2.Clinical study of Tuina combined with functional training to improve the clinical symptoms and balance function in patients with meniscus injury

Objective:To observe the effect of Tuina(Chinese therapeutic massage)combined with functional training in improving clinical symptoms and balance function in patients with meniscus injury. Methods:A total of 93 patients with grade Ⅰ-Ⅱ meniscus injury were randomly divided into a functional training group,a Tuina group,and a combination group,with 31 cases in each group.The Tuina group received Tuina manipulation.The functional training group underwent functional training under the supervision of physicians.The combination group received Tuina manipulation in addition to the functional training.The treatment course of all three groups was 8 weeks.The clinical symptoms were observed before the intervention and after 4 and 8 weeks of interventions,and the changes in knee injury and osteoarthritis outcome score(KOOS)and Y-balance test(YBT)were compared. Results:There was no statistically significant difference in the total response rate among the three groups after treatment(P>0.05).The total and subscale KOOS in the three groups were significantly higher than those before treatment after 4 and 8 weeks of interventions(P<0.05).There was a statistically significant difference in the total KOOS between the combination group and the functional training group after 4 and 8 weeks of interventions(P<0.05);there was a statistically significant difference in the score of daily living ability and sports and recreation function between the two groups after 8 weeks of interventions(P<0.05).The YBT balance coefficients of the affected and non-affected knees of the three groups were significantly improved after 4 and 8 weeks of interventions(P<0.05),and there was no statistically significant difference among the groups(P>0.05). Conclusion:Tuina,functional training,and their combination can improve clinical symptoms and balance function in patients with grade Ⅰ-Ⅱ meniscus injury.Tuina combined with functional training is significantly effective in improving daily living ability and sports and recreation function.
3.Advances in neoadjuvant therapy of esophageal cancer involved in organ preservation

Esophageal squamous cell carcinoma is one of the most common cancer in China, and locally advanced stage remains a heterogeneous and complex disease. Its treatments are constantly evolving. To date, radiochemotherapy and surgery are involved in the comprehensive treatment of this disease. However, because of the specifical anatomical location of esophagus, its physiological function will be impaired after surgical resection. If similar survival is obtained in esophageal cancer, an organ preserving strategy without surgery is of great significance for improving the quality of life. As a non-invasive tumor treatment method, neoadjuvant therapy plays an important role in the comprehensive treatment of preserving the esophagus. Recently, there are studies investigating the role of organ preserving strategy in the comprehensive treatment of esophageal cancer with neoadjuvant therapy, and the recognition of these advances will provide reference value for the comprehensive treatment of esophageal cancer.
4.Analysis of virulence and drug resistance characteristics of diarrheagenic Escherichia coli isolated from patients with infectious diarrhea in a hospital in Beijing

Objective To investigate the virulence and drug resistance characteristics of diarrheagenic Escherichia coli isolated from patients with infectious diarrhea in our hospital.Methods The preliminary identification of microbes was carried out by the VITEK-MS microbial mass spectrometry detection system and virulence genes were detected by the multiplex real-time PCR.Five types of diarrhea-genic Escherichia coli(DEC)clinically isolated from patients with infectious diarrhea in our hospital were identified.The drug resist-ance characteristics of DEC strains were detected by the microbroth dilution and E-test.The drug-resistant molecular characteristics were analyzed by the next-generation sequencing and bioinformatics.The Fisher exact probability method was used for statistical analy-sis.Results The detection rate of DEC in our hospital was 11.9%,with enteroaggregative E.coli(EAEC)accounting for 37.5%,a-typical enteropathogenic Escherichia coli(EPEC)accounting for 34.38%,enterotoxigenic E.coli(ETEC)accounting for 25.0%,and enteroinvasive E.coli(EIEC)accounting for 3.12%.None of enterohemorrhagic E.coli(EHEC)strain was detected.The resistance rates of 32 DEC strains to ampicillin,tetracycline,and trimethoprim/sulfamethoxazole were 53.12%,43.75%,and 37.5%,respec-tively.ESBLs(+)strains accounted for 18.75%,and the detection rate of multidrug-resistant strains was 83.83%,significantly higher than that of ESBLs(-)strains(P=0.042).A total of 25 ST genotypes were obtained from 32 DEC strains.The dominant genotypes were ST10(4 strains,12.5%),followed by ST28(2 strains,6.25%),ST31(2 strains,6.25%),ST3153(2 strains,6.25%),and the other 21 genotypes(1 strain,3.13%).One carbapenem resistant strain carrying the blaNDM-1 gene was detected in EAEC.Conclu-sion Four virulence genes such as aggR,pic,astA,and eae,are more common in the DEC of patients with infectious diarrhea in our hospital,with EAEC and EPEC as the main subtypes.The genotypes are highly polymorphic,and multidrug-resistant strains have been detected.
5.Analysis of clinical efficacy and safety of local radiotherapy in children with stage Ⅳ high-risk neuroblastoma

