1. Differential diagnosis of phyllodes tumor and fibroadenoma of breast using mammography and ultrasonography

Objective: To assess the clinical value of mammography and ultrasonography in diagnosis and differential diagnosis of breast phyllodes tumor and fibroadenoma. Methods Clinical, mammographic and sonographic findings were retrospectively analyzed and compared in 110 women with phyllodes tumor and 102 women with fibroadenoma confirmed by surgical pathology. Results The maximum diameter of phyllodes tumor ([4.61±3.35]cm) and fibroadenoma ([2.71±1.44]cm) were statistically different (P<0.001), and the difference of maximum diameter between benign and malignant phyllodes tumor was statistically significant (P=0.024). The mammogram parameters of tumor shape, margin, density and calcification were statistically different between phyllodes tumor and fibroadenoma (all P<0.05), and ultrasonography showed statistically significant differences in tumor shape, margin, internal cystic regions, posterior echo, blood flow classification (all P<0.05). Among the benign, borderline and malignant phyllodes tumors, the difference of cystic regions inside the tumor was statistically significant (P=0.006). Conclusion Combination of mammographic, sonographic and clinical findings can initially identify phyllodes tumor and fibroadenoma. When the tumor is large (>3.0 cm), lobulated, high density with internal cystic regions and abundant blood flow signal, phyllodes tumor should be considered.
3. Diagnostic performance of contrast-enhanced spectral mammography in suspected breast lesions based on histological results

Objective:
To assess the diagnostic performance of contrast-enhanced spectral mammography (CESM) in suspected breast lesions.
Methods:
A total of 97 patients with suspected breast cancer identified by clinical examination or screening underwent two-views CESM examination on the basis of digital breast tomosynthesis (DBT) combined with full-field digital mammography (FFDM), and they were finally confirmed by biopsy or pathology. Three senior radiologists analyzed images, including lesion visibility, lesion characteristics, enhancement type, degree of enhancement, BIRDS classification, etc. Finally, based on the pathology, we compared the CESM+DBT+FFDM and DBT+FFDM two models according to sensitivity, specificity and ROC for diagnostic performance.
Results:
There were a total of 120 lesions. Eighty-nine lesions were malignant, 31 benign; CESM was not enhanced in 2 cases, mild enhancement was performed in 22 cases, moderately intensive in 15 cases, highly intensive in 81 cases, and 2 cases were not enhanced; mass-enhanced in 96 cases, including ring-enhanced in 12 cases, 22 cases of non-mass type. The sensitivities of the combination of CESM and not combination of CESM were 91.0% and 80.9%, respectively, and the specificities were 93.5% and 87.1%, respectively. 5.Variant analysis of CCBE1 gene in a case of Hennekam lymphangiectasia-lymphedema syndrome type 1.

OBJECTIVE:
To explore the genetic etiology of a child with lymphangiectasia and lymphedema.
METHODS:
DNA sample of the patient was extracted and subjected to whole exome sequencing. Suspected variants were verified by Sanger sequencing.
RESULTS:
The patient was found to carry compound heterozygote variants (c.521G>A and c.472C>T) of the CCBE1 gene, which were respectively inherited from his parents.
CONCLUSION
The compound heterozygote variants of the CCBE1 gene probably underlie the disease in this child.
6.Strontium fructose 1,6-diphosphate (FDP-Sr) alleviates oligozoospermia induced by multiglycosides of tripterygium wilfordii in rats.

OBJECTIVETo investigate the effects of strontium fructose 1,6-diphosphate (FDP-Sr) on the multiglycosides of tripterygium wilfordii Hook. f. (GTW)-induced oligozoospermia in male rats.
METHODSForty SD male rats were randomly divided into 3 groups: model (n=10), FDP-Sr (n=10), and control (n=10). The model rats received intragastric administration of GTW at 30 mg/(kg x d) for 40 days to induce oligozoospermia, the rats of the FDP-Sr group orally administered FDP-Sr at 200 mg/(kg x d) for 30 days after GTW induction, while the control rats given but distilled water. Then we obtained the gonad indexes involving the testis, epididymis, preputial gland and seminal vesicle, and determined the count and motility of epididymal sperm, testicular pathomorphology, serum testosterone level and activities of succinodehydrogenase (SDH), lactate dehydrogenase (LDH) and acid phosphatase (ACP) in the testis.
RESULTSThe indexes of the testis and seminal vesicle in the control, model and FDP-Sr groups were (0.71 +/- 0.04) and (0.29 +/- 0.04)%, (0.37 +/- 0.04) and (0.25 +/- 0.05)%, and (0.45 +/- 0.07) and (0.31 +/- 0.06)%, respectively; the epididymal sperm counts were (59.87 +/- 11.28), (11.06 +/- 2.53) and (20.95 +/- 4.98) x 10(6)/ml; the serum testosterone levels were (85.31 +/- 7.41), (65.33 +/- 2.90) and (75.32 +/- 5.34) ng/L; and the activities of ACP, LDH and SDH were (95.64 +/- 19.27), (9574.73 +/- 3 578.06) and (6.39 +/- 1.93) U/g prot, (58.42 +/- 12.38), (4820.77 +/- 1 535.22) and (3.48 +/- 0.91) U/gprot, and (83.74 +/- 21.30), (7649.01 +/- 3 123.02) and (5.59 +/- 1.75) U/g prot. All the parameters above were significantly increased in the FDP-Sr group as compared with the GTW models (P < 0.05). Besides, FDP-Sr treatment significantly alleviated the injury of the testicular seminiferous epithelium.
CONCLUSIONFDP-Sr can alleviate GTW-induced oligozoospermia, which is closely related with its improvement of testicular function.

