1.A Case of Gastritis Cystica Polyposa, Presenting as a Submucosal Tumor - like Lesion.

Gastritis cystica polyposa (GCP) is a lesion characterized by all the histological features described for hyperplastic polyps, And there is s marked proliferation of muscular elements and entrapment of numerous epithelial cysts. These lesions have been described at gastroenterostomy stomas, at peptic ulcer edges, and in association with carcinoma. It is rare that GCP presenting as a submucosal tumar-like lesion develops in the absence of above mentioned associated conditions. We had experienced a 69 year-old man with GCP. Barium X-ray and endoacopic findings showed submucosal tumor in antrum, and endoscopic ultrasonograpic findings showed markedly thickened mucosal layer with scattered hypoechoeic areas and the submucosal layer well preserved. GCP was confirmed by histological examination of resected stomach.

3.Establishment of Reference Values for Platelet Activation Markers by Flow Cytometry.

BACKGROUND: This study was purposed to establish reference values for platelet activation markers and leukocyte-platelet aggregates in the evaluation of the platelet function tests using flow cytometry. METHODS: Whole blood samples were obtained from 30 volunteers of healthy adults. Diluted blood samples, either in the resting state or in activated state by the addition of agonist, 20 micrometer ADP or 100 micrometer TRAP, were stained with fluorescent conjugated monoclonal antibody of PAC1 or CD62P. Then, the percentages of expression for each marker were analyzed by flow cytometry. For leukocyte-platelet aggregates, monoclonal antibodies of CD41a, CD14 and CD45 were added simultaneously to undiluted whole blood. RESULTS: Reference values for the percentages of the expression of PAC1 and CD62P, respectively, were 0.1-12.5% and 0.0-4.7% at the resting state, 65.3-92.4% and 39.0-75.7% with the addition of 20 micrometer ADP, and 68.1-93.1% and 60.5-91.2% with the addition of 100 micrometer TRAP. Reference values for leukocyte-platelet aggregates, granulocyte-platelet aggregates, and lymphocyte-platelets aggregates were 2.8-23.6%, 5.3-34.2%, and 4.9-21.6%, respectively. CONCLUSIONS: The platelet activation markers at the resting or an activated state with agonists and leukocyte-platelet aggregates could be analyzed using flow cytometry. These reference values should be helpful in interpreting platelet function tests by flow cytometry.

PURPOSE: The purpose of this study is to evaluate the knowledge and attitudes of oncology ward nurses toward cancer pain management and to find ways to improve the educational program for nurses. METHOD: A total of 209 nurses working at the oncology ward of three hospitals in Seoul and a Gyenggi Province. The survey instrument used was the 32-item scale for evaluating nurses knowledge and attitudes originally developed by McCaffery and Ferrell'(1990), that was by Kim'(1997). RESULTS: In terms of the nurses knowledge of pain management, the result showed that the nurses scored an average of 67.8 out of 100 for phamacokinetics of opioids, 84.8 for classification of analgesics, 60.1 for pain assessment, and 70.7 for drug administration. 18.2% of the nurses hesitate to inject the narcotic agent because of concerns regarding the drug's potential side effects. there was significant difference in the knowledge of pain management according to the general characteristics of pain in terms of the nurses age(p=.001), position (p=.016), years of experiences(p=.002), experience of cancer pain education(p=.001). CONCLUSION: The also showed that nurses working at cancer ward lack knowledge. It is important to provide intensive education to nurses about cancer pain management.

6.A Case of Primary Adenosquamous Carcinoma of Stomach.

Adenoaquamous carcinoma of stomach is a extremely rare malignant tumor which is composed of the mixed giandular-epidermoid neoplastic comyonnts. The pathogenesis of this neoplasm remains obscure and has various different theories, such as the origin of squamous components are island of ectopic squamous epithelium in the, gastric mucosa, squamous metaplasia of gastric epithelium, squamous differentiation in a preexisting adenocarcinoma, endotheliai cell differentiated toward squamous elements, and totipotential undifferentiated cells of the gastric mucosa. Primary adenosquamous carcinoma(ASC) was classified into two types, differentiated type adenocarcinomatous component(DAC) and undifferentiated type adenocarcinomatous component(UAC), according to the degree of glandular formation of the adenocarcinomatous elements. As a consequence, with respect to biologic behavior, ASC with DAC was similar to the differentiated type adenocarinoma, and ASC with UAC to the undifferentiated type adenocarcinoma. Accodingly, the behavioristic features of ASC seemed to be governed by the adenocarcinomatous component. We reported a case of primary gastric adenosquamous carcinoma with undifferentiated type adenocarcinomatous component.

