1.Coagulation index in newborn with different age

Objective To expolore the changes of coagulation index in newborns of different age and its clinical significance. Methods We studied 129 newborn infants without complication admitted to the neonatal department in an all-around hospital in Gnangzhou from January to December in 2005. These neonates were divided into two groups, including 63 cases of full-term infants and 66 cases of premature infants. Prothrombin time (PT), activated partial prothrombin time (APTT), Fibrinogen (Fg), thrombin time (TT), D-dimer (D-D) of these newborn infants were assayed on the first day, the third day and the tenth day. Results In the postnatal three periods of time, there was no difference in PT and D-dimer between two groups(P >0.05), but APTT and TT in premature infants were higher than that in full-term infants( P < 0.05), and Fg in premature infants were lower than that of full-term irfants(P<0.05). PT,APTT, TT,Ddimer were in decrease tendency and Fg increased as age increased( P < 0.05 ). Conclusion Geststional age and age had effect on coagulation index in newborn infants, and both can provide clinical references to clinical diagnosis and therapy.
2.Effect of Acupuncture plus Medication on Pregnancy RateAfter IVF in Different Age Groups

ObjectiveTo observe the effect of Chinese medication and acupuncture-moxibustion plus Western medication on the pregnancy rate after in-vitro-fertilization and embryo transfer (IVF-ET) in different age groups.MethodTotally 810 eligible patients enrolled between 1st January 2010 and 31st December 2013 from the Reproduction and Infertility Department were categorized into 3 groups according to the age, respectively 20-29 years old, 30-39 years old, and 40-49 years old. The pregnancy rates of the three groups were compared, and the pregnancy rate of IVF-ET after the treatment of the integrated treatment of Chinese and Western medicine was also statistically analyzed.ResultThe pregnancy rate was 54.3% in the group of 20-29 years old, versus 46.4% in the group of 30-39 years old and 26.0% in the group of 40-49 years old, and the total pregnancy rate of IVF-ET was 47.2%. ConclusionThe younger, the higher the pregnancy rate of IVF-ET, and intervention of traditional Chinese medicine following the assisted reproductive technique can enhance the pregnancy rate of IVF-ET.

[ Abstract] Ischemic brain injury is closely associated w ith oxidative stress, neuron loss, and inflammatory reaction. The thioredoxin system is an important antioxidant system in human body. It plays the important roles in the process of fighting against oxidative stress damage, inhibiting apoptosis and inflammation. In recent years, the roles of thioredoxin system in cerebral ischemia have attracted more and more attention. This article review s the roles of thioredoxin in cerebral ischemia.
4.Effect of cholesterol on human meniscal fibrochondrocytes

Objective:To explore the effect of cholesterol on the expression of genes for matrix synthesis and degradation of human meniscal fibrochondrocytes and its mechanism.Methods:Meniscal tissue was taken from patients undergoing arthroscopic surgery to extract fibrochondrocytes. The cells were divided into a control group in which the normal cells were not processed, a positive control group in which interleukin-1 β was used to create a degeneration model, and 2 treatment groups which were subjected to treatment with 15 and 30 μg/mL cholesterol respectively. Safranin O staining, β-galactosidase staining and enzymic kits were used to detect the morphology and total cholesterol (TCH) content of meniscal fibrochondrocytes in the 4 groups. Immunofluorescence and western blot were used to detect the protein expression of type Ⅰcollagen precursor α1 (COL1A1) and type Ⅱ collagen precursor α1 (COL2A1). RT-qPCR was used to detect the mRNA expression of COL1A1, COL2A1, matrix metalloproteinase (MMP) 3, MMP9, MMP13, and genes related to cholesterol efflux pathways [like liver X receptor α (LXR α), ATP binding cassette transporter A1 (ABCA1) and ABCG1]. Results:There was no significant difference between the control and the positive control groups in the TCH content in human meniscal fibrochondrocytes ( P>0.05). The treatments with 15 and 30 μg/mL cholesterol resulted in significantly increased TCH contents in human meniscal fibrochondrocytes in the treatment groups ( P<0.05). Compared with the control group, the mRNA expression of LXR α, ABCA1 and ABCG1 was significantly decreased in the treatment groups ( P<0.05), and the meniscal fibrochondrocytes in the positive group and the treatment groups presented with a lower density, chaotic distribution and obvious signs of degradation. Compared with the control groups, the mRNA expression of matrix synthesis genes (COL1A1 and COL2A1) in the meniscal fibrochondrocytes was significantly inhibited while the mRNA expression of matrix degradation metalloenzymes (MMP3, MMP9 and MMP13) was significantly promoted ( P<0.05). Conclusion:Cholesterol may inhibit the cholesterol efflux pathways of meniscal fibrochondrocytes, and thus cause accumulation of cholesterol in the meniscal fibrochondrocytes, eventually leading to degeneration of meniscus.
5.Effects of Acupuncture on Hippocampal Synaptic Transmission Signal Molecules in Rats with Vascular Dementia

