1.Curative efficacy of sodium phosphocreatine combined with aminophylline in the treatment of postpartum hypertensive heart failure

Objective To investigate the curative efficacy of sodium phosphocreatine combined with amin-ophylline in the treatment of postpartum hypertensive heart failure.Methods 80 patients with postpartum hyperten-sive heart failure were randomly divided into the control group and the observation group equally by random number table method,40 cases in each group.The control group was given conventional treatment,such as oxygen inhalation, diuresis by furosemide,strengthening heart by cedilanid,vascular dilation by nitroglycerin,keeping water -electrolyte balance and so on.Besides that,the observation group was given sodium phosphocreatine in combination with amin-ophylline.Then,the curative efficacy,changes of blood pressure,indices of cardiac function and adverse reactions were compared.Results The total effective rate of the observation group was 95.0%,which was higher than 80.0% of the control group(χ2 =4.1 1 ,P <0.05).After treatment,SBP,DBP,LVEDD,LVESD,LVEF in the control group and the observation group were (1 25.7 ±6.5)mmHg vs (1 20.1 ±7.2)mmHg(t =3.651 ),(88.4 ±8.9)mmHg vs (81 .4 ± 9.0)mmHg(t =3.498),(64.0 ±6.8)mm vs (61 .3 ±6.2)mm(t =2.268),(53.7 ±6.3)mm vs (51 .0 ±5.6)mm (t =2.461 ),(38.6 ±9.0)% vs (42.5 ±8.7)%(t =2.476),respectively.In comparison with the control group, SBP[(1 25.7 ±6.5)mmHg vs.(1 20.1 ±7.2)mmHg],DBP[(88.4 ±8.9)mmHg vs.(81 .4 ±9.0)mmHg], LVEDD[(64.0 ±6.8)mm vs.(60.7 ±6.2)mm],LVESD[(53.2 ±5.3)mm vs.(50.2 ±5.6)mm]were signifi-cantly reduced and LVEF[(38.6 ±9.0)% vs.(42.5 ±8.7)%]was significantly increased in the observation group (t =3.651 ,3.498,2.268,2.461 ,2.476,all P <0.05).During the treatment,there was no severe adverse reaction. Conclusion Sodium phosphocreatine combined with aminophylline is effective for postpartum hypertensive heart fail-ure,which can significantly increase curative efficacy,decrease blood pressure and improve cardiac function with less adverse reactions.
2.Expression and its significance of aquaporin 8 in patients with functional constipation or constipated irritable bowel syndrome

Objective To detect the expression level of aquaporin 8 (AQP8) in patients with functional constipation(FC) or constipated irritable bowel syndrome (IBS-C),and the correlation between the expression of AQP8 and clinical features.Methods From March to December 2014,a total of 16 patients with IBS-C and 19 patients with FC met Rome Ⅲ criteria were collected,and nine healthy individuals were assigned to control group at the same period.The ascending and decending colonic tissues mucosa of FC,IBS-C and control group were taken under endoscope.The expression of AQP8 at mRNA and protein level was detected by real-time polymerase chain reaction (RT-PCR) and immunohistochemistry (IHC).The differences in AQP8 mRNA expression and AQP8 relative area were analyzed by Kruskal-Wallis test among groups,and Pearson correlation coefficient was performed for correlation analysis between the expression and clinical features.Results The relative expressions of AQP8 mRNA of ascending colon and descending colon of FC patients (1.38,0.61 to 4.09;2.65,0.82 to 7.52) and IBS-C patients (2.23,0.82 to 4.67;1.35,0.51 to 2.03) were higher than those of control group (0.56,0.19 to 0.97;0.38,0.21 to 1.19),and the differences were statistically significant (ZFc =-2.435,-3.149,ZIBS-C =-2.690,-2.152;all P<0.05).AQP8 mRNA expression of descending colon in patients with FC was higher than that of patients with IBS-C,and the difference was statistically significant (Z =-2.003,P =0.045).The expression of AQP8 in patients with FC and IBS-C was positively correlated with disease course (ascending colon r=0.57 and 0.53;descending colon r=0.49 and 0.54,all P<0.05),and was negatively correlated with frequency of defecation (ascending colon r=-0.82 and-0.61;descending colon r=-0.49 and-0.53,all P<0.05).There was no correlation between the expression of AQP8 and age,gender,onset age,presence of abdominal symptoms of the patients (all P> 0.05).Most of AQP8 of FC group was expressed in cytoplasm of colonic mucosa epithelial cells,while that of IBS-C group and control group was mostly expressed at apical membrane and basal membrane of epithelial cells.The results of semi-quantification demonstrated that AQP8 relative area of descending colon of FC and IBS-C group increased compared with that of control group (3.42% (1.24% to 5.61%),2.45%(1.72% to 4.27%) vs 1.18% (0.35% to 2.81%);Z=-2.534,-2.151,both P<0.05).Meanwhile,AQP8 relative area of ascending colon of FC group increased compared with that of control group (2.46%(1.48% to 4.18%) vs 1.14%(1.29% to 2.15%) Z=-2.041,P<0.05).Conclusion There are differences in AQP8 expression quantity and location in cells of descending colon between patients with FC and patients with IBS-C,which is a way for differentiation these two diseases.
3.Dysconnectivity of multiple resting-state networks in patients with schizophrenia with auditory verbal hallucinations

