixation via MAST Quadrant retractor. There were 38 female and 24 male ranging in age from 37 to 69 years (average 53.7 years). After operation, al patients were fol owed-up to evaluate the effect of the treatment. RESULTS AND CONCLUSION:The operative time was 90-210 minutes, average 145 minutes, and the amount of blood loss was 50-300 mL, average 120 mL. The hospitalization time was 5-9 days, average 6 days. Al incisions healed by first intention. Al patients were fol owed up 7.2 months on average (from 3 to 10 months). The preoperative JOA score was (10.25±2.34) points. The postoperative JOA score decreased to (18.31±3.12) points at the fol ow-up after 1 month and (25.35±2.61) points at the last fol ow-up, showing significant difference when compared with preoperative score (P<0.01). The preoperative VAS score was (8.24±1.15) points. The postoperative VAS score decreased to (2.97±1.12) points after 1 month and (1.13±0.39) points at the last fol ow-up, showing significant difference when compared with preoperative score (P<0.01). According to reforming Macnab standard, the results were excel ent in 53 cases and good in 9 cases at the last fol ow-up. The surgery including posterior lumbar spinal canal decompression, intervertebral disc resection, lumbar interbody fusion, and pedicle screw fixation via MAST Quadrant retractor is a safe, effective and minimal y invasive surgical technique in treating lumbar intervertebral disc protrusion concurring lumbar instability.
2.Clinical analysis of early rehabilitation treatment of 35 cases with acute myocardial infarction

The following report is on the early rehabilitation treatment of 35 cases with acute myocardial infarction,31 males and 4 females.The average age is 56.71±9.8 years.Among the 35 cases 15 are without complication of myocardial infarction and 20 are opposite.Besides the regular myocardial infarction treatment,the rehabilitation treatment is added after the initial period and before the patients leave the hospital, echocardiography, Holter monitor ECG and treadmill test are adopted to evaluate heart function.All the cases are without heart event during the early active treadmill test and rehabilitation treatment, so we conclude that it is safe and proper to early start treatment to acute myocardial infarction.The report will also discuss about the choice of the cases,the notes during the process and the evaluation of the cases after the treament and before leaving the hospital.
3.Preparation and Evaluation of a Hepatocyte Targeting pH-Sensitive Liposome

In order to obtain liposomes with properties of heptocyte-specificity and pH-sensitivity,four galactosylated derivatives were synthesized. A series of liposomes were prepared by mixing the galactosylated derivatives with DC-chol/DOPE respectively. The liposome 18-gal was proven to have favorable gene transfer efficiency to human hepatoma HepG2 cells, which was significantly inhibited in the presence of galactose solution, indicating that the liposomal transfection activity was mediated by asialoglycoprotein receptors. The liposome showed prominent pH-sensitivity and low cytotoxicity. Its optimum gene transfer conditions were also determined. The results showed that the liposome may be developed as a potential hepatocyte targeting pH sensitive delivery system for nucleic acid drugs.
4.The source and influential factors in signals of trans-esophageal oxygen saturation.

This paper is aimed to investigate the signal source and influential factors in signals of trans-esophageal pulse oxygen saturation (SeO2). The red light of the SeO2 probe was faced directly to the descending aorta (DA) of the mongrel dogs. The readings and waveform of SeO2 signals were recorded under following conditions: before and after DA was shield; before and after the blood supply of esophagus was cut off; under the different touch pressures between the SeO2 probe and the esophageal wall. The readings and waveform of SeO2 signals were also recorded respectively at both different esophageal depth and directions when mechanical ventilation was on and off. The tongue oxygen saturation (StO2) was recorded simultaneously as control. The waveform of SeO2 signals disappeared after DA was shield (P < 0.001). No significant difference was found in the SeO2 signals before and after the blood supply of esophagus was cut off (P > 0.05). Compared with the StO2 readings when the SeO2 probe was placed at different esophageal depth, the waldeyer ring, cervical area and thoracic inlet,the readings of SeOz significantly decreased (P < 0.05) while mechanical ventilation was on and off. However, there was no significant difference in the readings between SeO2 signals from DA, aortic arch (AA) and left subclavian artery and the StO2 signals recorded simultaneously. Mechanical ventilation had a remarkable effect on the SeO2 signals at different esophageal depth (P < 0.05), but the StO2 signals lay in its insensitivity to its influence. The readings of StO2 signals were significantly different from that of StO2 signals when the touch pressure between the SeO2 probe and the esophageal wall below 40 mmHg (P < 0.01). The directions of the optimum SeO2 signals acquired at different esophageal depth were not the same. The SeO2 signals were primarily derived from deeper arteries around the esophagus. All of Mechanical ventilation, location of the SeO2 probe in the esophagus and the touch pressure between the probe and esophageal wall can influence SeO2 signals.