Objective To retrospectively analyze the clinical efficacy,safety and influencing factors of radiotherapy in children with stage Ⅳ high-risk neuroblastoma (HR-NB).Methods A total of 120 children with HR-NB who were diagnosed and treated with local radiotherapy according to the BCH-HR-NB-2007 protocol in the Oncology Department of Beijing Children's Hospital from January 2014 to December 2017 were enrolled.Among them,56 children were male and 64 female with a median age of 43 months (9 -148 months).The treatment protocol consisted of 4 cycles of CAV chemotherapy,3 cycles of CVP chemotherapy,surgical resection after 4 cycles,autologous hematopoietic stem cell transplantation after 7 cycles,local radiotherapy at a dose of 15.0-30.6 Gy for 82 cases of primary tumors and 38 cases of primary and metastatic tumors,followed by 13 cis-retinoic acid as maintenance therapy.The entire treatment protocol endured for approximately 18 months.Results The median follow-up time was 21 months.The 3-year local control rate was 84.4%.Before radiotherapy,the 3-year event-free survival rate was 78.4% in children without metastases,significantly higher compared with 30.4% in the residual group (P=0.003).The 3-year event-free survival rate was 66.1% in patients who underwent radiotherapy within 6 months after surgery,significantly higher than 50.6% in their counterparts receiving radiotherapy at 6 months or more after surgery (P=0.018).Among the children with residual metastases before radiotherapy,the progression rate in children who did not receive radiotherapy was 66.6%,significantly higher compared with 20.0% in those receiving radiotherapy (P=0.001).All patients had no radiation-related adverse reactions in the liver,kidney and heart,etc.The incidence rate of grade Ⅲ-Ⅳ myelosupression was 24.5% at 1 week post-radiotherapy,and 8% at 2 weeks after radiotherapy.Conclusions Radiotherapy yields definite clinical efficacy in the local control of children with stage Ⅳ HR-NB.Early radiotherapy after surgery and radiotherapy for the metastatic lesions can improve the clinical prognosis.No vital organ injuries are observed during the short-term follow-up.At 2 weeks after radiotherapy,the myelosupression is gradually restored.
6.Pathogenesis and Traditional Chinese Medicine Treatment of Rheumatoid Arthritis Based on Theory of "Harmful Hyperactivity and Responding Inhibition"

Rheumatoid arthritis (RA) is a chronic autoimmune disease with a complex pathogenesis. Immune dysfunction, synovial inflammation, and bone destruction are the key pathological links. The theory of "harmful hyperactivity and responding inhibition" is a high-level summary of the coordinated development of things in nature and the generation and restriction of the five elements and the six factors in nature. People and all things have the same origin, and the theory of "harmful hyperactivity and responding inhibition" represents the intrinsic regulation mechanism of the human body's homeostasis, reflecting the unity of opposites of "hyperactivity" and "inhibition" and emphasizing coordination and stabilization. In the pathogenesis of RA, the excessive immune response disrupts the normal body balance, which is closely related to the process of "hyperactivity becoming harmful". Synovial inflammation, tissue hyperplasia, and bone destruction are pathological results of the dysregulation of the body's immune self-stabilization function and can be regarded as the process of "failing to inhibition". Therefore, the theory of "hyperactivity harmful hyperactivity and responding inhibition" provides a unique perspective for understanding the modern pathological mechanisms of RA. Based on the theory of "harmful hyperactivity and responding inhibition" and the pathogenesis of RA, the author analyzed the modern medical basis of RA from the perspective of traditional Chinese medicine (TCM) and revealed the intrinsic connection between TCM pathogenesis such as insufficiency of vital energy and blood, strong defensive Qi and weak nutrient Qi, and intertwined phlegm and blood stasis and modern research on autoimmune disorders, synovial inflammation, and bone destruction. With the therapeutic criterion of "harm inhibition and responsible supporting", the article summarized the mechanism of TCM in calming hyperactivity and supporting invincibility, which provided theoretical references for the clinical treatment of RA.
7.Adverse reactions of acupoint embedding and its treatment.

In this paper the adverse reactions of acupoint embedding and its treatment are summarized, including fever, infection, callosity, blood stasis, red swelling, numb swelling, pain, fester, ulcer, etc. It is consi-dered that adverse reactions are induced by the inappropriate operation, special constitution and embedding material. Thus we need to pay attention to standard manipulation, the contraindication, and cautious usage. Symptomatic treatment needs to be implemented timely once adverse reactions occur.
8.Human factors engineering of brain-computer interface and its applications: Human-centered brain-computer interface design and evaluation methodology.

