1.Diagnosis and Treatment of Cardiac Involvement in Dystrophinopathies (review)

Dystrophinopathies are a group of fatal X chromosome recessive genetic disease. With the application of ventilator technolo-gy, the main causes of death in patients with dystrophinopathies turned from respiratory failure into circulatory failure, and the importance of heart problems has become increasingly prominent. Myocardial damage can be defined with myocardial enzymes, electrocardiogram, ul-trasound cardiogram, cardiac magnetic resonance imaging, etc. Drugs for chronic heart failure remain the most important strategy, and heart transplant or left ventricular assist devices implant may be the alternatives for those in the end.
2.Study on the Chemical Consitituents of Different Parts of Mentha haplocalyx in Jiangxi by GC-MS

OBJECTIVE:To establish a method for analyzing the volatile constituents of different parts(root,stem and leaf)of Mentha haplocalyx. METHODS:HS-GC-MS was performed. GC conditions:the column was HP-5MS capillary quartz column, carrier gas was He (constant current mode),flow rate was 1.0 ml/min,volume injection was 1.0 μl,inlet was split mode with split ratio of 10∶1,temperature was 250 ℃(temperature programmed). MS conditions:ionization source was electron bombard-ment,energy was 1 905 V,temperature was 230 ℃,quadrupole temperature was 150 ℃,and the scanning range was m/z 30-550. RESULTS:Totally 10,14 and 48 volatile components were identified from the root,stem and leaf,which accounted for 90.03%, 92.79% and 95.93% of the capacity of the total essential oil from each part of it. And totally 51 volatile compounds was identified, including 7 kinds that shared the same chemical components that regard the piperitone oxide as the main ingredient. CONCLU-SIONS:The method is simple,stable and reproducible,and can be used for the analysis of volatile components of different parts of M. haplocalyx. The volatile components in different parts are quite different,and most is in the leaf.
3.Research and implementation of cooperative cache for PVFS

At present, there are many effective ways to achieve high performance in cluster system storage management, including server-end disk, server-end caching, local caching and cooperative caching. The cooperative caching mechanism shares caches among different clients so as to avoid expensive disk access costs and to improve overall throughput of cluster system. In this paper, a Single Copy Cooperative Cache model is proposed together with block lookup algorithm, block replacement algorithm and the consistency algorithm based on the model. Meanwhile, the prototype system of the model is implemented in PVFS file system. Finally, the performance of this system is tested in InfiniBand Framework, the result of which shows that in contrast to the original PVFS system, read performance of PVFS file system is improved by about two times, while write performance is reduced by nearly ten percent.
4.The association between peripheral blood circulating follicular helper T cells and antibody-secreting B cells in patients with ankylosing spondylitis

Objective To investigate the relationship between peripheral blood circulating follicular helper T cells and antibody-secreting B cells in patients with ankylosing spondylitis (AS). The role of circulating follicular helper T cells and antibody-secreting B cells in the pathogenesis of AS were explored. Methods Flow cytometry was used to detect the levels of peripheral blood CD4 +CXCR5+ T (cTfh), CD4+CXCR5+PD-1+ T, CD4+CXCR5+ICOS+ T, CD19+ B, CD19+CD38+ B cells in 59 patients with AS (including 36 cases of active AS patients and 23 cases of inactive AS patients). In addition, twenty healthy persons were selected as the controls. Data analysis were performed by independent-sample t test, One way ANOVA analysis, Pearson's and Spearman's correlation test. Results The percentages of cTfh [(26.8 ±10.4)%], CD4+CXCR5+PD-1+T [(12.1±14.0)%], CD4+CXCR5+ICOS+T [(13.6±9.5)%], CD19+CD38+B [(80.7±13.0)%] in the peripheral blood of AS group were significantly higher than healthy controls [(15.6 ±4.5)%, (6.4 ±2.4)%, (9.4 ± 4.5)%, (68.2±13.0)%] (t=6.663, P<0.01; t=2.999, P<0.01; t=2.573, P<0.05; t=2.712, P<0.01). The percentages of cTfh in the active AS group [(30.2 ±11.0)%] were significantly higher than those in the inactive AS group [(21.4±6.5)%] and HC (t=3.444, P<0.01;t=7.004, P<0.01). The percentage of CD19+CD38+B[(85.1±10.0)%] in the peripheral blood of active AS group was significantly higher than that of the inactive AS group [(73.8 ±14.2)%] and HC (t=3.561, P<0.01;t=5.410, P<0.01). The relationship between Bath AS disease activity index (BASDAI) and the percentage of cTfh, CD19+CD38+B was a positive correlation (r=0.442, P<0.01; r=0.405, P=0.001), and significant positive correlation was observed between the percentages of cTfh and CD19+CD38+B cells (r=0.420, P=0.001). Conclusion CD4+CXCR5+cTfh cells are significantly increased in peripheral blood in AS patients with aberrant CD19+CD38+ antibody-secreting B cells, suggesting that cTfh and CD19+CD38+antibody-secreting B cells may play an important role in the pathogenesis of AS .
5.Analysis of the clinicopathological characteristics and prognosis of 742 cases with primary IgA nephropathy