Objective To observe the effects of acupuncture on synaptic transmission signal molecules in rats with vascular dementia (VD), such as PKC, CaMKⅡ and NR2B, and discuss the molecular mechanism of acupuncture treatment for VD.Methods The multi-infarct dementia model was established by injection of emboli into the internal carotid artery. Experimental rats were randomly divided into normal group, model group, acupuncture group and non-acupoint group. For acupuncture group, acupuncture needles were penetrated into bilateral Zusanli. Non-acupoint group was given acupuncture treatment at the bilateral hypochondrium (10 mm above iliac crest). The rats in normal group and model group were performed to the same amount of capture stimulation as the acupuncture and non-acupoint groups. After treatment, the hippocampal PKC activity was detected by ELISA. Western blot was used to detect CaMKⅡ expression, and the protein expression of NR2B in CA1, CA3 and DG zones was assayed by immunohistochemical staining.Results Compared with normal group, PKC activity and NR2B expression in the hippocampus significantly decreased in the model group (P<0.01). After the acupuncture treatment, PKC activity increased significantly (P<0.05), and the protein expression of NR2B showed a trend to increase. There was no obvious difference in CaMKⅡ expression among all groups.Conclusion Acupuncture at Zusanli can enhance the activity of hippocampal PKC, a synaptic transmission signal molecule, which maybe one of the important molecular targets for the treatment of VD.
6.Effects of Acupuncture on Oxidative Stress and Neuronal Damage in Prefrontal Cortex of Vascular Dementia Rats

Objective To observe the effects of acupuncture on oxidative stress and neuronal damage in the prefrontal cortex of chronic cerebral hypoperfusion (CCH) rats; To investigate the molecular mechanisms of acupuncture in cognitive impairment of vascular dementia (VD) rats. Methods The animal model of VD was replicated by permanent bilateral common carotid artery occlusion (2VO) to induce CCH in rats. After modeling, rats were randomized into model group, acupuncture group and non-acupoint group. Sham-operation group was set as control group. Rats of acupuncture group and non-acupoint group were treated with needling for 2 weeks at 3 days after 2VO. Nissl and DHE staining were respectively used to evaluate the numbers of neurons and ROS level. SOD activity was examined by xanthine oxidase technique. Results Compared with sham-operation group, the number of neurons and SOD activity decreased in the model group, while the ROS level increased. Compared with the model group, the number of neurons and SOD activity increased in the acupuncture group, while the ROS level decreased. There were no obvious changes in all indexes in non-acupoint group. Conclusion Acupuncture can inhibit oxidative stress and improve neuronal damage and loss in the prefrontal cortex of VD rats.
7.A STUDY OF THE NUTRITIONAL STATUS AND ADEQUATE DIETARY NUTRIENTS INTAKES IN GRAVIDAS

The nutritional status, serum protein, albumin, calcium, zinc, iron, SF, FEP, vitamin A, activity of RBC transketolase, and blood glutathione redu-ctase of 349 normal primigravidas, 30 healthy non-pregnant women and 42 toxemic gravidas were estimated longitudinally from the 1st trimester to delivery. It was noted that serum protein, albumin, Ca, Zn, and Fe decreased markedly in the course of pregnancy. The content of serum vitamin A showed no variation or decrease in the 3rd trimester in 1/3 of the pregnant women observed. Insufficiency of thiamine and riboflavin were noted in 16.7% and 47% of the normal pregnant women respectively. The adequate nutrients intakes in pregnancy were considered to be: Energy, +200 kcal/d; Protein, + 15g/d in the 2nd trimester and +25g/d in the 3rd trimester; Calcium 1000 mg/d in the 2nd trimester and 1500mg/d in the 3rd trimester; Zinc, 20mg/d; Iron 26 or 30 mg/d by supplementation if possible; vitaminA 1000 ?gRE/d; both thiamine and riboflavin 1.8 mg/d.
8.Adult hemophagocytic syndrome in emergency department setting: the implications of hormone therapy on diagnosis and prognosis