Objective:To explore the traits of functional connectivity of resting-state networks in patients of schizophrenia with auditory verbal hallucinations (AVH) by using independent component analysis (ICA).Methods:All patients were met the Diagnostic and Statistical Mannal of Mental Disorders,Fourth Edition,Text Revision (DSM-IV-TR) diagnostic criteria for schizophrenia.Thirty schizophrenia patients with frequent AVH (AVH),24 schizophrenia patients without AVH (non-AVH) and 60 healthy controls (HC) matching with age and gender were analyzed by resting functional magnetic resonance imaging.The AVH were assessed by using Hoffman auditory hallucination scale.By using ICA,auditory network (AUN),language network (LAN),salience network (SAN),right front-parietal network (RFP) and default mode network (DMN) were selected as interesting networks for further analyses.Covariance analysis was used to compare the activity and functional connectivity of multiple resting state networks between three groups,and correlation analysis was used to evaluate the relations between abnormalities of brain and the severity of hallucination.Results:Compared to non-AVH,AVH group showed increased activity in left superior temporal gyrus (STG) and left postcentral gyrus (LPG) in the AUN,and showed decreased activity in right anterior cingulate cortex (ACC) in the SAN (P < 0.05,FDR corrected).The severity of AVH group was associated with activity of the left STG (r =0.43,P <0.05) and the right ACC (r =-0.48,P <0.01).Furthermore,compared to non-AVH group,there were decreased connectivity between LAN and AUN [(-0.29 ±0.21) vs.(-0.16 ± 0.17),P < 0.05,uncorrected],decreased connectivity between SAN and LAN [(-0.09 ± 0.22) vs.(-0.06 ± 0.18),P < 0.05,uncorrected],increased connectivity between SAN and AUN [(0.30 ± 0.18) vs.(0.15 ± 0.24),P < 0.05,uncorrected] in AVH group.Conclusion:There may be aberrant functional connectivities of AUN,LAN and SAN in schizophrenia patients with frequent AVH,and the occurrence of AVH seems to be related to brain areas involved in language production,speech perception and self-monitoring.
4.Relationship between bullying victimization and overweight/obesity among adolescents in Wuhan

Objective:
To analyze the prevalence of overweight and obesity among adolescents in Wuhan and its association with bullying victimization,and to provide information for relevant interventions.
Methods:
A total of 3 251 adolescents in senior grade of primary school, middle school and high school were selected by using random clustering sampling method and were surveyed by questionnaire, height and weight were measured.
Results:
Among the 3 251 students, 523(16.09%) were overweight, and 445 (13.69%) were obesity. In the past 30 days, 732 (22.52%) of the adolescents had ever been bullied and 101(3.11%) had often been bullied. The relationship between overweight/obesity and bullying victimization among boys was not statistically significant by controlling areas and age(P>0.05). While girls who were overweight or obesity were more likely to be bullied (OR=1.78,1.58, P<0.05).
Conclusion
Overweight and obesity have a certain influence on the physical and mental development of adolescents, which may increase the possibility of being bullied among girls. Strategies should be taken to prevent overweight and obesity among children and adolescents, and a healthy body shape appreciation should be established, to reduce the incidence of bullying.
5.Simultaneous determination of seven constituents in Chenxiang Lubailu tablet by HPLC