7.Telomere Length, Expression of MRE11 and Ku80 in Patients with Aplastic Anemia and Their Correlation with Pathogenesis.

OBJECTIVETo detect the expression levels of MRE11 and Ku80 mRNA, and telomere length in bone marrow mononuclear cells of aplastic anemia(AA) patients, and to explore their correlation with pathogenesis of aplastic anemia.
METHODSBone marrow mononuclear cells were collected from 40 cases of AA and 20 normal controls for detecting mRNA expression of MRE11 and Ku80 and telomere length by using real-time quantitative polymerase chain reaction (qPCR), then MRE11, Ku80 and telomere length were analyzed for their correlation.
RESULTSAs compared with controls, the expression levels of MRE11 and Ku80 in patients with AA were significantly reduced, and the telomere length in patients with AA was obviously shortened, respectively (P<0. 05). The telomere length was significantly shorter in the persons aged ≥45 years in comparison with the AA patients and normal control younger than 45 years old (P<0.05). For the AA patients older than or equal to 45 years and less than 45 years in comparison with the controls at the same age, the telomere length was significantly shorter(P<0.05). The expression levels of MRE11 and Ku80 didn't correlate with telomere length (P>0.05). The mRNA expression level of MRE11 correlated positively and significantly with that of Ku80 (r=0.863, P<0.05).
CONCLUSIONThe change of telomere length may play an important role in the pathogenesis and progression of aplastic anemia. The lower expression of MRE11 and Ku80 may be involved in the pathogenesis of aplastic anemia.
8.Investigation on anxiety and depression of employees in municipal and above administrative organs and public institutions in Qinghai region

BackgroundThe mental health problems of employees in administrative organs and public institutions are highly valued. However, there is a lack of understanding regarding the mental health status of employees in provincial and municipal administrative organs and public institutions in Qinghai region. ObjectiveTo analyze the anxiety and depression of employees in provincial and municipal administrative organs and public institutions in Qinghai region, so as to provide references for promoting their mental health and intervening in psychological problems. MethodsFrom October 23 to 28, 2022, a total of 3 096 employees in provincial and municipal administrative organs and public institutions in Qinghai region were enrolled using stratified cluster sampling technique, and were assessed using Self-rating Anxiety Scale (SAS) and Self-rating Depression Scale (SDS). ResultsA total of 1797 (58.04%) employees in municipal and above administrative organs and public institutions in Qinghai region completed a valid questionnaire survey. Depressive symptoms were noted in 579 (32.22%). SDS scores yielded statistical difference among the selected employees of different ethnicities, types of job positions and educational levels (F=9.074, 101.488, 4.477, P<0.01), and the severity of depressive symptoms also demonstrated statistical difference among the selected employees with different ethnicities, educational levels and types of job positions (H=49.196, 17.028, 160.848, P<0.01). Anxiety symptoms were detected in 711 (39.57%) employees. SAS scores exhibited statistical difference among the employees with different genders, ages, ethnicities, educational levels, marital status and types of job positions (t=-4.571, F=6.648, 3.950, 5.243, 12.008, 22.253, P<0.01). Statistical differences were also illustrated in the severity of anxiety symptoms among the selected employees with different ages, genders, ethnicities, educational levels, marital status and types of job positions (H=24.361, -3.788, 28.365, 24.268, 25.976, 56.204, P<0.01). ConclusionAbout one-third of employees in municipal and above administrative organs and public institutions in Qinghai region may have symptoms of depression and anxiety. The depression symptoms are more severe among Tibetan people, those with low education levels and ordinary civil servants.