Objective:To investigate the clinicopathological characteristics and influencing factors of kidney prognosis in primary IgA nephropathy (IgAN) patients.Methods:The data of primary IgAN patients diagnosed with renal biopsy in the First Affiliated Hospital of Anhui Medical University from January 2015 to September 2019 were retrospective analyzed. According to the level of baseline estimated glomerular filtration rate (eGFR) when performing renal biopsy, the patients were divided into group A[eGFR≥90 ml·min -1·(1.73 m 2) -1], group B[eGFR 61-89 ml·min -1·(1.73 m 2) -1] and group C[eGFR≤60 ml·min -1·(1.73 m 2) -1]. The clinical and pathological data were collected and compared among the three groups. Kaplan-Meier method was conducted for renal results, whereas the Cox proportional-hazards regression model was exploited to analyze the influencing factors of kidney prognosis in IgAN patients. Results:A total of 742 patients were included in the study, including 394 cases (53.1%) in group A, 203 cases (27.4%) in group B, and 145 cases (19.5%) in group C. There were 325 males (43.8%) and 417 females (56.2%). The median duration of renal biopsy was 6 (1, 24) months, and the median age was 36 years old (18-68 years old). As the baseline level of renal function decreased, the proportion of patients with nephrotic syndrome, hypertension, anemia and hyperuricemia and the levels of 24 h urinary protein, serum triglyceride and total cholesterol increased significantly (all P<0.05), while the proportion of gross hematuria episodes and the ratio of serum albumin to globulin significantly decreased (all P<0.05). For the aspect of pathological manifestations, the proportions of cell proliferation in capillaries (E1), segmental sclerosis or adhesion (S1), renal tubular atrophy or interstitial fibrosis (T1/2), globular sclerosis, renal arteriole wall thickening and vitreous degeneration, Lee's grade Ⅳ and Ⅴ increased with the decrease of baseline renal function (all P<0.05). Kaplan-Meier analysis showed that the cumulative renal survival rate decreased with the decline of baseline renal function (Log-rank χ2=88.510, P<0.001). As a result of multivariate Cox regression analysis, nephrotic syndrome ( HR=2.399, 95% CI 1.054-5.459, P=0.037), hypertension ( HR=1.806, 95% CI 1.071-3.048, P=0.027), low baseline eGFR (taking group A as the reference, group B: HR=2.383, 95% CI 1.053-5.392, P=0.037; group C: HR=6.878, 95% CI 3.074-15.393, P<0.001), IgG deposition ( HR=2.224, 95% CI 1.384-3.574, P=0.001) and globular sclerosis ( HR=2.075, 95% CI 1.230-3.501, P=0.006) were the independent influencing factors for renal progression in primary IgAN patients. Conclusions:The level of baseline renal function in primary IgAN patients can be used to predict the extent of clinic-pathological damage. Nephrotic syndrome, hypertension, low baseline eGFR, IgG deposition and globular sclerosis are the independent influencing factors for renal progression in primary IgAN patients.
6.The quality of life in man after treatment of advanced prostate cancer by surgical castration

Objective To investigate the quality of life in man after treatment of advanced prostate cancer by surgical castration. Methods A total of 69 patients with advanced prostate cancer treated by sur-gical castration completed the European organization for research and treatment of cancer quality of life questionnaire (QLQ-C30) and QLQ-prostate specific 25-item (PR25) module third at a 12-month interval.The assessment points were preoporative,6 months postoperative and 12 months postoperative. Results Although there were improvement on pain relief (P < 0.01 ) and symptom of urinary (P < 0.01 ) and global health (P < 0.01 ), the physical functioning (P < 0.05 ), role functioning (P < 0.01 ), emotional functioning (P < 0.01 ), cognitive functioning (P < 0.05 ), social functioning (P < 0.05 or < 0.01 ) were found significantly reduced , and insomnia (P < 0.05) and fatigue (P < 0.01 ) aggravated, treatment related symptoms (P <0.01 )appeared predominantly, especially deprivation of sexual functioning. Conclusion The surgical cas-tration when treating advanced prostate cancer did appear to impair the physical and psychological of patients, especially the sexual functioning.
7.Forging and training of interns’clinical thinking ability

8.Preliminary application of video head impulse test in the diagnosis of vertigo.