Objective To establish the method for the simultaneous determination of liquiritin, ammonium glycyrrhizinate, hesperidin, nobiletin;tangeretin;costunolide, dehydrocostuslactone in Chenxiang Lubailu tablet by HPLC. Methods ZORBAX Eclipse XDB-C18 chromatographic column (4.6 mm×250 mm, 5 μm) was used. The mobile phase was methanol-0.1% phosphoric acid solution. Gradient elution with flow rate of 1.0 ml/min was used. Column temperature was 35 ℃. Detection wavelength for liquiritin, ammonium, tangeretin, and costunolide was at 237 nm. Detection wavelength for glycyrrhizinate was at 283 nm. Detection wavelength for hesperidin and nobiletin was at 330 nm. Injection volume was 10 μl. 16 batches of samples were tested. Results The linear ranges for the detection of liquiritin, ammonium, glycyrrhizinate, hesperidin, nobiletin, tangeretin, and costunolide were 1.110 - 55.72 (r=0.9992), 22.15 - 1108 (r=0.9999), 6.140 - 307.2 (r=0.9995), 1.130 - 56.25 (r=0.9997), 0.3700 - 18.75 (r=0.9982), 0.5200 - 26.01 (r=0.9991), and 1.180 - 58.95 (r=0.9999) μg/ml respectively. The average recoveries were 98.71%, 98.12%, 98.44%, 98.22%, 99.17%, 99.18%, and 97.93%, and the RSDs were 0.16%, 0.67%, 0.57%, 0.62%, 0.48%, 0.56%, and 0.58% respectively. The contents of the seven components in 16 batches of samples were 0.1250 - 1.174, 2.354 - 7.426, 1.822 - 27.21, 0.0370 - 1.399, 0.0723 - 0.4433, 0.0140 - 0.1990, and 0.2207 - 1.407 mg/g respectively. Conclusion The method is accurate, reproducible and durable, which could be used to the quality control and evaluation of Chenxiang Lubailu tablet.
6.Effect of anesthesia factor on prognosis of cesarean section in parturients with medium and severe pulmonary arterial hypertension

Objective To compare the effects of general anesthesia and spinal anesthesia on prognosis of cesarean section in parturients with medium and severe pulmonary arterial hypertension (PAH).Methods Parturients with medium and severe PAH,at ≥ 24 weeks of gestation,aged 20-45 yr,undergoing elective cesarean section under general or epidural anesthesia from November 1,2011 to December 31,2017 in our hospital,were divided into general anesthesia group and epidural anesthesia group according to the anesthetic method.General anesthesia was combined intravenous-inhalational anesthesia.The highest temperature within 5 days after surgery,the lowest SpO2 (inhaling oxygen 2-4 L/min via a nasal tube) within 3 days after surgery,duration of intensive care unit stay,time of postoperative use of antibiotics,requirement for targeted drugs for pulmonary hypertension,results of laboratory tests (blood biochemistry,coagulation function),postoperative mechanical ventilation,length of hospital stay,and hospitalization costs were recorded.The Apgar score and weight of the newborn,postoperative complications and death of the newborn and parturients in hospital were recorded.Cox regression analysis was used to identify the risk factors after cesarean section in parturients with medium and severe PAH.Results Fifty-seven parturients were enrolled in this study,with 21 cases in general anesthesia group and 36 cases in epidural anesthesia group.Compared with general anesthesia group,the rate of postoperative mechanical ventilation was significantly decreased,the incidence of adverse events of parturients in hospital and mortality rate were decreased,the postoperative level of albumin was increased,activated partial thromboplastin time was shortened (P<0.05),and no significant change was found in the other parameters in epidural anesthesia group (P> 0.05).The results of Cox regression analysis showed that anesthetic method and preoperative SpO2difference were independent risk factors for cesarean section-related adverse events and death of parturients with medium and severe PAH.The risk of adverse events and death of parturients was significantly higher in general anesthesia group than in epidural anesthesia group (P<0.05).Conclusion Epidural anesthesia produces better prognosis than general anesthesia when used for cesarean section in parturients with moderate and severe PAH.
7.Influential factors of cerebral oxygen saturation in pediatric cardiovascular surgery

