1.Hospital Acquired Bacterial Infection in Intensive Care Unit (ICU):A Clinical Investigation

OBJECTIVE To study the bacterial infections in ICU,pathogens distribution and their drug-resistance characteristics,so as to offer a reference to the prevention and control of hospital infection.METHODS Specimens from ICU from Jan 2003 to Dec 2004 were underwent a process of germ culture,identification and drug-sensitivity test.Germ culture-positive cases were confirmed by Diagnostic Standards of Hospital Infections.RESULTS Ninety three pathogens were identified from ICU in two years.Of all cases,63.44% were Gram-negative,30.1% were Gram-positive,and 9.68% were epiphyte.Among them,19.35% were Pseudomonas aeruginosa and it was the main one of Gram-negatives;9.68% were Staphylococcus aureus and it composed the most of Gram-positives;6.45% were epiphyte.Of all 93 pathogens,60 gained from respiratory tract,and 23 gained from urinary tract.CONCLUSIONS To emphasize on the strictly control of ICU,hospital infections and high risk disease,and to adopt certain preventive measures are the ways to control and decrease hospital infections.
2.Adeno-associated vector mediated intracellular biological activity of human Kallistatin.

Human tissue kallikrein-binding protein (Kallistatin, KAL), a secretory protein that participates in the regulation of multiple signaling pathways by binding to the extracellular receptor, however, at present has not been reported about the intracellular activity, and whether it has the similar biological activity with extracellular activity. Here we constructed no signal peptide KAL (NSK) into the adeno-associated virus vector to explore the intracellular activity of KAL. Both the endothelial cell and lung cancer cells could express KAL, but not secreted after rAAV2-NSK transfection. The proliferation and migration of human umbilical vein endothelial cells (HUVECs) were inhibited, but the apoptosis rate was not affected. The proliferation rates, mobility and tubule formation of all the three tested lung cancer cells, such as NCI-H446, NCI-H460 and A549, were inhibited to different extents. This cellular study not only confirmed the intracellular activity, but also suggested it may serve as a kind of "balance factor" in multi-targeted controlling, which may provide a new train of thoughts to explain the regulatory contradiction in PI3K-Akt signaling pathways by KAL.
3.Activation of liver X receptors induced pancreatic β cell cycle arrest by up-regulating the expression of p27 protein

Objective To investigate the effects of liver X receptor (LXR) agonist on the proliferation of mouse pancreatic β cell line MIN6 cells.Methods The viability,changes of cell cycle,mRNA levels of S phase kinase associated protein 2 (Skp2) and p27,and protein levels of Skp2 and p27 in MIN6 cells treated with LXR agonist T0901317 were determined by the CCK-8 method,flow cytometry,real-time RT-PCR and western blot,respectively.Results The viability of MIN6 cells treated with 1 μmol/L,5 μmol/L and 10 μnol/L of T0901317 were (98.54 ±0.94)%,(87.03 ±0.93)% and (75.57 ± 1.85)% of the controls,respectively,and there was significant difference among them (F =301.90,P < 0.01).The percentages of G1 phase cells in the MIN6 cells treated with 0 μmol/L,1 μmol/L,5 μmol/L and 10 μmol/L of T0901317 were (35.93 ±2.25)%,(38.45 ±0.91)%,(45.46±1.34)% and (53.28 ± 1.14) %,respectively,and there was significant difference among them (F =80.83,P < 0.01).Similarly,the percentages of S phase cells in the MIN6 cells treated with 0 μmol/L,1 μmol/L,5 μmol/L and 10 μmoi/L of T0901317 were (52.87 ± 1.19) %,(48.65 ± 0.85) %,(36.31 ± 1.37) % and (31.45 ± 1.22) %,respectively,and there was also significant difference among them (F =221.30,P < 0.01).The protein levels of p27 in the MIN6 cells treated with 10 μmol/L of T0901317 (2.84 ± 0.14) were significantly higher than that in the controls (2.28 ± 0.10) (t =4.54,P < 0.05),while there was no significant difference in the mRNA levels of p27 between them (t =0.28,P > 0.05).However,10 μmol/L of T0901317 significantly decreased mRNA (0.52 ± 0.02,t =29.22,P < 0.01) and protein levels (0.98 ± 0.12 vs 1.89 ± 0.01,t =10.98,P < 0.01) of Skp2 in MIN6 cells.Based on the control siRNA transfection group as a reference (100%),the cell survival rates of the p27 siRNA transfection group,10 μmol/L of T0901317 treatment group and the intervention group (p27 siRNA transfection + T0901317 treatment) were (100.97 ± 1.08) %,(75.03 ± 1.83) % and (86.67 ± 2.45) %,respectively.There was no significant difference between the control siRNA and p27 siR-NA transfection groups (t =1.542,P > 0.05).Compared with the control siRNA transfection group,the cell survival rates of the T0901317 treatment group decreased (t =23.58,P < 0.01).There was also significant difference in the cell survival rates between the T0901317 treatment group and the intervention group (t =7.77,P < 0.01).Conclusion The activation of LXR may induce pancreatic β cell cycle arrest by up-regulating the expression of p27 and down-regulating the expression of Skp2.
4.Effect of acupuncture on macrophage polarization of white adipose tissue in obese mice induced by high-fat diet.

