Journal of International Oncology 2021;48(11):673-677
Esophageal squamous cell carcinoma is one of the most common cancer in China, and locally advanced stage remains a heterogeneous and complex disease. Its treatments are constantly evolving. To date, radiochemotherapy and surgery are involved in the comprehensive treatment of this disease. However, because of the specifical anatomical location of esophagus, its physiological function will be impaired after surgical resection. If similar survival is obtained in esophageal cancer, an organ preserving strategy without surgery is of great significance for improving the quality of life. As a non-invasive tumor treatment method, neoadjuvant therapy plays an important role in the comprehensive treatment of preserving the esophagus. Recently, there are studies investigating the role of organ preserving strategy in the comprehensive treatment of esophageal cancer with neoadjuvant therapy, and the recognition of these advances will provide reference value for the comprehensive treatment of esophageal cancer.

[Objective]To explore the role of estrogen and estrogen receptor (ER) in the pathogenesis and regulation of non-invasive fungal rhinosinusitis (NIFR).[Methods]Totally 60 patients with NIFS who met the inclusion criteria in Fuzhou Second General Hospital from November 2020 to November 2023 were selected as the NIFS group, while 30 healthy volunteers were recruited as the blank control group. Samples of each group were collected. The number of eosinophils and mast cells in each group were detected by HE staining;ER expression was detected by immunohistochemistry and immunofluorescence;mRNA expression levels of NF-κB,IKK and MASPIN were detected by qPCR;and protein expression levels of NF-κB,IKK and MASPIN were detected by immunohistochemistry and Western blot.[Results]In the NIFS group, the counts of eosinophils and mast cells were significantly increased respectively, compared with those in the control group, and the inter-group differences are statistically significant (P<0.01 and P<0.05,respectively). The Estrogen Receptor (ER) score in the NIFS group was significantly increased compared with that in the control group, and the inter-group differences are statistically significant (P<0.05). Additionally, the average high-density value in the NIFS group was significantly increased compared with that in the control group, and the inter-group differences are statistically significant (P<0.01). The expression levels of NF-κB,IKK,and MASPIN in the NIFS group were significantly increased respectively, compared with those in the control group, and the inter-group differences are statistically significant (P<0.01,P<0.05,and P<0.01,respectively). The mRNA expression levels of NF-κB,IKK,and MASPIN in the NIFS group were significantly increased, respectively, compared with those in the control group (P<0.01). Furthermore, the protein expression levels of NF-κB,IKK,and MASPIN in the NIFS group were increased, respectively, and the inter-group differences are statistically significant (P<0.01,P<0.01,and P<0.05,respectively).[Conclusion]Our results show that the significant increase in the number of eosinophils and mast cells, and in the expression levels of ER,NF-κB,IKK and MASPIN may indicate a significant increase in eosinophil and mast cell infiltration in ER positive patients, and suggest the involvement of estrogen and its receptors in the pathogenesis of NIFS.

Real world data (RWD) refers to the data generated in routine clinical practices, daily life, and real work environment and has been widely used in clinical or public health research. Still, issues related to the quality of RWD, such as incompleteness, inconsistency, and inaccuracy, would affect the validity of real-world research. To overcome the challenges due to the lack of standardization of real world source data, case report form based on clinical data interchange standards consortium (CDISC-CRF) on certain diseases was developed to promote the ecology construction of RWD based on the data standards set by the CDISC which has been widely used. Firstly, we described how to apply data standards to make up the gap between RWD and real world evidence. Then, the process was designed to build RWD ecology based on CDISC-CRF, in which the development technology of CDISC-CRF form is mainly introduced. Finally, the application prospect and significance of building real-world data based on disease-specific CDISC-CRF are described. It is believed that the present paper can provide a new idea for promoting the ecology construction of RWD in China.

Journal of Zhejiang Chinese Medical University 2024;48(7):862-871
[Objective]To establish a liquid chromatography-tandem mass spectrometry(LC-MS/MS)method for the determination of 5-hydroxytryptamine(5-HT)and 5-hydroxyindole acetic acid(5-HIAA)in mouse serum and hippocampus, applied to the evaluation of the antidepressant efficacy of traditional Chinese medicine.[Methods]The serum and hippocampus were taken and the concentrations of 5-HT and 5-HIAA were determined by LC-MS/MS after pretreatment of protein precipitation.Chromatographic separation was performed on Hypersil ODS(250 mm×4.6 mm,5 μm)at column temperature of 40℃.The mobile phase was 0.1%formic acid-water solution with 2 mmol·L-1 ammonium acetate and acetonitrile whereas flow rate was set at 1 mL·min-1.The mass spectrometry detection was performed using multiple reaction monitoring(MRM).One hundred and ninety-eight mice were randomly divided into normal control group, model group, fluoxetine hydrochloride group and 30 groups of Paeoniae Radix Alba with 6 rats in each group.Tail suspension test and forced swimming test were carried out.[Results]5-HT and 5-HIAA had good linear relationship within the concentration range of 0.1~1 000 ng·mL-1 and 1~500 ng·mL-1,respectively(r>0.999).Precision and recovery of this method were in line with the requirements of biological sample analysis. The degree of change in 5-HIAA/5-HT values in the Paeoniae Radix Alba group relative to the model group was correlated with the degree of change in indicators of the behavioral experiment, and the degree of change was significantly higher than that of the behavioral experiment, with a much smaller within-group relative standard deviation(RSD).[Conclusion]This LC-MS/MS method is simple and fast for operation, with accurate results, high sensitivity, good reproducibility and strong specificity which indicated that was suitable for the simultaneous determination of 5-HT and 5-HIAA in serum and hippocampus. It could be used for research and efficacy evaluation of neuroprotective drugs like Paeoniae Radix Alba.

Chinese Journal of Experimental Traditional Medical Formulae 2024;30(23):300-307
Rheumatoid arthritis (RA) is a chronic autoimmune disease with a complex pathogenesis. Immune dysfunction, synovial inflammation, and bone destruction are the key pathological links. The theory of "harmful hyperactivity and responding inhibition" is a high-level summary of the coordinated development of things in nature and the generation and restriction of the five elements and the six factors in nature. People and all things have the same origin, and the theory of "harmful hyperactivity and responding inhibition" represents the intrinsic regulation mechanism of the human body's homeostasis, reflecting the unity of opposites of "hyperactivity" and "inhibition" and emphasizing coordination and stabilization. In the pathogenesis of RA, the excessive immune response disrupts the normal body balance, which is closely related to the process of "hyperactivity becoming harmful". Synovial inflammation, tissue hyperplasia, and bone destruction are pathological results of the dysregulation of the body's immune self-stabilization function and can be regarded as the process of "failing to inhibition". Therefore, the theory of "hyperactivity harmful hyperactivity and responding inhibition" provides a unique perspective for understanding the modern pathological mechanisms of RA. Based on the theory of "harmful hyperactivity and responding inhibition" and the pathogenesis of RA, the author analyzed the modern medical basis of RA from the perspective of traditional Chinese medicine (TCM) and revealed the intrinsic connection between TCM pathogenesis such as insufficiency of vital energy and blood, strong defensive Qi and weak nutrient Qi, and intertwined phlegm and blood stasis and modern research on autoimmune disorders, synovial inflammation, and bone destruction. With the therapeutic criterion of "harm inhibition and responsible supporting", the article summarized the mechanism of TCM in calming hyperactivity and supporting invincibility, which provided theoretical references for the clinical treatment of RA.