Objective:To determine the intervention measures for the decrease of cerebral tissue oxygen saturation during anesthesia for the congenital heart disease in children.Methods:Twenty-eight children with cardiac surgery were enrolled.Anesthesia was deepened with propofol (3 mg/kg) intravenous injection.The data of cerebral tissue oxygen saturation(SctO2),mean arterial pressure (MAP),HR,bispectral index (BIS),arterial partial pressure of oxygen (PaO2),arterial partial pressure of carbon dioxide (PaCO2),hemoglobin (Hb) and middle cerebral artery (MCA) mean flow velocity (Vm) at different points were collected after intravenous injection ofpropofol at 3 mg/kg.The changes of SctO2 and the influential factors were analyzed.Results:SctO2 decreased by 4.99% after deepen anesthesia,with 95% CI 4.33% to 5.65% (P>0.05).There was no significant differince in MAP,PaO2,PaCO2,and Hb between the time points after deepen anesthesia and the baseline (P>0.05).MCA Vm decreased obviously after deepen anesthesia for 1,5,10 min (P<0.05).The decrease in MAP,HR,PaCO2 and MCAVm is positively correlated with the decrease in SctO2.Conclusion:The decrease of MAP,HR,PaCO2,and MCAVm is the risk factor for SctO2.To avoid the decrease,it needs to maintain the stability of SctO2 and prevent neurological complications.
8.Effect of ropivacaine combined with bilateral thoracic paravertebral nerve block on surgical rehabilitation in cardiopulmonary bypass heart valve surgery

Objective To observe the effect of ropivacaine on bilateral posterior thoracic paravertebral nerve block for cardiopulmonary bypass.Methods A total of 142 patients undergoing elective cardiac valve surgery,68 males and 74 females,were randomly divided into two groups:group C (general anesthesia,n =69) and group P (general anesthesia+ paraverteral nerve block with ropivacaine,n=73).The changes of hemodynamics,BIS,anesthesia dose and postoperative anesthesia were observed before and after anesthesia,the time of mechanical ventilation,the duration of vasoactive drug support,ICU stay time,length of stay and hospitalization costs were recorded.Results Compared with group C,heart rate and blood pressure were more stable in group P,and the dosage of sufentanil was less [(130.6±5.0) μg vs (202.8±7.4) μg,P<0.01],the patient was awakened quickly [(241.0±15.8) min vs (417.6±38.9) min,P<0.05],and the time of mechanical ventilation [(466.0±22.9) min vs (657.8±49.4) min,P<0.05] and ICU residence time [(22.1±1.0) h vs (34.1±3.6) h,P<0.05] were shortened,the cost of hospitalization decreased [(72 360±1 565) yuan vs (78 420±2 094) yuan,(P<0.05)].Conclusion Thoracic paravertebral nerve block anesthesia combined with general anesthesia is better than pure general anesthesia,in terms of maintaining intraoperative circulation stability,reducing the amount of anesthetic analgesic drugs,and early recovery of patients after surgery.
9.Effect of Huangkui extract powder on the expression of nephrin and podocin in podocyte induced by high glucose