OBJECTIVETo observe the effect of acupuncture at acupoints and non-acupoints on macrophage polarization of white adipose tissue in obese mice induced by high-fat diet(HFD).
METHODSFifty male C57BL/6 mice were randomly divided into a control group (=10), and other mice were fed with HFD for 16 weeks to establish model. Thirty model mice, which were successfully established and continuously fed with HFD for 8 weeks, were randomized into a model group, a non-acupoint group and an acupuncture group (=10/group). The mice in the control group were continuously fed with common food for 8 weeks. From the second day after model established, the mice of the acupuncture group were treated with acupuncture at "Guanyuan" (CV 4), "Zusanli" (ST 36) and "Weiwanxiashu" (EX-B 3); the mice of the non-acupoint group were treated with acupuncture at two non-acupoints in tail, 15 min a time, once a day for 8 weeks. After model establishment and on the second day after the last acupuncture treatment, the body weight was recorded. The level of serum triglyceride (TG), total cholesterol (TC) in eye socket blood were measured. The morphological structure of epididymis white adipose tissue (eWAT) in mice was observed by hematoxylin and eosin (HE) staining. The mRNA expressions of interleukin-6 (IL-6), monocyte chemotactic protein 1 (MCP-1), tumor necrosis factor α (TNF-α), IL-10, inducible nitric oxide synthesis (iNOS), CD206 in eWAT of mice were determined with real-time quantitative PCR (RT-qPCR). The protein expressions of iNOS and CD206 in eWAT were determined with immunohistochemistry staining (IHC).
RESULTSCompared with the control group, the body weight in the model, non-acupoint and acupuncture groups increased at the 16th and 24th weeks (all<0.05). Compared with the model group, the body weight of the acupuncture group at the 24th week decreased (<0.05). The serum TG and TC in the model group increased compared with those of the control group (both<0.05). The TC in the non-acupoint group decreased (<0.05), and the TG and TC in the acupuncture group decreased compared with those in the model group (both<0.05). Meanwhile, the mRNA expressions of MCP-1, TNF-α, IL-6 and iNOS in the model and non-acupoint groups increased compared with those in the control group (all<0.05), and the mRNA expressions of IL-10 and CD206 decreased (all<0.05). The mRNA expressions of MCP-1, TNF-α, IL-6 and iNOS in the acupuncture group were lower than those in the model group (all<0.05), and the mRNA expressions of IL-10 and CD206 were higher (both<0.05). HE staining showed alveolate adipose tissue in the control group with numbers of unilocular adipose cells, vacuolar polygon or circle. The adipose cells in the model group were larger and irregular with larger intercellular space. The adipose cells in the acupuncture group were minified with smaller intercellular space. Compared with those in the control group, the protein expressions of iNOS increased (both<0.05) and those of CD206 decreased (both<0.05) in the model and non-acupoint groups. Compared with those in the model group, the protein expression of iNOS decreased (<0.05) and that of CD206 increased (<0.05) in the acupuncture group.
CONCLUSIONAcupuncture can improve inflammation of obese mice through affecting macrophage polarization of white adipose tissue.
5.Occurrence of hyperactivity behavior and its relationship with neuropsychological development in children aged 3-6 in Yunnan Province