5.Arrhythmia in Patients with Cervical Spinal Cord Injury: 61 Cases Report

ObjectiveTo explore the arrhythmia happened in patients with cervical spinal cord injury (CSCI). Methods61 patients with CSCI were retrospectively analyzed. ResultsThe overall incidence of sinus bradycardia was 26.2%, which was higher in the cases within 3 months (52.4%) than over (12.5%); and was higher in cases injured the cord above C5(46.2%)than lower (C6-8,11.4%). ConclusionThe risk of arrhythmia is still high in chronic phase, which still need to monitor.
6.Relationship between Objectively Determined Daily Physical Activities and Obesity Indices in Adults

Objective To investigate the relationship between daily physical activities determined by walking-steps and the obesity indices including body weight,Body Mass Index (BMI),waist and hip circumference and their ratio in adults. Methods 125 individuals who participated in routine physical examinations of health were measured with body weight, BMI, waist and hip circumference/ratio, daily physical activities determined by walking steps. Results For males, daily physical activities correlated negatively with body weight (r=-0.397,P<0.01), BMI (r=-0.499,P<0.01), waist circumference (r=-0.490, P<0.01) and the ratio of waist vs. hip circumference (r=-0.478,P<0.01), whereas body weight(r=-0.342,P<0.01), BMI (r=-0.249,P<0.05), and hip circumference (r=-0.303,P<0.05) for females. Daily walking-steps were significantly different among the groups of normal weight (11491±6100) steps/d, overweight (9314±5722) steps/d, and obesity (6141±2985) steps/d (P<0.01). Conclusion Daily physical activities determined by walking steps correlated significantly with obesity indices.
7.Relationship Between Daily Walking-steps and Serum Lipids in Chinese Adults

Objective To investigate the relationship between daily walking-steps and serum lipids.Methods 120 individuals who participated in routine physical examinations of health were investigated, objective measurements included triacylglycerol (TG), total cholesterol (TC), low density lipoprotein (LDL) and high density lipoprotein (HDL), and daily walking-steps were determined by pedometer.Results The daily walking-steps were correlated negatively to TG ( r=-0.351, P<0.01), and positively to HDL ( r=0.284, P<0.05) for male after adjustment for age.Conclusion Daily walking-steps determined by pedometer are correlated negatively to TG levels, and positively to HDL levels for male. No correlations are found between daily walking-steps and levels of TC, LDL for male. No correlations are found between daily walking-steps and serum lipids for female.
8.Relationship between Blood Pressure and Objectively Determined Daily Physical Activities in Chinese Adults

Objective To investigate the relationship between objectively determined daily physical activities and blood pressure levels in Chinese adults. Methods 125 individuals who participated in routine physical examinations of health were investigated, include systolic pressure, diastolic pressure, pulse pressure, and daily walking steps determined by pedometer. Results The daily walking steps correlated negatively with systolic pressure(Male r=-0.613, P<0.01; Female r=-0.430, P<0.01) and diastolic pressure (male r=-0.686, P<0.01; female r=-0.388,P<0.01). Daily walking steps of normotensive, pre-hypertensive, and hypertensive groups were (18249±6066) step/d, (10395±3788) step/d, (6232±3967) step/d for males, and (12564±6927) step/d, (11458±5084) step/d, (6153±3569) step/d for females respectively (P<0.05). Conclusion The daily physical activity determined by walking steps correlated negatively with blood pressure in Chinese adults. Hypertensives often live in low active or sedentary lifestyle. Active lifestyle (≥10000 step/d) may help keep normal blood pressure for Chinese adults.
9.The growing features of rabbit VX2 hepatic carcinoma model prepared with modified inoculation method

Objective To establish a rabbit VX2 liver tumor model by transplanting tumor tissue mass into the rabbit's liver,to analyze and observe the growing features of the liver tumor.Methods The tumor tissue mass(about 106-108 VX2 liver tumor cells)was inoculated into the left hepatic labe in 20 rabbits to establish rabbit VX-2 hepatic carcinoma model.The observation included the following two respects.(1)The tumor's volume at 7,10,14,17 and 21 days after the procedure was measured by ultrasonography and the growth rate of tumor was calculated.(2)The morphological feature of the tumor was inspected both macroscopically and microscopically.Results The growing pattern of the tumor was compatible with the exponential curve.Seventeen days after transplantation the increase rate of the tumor volume was much higher than that of the tumor diameter. Histopathologjcally,the growing pattern of the tumor took the form of infiltrative way,with its appearance being quite similar to the VX2 squamous cell carcinoma.Conclusion Transplantation of tumor tissue mass is the technique of first choice to establish the VX2 liver carcinoma model in rabbits.This experimental model is a very ideal animal form for both clinical and fundamental studies of liver carcinoma.
10.Comparative analysis of the complications of laparoscopic and abdominal cervical cancer surgery

Objective Compare the complications between laparoscopic and abdominal cervical cancer surgery, and investigate the safety of laparoscopic cervical cancer surgery and complications classified by Clavien-Dindo classification, then analyze the risk factors. Method Clinical data of 215 cases of cervical cancer received surgery from March 2011 to October 2014 was collected, which include intraoperative, postoperative and postoperative fol﹣low-up data. All the cases were divided into two groups: LRH group (n= 116) and ARH group (n= 88), then ana﹣lyze and compare the difference of intraoperative, postoperative complications and postoperative follow-up data be﹣tween the two groups, assess the safety of the two groups, and statistically concluded related independent risk factors. Results The complications of 204 patients were classified into 4 grades. The criticality and morbidity of intraopera﹣tive and postoperative complications have no significant difference between the two groups (P> 0.05). Logistic re﹣gression analysis show that over criticality grade Ⅱ of LRH intraoperative complication related to operating time, over criticality grade Ⅱ of ARH intraoperative complication related to aged over 50 yr, over criticality grade Ⅱ of ARH intraoperative complication related to BMI> 25. Conclusions Through the analysis of laparoscopic operation and the criticality of complications, the safety of the two groups were no difference. The patients with high risk fac﹣tors should be evaluated comprehensively. And strictly grasp the contraindication and indication.
