1.Epidemiological features of leprosy in China from 2011 to 2015

Objective To analyze the epidemiological features of leprosy in China from 2011 to 2015,and to provide scientific evidences for prevention and treatment strategies.Methods An epidemiological analysis and a trend analysis were conducted based on the national leprosy surveillance data from 2011 to 2015.Results The leprosy detection rate in China decreased from 0.085 per 100 000 in 2011 to 0.049 per 100 000 in 2015,with an average annual decline rate being 12.9%.A total of 4 775 leprosy cases were newly detected during 2011-2015,including 106 (2.2%) children,1 499 (31.4%) females,518 (10.8%) floating people,4 041 (84.6%) multibacillary cases and 1 134 (23.7%) cases with grade 2 disabilities.From 2011 to 2015,328 relapsed cases were reported,including 153 (46.6%) cases recurring after combined chemotherapy.The prevalence rate of leprosy in China decreased from 0.407 per 100 000 in 2011 to 0.235 per 100 000 in 2015,with an average annual decline rate being 12.9%.By the end of 2015,there had been 3 230 registered leprosy cases and 124 counties with a prevalence rate above 1 per 100 000.Conclusions The detection rate and prevalence rate of leprosy in China were both decreasing continuously from 2011 to 2015.The high-epidemic provinces were Yunnan,Guizhou,Sichuan and Guangdong.However,leprosy control in middle-and low-epidemic provinces can not be ignored.
2.Differential expression profiles of microRNAs/mRNA and docking study in oral squamous cell carcinoma.

OBJECTIVETo construct and analyze the differential expression profiles of microRNAs (miRNA) and mRNA in oral squamous cell carcinoma (OSCC). To explore the miRNA and mRNA of OSCC development and progression.
METHODSThe differential expression profiles of miRNA and mRNA were built by high-throughput deep sequencing technology. Using Gene Ontology (GO) enrichment analysis, the roles of differentially expressed miRNA and mRNA in cell cycle, cell proliferation, cell differentiation and cell apoptosis were analyzed.
RESULTSSeventy-seven differentially expressed miRNA and 1 298 differentially expressed mRNAs were identified in OSCC. GO analysis showed that 73 miRNA had found target mRNA in cell cycle, cell proliferation, cell differentiation and cell apoptosis of OSCC. Moreover, a miRNA could regulate multiple mRNA.
CONCLUSIONThe differential expression profiles of miRNA and mRNA have close relationship with the development and progression of OSCC.

3.Visual analysis of the research status of ischemic optic neuropathy in China in the past 20 years

Objective:To gain an in-depth understanding of the research status, hotspots, and future development trends in the field of ischemic optic neuropathy (ION).Methods:Using "ischemic optic neuropathy" as the subject heading or keyword to search for relevant literature in Chinese and English databases from January 1, 2000, to December 31, 2022. The bibliometrics method and software were applied to construct the visualization map of authors, institutions, keyword co-occurrence, outburst words, and keyword clustering.Results:A total of 1 203 ION-related articles were included, 1 106 Chinese literature and 97 English literature were included; the number of published articles in this field has fluctuated and increased in the past 20 years, mainly Chinese literature and English literature have shown a low growth trend. Chinese literature involved a total of 2 171 authors, and English literature involved 368 Chinese authors. A core team represented by Wang Runsheng, Wei Shihui, Zhong Yong, and Wei Qiping was formed among the high-yielding authors. Chinese literature involved a total of 799 research institutions, and English literature covered 119 Chinese institutions. The Xian No.1 Hospital and Beijing Tongren Hospital Affiliated to Capital Medical University respectively ranked first in the number of Chinese and English literature published in this field; 121 and 23 high-frequency keywords in Chinese and English were identified. In addition to "ischemic optic neuropathy" , compound anisodine, visual field, vision, treatment, risk factors, pathogenesis, optic nerve and rAION also appeared more frequently. The Chinese literature obtained 13 emergent words, and the English literature keywords formed 11 clusters. From the perspective of research type, the Chinese and English literature in this field mainly focued on the clinical efficacy observation of nonarteriotic Anterior ischemic optic neuropathy (NAION).Conclusions:In the past 20 years, clinical studies of ION in China have mainly focused on the treatment of NAION, risk factors, and the application of auxiliary examinations in disease diagnosis. The combination of drugs in treatment, the application of optical coherence tomography angiography, and the research on pathogenesis is still a future research trend in this field.
4.Factors Associated with Echinococcosis-Induced Perioperative Anaphylactic Shock.