Objective:
To investigate the prevalence of hyperactivity behavior in children aged 3-6 in Yunnan Province, to explore its relationship with neuropsychological development, so as to provide clues for early prevention and intervention of attention deficit hyperactivity disorder (ADHD) in children.
Methods:
A total of 1 321 children aged 3 to 6 from 10 kindergartens in 5 prefectures (cities) of Yunnan Province were selected by stratified random sampling method from October 2022 to May 2023. Teacher Rating Scale (TRS) was used to investigate childrens hyperactive behavior and coexistent behavior. A qualified evaluator applied the Developmental Scale for Children Aged 0-6 Years to assess the development of 5 ability areas of gross motor movement,fine movement,adaptive ability,language and social behavior. Statistical analysis was performed using Wilcoxon rank sum test and χ2 test. Binary Logistic regression was applied to analyze the score of their hyperactivity behavior and its relationship with other behavior problems and neuropsychological development.
Results:
The detection rate of hyperactivity behavior was 8.6% in children aged 3 to 6 years, 12.8% in boys and 4.1% in girls (χ2=31.53, P<0.01). The detection rate of hyperactivity in 3yearold children was 13.9%, which was higher than that in 4yearold (9.2%) and 5yearold children (7.0%) (χ2=8.73, P<0.05). The detection rate of inattentionpassivity of rural children (14.6%) was higher than that of urban children (5.9%) (χ2=22.23, P<0.01). Binary Logistic regression analysis showed that the higher the level of adaptive development, the lower the risk of hyperactivity (OR=0.58, 95%CI=0.39-0.86), the higher the risk of hyperactivity (OR=0.57, 95%CI=0.35-0.91), the higher the risk of conduct problems (OR=0.57, 95%CI=0.37-0.87), inattentionpassivity (OR=0.49, 95%CI=0.33-0.74) were also at lower risk (P<0.05). Children with higher levels of fine motor development had a lower risk of inattentionpassivity (OR=0.59, 95%CI=0.37-0.93, P<0.05).
Conclusions
Hyperactivity in boys and inattentionpassivity in rural children requires more attention. It is necessary to strengthen childrens early adaptive ability and fine motor training to prevent hyperactive behavior and inattention.
6.Current status and prospect of imaging examination methods for rejection after lung transplantation

Lung transplantation is the only effective therapeutic option for end-stage lung diseases, and postoperative rejection is the main factor affecting clinical prognosis of the recipients. Imaging examination can be utilized as a noninvasive tool to assist other examinations in monitoring rejection after lung transplantation. At present, multiple imaging examination methods have been reported. The advantages and disadvantages of various imaging examinations have been clarified, which may promote early diagnosis of rejection, deliver timely treatment for lung transplant recipients and improve the quality of life and clinical prognosis. In this article, the advantages, disadvantages and research progress upon different imaging examinations for rejection after lung transplantation were reviewed, aiming to provide reference for identifying the optimal noninvasive examination approach for rejection after lung transplantation and enhance the long-term survival of the recipients.
7.Learning curve and key procedures analysis of modified rat models of orthotopic left lung transplantation

Objective To summarize and analyze the key procedures of the modified rat model of orthotopic left lung transplantation, aiming to provide more experience for the establishment of rat models of lung transplantation. Methods Two surgeons (A and B) performed complete transplantation in consecutive 15 rat models, and every 5 surgeries were divided into 1 practice stage. The operating time of each transplantation procedure was recorded. The differences of overall success rate and 1-week survival rate were calculated among different practice stages. The learning curve was delineated by the cumulative sum method. Results For surgeons A and B, the number of the first successful transplantation was the 5th and 6th time, the overall success rates of transplantation were 80% and 87% respectively, and the 1-week survival rates of rats both were 92%. Lung transplantation has become the most effective treatment of end-stage lung diseases. Along with persistent optimization of lung transplantation technique and perioperative management, the short-term clinical efficacy after lung transplantation has been significantly improved, whereas the long-term clinical prognosis remains unoptimistic. Besides chronic lung allograft dysfunction, postoperative malignant tumors also threaten the long-term survival of the recipients. Common malignant tumors following lung transplantation include nonmelanoma skin cancer, posttransplant lymphoproliferative disease and lung cancer. After solid organ transplantation, a large majority of the recipients require lifelong immunosuppressive therapy. The intensity of immunosuppressive therapy for the lung transplant recipients is generally higher than other organ transplant recipients. Immunosuppression is the main factor which leads to the impairment of anti-tumor immune monitoring function and promotes the incidence and development of malignant tumors. In this article, the risk factors, prevention and treatment of the most common malignant tumors after lung transplantation were reviewed, aiming to provide reference for comprehensive diagnosis and treatment of malignant tumors following lung transplantation.
9.Willingness to preventive treatments and related factors among college freshmen with latent tuberculosis infection in Changzhou

