1.A PreIiminary Study of the CIinicaI Significance of Word Recognition Scores in EvaIuating the Curative Effects of Sudden Hearing Loss

Objective To preliminarily explore the clinical significance of word recognition scores in evalua_ting the curative effects of sudden hearing loss .Methods Pure tone averages (PTA) and word recognition scores (WRS) in 11 cases of sudden hearing loss were obtained on the pre-treatment day and the post -treatment day , and the results were compared .ResuIts The average PTA of 11 cases was 62 .27 ± 15 .36 dB HL on the pre-treat_ment day ,and 60 .23 ± 14 .32 dB HL on the post -treatment day .The average WRS in 11 cases was 42 .91% ± 30 .83% on the pre-treatment day ,and 72 .73% ± 26 .72% on the post-treatment day .There were no significant differences in PTA and the intensity of the sound for testing WRS before and after treatment(P> 0 .05) ,and the difference of the WRS between the pre -treatment and the post -treatment day was statistically significant(P<0 .05) .ConcIusion The WRS could be used in evaluating the curative effect of sudden hearing loss ,which is a sup_plement for PTA .
2.The change of the serum myeloperoxidase and lipoxin A4 in patients with coronary heart disease

Objective To investigate the changes and significance of the serum myeloperoxidase (MPO) and lipoxin (LXA4) in patients with coronary heart disease (CHD). Methods From December 2010 to February 2011,143 patients with CHD (CHD group) and 44 patients without CHD (control group) were selected. The serum high sensitive C-reactive protein (hs-CRP), MPO and LXA4 levels were assessed, and the ratio of MPO and LXA4 (M/L) was calculated. These indexes were compared between 2 groups. The influencing factors of MPO, LXA4 and M/L levels were analyzed by multifactor Logistic regression analysis. Results The serum levels of hs-CRP, LXA4, MPO, and M/L in CHD group were significantly higher than those in control group: 3.25 mg/L vs. 0.99 mg/L, 229.88 ng/L vs. 178.63 ng/L, 422.58 U/L vs. 186.85 U/L and (1.78 ± 0.52) U/ng vs. (1.02 ± 0.17) U/ng, and there were statistical differences (P<0.05). Multifactor Logistic regression analysis result showed: LXA4 level, unstable angina, acute myocardial infarction and severe elevation of white blood cell count were the impact factors for the elevation of MPO (P<0.01 or<0.05);there were no correlations between MPO level and stable angina, mild and midrange elevation of white blood cell count (P>0.05). MPO was the impact factor for the increase of LXA4 (P<0.01), and stable angina was the impact factor for the decline of LXA4 (P =0.019);there were no correlations between LXA4 level and unstable angina, acute myocardial infarction and other factors (P>0.05). Stable angina, unstable angina and acute myocardial infarction were the impact factors for the elevation of M/L (P<0.01); there were no correlations between M/L and other factors (P>0.05). Conclusions The inflammatory response in patients with CHD is accompanied by the presence of inflammation and resolution imbalance.
3.O-POSSUM and P-POSSUM for evaluation of operative risk of aged patients in orthopedic department

Objective To retrospectively review the mortality and morbidity after orthopedic surgery for aged patients with reference to orthopedic version physiological and operative severity score for the enumeration of mortality and morbidity (O-POSSUM) and Portsmouth POSSUM (P-POSSUM) and validate the score' s accuracy in operative risk prediction.Methods O-POSSUM and P-POSSUM for mortality and morbidity were applied to 839 aged patients who had undergone orthopedic surgery between January 2008 and December 2011.Observed mortality and morbidity were compared with the expected values.Results O-POSSUM predicted 230 morbidities,but the observed value was 182 (x2 =2.718,P > 0.05).O-POSSUM predicted 46 deaths,higher than the observed 3 deaths (x2 =38.870,P<0.01).P-POSSUM predicted 16 deaths,higher than the observed 3 deaths (x2 =8.997,P < 0.01).Conclusions O-POSSUM accurately predicts postoperative morbidity in aged orthopedic patients.However,O-POSSUM and P-POSSUM overpredicts the mortality and should be used with caution.
4.Establishment and application of a tandem affinity purification system of innate immune regulatory protein PKR