OBJECTIVE:
To investigate clinical application of head impulse test with video recording eye movements in the diagnosis of vertigo.
METHOD:
The video head impulse test(vHIT) was used to measure the eye saccades and velocity gain in 95 patients with vertigo which were divided into two groups, peripheral vertigo (47 cases) and central vertigo(48 cases); the characteristics of eye saccades and velocity gain of six semicircular canals in different patients with vertigo were analyzed, and were compared between the two groups.
RESULT:
The vHIT result in patients with peripheral vertigo: in 22 patients (23 affected ears) with Meniere's disease, 21 ears were abnormal (91. 3%); the vHIT results in 4 patients with vestibular schwannoma, 2 patients with vestibular neuritis, 5 patients with delayed endolymphatic hydrops, 6 patients with sudden hearing loss accompanied vertigo, and 8 patients with vestibular dysfunction, were abnormal with correct saccades and/or lower velocity gain of vHIT. The abnormal vHIT results were also found in 35 of 48 patients (72. 9%) with central vertigo, which including posterior cerebral circulation ischemia(7 patients), cerebral infarction/stroke(6 patients), and dizziness with vertigo(17 patients) and others(18 patients). Abnormal rate of vHIT in patients with peripheral vertigo was 95. 7% (45/47), which was significantly higher than that (72. 9%) in patients with central vertigo.
CONCLUSION
It is easy to perform the vHIT which without adverse reactions. We can record high-frequency characteristics of vestibular-ocular reflex among six semicircular canals through vHIT. The vHIT results which show the function of vestibular ocular reflex in different diseases with vertigo, can help discriminate peripheral vertigo from central vertigo, and it is a practical assessment method for vertigo.

9.Effect of Dihydromyricetin on High Glucose-induced Glomerular Mesangial Cell Proliferation and Fibro-nectin Accumulation

OBJECTIVE:To study the effect of dihydromyricetin(DMY)on high glucose(HG)-induced glomerular mesangial cell(MCs)proliferation and fibronectin(FN)accumulation,and explore its mechanism for diabetic nephropathy glomerulosclero-sis. METHODS:Cells were divided into normal group(5.5 mmol/L glucose),HG group(30 mmol/L glucose),DMY low-concen-tration,medium-concentration,high-concentration groups (30 mmol/L glucose+22.5,45,90 μmol/L DMY). After incubating 48 h,MTT was used to detect the proliferative activity [reflected by the optical density(OD)value] of cells;molecular docking meth-od was adopted to conduct simulation analysis for DMY binding state with Smad2. Cells were divided into normal group(5.5 mmol/L glucose),HG group (30 mmol/L glucose),DMY group (30 mmol/L glucose+45 μmol/L DMY) and DMY control group (5.5 mmol/L glucose+45 μmol/L DMY). After incubating 5 h,Western blot was used to detect the expression levels of phosphorylated Smad2 (p-Smad2) and extracellular matrix protein FN. RESULTS:Results of MTT detection showed,compared with normal group,OD values in HG group were significantly increased(P<0.05);compared with HG group,OD values in DMY each con-centration group were significantly reduced(P<0.05). The Gibbs free energy(ΔG)of DMY and Smad2 protein was -5.64 kJ/mol, Ki was 73.53 μmol/L,and there were hydrogen bond donor and receptor binding in No. 465,464,461,458 amino acid residues. Results of Western blot showed,compared with normal group,expression levels of p-Smad2 and extracellular matrix protein FN in HG group were significantly increased (P<0.05);compared with HG group,expression levels of p-Smad2 and extracellular ma-trix protein FN in DMY group were significantly decreased(P<0.05). CONCLUSIONS:DMY inhibits HG-induced MCs prolifera-tion and improves diabetic nephropathy glomerulosclerosis by combining with Smad2 and inhibiting Smad2 phosphorylation to re-duce the extracellular matrix protein FN expression.
10.Lipophilic constituents from the stem bark of Toona sinensis

Ten compounds were isolated from the 95% ethanol extract of the stem bark of Toona sinensis by silica gel, ODS Sephadex LH-20 column chromatographies and preparation TLC methods. Their structures were elucidated by spectroscopic methods and chemical properties as betulinic acid(1), stigmastane-3, 6-dione(2), stigmast-4-en-3-one(3), 7β-hydroxysitosterol(4), 6-hydroxystigmast-4-en-3-one(5), stigmast-4-ene-3β, 6β-diol(6), ergosterol peroxide(7), 7-methoxy-2-(3, 4-methylenedioxyphenyl)benzofuran-5-carboxylate(8), canthin-6-one(9), and α-acetylamino-phenylpropyl α-benzoylamino phenylpropionate(10). All compounds were isolated from T. sinensis for the first time.
