1.Crime Offender With Mental Retardation: A Case Report

Objective: This case report aims to discuss the diagnosis of mental retardation as insanity defence in a crime offender. Methods: We report a gentleman who committed murder and rape 9 years ago, and currently being treated at a mental institution. Results: Patient was certified to have mental retardation, and was pleaded on the defence of unsoundness of mind because he had defect of reason
at the time of alleged offence. Conclusion: Mental retardation does fulfil the McNaughton’s rule. Unsoundness of mind becomes the insanity defence even for murder under section 84 of the Penal Code.

2.Impact Of Methadone Maintenance Therapy On Hiv Incidence In Perak, 2008 - 2017: A Cox Regression Analysis

In 2005, Ministry of Health introduced the Needle Syringe Exchange Program (NSEP) and Methadone Maintenance Therapy (MMT) program as a part of Harm Reduction program to combat HIV infection in Malaysia among people who inject drug (PWIDs). Expenditures were estimated approximately RM10 millions per year to establish and sustain the NSEP and MMT centres. This study examined the impact of MMT program on preventing HIV seroconversion among registered MMT clients that are people who inject drug (PWIDs), and to identify other predictors of HIV seroconversion among this group. This was a retrospective cohort study done in the state of Perak involving a total of 212 randomly selected MMT clients registered between 2008-2017 in 6 clinics. This study looks at data collected from the last ten-year cohort from baseline to follow-up. A questionnaire was used to obtain socio-demographic data, sexual and drugs abuse history. Test results for HIV were obtained from medical records. Cox regression analysis was performed to examine factors associated with seroconversion and Kaplan-Meier analysis to estimate HIV survival time. This study displayed that both Methadone take home supply (HR 10.4, 95% CI: 1.6 – 68.8) and unprotected sexual practice (HR 5.9, 95%CI: 1.1 – 31.5) shown higher risks of HIV seroconversion compared to DOTS and condom practice among MMT clients respectively. Mean survival for HIV seroconversion among MMT clients was 104.44 (95%CI: 101.85 – 107.04) months. This study provides reliable evidence that MMT program markedly reduces incidence of HIV infection among people who inject drug (PWIDs).

Objective: To determine the anti-proliferative activity of Abrus precatorius (A. precatorius) leaf extracts and their effect on cell death. Methods: A. precatorius leaves were extracted successively with hexane, ethyl acetate and methanol by Soxhlet extraction. Aqueous extract was prepared by decoction at 50 ℃. Extracts of A. precatorius leaves were used to treat selected cancer and normal cell lines for 72 h. Furthermore, 3-(4,5-dimethyl thiazol-2-yl) 2,5-diphenyl tetrazolium bromide assay was performed to determine cell viability. Analysis of cell cycle arrest, apoptosis assay and apoptosis protein expressions were determined by flow cytometry. Results: Methanolic extract of A. precatorius leaves showed the lowest IC50 on MDA-MB-231 cells at (26.40±5.40) μg/mL. Flow cytometry analysis revealed that cell arrest occurred at G0/G1 phase and the apoptosis assay showed the occurrence of early apoptosis at 48 h in MDA-MB-231 cells treated with methanolic extract of A. precatorius leaves. Methanolic extract of A. precatorius leaves induced apoptosis by upregulation of Bax, p53 and caspase-3 and downregulation of Bcl-2. Conclusions: Methanolic extract of A. precatorius leaves promotes MDA-MB-231 cell death by inducing cell cycle arrest and apoptosis possibly via the mitochondrial-related pathway.

