1.Perioperative nursing of transurethral bipolar plasmakinetic resection of prostate

Objective To evaluate the perioperative nursing of transurethral bipolar plasmakinetic resection of prostate(TUPKRP).Methods One hundred and thirty BPH patients underwent transurethral prostatectomy with the bipolar plasmakinetic technique.All patients received preoperative mental nursing,holistic nursing care and health education before operation.Results All patients passed through the surgery and were given nursing intervention.They were discharged after rehabilitation.Conclusions TUPKRP is an important miro-invasion operation for treatment of BPH.The important points are preoperative mental nursing,rational use of drug,postoperative bladder irrigation and prevention of hemorrhage.
2.Research progress on the epidemiology of chronic kidney disease

Chronic kidney disease (CKD) is characterized by long course, poor prognosis, multiple complications and high cost of treatment, thus it has become a major public health problem. Based on review of publications pertaining to the epidemiological study of CKD from February 2002 to March 2023, this article summarizes the epidemiological characteristics, risk factors, prevention and control strategies of CKD. It is found that China has the largest number of adult patients with CKD in Asia, and the prevalence of CKD is higher among females and elderly people. The influencing factors for CKD include cardiovascular diseases, diabetes, hypertension and coronavirus disease 2019. Henceforth, it is of vital importance to emphasize three levels of prevention and optimize the CKD management, so as to support for prevention and control of CKD.
3.Effect of Bushen Tiaojing Recipe Extract on Sexual Hormone Levels of Premature Ovarian Failure Patients with Kidney Deficiency Syndrome

Objective To investigate the clinical efficacy of Bushen Tiaojing Recipe Extract(an extract preparation of Chinese herbal medicine with the actions of tonifying kidney and regulating menstruation) for the treatment of kidney deficiency type of premature ovarian failure (POF),and to observe the effect of the extract formula on sexual hormone levels of POF patients.Methods A total of 75 POF patients were randomly divided into Chinese medicine (CM) decoction group,CM extract group and western medicine (WM) group,25 cases in each group.CM decoction group was treated with the decoction of Bushen Tiaojing Recipe (a decoction recipe for tonifying kidney and regulating menstruation),CM extract group was treated with Bushen Tiaojing Recipe Extract,and WM group was treated with hormone replacement therapy(HRT).Twenty-eight days constituted one treatment course,and the treatment for the 3 groups lasted 3 continuous courses.After treatment,the patients were followed up for 3 months.Clinical efficacy of the 3 groups was evaluated,and clinical symptom scores,the improvement of menstruation and the changes ofsexual hormones ofestrogen(E2),follicle-stimulatinghormone(FSH) and luteinizing hormone (LH) were observed before and after treatment.Results (1) Two cases of each group dropped out,and a total of 69 patients completed the study.(2) The total effective rate of CM decoction group and CM extract group was 73.91%,78.26% respectively,higher than 52.17% of WM group (P < 0.05).(3)After treatment,clinical symptoms of CM decoction group and CM extract group were obviously improved (P < 0.05 or P < 0.01 compared with those before treatment),while WM group only had an effect on improving the abnormal menstruation (P < 0.01 compared with that before treatment).CM decoction group and CM extract group had better effect on improving the clinical symptoms than WM group (P < 0.05 or P < 0.01).(4) CM decoction group and CM extract group had higher menstrual improvement rate,shorter menstrual-improvement time and lower recurrence rate than WM group (P <0.05).(5) CM decoction group and CM extract group had better effect on increasing serum E2 level and decreasing FSH and LH levels than WM group (P < 0.05 or P < 0.01).(6) The adverse reaction incidence of CM decoction group and CM extract group was all 4.35%,lower than 13.04% of WM group (P < 0.05).Conclusion Bushen Tiaojing Recipe Extract has high drug concentration,and its effect is stable,mild and lasting,having the advantages of convenience for carrying and medication.The extract exerts certain clinical efficacy for the treatment of POF,and is effective on alleviating clinical symptoms,recovering normal menstruation,regulating serum sexual hormone levels,with lower abnormal menstruation relapse rate and less adverse reaction.
4.Effect of catestatin on inhibiting C.albicans biofilm formation by down-regulating the expression of HWP1