This retrospective case-control study explored the factors associated with anaphylactic shock during surgery for cystic echinococcosis (CE) at the First Affiliated Hospital of Xinjiang Medical University between October 2008 and September 2013. Patients who suffered from anaphylactic shock (n=16) were age-matched 3:1 to patients who did not (n=43). Multivariate analysis suggested that IL-4 levels (odds ratio=1.096; 95% confidence interval=1.015–1.185; P=0.02) and cyst size (odds ratio=3.028, 95% confidence interval=1.259–7.283, P=0.013) were independently associated with CE-induced perioperative anaphylactic shock. Using the receiver operating characteristic (ROC) curves and a cut-off value of 415.7 ng/ml, IL-4 showed an area under the ROC (AUC) of 0.926, sensitivity of 75.0%, and specificity of 97.7%. Using a cut-off value of 7.8 cm, cyst size showed an AUC of 0.828, sensitivity of 81.3%, and specificity of 76.7%. In conclusion, results suggest that levels of IL-4 and cyst size were independently associated with echinococcosis-induced perioperative anaphylactic shock. These results could help identifying patients with echinococcosis at risk of anaphylactic shock in whom appropriate prophylaxis could be undertaken.

5.Prognostic factor analysis of pneumonectomy for non-small cell lung cancer.

OBJECTIVESTo identify predictors of survival following pneumonectomy for non-small cell lung cancer (NSCLC) and provide evidence for the revision of patient selection criteria.
METHODS81 cases of pneumonectomy for NSCLC from January 1990 to May 1996 at our hospital were reviewed retrospectively. There were 65 men (80.2%) and 16 women (19.8%), with a mean age 53.4 +/- 9.4 years (range 20 - 68 years). Predominant histological types included squamous cell carcinoma (54.3%), adenocarcinoma (24.7%), and squamoadenocarcinoma (17.3%). After follow-up for more than 5 years, data were examined using the chi-square test, Kaplan-Meier method, and Cox-mantel test. The possible factors affecting survival were tested with univariate and multivariate analysis.
RESULTSThe 5-year survival of N(0), N(1) and N(2) disease of NSCLC following pneumonectomy was (20.8 +/- 9.9)%, (15.4 +/- 10.0)% and (4.0 +/- 2.8)%, respectively. There was no perioperative death. The operative complications morbidity was 22.2%. Factors adversely affecting survival with univariate analysis included age over 60 years for right pneumonectomy, cardiopulmonary complications, adenocarcinoma, peripheral location, tumor greatest dimension more than 10 cm, chest wall involvement and N(2) disease. Factors adversely affecting survival with multivariate analysis included cardiopulmonary complications, greatest tumor dimension more than 10 cm, chest wall involvement and N(2) disease.
CONCLUSIONSPneumonectomy provides survival benefit with a high operative complications morbidity. Old age (>/= 60 years) for right pneumonectomy, cardiopulmonary complications, adenocarcinoma, and N(2) disease may be negative prognostic factors of long-term survival. Patient selection should be based on cardiopulmonary evaluation and the stage of disease.

