1.Treating acute lung injury by strategy of conservative fluid management combining with lung-protective ventilation

Objective To explore the role of strategy of conservative fluid management combining with lung-protective ventilation in treating acute lung injury. Methods 40 cases with final diagnosis of acute lung injury were selected and randomly divided into experimental group(22cases) and control group(18cases). Between two groups,on the basis of lung-protective ventilation,therapy policy of conservative fluid management was carried out in experimental group, and strategy with a liberal fluid management was taken in the other group. Hie duration of mechanical ventilation and intensive care together with the incidence rate of nonpulmonary complications( congestive heart failure,renal failure and liver failure) were compared between both groups. Results When compared with the control group,the experimental group had the shorter duration of mechanical ventilation and intensive care( P < 0.05 ). And the incidence rate of nonpulmonary complications were similar in both groups. Conclusion Strategy of conservative fluid management combining with lung-protective ventilation could shortened the duration of mechanical ventilation and intensive care for patients with acute lung injury without increasing the incidence rate of nonpulmonary complications, which was an effective and safe treatment and deserved consulting in clinical work.

Reliable data support, information technology and its products are needed to achieve the target of big data-based precision medicine.The mature clinical decision supporting products , such as seamless information product and nursing decision supporting products (my cancer genome and Watson) and studies on their related evi-dence in foreign countries were thus described in this paper with suggestions put forward for scientific achievements in genomes and in docking and integrating clinical electronic medical records, such as training learned bioinformatics professionals, sharing medical data, working out need-guided research strategies, constructing basic bioinformatics framework and cancer knowledge network.
3.Comparison of open access publication environments in domestic and foreign academic journals

After the characteristics and significance of domestic and foreign open access publication were analyzed, the open access publication environments in domestic and foreign academic journals were compared in aspects of open access policy, scientific research management evaluation system and awareness of open access publication in persons engaged in scientific research, journal management system and journal management model, open access publication journals and open access publication papers in order to know the advantages of open access publication and the difficulties it is faced.Certain suggestions were proposed for its future development.
4.Radiation dosimetry estimates of18F-fluoroestradiol based on whole-body PET imaging of mice

Background and purpose:In preparation for using this tracer in humans, this study estimated thedosimetry of18F-FES with the method established by MIRD based on whole-body PET imaging of mice.Methods:Three female mice receivedⅣ tail injections of18F-FES and were scanned for 160 min in an Inveon dedicated PET/CT scanner. This study selected some important organs (brain, lung, liver, heart wall, small intestine, large intestine, kidney and urinary bladder), computed their residence times. Then, the residence times in mice organs were converted to human values using scale factors based on differences between organ and body weights. OLINDA/EXM 1.1 software was used to compute the absorbed human doses in multiple organs for both adult female and adult male body phantoms. Results:The highest absorbed doses in gallbladder wall, urinary bladder wall, small intestine, upper large intestine and liver are 0.072 5, 0.044 5, 0.043 0, 0.031 5 and 0.028 2 mGy/MBq, respectively. The organs which have the lowest ab-sorbed doses were brain (0.005 2 mGy/MBq), followed by skin (0.001 1 mGy/MBq), breast (0.001 1 mGy/MBq), heart wall (0.001 2 mGy/MBq) and thyroid (0.001 2 mGy/MBq). The mean absorbed doses for the other major organs ranged from 0.009 5 to 0.023 5 mGy/MBq. The total mean effective dose is 0.019 0 mSv/MBq and the mean effective doses equivalent is 0.025 0 mGy/MBq. A 370-MBq injection of18F-FES leads to an estimated effective dose of 7.03 mSv for the female. There was no statistical difference in the doses results obtained from direct measurement of18F-FES ab-sorption in normal people between previous publications by others and our work.Conclusion:The whole-body mouse imaging can be used as a preclinical tool for initial estimation of the absorbed doses of18F-FES in humans. Furthermore, the potential radiation risk associated with18F-FES imaging is well within the accepted limits.
5.The molecular epidemiology of a local dengue fever outbreak in Shenzhen