Objective:To assess the implications of hormone therapy on confirmation of clinical diagnosis and prognosis of adult hemophagocytic syndrome (HPS) in the emergency department setting.Methods:The eligible 34 patients admitted with suspected HPS in the Emergency Department of Peking University People's Hospital from September 2019 to August 2021 were respectively collected. The patients were divided into the death group and survival group according to the prognosis and divided into early hormone therapy group and standard hormone therapy group according to the timing of hormone application. Patients in the early hormone therapy group were divided into the routine 4 criteria group and non-routine 4 criteria group according to the conditions of meeting the four HLH-2004 diagnostic criteria. Medical records of the following were collected and statistically analyzed: complete blood count, blood biochemical index, coagulation function, serum ferritin, NK cell activity, sCD25 level, peripheral blood smear, bone marrow biopsy, abdominal ultrasound scan, and abdominal CT on admission, and recheck the clinical indicators such as blood count, blood biochemical index and blood coagulation dunction 5-7 days later.Results:①Patients from the death group were older, with higher APACHEⅡ scores and SOFA scores, higher total bilirubin, and lower serum albumin. ② Univariate Logistic analysis showed age ( OR=1.098, CI: 1.019-1.183, P=0.014), APACHE Ⅱ score ( OR=1.144, CI: 1.017-1.285, P=0.024), SOFA score ( OR=1.441, CI: 1.079-1.925, P=0.013) were associated with the risk of death. Multivariate Logistic analysis showed that age ( OR=1.099, CI: 1.014-1.190, P=0.021) was associated with the risk of death. There was no significant correlation between early hormone therapy and clinical prognosis. Kaplan-Meier survival curves showed that there was no difference in the 60-day survival rate between the early hormone therapy group and the standard hormone therapy group. ③ The level of triglyceride still increased after early hormone therapy, and the number of indexes meeting the diagnostic criteria of HLH-2004 increased significantly. All patients met the criteria of Hscore>169, and 3 patients did not meet at least 5 diagnostic criteria of HLH-2004, accounting for 16.7% of the total cases of early hormone therapy. ④ Starting hormone therapy when the four HLH-2004 diagnostic criteria were met could reduce the length of hospital stay. Prothrombin time and activited partial thomboplastin time were closer to normal levels in patients 5-7 days after treatment. Early hormone therapy had no significant effect on treatment response and in-hospital death risk. There were no significant differences in APACHE Ⅱ score, SOFA score, confirmation of diagnosis, treatment response, clinical prognosis, and related clinical indicators after hormone therapy between the routine 4 criteria and non-routine 4 criteria groups. Conclusions:Initiation of early hormone therapy has no significant effect on the confirmation of clinical diagnosis, treatment response, in-hospital mortality, and 60-day survival rate of patients with HPS, and can quickly correct coagulation dysfunction and effectively reduce the length of hospital stay. An earlier start of hormonal therapy (meeting the four HLH-2004 diagnostic criteria) may be considered by the emergency physician when a patient is highly suspected of HPS diagnosis
9.Clinical diagnosis value of echocardiography and prognosis evaluation to fetus with persistent truncus arteriosus

Objective To evaluate prenatal diagnosis value of echocardiography in pathological types,differential diagnosis and accompanied malformations of fetal persistent truncus arteriosus(PTA).MethodsTwenty-four cases of PTA selected from 1 392 cases were analysed,who were definitely diagnosed to be suffered from cardiovascular malformation by fetal echocardiography.The ultrasound findings,pathological results and followed up were analysed.According to Van Praagh classification,the type IV PTA was excluded in this study which was classified into pulmonary artery atresia.Results The total PTA were 24 cases,in which 10 cases of A1 type,3 cases of A2 type,9 cases of A3 type,and 2 cases of A4 type.Nine cases of PTA accompanied other cardiac anomalies,and 1 case of PTA accompanied both cardiac anomalies and extracardial malformations.Two PTA cases were born,one was A1 type underwent surgical intervention,and the other was died due to multiple organ-failure.Fourteen PTA cases were termination and 7 cases were confirmed by pathology.Seven women pregnant again,of which 5 cases were born while only one was diagnosed atrial septal defect after birth,2 pregnant women were still during follow-up.Eight PTA cases follow-up were lost.Conclusions A1 type and A3 type of PTA have high incidence in fetus.Accompanied cardiac anomalies is certainly related to different types.Combination of multiple ultrosund techniques can diagnose PTA prenatally,make accurate classification and detect accompanying malformations,which is of great significance to offer proper pregnancy counselling and postpartum treatment.
10.Progress in efficacy and safety of vigabatrin in treatment of epilepsy associated with tuberous sclerosis complex in children

Tuberous sclerosis complex(TSC)is an autosomal dominant skin-nerve syndrome with diverse clinical manifestations.Epilepsy is the most common neurological manifestation.Vigabatrin(VGB)is an inhibitory neurotransmitter γ-aminobutyric acid(GABA)analogue.It reduces the degradation of GABA by irreversibly binding to GABA transaminase, thereby increasing the level of GABA in the central nervous system and exerting anti-epileptic effects.It is suitable for the treatment of various types of epilepsy related to TSC.In recent years, the preventive effect of VGB in TSC-related epilepsy has attracted wide attention.There is a certain degree of adverse reactions during the use of VGB, the most noteworthy of which is the visual field and central nervous system changes.This article reviews the efficacy and safety of VGB in the treatment of TSC-related epilepsy.