Pesticide exposure can lead to low trace elements levels in human body. Trace element plays important role in body metabolism. The aim of this study was to study the levels of selenium, zinc and chromium among paddy farmers who expose to pesticide in Wilayah I, MADA, Perlis. This cross sectional study involved 70 males paddy farmers and 57subjects living in fisherman village as control group who were aged between 21 to 80 years old. Subjects were interviewed to obtain information on their demographic data by using validated questionnaire. Subjects also were examined for their blood pressure and glucose level. Selenium, zinc and chromium levels were analyzed by using acid digestion method and Inductively Coupled Plasma Mass Spectrometry (ICP-MS). Results showed that selenium levels in hairs (5.11 ± 17.05 μg/L) and nails (4.92 ± 2.17 μg/L) were significantly (p < 0.05) lower compared to selenium levels in hairs (15.67 ± 10.59 μg/L) and nails (6.67 ± 2.81 μg/L) in control group. Chromium levels in hairs (31.83 ± 15.17 μg/L) and nails (87.64 ± 23.30 μg/L) were also significantly lower (p < 0.05) compared to chromium levels in hairs (85.19 ± 56.90 μg/L) and nails (99.36 ± 56.89 μg/L) of control group. However there were no significant different (p>0.05) between all trace element levels and duration of pesticide exposures. In conclusion, levels of trace elements were lower in nails and hairs of paddy farmers than fisherman community group

Objective: To identify the phytochemical compounds from Annona muricata (A. muricata) and to determine their in vitro anti-proliferative activities against breast cancer cells, MCF7 and MDA-MB-231. Methods: A. muricata leaves were successively extracted by soxhlet method using n-hexane, ethyl acetate and methanol, and decocted with water. Each extract was analysed by gas chromatography mass spectrometry (GCMS) and characterized with Wiley and NIST library searches. Anti-proliferative activity of each extract was evaluated on MCF7 and MDA-MB-231 breast cancer cells using MTT assay. Results: The GCMS analysis of different solvent extracts of A. muricata leaves showed presence of different chemical groups of compounds such as steroids, terpenoids, phenolic compounds, sugars, sugars alcohol and others including vitamin E. presented with history of cough and haemoptysis for two months. The chest X-ray and bronchoscopy revealed left lung mass. While tuberculosis showed negative result and no growth observed in the tissue culture, the
histopathological examination (HPE) finding was suggestive of fungal infection. The formalin-fixed
paraffin-embedded (FFPE) tissue was sent for molecular testing, which revealed Cryptococcus neoformans.
This report emphasises on the advantages and limitations of polymerase chain reaction (PCR) as an alternative
method to confirm the diagnosis in cases of culture-negative fungal infection.
8.Spatial and Temporal Distributions Pattern of Dengue Fever Cases: A Ten Years Trends in Kuantan, Pahang

Introduction: Dengue fever (DF) is a prominent vector-borne disease spread by mosquitos of the Aedes genus (mainly Aedes aegypti, and even Aedes albopictus), a tropical regions vector. The purpose of this research was to establish the spatial and temporal distribution patterns of DF cases in the study area between 2010 and 2020. Methods: The correlation between the Kuantan sub-district and dengue haemorrhagic fever (DHF) incidence is examined in this study using spatial analysis. The correlation was calculated using spatial autocorrelation, Moran’s Index (Moran’s I) and Spatial Autocorrelation of Local Indicators (LISA). Moran’s index is a worldwide indicator used to determine whether or not disease transmission has geographical autocorrelation in disease transmission. Results: The results indicated that between 2011 and 2020, the monthly Moran’s I of dengue transmission in Kuantan was estimated to range between -0.685 and 0.338. The lowest reading of Moran’s index was -0.685 in May 2015, whereas the highest reading was 0.338 in May 2019. This reflects the strong spatial autocorrelation of dengue transmission in Kuantan over the last decade. The LISA analysis revealed significant spatial autocorrelations on DF cases in Kuantan for three (3) out of six (6) sub-districts (50%) with a significance level of 2%. This suggests that there are spatial autocorrelations in Kuala Kuantan, Beserah and Penor sub-district that influence the distribution of DHF transmission. Conclusion: The results reveal that the spatial autocorrelation analysis method can be a tool for relevant researchers to understand the pattern of DF transmission study and establish the direction for further study.