Objective To determine the pathogen of a local dengue fever outbreak in Shenzhen city in 2010,and to analyze the molecular characteristics of the epidemic dengue virus strain as well as explore the possible origin.Methods The serum samples collected from the suspect dengue fever cases were detected for IgM, IgG by enzyme-linked immunosorbent assay ( ELISA ),immunochromatography and dengue virus nucleic acid by real-time polymerase chain reaction (PCR).Serum samples from patients with early stage dengue fever were used to isolate virus with C6/36 and BHK-21 cell lines.The type of isolated virus strain was determined by RT-semi-nested-PCR and realtime PCR.E gene of isolated virus strain was amplified by RT-PCR and sequenced.Homology and phylogenetic tree of E gene of Shenzhen dengue virus with the strains isolated from other areas were constructed.Results IgM,IgG and RNA of type 1 dengue virus were detected in serum samples from dengue fever suspected patients.Type 1 dengue virus named DEV1-SZ1029 was successfully isolated from the serum sample.The homology of nucleotide sequence of E gene of SZ1029 strain with standard type 1 dengue virus HAWAII 45,Fj231/04,GD14/97 and GD05/99 were 94.8%,99.6%,97.7% and 98.5 %,respectively.The phylogenetic tree indicated that SZ1029 had the greatest similarity with the D1/Malaysia/36000/05 strain,SG(EHI)DED142808 strain and Fj231/04 strain and they lied in the same branch of the phylogenetic tree.The isolated dengue virus type 1 belonged to genetype Ⅰ with GZ/80,Taiwan87,All patients lived in a certain construction site in Shenzhen and had no recent travel history outside the area in one month before infection.Conclusions The virological,serological and molecular features all identify that the local dengue fever outbreak in Shenzhen in 2010 is caused by type 1 dengue virus and SZ1029 strain may be transferred from Southeast Asian region,and there may be a plague focus in Shenzhen.
6.The effects of willed movement on the extracellular signal-regulated kinase and cAMP response element binding protein pathway following focal cerebral ischemia

Objective To explore the effects of willed movement on neurological performance and the extracellular signal-regulated kinase (ERK) and cAMP response element binding protein (CREB) pathway in rats following focal cerebral ischemia.Methods Reversible middle cerebral artery occlusion was induced in 144 male SpragueDawley rats using intraluminal sutures,and they were randomly divided into a control group,a swimming exercise group,an environment modification group,and a willed movement group.The observation time points were at 7,15 and 30 days after reperfusion.A behavioral test was performed to evaluate any neurological deficiency.Reverse transcription PCR (RT-PCR) and immunofluorescence were used to detect the ERK and CREB responses in terms of mRNA and phosphorylated ERK (pERK) and phosphorylated CREB (pCREB) protein in the peri-ischemic brain tissue.Results The climbing frequency of the willed movement group was significantly higher than that of the environment modification group.Three days after reperfusion the neurological deficit scores of all groups began to decrease,and that of the willed movement group had decreased significantly more than in the other three groups at all time points.ERK/CREB mRNA and pERK and pCREB protein expression were dramatically up-regulated in the willed movement group at 7,15 and 30 days after reperfusion,significantly more than in the other three groups.Conclusions Willed movement may promote motor recovery by up-regulating and activating the ERK/CREB pathway following focal cerebral ischemia.
7.Biocompatibility of rat osteoblasts with chitosan/hydroxyapatite composite scaffold degradation products

BACKGROUND:The in vivo degradation process of chitosan/hydroxyapatite composite porous scaffolds is not very clear. Research on the effects of rat osteoblasts and degradation products is less.
OBJECTIVE:To analyze the biocompatiblity of rat osteoblasts with degradation products of chitosan/hydroxyapatite composite porous scaffolds.
METHODS:The second generation of cultured rat osteoblasts were respectively cultured in the extract of degradation products of chitosan/hydroxyapatite composite scaffolds (experimental group) and Dulbecco’s modified Eagle’s medium containing 10%fetal bovine serum (control group). At 2, 4, 6, 8, 10 days of culture, cel counting was measured by 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide assay. Alkaline phosphatase activity was measured by the recommended method of determination of the Federation, and total protein was determined by BCA method.
RESULTS AND CONCLUSION:The proliferation speed, alkaline phosphatase activity, total cel ular protein synthesis and ratio of alkaline phosphatase to total protein in rat osteoblasts cultured in the experimental group were significantly higher than those in the control group (P<0.05). This experiment showed that the degradation products of chitosan/hydroxyapatite composite porous scaffolds cannot only promote rat osteoblast adhesion, growth and proliferation, but also enhance its ossification function, with good biocompatibility.
8.Practice of intensive clinical skill training for non-mainland medical interns and its effect

Objective To conduct intensive clinical skill training for non-mainland medicine interns and to explore the effects of training.Methods Twenty-six new clinical interns in the first affiliated hospital of Jinan university from June 2019 to June 2011 were enrolled and the 96 hours of intensive training was conducted.Results The scores of theoretical examination,puncture operation,physical examination and complete medical records of the 26 non-mainland interns were improved after training ( P < 0.05 ) and were higher than those of mainland interns ( P < 0.05 ).The enhancement in complete medical recording skills was the greatest and in puncture operation was the smallest among the four techniques.Conclusions The intensive training can effectively improve the clinical skills of nonmainland medical interns and the improvement of clinical skills in non-mainland interns is greater than that in mainland students.
9.Synthesis of a novel long-circulating dual-receptor targeting and dual-modal molecular probe:an in-vitro study