Objective:
To investigate the willingness to accept preventive treatments and its related factors among college freshmen with latent tuberculosis infection (LTBI), so as to provide the evidence for preventive treatment intervention measures for students with LTBI.
Methods:
Cluster sampling method was used to select 368 LTBI freshmen from 8 colleges and universities in Changzhou in September 2023, who conducted a questionnaire survey on the willingness to receive preventive treatment. General demographic data were collected and relevant data were collected using tuberculosis knowledge scale, General Anxiety Disorder-7 (GAD-7), Patient Health Questionnaire-9 (PHQ-9), Adaptation, Partnership, Growth, Affection and Resolve (APGAR), and a self developed Stigma Scale. A binary Logistic regression model was constructed with the willingness to accept preventive treatment as the dependent variable to analyze the willingness to accept preventive treatment and the influencing factors.
Results:
A total of 253 LTBI college freshmen were willing to take preventive treatment, the acceptance rate was 68.75%. The rate of willingness to accept preventive treatment for LTBI was higher among students whose fathers had an education level of high school, compared to those whose fathers had an education level of junior high school or below ( OR =2.16, P <0.05). LTBI students whose per capita family income was >5 000-10 000 yuan and >10 000 yuan were more willing to accept LTBI preventive treatment than those whose per capita family income was <3 000 yuan ( OR =2.72, 4.46, P <0.05). LTBI students who engaged in physical exercise for more than 2 hours per week were more willing to accept than those who exercised less than 0.5 hours per week ( OR =1.91, P <0.05). LTBI students with high levels of tuberculosis knowledge and stigma were more likely to receive preventive treatment ( OR =1.18, 1.11, P < 0.05). LTBI students with high PHQ-9 ( OR =0.85) and GAD-7 ( OR =0.92) scores were more likely to refuse preventive treatment ( P <0.05).
Conclusion
The present study revealed a moderate level of willingness of LTBI students to preventive treatment in Changzhou City, and the acceptance is affected by family factors, healthy lifestyles, tuberculosis knowledge and psychological status.
10.Analysis of current status of internet-based patient education materials on labor analgesia in China

Objective:To analyze the current status of internet-based patient education materials related to labor analgesia in China.Methods:Labor analgesia-related materials were retrieved and screened according to the search habits of Chinese search engine users. The coverage and accuracy of the content were evaluated by 3 anesthesiologists. The Chinese version of the Patient Education Materials Assessment Tool for Print Materials was used to subjectively assess comprehensibility and operability from a medical text perspective. The consistency of the evaluation results of the three anesthesiologists was analyzed using the intraclass correlation coefficient method. A machine learning method combined with ChatGPT-4.0 was employed to establish a Chinese readability classification model to objectively evaluate the readability difficulty of the included materials from a Chinese text perspective.Results:A total of 97 web pages were retrieved, with 21 valid materials included in the study. The coverage rate of contraindications for labor analgesia was only 62% (13/21), and the accuracy rate of materials exceeding 90% was 71% (15/21). Internet-based materials that were easy to understand accounted for 81% (17/21), while the constituent ratio of internet-based materials with instructional significance were only 5% (1/21). The intraclass correlation coefficient values of consistency evaluation for coverage rate, accuracy rate, and comprehensibility and operability were 0.975, 0.833, 0.758, and 0.773, respectively ( P<0.001). Internet-based materials suitable for compulsory education level were only 5% (1/21), while those suitable for high school and above education level accounted for 43% (9/21). Conclusions:There are numerous internet-based patient education materials related to labor analgesia in China, but the quality needs improvement. In the future, a collaborative model of " anesthesiology+ linguistics" should be developed to provide patients with more comprehensive, accurate, and pregnant-friendly patient education materials.