Brain-computer interface (BCI) is a revolutionizing human-computer Interaction, which is developing towards the direction of intelligent brain-computer interaction and brain-computer intelligent integration. However, the practical application of BCI is facing great challenges. The maturity of BCI technology has not yet reached the needs of users. The traditional design method of BCI needs to be improved. It is necessary to pay attention to BCI human factors engineering, which plays an important role in narrowing the gap between research and practical application, but it has not attracted enough attention and has not been specifically discussed in depth. Aiming at BCI human factors engineering, this article expounds the design requirements (from users), design ideas, objectives and methods, as well as evaluation indexes of BCI with the human-centred-design. BCI human factors engineering is expected to make BCI system design under different use conditions more in line with human characteristics, abilities and needs, improve the user satisfaction of BCI system, enhance the user experience of BCI system, improve the intelligence of BCI, and make BCI move towards practical application.

INTRODUCTION:
Three doses of SARS-CoV-2 mRNA vaccines have been recommended for cancer patients to reduce the risk of severe disease. Anti-neoplastic treatment, such as chemotherapy, may affect long-term vaccine immunogenicity.
METHOD:
Patients with solid or haematological cancer were recruited from 2 hospitals between July 2021 and March 2022. Humoral response was evaluated using GenScript cPASS surrogate virus neutralisation assays. Clinical outcomes were obtained from medical records and national mandatory-reporting databases.
RESULTS:
A total of 273 patients were recruited, with 40 having haematological malignancies and the rest solid tumours. Among the participants, 204 (74.7%) were receiving active cancer therapy, including 98 (35.9%) undergoing systemic chemotherapy and the rest targeted therapy or immunotherapy. All patients were seronegative at baseline. Seroconversion rates after receiving 1, 2 and 3 doses of SARS-CoV-2 mRNA vaccination were 35.2%, 79.4% and 92.4%, respectively. After 3 doses, patients on active treatment for haematological malignancies had lower antibodies (57.3%±46.2) when compared to patients on immunotherapy (94.1%±9.56, P<0.05) and chemotherapy (92.8%±18.1, P<0.05). SARS-CoV-2 infection was reported in 77 (28.2%) patients, of which 18 were severe. No patient receiving a third dose within 90 days of the second dose experienced severe infection.
CONCLUSION
This study demonstrates the benefit of early administration of the third dose among cancer patients.

10.Mechanism of Total Glucosides of Paeony in Attenuating Neurotoxicity of Aqueous Extract of Strychni Semen via GRIN2A/PLCB1/PRKCG Signaling Pathway

ObjectiveTo investigate the effect of total glucosides of paeony (TGP) on neurotoxicity induced by aqueous extract of Strychni Semen (SA) in mice and to explore its mechanism. MethodThirty-two male KM mice were randomly divided into normal group,SA group (19.5 mg·kg-1),TGP group (225 mg·kg-1),and SA+TGP group (SA 19.5 mg·kg-1+TGP 225 mg·kg-1). The open field test and beam walking test were used to observe the behavioral changes in mice. Pathological changes in the Nissl bodies of the cerebral cortex were assessed through Nissl staining. The levels of malondialdehyde (MDA),glutamate (Glu) in the mouse brain tissue,and serum levels of 5-hydroxytryptamine (5-HT) were detected using enzyme-linked immunosorbent assay (ELISA). Transcriptome sequencing was employed to analyze gene expression profiles in the brain tissue. Common differentially expressed genes (DEGs) underwent gene ontology (GO) and kyoto encyclopedia of genes and genomes (KEGG) enrichment analyses. The mRNA expression levels of key targets were determined using quantitative real-time polymerase chain reaction (Real-time PCR). ResultCompared with the normal group,the SA group exhibited significant increases in side-to-side distance and average speed in the open field test,as well as increased walking time on the balance beam. The axons of cortical neurons were absent,and the levels of Glu and MDA in the brain tissue were significantly elevated (P<0.05,P<0.01),along with a notable increase in serum 5-HT levels (P<0.05). In contrast to the SA group,the SA+TGP group significantly reduced the side-to-side distance,average speed,and balance beam walking time (P<0.05 or P<0.01). The neuronal axons were clearly visible,and levels of 5-HT,Glu,and MDA were decreased (P<0.05,P<0.01). Transcriptome analysis indicated that TGP could regulate the glutamate receptor,ionotropic,N-methyl-D-aspartate 2a (GRIN2A)/phospholipase C β1 (PLCB1)/protein kinase C,gamma (PRKCG) signaling pathway. Compared with the normal group,SA significantly decreased the expression of GRIN2A,PLCB1,and PRKCG genes in the mouse brain (P<0.01),while the mRNA levels of GRIN2A and PRKCG significantly increased after TGP administration (P<0.05,P<0.01). ConclusionSA induces significant neurotoxicity in the mouse brain,and TGP significantly alleviates SA-induced neurological damage,potentially through the GRIN2A/PLCB1/PRKCG signaling pathway.