Objective To establish a tandem affinity purification(TAP) system of innate immune-regulatory protein PKR and analyze PKR function, for the future screen and identification of novel PKR-interaction proteins. Methods PKR gene was amplified by PCR, and then cloned into a mammalian expression vector pcTAP-A. Recombinant pcTAP-PKR was transfected into PKR knock-down(PKRkd) HeLa cells by LipofectAMINE 2000,and the PKR overexpressed HeLa cells were harvested for mitogen-activated protein kinases(MAPK) activation analysis. Cell extracts of PKR overexpressed cells were purified using TAP kit and examined by Western blot. Results Cal modulin resin(CBP) and streptavidin resin(SBP) tagged PKR was detected in PKRkd HeLa cells as early as 24 h upon transfection with pcTAP-PKR, and its expression decreased at later time points. The overexpression of PKR was autophosphorylated, and thus involved in the regulation of MAPK actviation. After small-scale TAP kit purification, PKR protein was detectable by Western blot. Conclusion We have successfully established a TAP system that over-expresses functional PKR, providing a useful tool for the future study on the identification of PKR interacting proteins.
5.The correlation of uric acid level and the circadian rhythm of blood pressure in hypertensive patients

Objective To investigate the correlation of uric acid(UA) level and the circadian rhythm of blood pressure in hypertensive patients. Methods Among the individuals who presented to the cardiology clinic, 70 patients who had hypertension and were diagnozed with non- dipper hypertension (non-dipper hypertension group) by 24 h ambulatory blood pressure monitoring (ABPM), 70 patients with dipper hypertension patients (dipper hypertension group), and 52 normotensive individuals (control group) were enrolled in this study. Peripheral venous blood samples were collected from all the patients in order to evaluate the hematological and biochemical parameters. All the assessed parameters were compared among three groups. Results The level of UA in non-dipper hypertension group was the highest, in dipper hypertension group was higher and in contrl group was the lowerst:(393.57 ± 53.52), (280.57 ± 41.64), (267.66 ± 59.38) μmol/L, and there were significant differences (P<0.01). Multivariate Logistic regression analysis revealed that the level of UA was an independent risk factor for non-dipper circadian rhythm of blood pressure (P = 0.003, OR = 2.26, 95% CI: 1.34- 3.89). Conclusions The higher level of UA may be a risk factor for non-dipper circadian rhythm of blood pressure in hypertension patients.
6.Clinical Study of 5% D-fructose Injectio for Energy Supply in Surgery Patients

OBJECTIVE: To observe the clinical effect of 5% D-fructose injectio on energy supply in surgery patients. METHODS: By setting 5% glucose injectio as control,the influence of 5% D- fructose injectio on blood sugar level,liver and kidney function indices was detected.RESULTS: 5% D-fructose injectio did not influence liver and kidney functions, serum uric acid and RESULTS: of routine examination of blood and urine.Compared with control group, the change of blood sugar level in experiment group was slighter.CONCLUSION: 5% D-fructose injectio is effective and safe for energy supply in surgery patients.

OBJECTIVE:
To investigate the change of serum myeloperoxidase (MPO) and lipoxin A4 (LXA4) in coronary heart disease (CHD) patients with anxiety and depression and its clinical significance.
METHODS:
From December 2010 to February 2011, 143 CHD patients and 44 non-CHD patients (the control group) hospitalized in the Department of Cardiology at the Second Xiangya Hospital were enrolled. The hospital anxiety and depression scale (HADS) was used to evaluate the psychological state of all patients and the CHD patients were assigned to an anxiety and depression group (n=57) or a non-depression and anxiety group (n=86). The serum levels of high sensitive C-reactive protein (Hs-CRP), MPO, and LXA4 were examined, and the ratio of MPO and LXA4 (M/L) was calculated.
RESULTS:
The levels of Hs-CRP, MPO, and LXA4 as well as M/L ratios in both CHD groups were significantly higher than those in the control group (all P<0.01). Compared with the non-anxiety and depression group, the levels of MPO and LXA4, and M/L ratios in the anxiety and depression group increased (all P<0.05). Correlation analysis showed that MPO was positively correlated with the score of HADS-total (HADS-t), HADS-anxiety (HADS-a), or HADS-depression (HADS-d), while LXA4 was negatively correlated with HADS-t or HADS-d. Multiple ordinal logistic regression analysis revealed that higher HADS-t score, stable angina, unstable angina, and acute myocardial infarction were the independent impact factors for the elevation of M/L ratio.
CONCLUSION
Anxiety and depression may aggravate the inflammatory response in CHD patients. The imbalance between inflammation and anti-inflammation may be part of the mechanism.