6.Diagnostic efficacy of prostate imaging reporting and data system version 2.1 based on multiparametric MRI in detection of clinically significant prostate cancer

Objective:To evaluate the diagnostic performance of the prostate imaging reporting and data system version 2.1 (PI-RADS v2.1) based on multiparametric MRI (mpMRI) in the detection of clinically significant prostate cancer (csPCa).Methods:A total of 561 patients who underwent prostate mpMRI in the First Affiliated Hospital of Guangxi Medical University from June 2015 to December 2020 due to elevated prostate specific antigen were collected ambispectively. The patients were divided into csPCa group (276 cases) and non-csPCa group (285 cases) according to pathological findings. Prostate were scored according to the PI-RADS v2.1 scoring standard by a junior and a senior radiologist. The prostate volume was measured and the prostate specific antigen density (PSAD) was calculated. The diffusion-weighted imaging and dynamic contrast-enhanced MRI images were processed to measure the quantitative parameters of the index lesion, including apparent diffusion coefficient (ADC), volume transfer constant (K trans) and rate constant (K ep) values. The Mann-Whitney U test was used to compare the difference in parameters between the two groups. The predictors of csPCa were screened by logistic regression analysis. Predictive model of multi-parameter was established. The receiver operator characteristic curves were used to evaluate the efficacy of PI-RADS v2.1 and the model in diagnosing csPCa, and the comparisons of area under the curve (AUC) were conducted by DeLong test. Results:Compared with non-csPCa group, the patients in csPCa group had higher PI-RADS score of senior physician, PSAD, K trans and K ep value, lower ADC value ( Z=-16.69, -12.49, -3.43, -4.67, 13.91, all P<0.001). The PI-RADS scores of senior physician (OR=3.064, 95%CI 2.428-3.866, P<0.001), PSAD (OR=1.554, 95%CI 1.170-2.064, P=0.002) and ADC value (OR=0.095, 95%CI 0.032-0.288, P<0.001) were the predictors of csPCa. The AUC of junior, senior physician PI-RADS and combined prediction model were 0.861 (95%CI 0.830-0.892), 0.895 (95%CI 0.868-0.922) and 0.923 (95%CI 0.898-0.944). The pairwise difference was statistically significant (the PI-RADS score between the junior and senior physicians Z=3.24, P=0.001, the difference between the PI-RADS score of junior physician and prediction model Z=5.54, P<0.001, the difference between the PI-RADS score of senior physician and prediction model Z=4.20, P<0.001). Conclusion:Based on mpMRI, the application of PI-RADS v2.1 by junior and senior radiologists has the high diagnostic efficacy for csPCa, and the multi-parameter model has the best diagnostic efficacy for csPCa.
7.Perinatal management and genetic testing and for a puerperant affected with ATP7B gene variation in Wilson disease

Objective:To explore the perinatal management of patients with WD during pregnancy, and to determine its genetic etiology and the possibility of fetal morbidity using the genetic detection of amniotic fluid and umbilical cord blood.Method:In terms of fine management during the perinatal period, a case of K-F ring was found in the Ophthalmology Department of Beijing Friendship Hospital, Capital Medical University in March 2019 due to eye astringency and eye swelling, and the hepatology department further diagnosed WD for one artificial abortion. After the second pregnancy in October 2020, multidisciplinary consultation and standardized treatment during pregnancy including gynecology and obstetrics, liver disease center, anesthesiology department, gastroenterology department and nutrition department were carried out. The genomes of patients' venous blood, amniotic fluid and umbilical cord blood were extracted and analyzed for ATP7B gene variation by Sanger sequencing.Result:Through multi-disciplinary collaborative management, the patient gave birth successfully in the case of pregnancy complicated with liver cirrhosis, portal hypertension, splenomegaly with hyperfunction, thrombocytopenia, anemia, esophageal and gastric varices and other complications. The phenotype of the newborn was normal, and the Apgar score was 10-10-10. Sequencing results showed that the patient had ATP7B p.Arg778Leu and p.Val890Met, which were missense heterozygous variants reported in the mutation database, and ACMG was classified as pathogenic variants. The results of amniotic fluid and umbilical cord blood showed that the fetus had only p.Arg778Leu single heterozygous variation, and it was predicted that there would be no clinical phenotype of WD.Conclusion:Perinatal multidisciplinary collaborative management has important protective significance for the successful pregnancy of patients with WD. Genetic screening of amniotic fluid and umbilical cord blood is conducive to early detection of fetal WD.