Objective To investigate whether catestatin (bovine chromogranin A 344-364) can inhibit the biofilm formation of Candida albicans and examine its relationship with the expression of adhesion gene HWP1.Methods Clinical strains and standard strain ATCC 10231 of C.albicans were studied.XTT [2,3-bis-(2-methoxy-4-nitro-5-sulfophenyl)-2H-tetrazolium-5-carboxanilide] method was used to assess the ability of C.albicans biofilm formation.Antifungal activity against planktonic Candida ceils was evaluated in terms of minimum inhibitory concentrations (MICs) according to the description in CLSI-M27-A3.XTT assay and colony count were used to assess the effect of catestatin on inhibiting C.albicans biofilm formation.The lowest concentration showing 50 % inhibition on biofilm formation (BIC50) was decided by calculating the metabolic activity.The adhesion of C.albicans reduced by catestatin was visualized under an inverted microscope and quantified by colony count.The expression of HWP1 was analyzed by RT-PCR.One-way analysis of variance (ANOVA) and Dunnett's T3 test were used to compare the results.Results Clinical strains and standard strain ATCC 10231 of C.albicans showed strong ability in forming biofilm.Catestatin exhibited MICs ranging from 40 μmol/L to 80 μmol/L against planktonic C.albicans cells,and BIC50 of 80-160 μmol/L in inhibiting C.albicans biofilm formation.Catestatin reduced the adhesion of C.albicans.The colony forming unit (CFU) was 27 822.22-±-2 472.74 in blank control group,while the CFU was 5 355.55± 1 264.03,11 377.78±2 232.58,17 488.89±1 136.27,22 377.78±3 521.99,and 26 044.44±1 329.57 in the presence of 160,80,40,20,and 10 μmol/L catestatin,respectively (F=147.018,P=0.001).The difference between control group and 160,80,and 40 μmol/L catestatin was statistically significant (P<0.05).RT-PCR found the expression of HWP1 in the presence of 160 μmol/L catestatin was about 12.24% of that in blank control group.Conclusions Catestatin can effectively prevent C.albicans biofilm formation.This effect may be related to the down-regulated expression of adhesion gene HWP1 by catestatin,which results in reduced adhesion of C.albicans.Promising clinical prospect is expected for this finding.
5. Nursing of a case of male patients with periodic psychosis

Objective:
To summarize the nursing methods of one male patient with periodic psychosis.
Methods:
The nursing methods of a male patient with periodic psychosis were summarized and analyzed.
Results:
According to the change rule of the disease, different nursing measures were taken, no adverse nursing events occurred during the hospitalization, which improved the treatment compliance and treatment effect.
Conclusions
According to the condition characteristics of periodic psychosis, nursing by stages was put forward, which provided the basis for definite diagnosis and promoted the patients' condition recovery.
6.The clinical application of modified rhytidectomy incision in superficial parotid tumor surgery.

OBJECTIVE:
To evaluate the modified rhytidectomy incision in superficial parotidectomy.
METHOD:
Thirty-five patients with superficial parotid tumor were included in this study. A modified rhytidectomy incision often used in facial plastic surgery was used for superficial parotidectomy and subtotal superficial parotidectomy with preservation of facial nerve and great auricular nerve. The follow-up study included the exposed region, the cosmetic effect of this approach and the rate of complication.
RESULT:
All patients healed without salivary fistula, and were satisfied with this modified approach. Temporary paralysis of the marginal mandibular branch of facial nerve were found in five patients, and six patients felt insensible around earlobe after operation. They all recovered in 1 to 3 months after surgery, no recurrence was happened during follow-up in 36 to 60 months (median follow-up period was 48 months).
CONCLUSION
The modified rhytidectomy incision provided good exposure, had less complication and better cosmetic outcome.

7.Clinical significance of determination of serum neutrophil gelatinase associated lipocalin in the diagnosis of contrast induced nephropathy

Objective To discuss the clinical significance of serum neutrophil gelatinase associated lipocalin(NGAL)detection in the diagnosis of contrast induced nephropathy .Methods A total of 299 inpatients with contrast medium in department of inten‐sive care unit(ICU) ,department of cardiovascular disease ,department of urology from Guangdong Province Hospital of Traditional Chinese Medicine were enrolled ,and the fasting blood glucose and lipids were detected .Sera were collected before examination of contrast medium and 1 ,2 and 6 d after examination of contrast medium respectively .Serum creatinine(SCr)and cystatin C(CysC) were detected ,the concentration of NGAL was determined .CIN was defined as of increased by 44 μmol/L or 25% from baseline in SCr within 24-48 h .The NGAL detection variation tendency was analyzed in different time point .Results In 299 cases ,CIN oc‐curred in 28 patients ,the incidence rate was 9 .36% .The diabetes patients in the incidence of CIN was 16 .21% (12/74) ,no inci‐dence of diabetic patients was 7 .11% (16/225) .When compared with that of before contrast ,CIN in serum of patients with Cr and CysC coronary angiography in 2 d significantly increased after contrast medium ,returned to the level before angiography on 6 d .But the serum NGAL in angiography 1 d after it was significantly increased (P<0 .05) ,statistically significant differences compared with pre contrast ,and was still at a high level on sixth day .Conclusion Prediction of CIN level of serum NGAL in contrast after 1 d could be a good predictor for CIN ,and the prediction time is earlier than the serum levels of Cr and CysC .The level of serum NGAL can be used for early diagnosis of CIN .
8.Clinical outcomes of modified 360° fusion for lumbar spondylolisthesis