7.A Case of Mixed Long Carcinoma Composed of Three Cell Type.

Carcinoma of lung is classified by histologic cell type, this is based on predominant major cell population by tissue specimen. But in fact a range of 13% to 63% of lung cancer has been appeared to be heterogeneous cell type at the light microscopic level. After treatment of the mixed lung cancer and/or with time there are reports that cell type is changed. Because it is possible that the tumor were pleomorphic and that the antitumor therapy eliminated the more sensitive cell population and permitted the subsequent emergence of the more resistant cell population. The authors reported here, a case of 32 year old female patient with mixed lung cancer composed of three cell type, and this is the first case in Korea.

8.Thyroid Dysfunction in Obese Children in Jeonju, Korea.

PURPOSE: The aim of this study was to evaluate thyroid dysfunction in obese children in Korea. METHOD: We measured body mass index and thyroid hormone in obese, overweight, and normal weight children who visited a pediatric clinic in Jeonju, Korea, from January 2009 to September 2012. RESULTS: The free thyroxoine (FT4) level of the obese group (16.35 +/- 2.79 pmol/L) was lower than levels of the overweight (17.65 +/- 2.31 pmol/L) and normal weight (17.28 +/- 2.35 pmol/L; P=0.012) groups. The average value of FT4 in severely obese children (15.71 +/- 1.68 pmol/L) was lower than those of mildly and moderately obese children (16.40 +/- 2.88 pmol/L). We found no significant difference in thyroid stimulating hormone level between the obese group and normal weight group. CONCLUSION: The FT4 level is lower in obese children than in normal weight children.

9.Laparoscopic Totally Extraperitoneal Hernia Repair for Inguinal Hernia Patients: Results of 92 Cases.

PURPOSE: Laparoscopic totally extraperitoneal herniorrhaphy (TEP) was developed as an alternative treatment of inguinal hernias to open hernia repair. This study evaluated 92 cases of laparoscopic surgery to determine the effectiveness and safety of laparoscopic TEP. METHODS: Laparoscopic TEP was performed on 92 patients with inguinal hernias from January 2008 to December 2010. Through a retrospective study of these patients, information om TEP repair was collected including the patients' characteristics, operation time, hospital stay, analgesic use and related complications. RESULTS: Laparoscopic herniorrhaphy were performed on a total of 92 patients (85 men and 7 women, age ranging from 16 to 83 years, with a mean of 56 years). The mean operation time for a unilateral inguinal hernia and bilateral inguinal hernia was 58.7 and 84.2 min, respectively. The mean postoperative hospital stay was 4.0 days (range, 2~9 days). Thirty nine patients were discharged without an analgesic injection, whereas 36 patients were injected with analgesic on the day of surgery. Of these 92 procedures, 10 complications were recorded; one granuloma complication, two patients with operation site discomfort, five with urinary retention issues, one patient with a scrotal seroma, and one patient with scrotal edema. CONCLUSION: TEP repairs have minimal morbidity and are more effective with less pain than the open procedure. TEP repair can be considered a favorable procedure for patients who request minimally invasive procedures for inguinal hernia repairs.

10.Effects of a BeHaS(Be Happy and Strong) Exercise Program on Pain, Health Status and Weight in Middle Aged Women with Osteoarthritis of the Knee.

PURPOSE: The purpose of this study was to identify the effects of a BeHaS exercise program on pain, health status, body weight in middle-aged women who have osteoarthritis in their knees. METHOD: The participants in this study were eighteen women whose mean age was 56.22years and who had osteoarthritis in the knees. The BeHaS exercise program consists of approximately one and half hour of exercise, two days per week, for twelve weeks. Data were gathered from September 2005 to December 2005 using a questionnaire and measuring VAS for pain, KWOMAC for health status, body weight for scale. Data were analyzed with the SPSS Win 11.0 using frequency and paired t-test for differences between pre-exercise and post-exercise. RESULTS: The results of this study are as follows: Scores for pain on post test were statistically lower than pre test(t=4.202, p=0.001), health status was statistically improved after exercise program (t=3.479, p=0.002). But body weight did not change(t=-.774, p=0.222). CONCLUSION: From these results, it is concluded that the BeHaS exercise program can be effective in decreasing pain and increasing health status in middle-aged women with knee osteoarthritis.