Objective To explore a novel long-circulating dual-receptor targeting and dual-modal molecular probe and investigate its physicochemical properties and targeting effect on breast cancer cells in vitro. Methods Dual-receptor targeting and dual-modal molecular probe RGD@BBN-lipo(QDs)-SPIO was synthesized in the following steps: long-circulating liposome was prepared by film dispersion method;water-soluble superparamagnetic iron oxide (SPIO) nanoparticles and Quantum dots (QDs) were loaded in the hydrophilic and hydrophobic layer of liposome, respectively;RGD and BBN polypeptides were coupled on the former functional magnetic/fluorescent liposomes. Stability of the probe in different physiological solutions was investigated. Transmission electron microscopy (TEM) and particle size analyzer were used to measure nanoparticle sizes and the Zeta potential. Characterization of RGD and BBN was investigated through 1H-NMR and elemental analysis. The MRI T2 relaxivities (1/T2) of RGD@BBN-lipo(QDs)-SPIO was measured through T2 map scanning on 3.0 T MR system. HUV-EC-C cells were used for assessment of cells viability by MTS assay. Prussian blue staining and fluorescence imaging were carried out to determine the targeted breast cellular uptake of RGD@BBN-lipo(QDs)-SPIO nanoparticles. Results The targeting magnetic/fluorescent dual-model molecular probes appeared spherical or para-spherical,with a mean diameter of(118.2±3.9)nm,Zeta potential of (-24.78±1.68) mV,MR T2 magnetic relaxation rate of 0.498 1× 106 M-1 · s-1.RGD and BBN polypeptides were successfully coupled on the former functionally magnetic/fluorescent liposomes with the bind rates of 33.05%and 45.06%, respectively. There was low cytotoxity of the molecular probe on human umbilical vein endothelical cells(HUV-EC-C)by MTS study. Prussian blue staining and fluorescence imaging studies showed that the RGD@BBN-lipo(QDs)-SPIO nanoparticles could target any αvβ3 or gastrin releasing peptide receptor overexpression breast cancer. Conclusions RGD@BBN-lipo(QDs)-SPIO is a novel long-circulating dual-receptor targeting and dual-modal molecular probe and has excellent physicochemical properties and stability, high T2 relaxivities and strong targeting effect on cancer cells and has laid a solid foundation for early diagnosis of breast cancer.
10.The surgical outcome of sacral decompression and lumbopelvic fixation for H-shaped sacral fracture and correlation factors analysis

Objective To investigate the correlation factors for surgical outcome of sacral decompression and lumbopelvic fixation in H-shaped sacral fracture and the methods to prevent and treat the complications.Methods From January 2008 to January 2016,45 patients with H-shaped sacral fracture treated by sacral decompression and lumbopelvic fixation were respectively analyzed,including 29 men and 16 women,mean age 41.2 (range,24-53 years),mean follow-up time 52.6 months (range,16-93 months).The surgical outcome was evaluated by pelvic outcome score,and correlation factors were analyzed.We analyzed whether each factor was in correlation with pelvic outcome score.Then we integrated the statistically significant indicators into Logistic regression equation to determine the related factors.Postoperative complications were all recorded.Results The average operation time was 161.2 min (range,100-220 min),average blood loss was 491.6 ml (range,370-1 000 ml),injury-surgery interval was 7.2 d (range,1-23 d).In terms of pelvic outcome score,31 (68.9%) patients had satisfactory result and 14 (31.3%) patients had unsatisfactory result.Univariate x2 analysis suggested that cauda equina injury,Roy-Camille classification,L5S1 facet injury,fracture line,kyphotic angle,injury-surgery interval,decompression approach and inserting sacral screws were correlated with pelvic outcome score,but gender,age,injury mechanism and L5 pedicle fracture had no relation with pelvic outcome score.The multivariate Logistic regression analysis showed that cauda equina injury,Roy-Camille classification,fracture line,kyphotic angle,injury-surgery interval,decompression approach and inserting sacral screws were closely related to pelvic outcome score,but L5S1 facet injury was excluded.2 patients required early surgical procedures with proper antibiotics for deep wound infection;3 patients complained pain related to hardware prominence and the pain subsided after removal of implants;1 patient got unilateral rod breakage at 3-6 months and achieved bony fusion after nine months of observation.Conclusion Sacral decompression and lumbopelvic fixation is effective in neurological recovery and early ambulation in treating H-shaped sacral fracture.Better surgical outcome is related to timely surgery,preoperative or intraoperative bone traction,sacral screws insertion,incomplete cauda equina injury,Roy-Camille type Ⅱ,fracture line penetrating S2 and kyphotic angle less than 40°.