8.A prospective randomized study of adjuvant chemotherapy in completely resected stage IIIA-N2 non-small cell lung cancer.

BACKGROUNDRecently, results from IALT, JBR10 and CALGB9633 showed that postoperative adjuvant chemotherapy improved survival rate of patients with non-small cell lung cancer (NSCLC) after complete resection. The aim of this study is to evaluate the effect of postoperative adjuvant chemotherapy on survival after complete resection for stage IIIA-N2 NSCLC.
METHODSFrom Jan 1999 to Dec 2003, one-hundred and fifty patients with stage IIIA-N2 NSCLC were randomly divided into two groups. The chemotherapy group received four cycles of chemotherapy with navelbine or paclitaxel plus carboplatin, while the observation group did not receive chemotherapy after operation.
RESULTSIn the chemotherapy group, 86.1% (68/79) of patients finished 4 cycles of chemotherapy, and no one died of toxic effects of chemotherapy; 25% of patients had grade III-IV leukopenia, 2% of patients had febrile leukopenia. The median survival time for the entire 150 patients was 879 days, and 1-, 2- and 3-year survival rate was 81%, 59% and 43%. There was no significant difference in median survival between the chemotherapy and observation groups (P= 0.0527), but there was significant difference in the 1- and 2-year survival rate (94.71% and 76.28% vs 88.24% and 60.13%, P < 0.05). The most common site of recurrence was the brain. Twenty-six percent (39/150) of patients recurred in the brain as their first site of failure, and 22.8% (18/79) for the chemotherapy group, 29.6% (21/71) for the observation group. The median survival time for patients who developed brain metastasis was not significantly different between the chemotherapy and observation groups (812 days vs 512 days, P=0.122), but there was significant difference in the 2-year survival rate (66.7% vs 37.6%, P < 0.01). The median survival was 190 days for the patients since brain metastasis appeared.
CONCLUSIONSPostoperative adjuvant chemotherapy dose not significantly improve median survival among patients with completely resected stage IIIA-N2 NSCLC, but significantly improves the 1- and 2-year survival rate. It also dose not decrease the incidence of brain metastasis but puts off the time of brain metastasis.

10.Application of pulsed electromagnetic fields in the treatment of orthopedic diseases

BACKGROUND:Pulsed electromagnetic field is a non-invasive and non-radiative treatment method.Clinical use of pulsed electromagnetic fields in the treatment of orthopedic diseases has achieved certain results. OBJECTIVE:To review the current clinical application of the pulsed electromagnetic field in the treatment of orthopedic diseases,providing a scientific theoretical basis for the clinical treatment of orthopedic diseases. METHODS:The first author used a computer to search PubMed,CBM,Cochrane Library,CNKI,and WanFang Data for related studies on the pulsed electromagnetic field in the treatment of orthopedic diseases,using the keywords of"pulsed electromagnetic field,orthopedics,osteoarthritis,osteoporosis,bone healing,electromagnetic navigation"in English and Chinese.For the literature related to the same content,recent publications were selected.A total of 69 articles were selected from the search results for review. RESULTS AND CONCLUSION:Pulsed electromagnetic field has a definite curative effect on fracture healing.It can be used in the treatment of osteomyelitis by antibacterial,bactericidal,anti-inflammatory and promoting bone healing,and can inhibit osteoporosis and its progress.In addition,the treatment of early osteoarthritis,femoral head necrosis and postoperative rehabilitation of late joint replacement through various ways can become a treatment for orthopedic diseases.However,the therapeutic mechanism of the pulsed electromagnetic field for a variety of orthopedic diseases is still unclear,and most of the research is still in the primary stage.In the future,it is still necessary to obtain more reliable evidence from high-quality research and clinical trials to provide a more perfect basis for the clinical treatment of orthopedic diseases.