Objective To investigate the effect of Huangkui extract powder (HK) on the expression of nephrin and podocin proteins in mouse podocytes induced by high glucose,which is involved in the treatment of diabetic nephropathy (DN).Methods Cultured mouse podocytes (MPC5) were incubated in high glucose and HK at 5.6 mmol/L NG,5.6 mmol/L NG + 0.45 g/L HK,25 mmol/L HG,25 mmol/L HG + 0.45 g/L HK,respectively.The 5.6 mmol/L NG group was used as normal control.After 24 hours of intervention,we detected podocyte apoptosis by Annexin-V FITC/PI double staining,measured the mRNA and protein expression of nephrin and podocin by qRT-PCR and Western blot.Results Compared with the control group (5.6 mmol/L,NG),the apoptosis rate of podocytes in the high glucose concentration group (25 mmol/L,HG) was significantly higher [(20.39 ± 0.03) % vs.(17.70 ± 0.91) %,t =2.947,P < 0.05)].The apoptosis rate of podocytes in the 25 mmol/L HG + 0.45 g/L HK group was significantly lower than that in the 25 mmol/L HG group [(11.96 ± 1.11) % vs.(20.39 ± 0.03) %,t =7.586,P < 0.01].The results of qRT-PCR and Western blot showed that the expression of nephrin and podocin was significantly inhibited by high glucose concentration compared with the control group[(0.489 ±0.040) vs.(0.721 ±0.022),t =4.992,P <0.01;(0.387 ±0.014) vs.(0.778 ±0.036),t =10.050,P <0.01],and the expression of podocin and nephrin was increased by appropriate concentration of H K [(0.603 ± 0.013) vs.(0.489 ± 0.040),t =2.653,P<0.05;(0.640±0.024) vs.(0.387 ±0.014),t=8.946,P<0.01].Conclusion Podocyte apoptosis can be induced by prolonged high glucose treatment,but a certain concentration of HK can inhibit podocyte death induced by high glucose.The possible mechanism is that HK may inhibit the apoptosis of podocytes by regulating the expression of podocin and nephrin in podocytes at high glucose concentration,thus plays a protective role on podocytes.
10.Expression of miR-1269a in ESCC tissues and its effect on malignant biological behaviors of KYSE30 cells

Objective: To investigate the expression of miR-1269a in esophageal squamous cell carcinoma (ESCC) tissues and its effect on the malignant biological behaviors of ESCC KYSE30 cells, as well as to explore the underlying mechanism. Methods: Ninety specimens of ESCC tissues and adjacent para-cancerous tissues were obtained from patients underwent surgery in Fourth Hospital, Hebei Medical University. In addition, normal esophageal immortalized epithelial cells and esophageal cancer cell lines were also collected. The expression level of miR-1269a in above mentioned tissues and cell lines was examined by Real-time fluorescent quantitative PCR. After being transfected with miR-1269a mimics and inhibitors, the effects of miR-1269a on proliferation, migration, invasion and colony formation of KYSE30 cells were detected by MTS, Transwell and colony formation assay, respectively. The bioinformatics tool was used to predict the possible target genes of miR-1269a. Then the regulation effect of miR-1269a on target gene expression was validated by WB and Dual-luciferase reporter assay. After being transfected with SOX6 plasmid, the effects of SOX6 on the proliferation, migration, invasion and colony formation of KYSE30 cells were detected by MTS, Transwell and colony formation assay, respectively. At last, rescue assay was used to confirm the results. Results: The expression level of miR-1269a in ESCC tissues was significantly higher than that in adjacent para-cancerous tissues (P<0.05), and the expression level of miR-1269a in ESCC cell lines was significantly elevated compared with the normal epithelial cells (P<0.05 or P<0.01). The capacities of proliferation, invasion, migration and colony formation of KYSE30 cells in miR-1269a mimics transfection group were obviously higher than those in mimics NC group, while those abilities in miR-1269a inhibitor transfection group were significantly lower than those in inhibitor NC group (P<0.05 or P<0.01). Bioinformatics analysis showed that miR-1269a could combine with 3’UTR region at SOX6 gene; and after miR-1269a over-expression, the expression level of SOX6 and luciferase activity in KYSE30 cells were significantly reduced (P<0.05). Rescue assay showed that miR1269a over-expression could promote the proliferation, invasion and migration of KYSE 30 cells, while simultaneous transfection of SOX6 could partially reverse the promotion effect of miR-1269a mimics. Conclusion: The expression level of miR-1269a in ESCC tissues and cell lines is significantly increased, and it could enhance proliferation, migration, invasion and colony formation of KYSE30 cell line.And its mechanism may be related to the suppression of its target gene SOX6.