Objective To analyze the clinical results of 360° fusion for spondylolisthesis.Methods Data of 76 patients from March 2005 to November 2012,including 26 males and 50 females with a mean age of 54.3 years were retrospectively analyzed.All patients had undergone modified 360° fusion.The clinical outcomes were evaluated by the Japanese Orthopaedic Association scores (JOA) and visual analogue scale (VAS).The fusion status and loss of correction were assessed by CT and plain radiographs.Results All patients had been successfully followed up for at least one year.Post-operative reduction rate was 96.8%,with a mean loss rate of 1.1% at six months and 0.8% at one year follow-up separately,but the difference was not significant.Postoperative percentage of slip,lumbar lordosis,intervertebral disc height and slip angle had significant differences,compared with those of the pre-operative,but the difference between one week post-operative,six months follow-up and one year follow-up were not significant.The fusion rate was 76.3% at six months follow-up and 98.7% at one year follow-up,and there were significant differences.Both JOA and VAS score at six months and one year follow-up indicated significant differences in contrast to that of preoperative.Dural injury was found in one patient,and subsidence was found in one patient.One received a second operation at 3 months after the surgery due to screw loose.Conclusions Modified 360° fusion shows great clinical outcome and fusion rate,which can be a dominant procedure for treating spondylolisthesis.
9.Adeno-associated vector mediated intracellular biological activity of human Kallistatin.

Human tissue kallikrein-binding protein (Kallistatin, KAL), a secretory protein that participates in the regulation of multiple signaling pathways by binding to the extracellular receptor, however, at present has not been reported about the intracellular activity, and whether it has the similar biological activity with extracellular activity. Here we constructed no signal peptide KAL (NSK) into the adeno-associated virus vector to explore the intracellular activity of KAL. Both the endothelial cell and lung cancer cells could express KAL, but not secreted after rAAV2-NSK transfection. The proliferation and migration of human umbilical vein endothelial cells (HUVECs) were inhibited, but the apoptosis rate was not affected. The proliferation rates, mobility and tubule formation of all the three tested lung cancer cells, such as NCI-H446, NCI-H460 and A549, were inhibited to different extents. This cellular study not only confirmed the intracellular activity, but also suggested it may serve as a kind of "balance factor" in multi-targeted controlling, which may provide a new train of thoughts to explain the regulatory contradiction in PI3K-Akt signaling pathways by KAL.
10.Effects of Spleen-warming and Kidney-tonifying Therapy on TLR4 Signaling Pathway of Cerebral Infarction Sequelae Patients

Objective To observe the effect of spleen-warming and kidney-tonifying therapy on Toll-like receptor 4 (TLR4) signaling pathway of cerebral infarction sequelae patients. Methods A total of 60 cerebral infarction sequelae patients with spleen and kidney(yang) deficiency were divided into control group and treatment group, 30 cases in each group. The two groups were given function rehabilitation training following by the theory of modern rehabilitation medicine, and additionally the treatment group was treated with spleen-warming and kidney-tonifying herbs orally. The treatment period for the two groups averaged 3 months. Before and after treatment, the scores of traditional Chinese medicine (TCM) symptoms and signs, peripheral blood TLR4 mRNA expression level, and the inflammation-related factors in the two groups were observed, and the observation parameters were compared with 30 healthy volunteers. Meanwhile, the changes of modern rehabilitation evaluation indexes were also evaluated before and after treatment. Results (1) After treatment, the Fugl-Meyer scores for motor function and Modified Barthel Index(MBI) scores for activities of daily living in the treatment group were obviously increased(P < 0.05 or P < 0.01 compared with those before treatment); the MBI scores in the control group was much increased(P < 0.05 compared with those before treatment), but the Fugl -Meyer scores only showed an increasing trend(P > 0.05). (2) After treatment, the predominant symptoms and signs of hemiplegia, facial distortion and dysphasia in the two groups were much improved(P < 0.01 compared with those before treatment), and the improvement in the treatment group was superior to that in the control group(P < 0.05). The control group had no significant effect on relieving the symptoms and signs of dizziness and blurred vision, numbness of limbs, spontaneous sweating, shortness of breath and weakness, and darkish tongue(P > 0.05 compared with those before treatment); the treatment group had obvious effect on the above symptoms and signs, and the effect was superior to that in the control group(P < 0.05 or P < 0.01).(3) For the inflammation-related factors, tumor necrosis factor alpha(TNF-α) level in the treatment group was much decreased and even arrived to the normal level(P > 0.05), and TNF-α level in the control group was decreased but did not arrive to the normal level(P < 0.05); levels of interleukin(IL)-1β, IL-6 and TLR4 mRNA in the two groups were decreased but did not arrive to the normal(P < 0.01), and the decrease in the treatment group was superior to that in the control group(P < 0.05 or P < 0.01). (4) During the treatment period, no obvious adverse effect was found in the two groups. Conclusion Oral use of spleen -warming and kidney –tonifying Chinese medicine combined with modern rehabilitation therapy is effective for the treatment of the sequelae of cerebral infarction patients by relieving the clinical TCM symptoms, and one of the therapeutic mechanisms is probably related with the TLR4 signaling pathway through regulating the expression of inflammatory factors.
